Sedentary Lifestyle (SL) / Inactiviteit WHO fourth leading risk factor for global mortality, accounting 6 % of deaths globally. Less active and less fit people have greater risk to develop CHD (even after accounted for smoking / alcohol, and diet) Greater change for developing cancer. SL is associated with several musculoskeletal disorders.
Gevolgen SL / PI Inactive adults experience a 3 8 % loss of muscle mass per decade, accompanied by resting metabolic rate reduction and fat accumalation. (Westcott, 2012) Sarcopenie groot risico
Sarcopenie
Muscle loss / SP Dergelijke (patho) fysiologische processen geassocieerd met: Metabool Syndroom Diabetes Mellitus Type 2 (Hu 1999, Siegel 2009) Chronische (lage rugpijn) (Kell 2012, Westcott 2012, Mobasheri 2013)
Exercise is Medicine (EIM) EIM is a global health initiative managed by the American College of Sports Medicine (ACSM) that is focused on encouragaging primary care physicians and other health care providers to include physical activity when designing treatment plans for patients. www.exerciseismedicine.org
Enough Exercise? Prescribing physical activity in the right dosage is a highly effective prescription for the prevention, treatment and management of more than 40 of the most common chronic health conditions encountered in clinical practice. Bewegen? / sportief bewegen? en / of sport?
Physical activity vs Exercise Physical Activity Physical activity can be defined as any activity that involves some form of physical exertion and voluntary movements that burn calories. Such an activity causes a person s body to work harder than normal.
Physical Activity vs Exercise Exercise Exercise also involves physical exertion, voluntary movements and burning calories. This form of physical activity, however, is specifically planned, structured and repetitive. It does not usually involve any kind of competition.
Exercises Aerobic exercise Resistance exercise Average individuals saw no reason to engage in weight training, and participants in sports (other than weightlifting) typically felt that lifting weights actually would hinder their athletic performance. (Westcott 2012)
Resistance training / Strength Training? = Meer dan gewichtheffen ( = weight training) Resistance training is a form of physical activity that is designed to improve muscular fitness by exercising a muscle or a muscle group against external resistance. (ACSM 1998) Bijv. Cross fit / Bootcamp

Fysiotherapie Richtlijnen Lage rugpijn (2013) KANS (2010) Therapeutische interventie oefenen Graded activity
Bv. - Welke vorm bewegen / activiteit? - Welke Intensiteit? - Welke frequentie?

Vicieuze cirkel The physiology of inactivity (Painter 1994) Impaired health Inactivity Deconditioning Disability
Fysiotherapie (Oefenen?) Exercise training is a subcategory of physical activity or physical activity that is planned, structured, repetitive, and purposive in the sense that improvement or maintenance of one or more components of physical fitness is the objective (Caspersen et al., 1985).
Fysiotherapeutische interventie? Exercise training is one key management strategy used to address impairments (problems with body function or structure such as pain or weakness), activity limitations (the difficulties an individual may have in activities such as walking, sitting, and standing) and participation restrictions (problems an individual may experience in everyday life situations such as working, playing sports or socializing) (World Health Organization, 2001).
Fysiotherapeutische interventie? Multiple meta-analyses on the effects of exercise on various painful conditions report that exercise reduces pain and increases aerobic capacity and physical function. The mechanisms behind these effects are not completely clear and are most likely due to multiple factors (Wittink and Verbunt, 2008).
Sneller actieve benadering
Kristensen 2012 12 RCT s (1991 2010) Ruim 350 personen Gem leeftijd: rond de 40 jaar Outcome measure Muscle Strenght Pain score Functional Ability

Searle 2015 Coördinatie / Stabilisatie En Strength / Resistance Beide positieve effecten. (Pijn en ADL) Largest effect size of all the exercise groupings was noted in the strength / resistance trials that concentrated on whole body and trunk.
Searle 2015 Time frame (trainingsperiode > 12 weken) Losse gewichten / Vrije ruimte oefeningen ACSM guidelines (2011) Geen toegenomen risico m.b.t. blessures
2 minuten.
Conclusie RT is a valid therapeutic tool in the treatment of the most common musculoskeletal injuries, especially those of a chronic variaty. RT is effective across age and gender. A high approach (> 70 % of 1 RM) more effective.
