Aanleiding Welk teugvolume is voor non ALI/ARDS patiënten het meest veilig tijdens mechanische beademing? Wat zegt de literatuur hierover? 5
Hoe is de beademing geregeld? Op dit moment geen protocol of richtlijn. Mondelingen afspraak: - alle gecontroleerd beademde patiënten worden met 6 ml/kg ideaallichaamsgewicht beademd. Volwassen man: IBW (kg) = 0.9079 x lengte (cm) - 88.022 cm x kg/cm -Volwassen vrouw: IBW (kg) = 0.9049 x lengte (cm) - 92.006 cm x kg/cm 6
Probleemstelling Mechanische ventilatie veroorzaakt een beademing gerelateerde inflammatoire reactie. 7
Doel studie Door literatuurstudie inzicht te krijgen in het meest optimale teugvolume bij non ALI/ARDS patiënten in relatie tot de inflammatoire reactie. Door retrospectief onderzoek, om inzicht te krijgen in de gebruikte teugvolumina bij non ALI/ARDS patiënten op de intensive care van het Medische Centrum Leeuwarden. 8
Vraagstelling Wat wordt in de literatuur beschreven als het meest ideale teugvolume voor non ALI/ARDS patiënten? Met welk teugvolume (in ml/kg ideaal lichaamsgewicht) worden patiënten beademd op de IC van het Medische Centrum Leeuwarden? 9
Literatuur. Schultz 4 Ognjen Gajic Tenney SM 10 Schultz 11 Grote teugvolumina geven longschade. 9 ALI risicofactoren: grote teugvolumina en bloedproducten Kleine volumina - normale volumina. Kleine volumina ook voor korte beademingen. Determann14 Teugvolume wordt mede bepaald door lengte en geslacht. Determann 15 Teugvolume is een onafhankelijke voorspeller van ALI/ARDS in ontwikkeling. Alle studies en reviews overwegend. 10

Conclusies uit praktijk onderzoek Er is geen richtlijn voor beademing in het Medische Centrum Leeuwarden Beademen met een te groot teugvolume Grote teugvolumina bij vrouwen PEEP is gemiddeld hoger dan 10 cmh 2 O Piekdruk kleiner 30 cmh 2 O FiO 2 is gemiddeld kleiner dan 0,6 20
Conclusie totaal Er wordt beademd met te grote teugen. Er is geen goede richtlijn voor non ALI/ARDS patiënt. Verkorting beademingsduur Mortaliteit Juiste teugvolume? 21
