Aortaklepstenose - Etiologie Age <70 years Bicuspid AV (50%) Postinflammatory (25%) Degenerative (18%) Unicommissural (3%) Hypoplastic (2%) Indeterminate (2%) Age >70 years Degenerative (48%) Bicuspid (27%) Postinflammatory (23%) Hypoplastic (2%)
Aortaklepstenose Aortaklepsclerose 25-30% >65y Tot 40% >75y Verdikte cusps, bewaarde mobiliteit Geen significante obstructie LV outflow Meestal Peak Doppler velocity <2.5m/s Stenose in ongeveer 2-9% leeftijdsgroep >65y Cardiovasc Ultrasound. 2006 Jul 1;4:27. Epidemiology and cardiovascular risk factors of aortic stenosis. Faggiano P 1, Antonini-Canterin F, Baldessin F, Lorusso R, D'Aloia A, Cas LD.
Aortaklepstenose Natuurlijk verloop Survival vanaf onset symptoms RSP 5 jaar Syncope 2-3 jaar CHF 1-2 jaar From Townsend CM Jr, Beauchamp RD, Evers BM, Mattox KL: Sabiston textbook of surgery: the biological basis of modern surgical practice, ed 18, Philadelphia, 2008, Saunders.
Aortaklepstenose - Diagnose Transthoracale echocardiografie mild AVA 1.5 2.0 cm 2 mean gradient 12 25 mm Hg <3 m/s moderate AVA 1.0 1.5 cm 2 mean gradient 25 40 mm Hg 3-4 m/s severe AVA 0.6 1.0 cm 2 mean gradient 40 50 mm Hg >4 m/s critical AVA <0.6 cm 2 mean gradient >50 mm Hg CAVE lage gradient bij slechte LV functie! TEE Ventriculografie Cardiac catheterization MRI
Aortaklepstenose - Behandeling Surgical aortic valve replacement (SAVR) Gouden standaard Transcatheter aortic valve replacement (TAVR) High risk Medicamenteus Diuretics, digoxin, ACE inhibitors, or angiotensin receptor blockers
Aortaklepstenose - risicoinschatting Comorbiditeiten Euroscore II Conditie en wensen van de patiënt en familie Multidisciplinair overleg -> TAVI?
Aortaklepstenose Indicatie AVR Class I indicaties Symptomatisch ernstig AS (AVA <1,0cm 2 of mean gradient >40mmHg of peak velocity >4m/s) Moderate AS, bij andere cardiale ingreep (AVA 1,0-1,5cm 2 of mean gradient 25-40mmHg of peak velocity 3-4m/s) Asymptomatisch ernstig AS met LV systolische dysfunctie Class II indicaties Asymptomatisch AS met Abnormale inspanningstest (BD, S/) AVA <0,6cm LV hypertrofie >15mm Zeer hoge gradiënt
Aortaklepvervanging Valves Mechanisch versus Biologisch Stented versus stentless Sutured versus sutureless (Perceval) Porcine (valve) versus Bovine (pericardium) (homografts) (pulmonary autograft -> Ross procedure)

Aortaklepvervanging - Mech vs Bio Nadelen biologische kleppen Valvulair falen -> afhankelijk van leeftijd (40 10% op 12 jaar) Hemodynamisch profiel -> meer obstructief dan mechanisch Calcificaties -> thv mechanische stressplaatsen -> falen Nadelen mechanische kleppen Thromboembolische vs Hemorrhagische events (2-4% per jaar) Prosthetic valve endocarditis (hoger risico eerste 3 maanden!) Mechanical versus bioprosthetic aortic valve replacement. Head SJ, Çelik M, Kappetein AP. Eur Heart J. 2017 Jul 21;38(28):2183-2191. doi: 10.1093/eurheartj/ehx141. Review.
Aortaklepvervanging - Mech vs Bio Speciale overwegingen Jonge patiënt (40-60y) Shift naar eerder biologisch Nieuwe generatie implants (cfr Inspiris) Zwangerschap Keuze 1: Bio met waarschijnlijk nood aan redo-avr Keuze 2: Mech met heparine (cave Teratogeniciteit Coumarines) Mechanical versus bioprosthetic aortic valve replacement.
