Tele-Cardiologie 6 Geïmplanteerde sensoren 400 patiënten met HF en/of hartritmestoornissen met geïmplanteerde ICD/CRT devices worden opgevolgd in klinisch callcenter van ZOL/ Jessa. Externe sensoren HF en hypertensie 132 patiënten met HF of te hoge bloeddruk worden op afstand gevolgd door het registreren van fysiologische parameters zoals gewicht, hartslag en bloeddruk. Tele-Revalidatie 61 patiënten worden momenteel opgevolgd tijdens hun hartrevalidatie m.b.v. een accelerometer en een interactieve webapplicatie.
