8 Definitie CMS (Consensus meeting PFS society 2015) Post-operative pediatric CMS is characterized by delayed onset mutism/reduced speech and emotional lability after cerebellar or 4th ventricle tumor surgery in children. Additional common features include hypotonia and oropharyngeal dysfunction/dysphagia. It may frequently be accompanied by the cerebellar motor syndrome, cerebellar cognitive affective syndrome and brain stem dysfunction including long tract signs and cranial neuropathies. The mutism is always transient, but recovery from CMS may be prolonged. Speech and language may not return to normal, and other deficits of cognitive, affective and motor function often persist.

10 Lange termijn gevolgen na CMS: niet alleen spraak!! Patienten met CMS die langer dan 4 weken duurt hebben een verhoogd risico op sspraak en taal pathologie (Robertson et al 2006 ) Patients met CMS, die behandeld worden voor een medulloblastoom hebben meer neurocognitieve defecten dan kinderen met een medulloblastoom zonder CMS postoperatief. 13 Cerebellum en taal/ spraak Transneuronale tracing technieken toonden aan dat er verbindingen zijn tussen het cerebellum en de cerebrale hemisferen R >L cerebellaire hemisfeer zijn betrokken bij verschillende componenten van taal productie.

14 Er is ovetuigend bewijs dat CMS ontstaat door bilaterale schade aan het outflow circuit van cerebellum naar de thalamus.

15 Dentato-thalamo-cortical tract (DTC) Bilaterale beschadiging ergens in dit circuit leidt to CMS Concept bevestigd door veel radiologische studies (DTI, perfusie, diffusie)

25 CCAS en CMS symptomatologie liggen in elkaars verlengde CMS is een hyperacute vorm van CCAS Dus luister bij kinderen met beschadiging van het cerebellum niet alleen naar spraak, maar ook naar taalstoornissen. Daarnaast verdienen ze een volledig neuropsychologische analyse.
