7 Farmacokinetiek Geurloos, niet-toxisch, niet-explosief en cardiovasculair stabiel. Niet gemetaboliseerd in het lichaam. Eliminatie via de longen. Geen verlenging van niet-depolariserende neuromusculaire blokkers. Opstapeling in luchthoudende compartimenten (cfr. N 2 O). MAC 70% naar 63% 65j: 69%, 51%.

8 Farmacodynamiek N-methyl-D-aspartate-receptor antagonist Inhibitie van 5-HT3 serotonine-receptor. AMPA, kainate, nicotine acetylcholine-receptor en TREK-1 kalium kanalen. Analgetisch 50% meer potent dan N 2 O.

9 Cardiaal Niet geblindeerde RCT, 160 ASA I of II patiënten. Lagere hartslag in xenon-groep. Hogere systolische en diastolische bloeddrukken in xenon-groep.

13 Cerebraal Excessieve NMDA-receptor stimulatie => Ca 2+ -influx => apoptose-cascade Xenon beschermt zonder de neurotoxische bijwerkingen. N 2 O en ketamine neurotoxiciteit via dopaminerge pathways. Xenon beschermt tegen dopamine-geïnduceerde schade.

14 Cerebraal Xenon is neuroprotectief bij ratten die tijdelijke ischemie ondergingen van de arteria cerebri media. Preconditionering met xenon heeft een gunstig effect op de infarctgrootte bij muizen. TREK-1 knockout muizen zijn minder gevoelig voor anesthesie. Bij geïnduceerde ischemie en epilepsie worden significantere neurotoxiciteit waargenomen.

15 Eigenschappen Milieuvriendelijk. Geen effect op coagulatie of bloedplaatjes. Geen effect op immuunsysteem. Geen hepatische of renale toxiciteit. Geen indicatie op mogelijk uitlokken van maligne hyperthermie. PONV ondanks inhibitie van 5-HT3 receptor.

16 Kostprijs Anesthesietoestel. 20 / L. Hoogste verbruik tijdens wash-in fase. Closed-circuits meest efficiënt. Recycling systemen. Belang van voldoende preoxygeneren.

17 Conclusie Xenon heeft een meerwaarde t.o.v. andere anesthetica. De grootste handicap blijft de kostprijs. Om dit te rechtvaardigen zullen langdurige operaties met een hoog risico op cerebrale en myocardiale ischemie als eerste in aanmerking komen. Meer onderzoek is nodig o.a. naar kosten-batenanalyse.
