Neuropsychiatrische aandoening bij uitsteek Motor-symptoms Beven Traag en onhandig Non- Motor-symptoms: e.g. cognitive changes psychiatric symptoms autonomic dysfunction sleep disturbances
Prevalentie psychiatrische stoornissen Apathie 17-70% Depressie 10-40% 30% bij debut Angst 30-70% Psychose 40-50%
Relatie met Parkinson subtype PD patients can be accurately and reliably classified into four different subtypes: rapid disease progression subtype, young-onset subtype, non-tremor-dominant subtype with more psychopathology and a tremor-dominant subtype. Cognitive deterioration, depressive and apathetic symptoms, and hallucinations all cluster within the non-tremordominant motor subtype, that is characterized by hypokinesia, rigidity, postural instability and gait disorder. (Reinders et al., Parkinsonism Relat Disorder 2009;15(5):379-82).
Bimodaal beloop psychiatrische stoornissen Depressie Angst Early peak Depressie Psychose Late peak
Neural Circuits: Dopamine Rodrigues-Ores et al., Lancet Neurology 2009;8:1128-1139
Neuropathologische stagering Braak et al., Cell and Tissue Research (2004), 318 (1): 121-134
Depressie Major depression (10-30%) Minor depression (10-20%) Dysthymia (10-20%) Off fase depressie 75% motor fluctuatie ook mood fluctuatie
Angst Research suggests that anxiety is not just a reaction to the motor symptoms of PD, but a manifestation of the disease itself. Anxiety may precede the onset of motor symptoms of PD by years. Abnormalities in PD patients in nerve transmission may also be responsible for anxiety Norepinephrine Serotonin Dopamine
Angst Angststoornis bij parkinson zijn onderbelicht Paniekstoornis (4-30%) Generele Angststoornis (10-20%) Sociale fobie (~15%) Off fase angst
Angst: Diagnostiek en behandeling Recente vergelijking van Beck anxiety inventory (BAI), the hospital anxiety and depression scale (HADS), the Zung selfrating anxiety scale (SAS), and anxiety status inventory (ASI), the Spielberger state trait anxiety inventory (STAI), and the Hamilton anxiety rating scale (HARS).
Apathie Inability: to associate emotion/affect with behavior to accurately decode the affective context that guides behavior to evaluate the consequences of actions in terms of positive or negative outcome
Apathie Quantitative reduction of voluntary actions associated with: emotional blunting (reactivity to emotional situations is poor and short live) loss of interest to daily-life activities, situations or stimuli that were previously considered as motivating decreased reward sensitivity, assessed by the reversal tasks (Rolls et al., 1994) decreased involvement in affective aspects of life (social interpersonal conduct, sexual life, decline in personal hygiene)
Apathie no depressive state not necessarily associated with cognitive deficits not necessarily reversed by strong external solicitation Location of the lesions or dysfunctions associated with this mechanism: Orbital and medial PFC (BA 13, 14, ventral 10) (Limbic territories of the basal ganglia [ventral striatum, ventral pallidum])
Apathie: Diagnostiek en behandeling Apathy Evaluation Scale (AES), Apathy Scale (AS), Apathy Inventory (AI), Lille Apathy Rating Scale (LARS), UPDRS Item 4 (screening), Item 7 NPI Apathy subscore].
Psychose: Behandeling Indien mild, niets doen / Psychoeducatie Parkinson medicatie verlagen Zn levodopa monotherapie Atypische neuroleptica Quality Standards Subcommittee of the American Academy of Neurology (AAN) clozapine should be considered effective (Level B), quetiapine may be considered effective (Level C) (Miyasaki et al. Neurology 66 (2006), 996 1002) Rivastigmine
Impulscontrolestoornis Dopamine reward system Projections between VTA and nucleus accumbens is impaired reduced hedonic tone. apathy hedonic overcompensation. ICDs
Range of Impulse Control & Compulsive Disorders ( ICDs ) in PD GSES or behavioural addictions Pathological gambling Binge eating Compulsive shopping Hypersexuality/paraphilias Punding, hobbyism Dopamine addiction
X-sectional study (Weintraub et al, 2008) Multi-centre study (> 3000 pat.; presence of ICDs x 6m At least one ICD: 14% compulsive buying: Pathological gambling: Binge eating: Compulsive sexual behaviour: 6% 5% 4% 4%
X-sectional study (Weintraub et al, 2008) No difference in ICD for the different dopamine agonists Dopamine agonist treatment had 2-3 x increased risk of ICD (class association)
