EPA s in de Common Trunck van de herziene opleiding anesthesiologie Een Entrustable Professional Activity (EPA) is een taak, die deel uitmaakt van de normale werkzaamheden van een professional, die afzonderlijk kan worden toevertrouwd aan iemand, die over de vereiste competenties beschikt. Voor elke EPA zijn competenties uit verschillende competentiegebieden noodzakelijk. EPA s in een opleiding zijn te beschouwen als essentiële bouwstenen. Door de EPA s te combineren kan een huis (de opleiding) worden gebouwd. Echter, voor een huis zijn er ook onderdelen nodig, naast de bouwstenen, zoals deuren, ramen, waterleiding en bedrading. In een opleiding is het niet anders, de verbinding tussen de EPA s bepaalt uiteindelijk hoe de opleiding eruit ziet en wat het rendement van de opleiding zal zijn. In het Project Herziening Opleidingseisen Anesthesiologie is er voor gekozen om de Common Trunck te bouwen rond een 20-tal EPA s. Deze EPA s zijn aangegeven in onderstaande lijst. Met deze EPA s worden in algemene termen de belangrijkste perioperatieve taken van een anesthesioloog (in opleiding) benoemd. Daarbij is aangegeven bij welk thema uit de opleiding deze EPA s behoren. Tevens, aan het eind van welk opleidingsjaar deze EPA door een AIOS dient te worden beheerst op het niveau van indirecte supervisie. Let wel, supervisie is er tot het eind van de opleiding. Maar cruciaal is het moment dat de supervisor besluit dat deze niet meer steeds fysiek aanwezig hoeft te zijn op het moment dat een specifieke AIOS de betreffende taak uitvoert. In de tabel is aangegeven welke A-kliniek tijdens de ontwikkelingsfase (maart-september 2015) primair verantwoordelijk is voor het uitwerken van de betreffende EPA. Het betreft hier een voorstel, klinieken kunnen in onderling overleg veranderingen in deze verdeling aangeven. Het voornemen is dat onder de verantwoordelijkheid van de diverse A-opleiders de betreffende EPA s worden uitgewerkt door stafleden en AIOS uit de verschillende A-klinieken en/of daarbij behorende clusterziekenhuizen. Een format voor het uitwerken van de EPA s is hieronder geleverd. Streven is dat de uitgewerkte EPA s is alle opleidingsclusters kunnen worden uitgevoerd, inclusief de beschreven beoordelingssystematiek. Tijdens de ontwikkelingsfase dient deze beoordelingssystematiek te worden getest, om te bewaken dat deze in de praktijk uitvoerbaar is. Toegevoegd is een drietal EPA s als voorbeeld. Deze voorbeelden werden ontwikkeld in het kader van een internationale studie naar workplace-based assessment, vandaar dat ze in de Engelse taal zijn gesteld. 1

Format voor EPA s in de Common Trunck Titel van de EPA Gedetailleerde beschrijving Welke specifieke kennis, vaardigheden en attitudes zijn er nodig om de EPA op het gewenste niveau te beheersen? Welke CanMeds competentiegebieden zijn het meest relevant voor deze EPA? Hoe kan de ontwikkeling van de AIOS in het beheersen van deze EPA worden bepaald? Aan welke criteria moet zijn voldaan door de AIOS om deze EPA op het niveau van indirecte supervisie te mogen verrichten? Een korte beschrijving in maximaal 15 woorden Beschrijving van de taak die wordt toevertrouwd, inclusief een beperking tot specifieke patiëntengroepen of context. De taak moet binnen een bepaald tijdvak kunnen worden afgerond, moet kunnen worden geobserveerd en beoordeeld. De taak dient voldoende breed te zijn geformuleerd dat deze enige universele toepasbaarheid heeft (dus niet toegespitst op een unieke patiënten situatie) maar weer niet zo breed dat deze grenzeloos wordt. Er dient te worden uitgegaan van het gewenste niveau van functioneren van een AIOS in deze opleidingsfase. Wat moet de AIOS kennen en kunnen voor de EPA? Denk aan leerstof, vaardigheden en trainingen, en aan eisen die kunnen worden gesteld aan non-technical skills. Een EPA bestrijkt altijd meerdere competentiegebieden. In de loop van de opleiding moeten alle competentiegebieden voldoende aan bod komen. Het beoordelen van de ontwikkeling is een essentieel onderdeel van elke EPA. Denk hierbij aan kpb s, 360 graden feedback, case based discussions, OSAT s, simulatie sessies enzovoort. Een concrete opsomming van de eisen waaraan moet worden voldaan, wil deze taak worden toevertrouwd. Welke stages en trainingen moeten zijn gevolgd, welke resultaten moeten zijn behaald voor de afgesproken beoordelingsmethoden? Uiteindelijk zal een X aantal leden van de opleidingsgroep moeten tekenen voor het bereiken van het gewenste niveau door de AIOS. 3
