Multiple Sclerose Behandeling: Het verleden, heden en toekomst Momenteel is er geen bekende curatieve behandeling voor MS. Echter, wel zijn er een aantal bewezen behandelmogelijkheden beschikbaar, die: De frequentie en de ernst reduceert van de MS exacerbaties De progressie vertraagt van MS En de mate van handicap vertraagt 14
De bewezen MS behandelingen worden in 2 groepen verdeeld: Immunomodulatoren, die het immuunsysteem veranderen. Immunosuppressiva, die het immuunsysteem onderdrukken. De huidige bewezen behandelingen voor MS worden 1 of enkele keren per week subcutaan (= onder de huid) of intramusculair (= in de spier) geïnjecteerd of door een infuus direct in de bloedbaan. Verschillende nieuwe therapieën zijn in ontwikkeling voor de behandeling van MS, die zowel per injectie dan wel oraal gegeven worden. 15
Immunomodulatoren Bij patienten met MS die immunomodulatoren gebruiken wordt het ontstekingsproces dat zenuwbeschadiging teweegbrengt geremd. Vier immunomodulatoire therapieën zijn wereldwijd geregistreerd voor de behandeling van MS: 1. Interferon (IFN) beta-1a (Rebif [Merck Serono] and Avonex [Biogen Idec]) 2. IFN beta-1b (Betaferon [Bayer Schering Pharma]) 3. Glatirameer-acetaat (GA; Copaxone [Sanofi/Teva Pharmaceuticals]) 4. IFN beta en Glatirameer-acetaat zijn eerste lijnstherapie voor MS. 16
Doseringsschema s van de huidige beschikbare MS behandelingen Interferon (IFN) beta-1a: Injectie 1 keer per week intramusculair (Avonex ; Biogen Idec) of subcutaan 3 keer per week (Rebif ; Merck Serono) Interferon beta-1b (Betaferon ; Bayer Schering Pharma): Injectie subcutaan om de dag. Immunosuppressieve therapie Immunosuppressieve therapie remt de cellen van het immuunsysteem die de schade veroorzaakt van MS. In het algemeen geldt het als tweede lijns therapie, dus als andere therapie niet geholpen heeft of bij patienten die van het begin van de MS een snel progressieve ziekte hebben. De beperkingen van de huidige therapieën heeft geleid tot de ontwikkeling van verschillende orale medicaties die elk op een andere manier ingrijpen in het MS ziekteproces. Oraal fingolimod (FTY720): een nieuwe behandeling voor Multiple Sclerose Oraal fingolimod (sphingosine 1-fosfaat receptor modulator) is de eerste van een nieuw type medicijnen voor de behandeling van MS. Conclusies: - Veel nieuwe MS geneesmiddelen op komst, die effectiever en gebruiksvriendelijker zijn (bv tabletten). - Eerste orale MS medicijn in NL zal wrs Fingolimod (Gilenya) zijn. Verwachting is eind 2011/ begin 2012. - Door effectievere middelen meer ernstige bijwerkingen (PML, opportunistische infecties, herpes-infecties) - Meer medicatie-mogelijkheden voor de patient en neuroloog. Meer individueel gerichtere therapie mogelijk. - Rol MS centra steeds groter. Meer ervaring en expertise in behandelmogelijkheden.
