4 Gebruikte criteria voor granulocytentransfusies Patiënten met granulopenie(< 0,5 x 10 9 /l) gedurende tenminste 72 hr En verwachting dat ze dit nog minstens 5 dagen zijn Koorts (>38.0 C) EN een levensbedreigende infectie: - Proven of probable invasieve schimmel infectie - Invasieve weefsel bacteriele infectie - Bacteriëmie (met ziekte-veroorzakend pathogeen) Patiënten reageren niet op systemische antimicrobiële therapie voor tenminste 48 hr Patiënten met levensverwachting > 3 maanden (in afwezigheid infectie) Patiënten worden verwacht te herstellen van granulocytopenie

7 Geen wetenschappelijk bewijs GTX Implications for practice There is insufficient evidence from randomised controlled trials (RCTs) to support or refute the use of granulocyte transfusion therapy in patients with neutropenia and severe infection to reduce mortality. None of the studies in this review assessed the use of granulocytes derived from whole blood donations. the use of granulocyte transfusions should still be regarded as investigational and should ideally be conducted in the context of ongoing prospective trials designed to answer the question of effectiveness.

Algorithm invasive infections 1 Does the patient have a significant positive isolate from bacterial blood cultures with a disease-causing pathogen (excluding positive blood cultures disappearing after removal of IV catheters/devices)? 2 Does the patient have clinical signs and symptoms compatible with an invasive tissue bacterial infection > 5 cm in diameter? 3 Does the patient have a (suspicion) for invasive fungal disease? 4 Does the patient have a strong clinical suspicion for invasive tissue infection despite not clearly fulfilling the criteria 1-3 above? 10 Casus I Meisje X, 8 jaar (2014) Komt 16-9 over uit een ziekenhuis elders In mei 2014 gediagnostiseerd met SAA. Veel infectieproblematiek, waaronder aspergillus. Heeft geen ATG gehad vanwege infectierisico. Vanwege progressieve longafwijkingen/ necrose door aspergillus is de linker bovenkwab in juli van dit jaar chirurgisch verwijderd. X. heeft een urgente diagnose voor SCT, en gezien aspergillus is verzoek dit onder granulocytentransfusies te doen

15 Refractair op granulocytentransfusies 1-10: pte heeft HLA klasse II antistoffen ontwikkeld: DQ1 NB klasse II zit niet op granulocyten.. maar kan wel koortsreacties verklaren. In loop der dagen uitbreiding antistofvorming: 8-10: DR11,DQ1,DQ : DR3,DR5,DR6,DR8,DQ1,DQ7,DP Donor 2 typering: DQB1*02:01/02:07/02:08+, DQB1*03:01/03:19/03:09+; Gevonden DQB1*03 allel is een matchdeterminant van een DQ7 Besloten omdat geen donoren te vinden die aan typering voldoen (<5% populatie) granulocytensupport te staken beenmergtransplantatie onverwant 9/10 gematcht Goed beloop: stabiele engraftment met % donor chimerisme

16 Casus II Dhr Y. 56 jaar 2011 MDS-RAEBII, behandeling HOVON allogene non-myeloablatieve MUD SCT 10/ pancytopenie na eerdere repopulatie stamcelboost zonder effect. BM biopt: geen hematopoiese Infectieuze problemen Recidiverend bacteriemiën Schimmelinfectie KNO gebied (Alternaria) MRI: Geen botdestructie, wel infiltratie vetweefsel + pijn rechter oog Plan: overbrugging met granulocytentransfusies naar 2 e allogene SCT (14 dec) met andere stamceldonor

21 Conclusies casus II Ontstaan antistoffen tegen granulocyten waardoor geen opbrengst Ontstaan HLA klasse 2: anti-dp a.s. Blijkt gericht tegen DP mismatch 1 e stamceldonor NB ook anti-e gevormd Gezien 2 e allo SCT in de planning en actieve antistofvorming GTX gestaakt

22 Beloop casus II 2 e allogene SCT Hier ook een DP mismatch. Pt heeft DPB1*05:01:01 en ;06:01 en donor heeft DPB1*02:01:02. Geen a.s, tegen DPB1*02 in luminex. Gecompliceerd door neurologisch beeld en infectieuze problematiek. Overleden op IC

23 Alloimmunisatie bij granulocytentransfusies, een probleem? 2 casus met ontstaan allo antistofvorming tijdens support GTX en leidend tot het niet kunnen vinden van geschikte donoren o Patiënten in deze setting kunnen allo antistoffen maken! o Dit leidt tot verminderde opbrengst op granulocyten transfusies o En eventueel transfusiereacties o Er is risico op TRALI o HLA antistoffen zijn geassocieerd met rejectie/graft failure stamceltransplantaat en verminderde survival, daarom antistofvorming voorafgaande aan geplande SCT ongewenst

25 Take home messages Alloimmunisatie kan ontstaan door granulocytentransfusies HLA a.s. en granulocyten a.s. Dit kan leiden tot verminderde opbrengst en transfusiereacties Cave toekomstige SCT-kandidaten Advies is om patiënten actief te volgen in ontstaan van HLA a.s. (wekelijks) en bij refractoriteit ook naar granulocyten a.s. te zoeken Geef alleen transfusies van donoren die geen antigenen hebben waartegen de antistoffen gericht zijn. Luminex technologie bij patiënt met gerichte donor HLA typering heeft CDC lymfocytenkruisproef overbodig gemaakt 25 Insert > Header & footer 19-mei-17
