Voorbereiding CTC-OC CTC OC 2 dagen voor het onderzoek Restenarm dieet Restenarm dieet 1 dag voor het onderzoek Laag residu dieet (Nutra Prep) Restenarm dieet + 3 x 20 ml Tagitol (Barium) 18 uur: 18 g magnesium citraat 18 tot 20 uur: 2 l water met Klean Prep (Loso Prep) in 250 ml water (1 glas van 250 ml om de 15 minuten) 19.30 uur: 4 tabletten bisacodyl (Dulcolax)

Voor- en nadelen CTC Voordelen 1. Minimaal invasieve techniek - Intraveneuze toegangsweg (Buscopan) - Dunne rectale canule 2. Gering risico op bloeding of perforatie 3. Beter getolereerd dan optische colonoscopie - Kortere voorbereiding die minder belastend is - Geen sedatie - Geen recovery periode na de procedure 4. Uitvoerbaar wanneer optische colonoscopie niet lukt (10%) - Dolichocolon - Colonstenose of colonobstructie - Vergroeiingen 5. Detectie van klinisch relevante afwijkingen buiten het colon (5%)
Voor- en nadelen CTC Nadelen 1. Röntgenstraling +/- 5 msievert = dosis achtergrondstraling van 20 maanden 2. Perforatie (gering risico) 3. Geen resectie van eventuele poliepen mogelijk (7 à 8 % van de procedures)

CTC en screening: recente wetenschappelijke gegevens Screening and Surveillance for the Early Detection of Colorectal Cancer and Adenomatous Polyps, 2008: a Joint Guideline from the American Cancer Society, the US Multi Society Task Force on Colorectal Cancer, and the American College of Radiology, Cancer J Clin, 5 maart 2008 In terms of detection of colon cancer and advanced neoplasia, which is the primary goal of screening for colorectal cancer, recent data suggest CTC is comparable to OC for the detection of cancer and polyps of significant size when state of the art techniques are applied. In previous assessments of the performance of CTC, the American Cancer Society concluded that data were insufficient to recommend screening with CTC for average-risk individuals. Based on the accumulation of evidence since that time, the expert panel concludes that there are sufficient data to include CTC as an acceptable option for CRC-screening.
