Casus A 23 year old male with right sided pneumothorax. Video assisted thoracoscopy and pleurectomy were performed Postoperatively hypotension and Hb level of 3,4 mmol/l. Chest X ray: massive right sided pleural effusion: haemothorax Six units of EC s and 2 units of FFP transfused
Casus Postoperatively patient became febrile, blood, sputum and urine cultures were taken and i.v antibiotics started Progressive circulatory shock: noradrenaline and dopamine started. DIC, renal function deteriotion. Mechanical ventilation.
Casus Dx: Severe sepsis with multiple organ failure Blood cultures: Yersinia enterocolitica type III Patient s stool and early antibody tests were negative for Yersinia Partner s blood and stool negative
Casus Donors: One donor: Yersinia IgG positive One donor : Yersinia IgG and IgA and stool cultures positive. Donor had no symptoms. DNA testing revealed a different strain of Yersinia.
Conclusion Case of Yersinia enterocolitica bacteriaemia very likely due to contaminated transfusion of red cells, leading to severe sepsis and multiple organ dysfunction syndrome
Recommendation of the authors We strongly advocate to further reduce risks and complications of our treatments, and suggest more effort is put into the reduction of bacterial contamination of blood transfusions, even if this incurs higher costs. The time has come not to selectively culture, but to culture all blood transfusions to further reduce this risk

Bacteriële contaminatie Definitie: Bacteriëmie aansluitend aan een bloedtransfusie. Klinische verschijnselen soms niet te onderscheiden van een hemolytische transfusie reactie. Indien dezelfde bacterie gevonden wordt bij de patient en in de zak (mits onder juiste condities bewaard) is de contaminatie zeker ; bij voorkeur wordt onderzoek verricht naar dezelfde identiteit van de stammen.
