THE ISSUE IS TISSUE
EBUS / EUS MEDIASTINOSCOPIE Voordeel van deze technieken is dat ze ambulant kunnen gebeuren onder locale verdoving (meestal met tevens gecontroleerde sedatie) en met lagere morbiditeit/mortaliteit dan mediastinoscopie.
GROEP A Patiënten met inoperabele majeure mediastinale infiltratie door tumor (T4) of niet meer aflijnbare klierpakketten: - CT evaluatie voltstaat, invasieve stagering niet nodig - Zo nog nood APO voor diagnose: EBUS-TBNA of EUS-FNA zijn mogelijke alternatieven voor mediastinoscopie.
GROEP B - Patienten met CT vergrote klieren (korte as > 1cm) ( PET +/-) STEEDS invasieve staging: afhankelijk van lokale expertise mediastinoscopie of EBUS-TBNA / EUS-FNA. - Patienten met CT klieren (korte as < 1cm) (PET +) STEEDS invasieve staging: EBUS-TBNA / EUS-FNA als initiele approach Een negatief resultaat van endoscopische techniek moet altijd bevestigd worden ahv mediastinoscopie!
GROEP C - Patienten met centrale tumor of clinical N1 (> 1 cm of PET +) met een normaal mediastinum: STEEDS invasieve stadiering: mediastinoscopie bij voorkeur.
GROEP D - Patienten met kleine perifere tumor (< 3 cm) (bij voorkeur Spino) zonder CT vergrote klieren EN zonder PET + klieren thv hilus en mediastinum Invasieve stadiering kan achterwege gelaten worden. (FN 5%)
A randomized trial comparing endosonography followed by surgical staging versus surgical mediastinal staging alone in non-small cell lung cancer: The ASTER study. Results: 241 patients were randomized as planned: 123 to ES+SS and 118 to SS alone. Nodal metastases were found in 62 patients (50%) (56 by EUS/EBUS + 6 by subsequent SS) vs. 41 (35%) by SS alone (p = 0.019). The sensitivity for mediastinal metastases for ES+SS was 94% (95% CI, 85-98) versus 80 % (95 CI, 68-89) for SS alone (p = 0.042). Thoracotomy was futile in 8 patients (7%) staged by ES+SS vs. 21 (18%) in those staged by SS alone (p = 0.009). The rate of complications during staging was similar in both arms (6 vs. 7 patients, p = 0.8), however 12/13 were due to SS. Conclusions: Starting mediastinal staging with endosonography in resectable NSCLC 1) improves the detection of nodal metastasis 2) reduces futile thoracotomies 3) and has a similar complication rate, as compared to surgical staging alone. Systematische lymfeklierdissectie wordt aangeraden naast intrapulmonale en hilaire lymfeklieren: - voor tumoren van de rechter long: stations 2, 4, 7, 8, 9 - voor tumoren van de linker long: stations 4, 5, 6, 7, 8, 9
