2016 verschillende (systematic) reviews conservative treatment of carpal tunnel syndrome
1. Patient education - First line management approach - Limitation of wrist movement and reduction of heavy work activities - Ergonomic working tools (little adequate evidence) Eg. the effectiveness of ergonomic keyboards in cts is unknown
Pharmacotherapy - Local corticosteroid injections - Reduce oedema, improving spatial relation (nerve tendons) - RCT 111 patients: methylprednisolone (80 mg or 40 mg) injection more effective than placebo, reducing symptoms severity and rate of surgery at 1 year - The palmar approach is less painful (vs proximal) - No difference in objective measures, such as nerve conduction findings
Surgical or non-surgical RCT local corticosteroid injection vs surgical decompression: - Similarly effective at alleviating symptoms, with corticosteroids being more effective in short-term follow- up (3 months) and surgical release having additional benefit for symptom resolution in the long term (2-year follow-up) - Clinical outcomes significantly improve after decompressive surgery and several non-surgical treatments (eg. splints and low-level laser therapy), but decompressive surgery shows a higher long-term effectiveness - Surgery is also more effective than non-surgical treatment at improving electro- physiological measures
