Mission of the newborn screening : The newborn screening is committed - to ensuring every newborn receives high quality and timely screening and treatment for serious diseases - to providing a comprehensive program of - education : parents (prenatal), personnel - analysis - follow-up (communication) - management - evaluation - research and development The NBS procedure is a very complex one, still more complex since the implementation of extended newborn screening. Many traps and pitfalls can occur along the whole procedure.
Criteria van Wilson en Jungner 1. De opgespoorde ziekte moet ernstig zijn; de ziekte moet eveneens voldoende frekwent zijn in de bestudeerde populatie* expanded newborn screening 2. Er moet een behandeling bestaan* genetisch advies, prenatale diagnose, zwangerschapsonderbreking 3. Bijkomende onderzoeken maken het mogelijk een preciese diagnose te stellen 4. De ziekte kenmerkt zich door een pre-klinisch asymptomatisch stadium WGO, 1968
Criteria van Wilson en Jungner 5. Het onderzoek voor de neonatale opsporing is eenvoudig, goedkoop en betrouwbaar (gevoelig en specifiek), en aanvaardbaar voor de bestuurdeerde bevolking 6. De natuurlijke evolutie van de aandoening is goed gekend 7. Er moet duidelijk afgesproken worden welke «ziekte» opgespoord wordt 8. Vroegtijdige behandeling verbetert de prognose 9. De vroegtijdige behandeling bespaart kosten aan de maatschappij 10. De opsporing is een eenmalige activiteit die niet moet herhaald worden. WGO, 1968
Neonatal screening in Belgium Neonatal screening for phenylketonuria was - initiated in Brussels in 1965 by Prof. HL Vis - HSP, ULB - organized at a national level from 1968 onwards. A national committee was in charge of the screening program until 1993. Requirements for agreement and funding of the screening laboratories were defined with regard to - techniques, personnel and organization, - the follow-up of children with a positive screening. The law was modified in 1974 and the neonatal screening extended to diseases characterized by high levels of tyrosine, leucine, histidine, methionine and galactose. The systematic screening for congenital hypothyroidism started in 1980. Pilot projects: CF, CAH, biotinidase deficiency... NB: any additional screening required another procedure.

Tandem massa spectrometer (MS/MS) - bestaat uit twee massa spectrometers in serie - maakt het mogelijk een zeer nauwkeurige afzondering te bekomen van een groot aantal metabolieten die vervolgens met grote precisie kunnen gekwantificeerd worden - vertrekkend van een staal gedroogd bloed op filterpapier - de methode kan volledig geautomatiseerd worden - een zeer groot aantal stalen kan onderzocht worden 24 u /24 u => radikale omwenteling op het screeningslaboratorium
Tandem massa spectrometrie (MS/MS) op het screeningslaboratorium - laat toe, in routine, door één enkel onderzoek, een veertigtal moleculen te onderscheiden en te kwantificeren : - de meeste aminozuren, - vrije carnitine en een groot aantal acylcarnitines, intermediaire metabolieten van de beta-oxidatie van de vetzuren, - acylcarnitines waarvan de bepaling de diagnose toelaat van enkele organische acidurieën. de vraag, wanneer een nieuwe screening wordt voorgesteld, stelt zich dus niet meer in termen van extra-kosten zoals voordien, toen voor elke nieuwe screening op een nieuwe techniek moest beroep gedaan worden, maar in termen van «komt een ziekte in aanmerking om opgespoord te worden?»

Vlaams bevolkingsonderzoek naar aangeboren aandoeningen bij pasgeborenen via een bloedstaal Werkingsgebieden organisades met terreinwerking
Opdrachten van de organisades met terreinwerking een aanbod van screening aan alle pasgeborenen in Vlaanderen; het informeren van (toekomsdge) ouders over het bevolkingsonderzoek; het bevorderen en bewaken van kwaliteit van staalafname; de analyse van de stalen en interpretade van de screeningsresultaten; de resultaatsmededeling bij afwijkend screeningsresultaat ; het nagaan of na afwijkend screeningsresultaat een vervolgonderzoek of behandeling wordt ingezet; de kwaliteitsbewaking van de staalafname en analyse van de stalen; de coördinade van het bevolkingsonderzoek, in samenwerking met het Vlaams agentschap Zorg en Gezondheid en de Vlaamse werkgroep Bevolkingsonderzoek naar aangeboren aandoeningen. Om die opdrachten te realiseren, voorzien de organisades met terreinwerking in informade- materiaal en - kanalen en een communicadeplan, bieden ze ondersteuning aan staalafnemers (bv. vorming of feedback over de kwaliteit van staalafname) en beheren ze een registradesysteem met gegevens over alle gescreende pasgeborenen in Vlaanderen. De organisades met terreinwerking volgen de procedures zoals beschreven in het Draaiboek 2012.

Continuous dripfeeding; subsequently every 2 hours.
3-methylcrotonyl-CoA carboxylase (MCC) deficiency Isolated biotin-resistant MCC deficiency is an autosomal recessive disorder of leucine catabolism that appears to be the most frequent organic aciduria detected in TMS-based neonatal screening programs (C5OH, hydroxy-isovaleryl carnitine). The phenotype is variable, ranging from neonatal onset with severe neurological involvement to asymptomatic adults. MR Baumgartner et al, JCI 2001
Treatable disease but with variable penetrance : 3-methylcrotonyl-CoA carboxylase deficiency Arnold GL et al, 2012 - Retrospective analysis of 35 cases of 3-MCC deficiency identified by newborn screening and diagnosed by enzyme or molecular analysis. - Strong inverse correlation between initial C5OH level and residual enzyme activity; free carnitine deficiency relatively common. - Few reports of hypoglycemia, ketosis, poor feeding/failure to thrive or fasting intolerance; no clear relationship between symptoms and residual enzyme activity. - No apparent relationship between residual enzyme activity and developmental outcome Conclusion : - Residual enzyme activity clearly related to metabolite elevation ; no apparent relationship with other measures of outcome. - Concerning frequency of neurologic abnormalities or metabolic symptoms (poor feeding, hypoglycemia, fasting intolerance, etc.), but unclear significance.
Treatable disease but with variable penetrance : 3-methylcrotonyl-CoA carboxylase deficiency The majority of children diagnosed by NBS have been reported to have remained asymptomatic so far [6,7,11,32]. Moreover, several asymptomatic MCC-deficient mothers have been identified only by detection of abnormal metabolites in the neonatal-screening sample from their healthy babies [3,6,11,22] and a number of asymptomatic affected siblings have been identified by family screening [33-35]. The comparative analysis of published case reports with German NBS data indicated that probably less than 10% of affected individuals develop symptoms [6]. Therefore, MCC deficiency may be considered to be a genetic condition with low penetrance (Grünen SC et al, 2012).

Improved quality : analytical stage De MS/MS technologie laat soms toe, dank zij een tweede onderzoek op dezelfde staal (second tier test), de efficiëntie en in dit geval de specificiteit - van de neonatale opsporing te verbeteren : Neonatale opsporing en diagnose, met vervroegde instellen van de behandeling, van propion- en methylmalonzuuracidurie, door middel van de bepaling, op dezelfde staal gedroogd bloed, van propion- en methylmalonzuur bij elk kind waarbij een abnormaal hoge concentratie propionylcarnitine gevonden wordt.
