Opmerking in commentaar: penicilline: MIC 0.12 mg/ml of ǿ 29 mm: β-lactamase opsporen Il s agissait d un germe sensible: uniquement des problèmes pour la pénicilline chez les utilisateurs du Vitek. Remarque dans le commentaire: pénicilline: CMI 0.12 mg/ml ou ǿ 29 mm: rechercher β-lactamase 2

3 Microbiologie: antibiogram/antibiogramme Acinetobacter baumanni M/10639 Multiresistente kiem: enkel colistine S Resultaten waren uitstekend amikacine: R van beperkt niveau! Steeds naar ref.centrum sturen Germe multiresistant: uniquement colistine S Les résultats étaient excellents amikacine: R de niveau limité! Toujours envoyer au centre réf Microbiologie: antibiogram/antibiogramme Enterococcus faecium M/10848 VRE (vancomycine R), andere AB S Resultaten waren uitstekend. VRE (vancomycine R), autres AB S Les résultats étaient excellents. 3

Geen grote problemen voor labo s (nitro iets moeilijker) Pas de grands problèmes pour les labos (nitro un peu plus difficile) Microbiologie: antibiogram/antibiogramme Escherichia coli M/11025 (2) Belangrijk: - wereldwijde progressieve toename van de resistentie tegen chinolones bij Escherichia coli contra-indicatie voor het empirisch gebruik van deze antibioticaklasse in de 1e lijn. - Norfloxacine: enkel voor urinaire isolaten testen; ciprofloxacine en levofloxacine: intrinsieke activiteit gelijkaardig op enterobacteriaceae en meestal kruisresistentie; moxifloxacine: hoofdzakelijk beperkt tot de behandeling van community acquired pneumonieën bij patiënten die allergisch zijn aan penicilline Important: - augmentation progressive de la résistance aux quinolones chez Escherichia coli contre-indique l utilisation empirique de cette classe d antibiotiques en première intention - Norfloxacine: tester uniquement sur les isolats urinaires ciprofloxacine et lévofloxacine: activité intrinsèque très similaire sur les entérobactéries et résistances habituellement croisées; moxifloxacine: essentiellement limité au traitement de pneumonies communautaires chez des patients allergiques aux pénicillines 4

Ampi-S detectie vooral probleem bij Vitek (27 % R); onderzoek biomérieux: «we can see a very weak growth on strain in the PC well. This observation explains the variability of MIC results (>,<) which leads to some false resistances for ampicillin. Microbiologie: antibiogram/antibiogramme Shigella sonnei M/11022 (2) Epidemiologische gegevens: Cipro-R 24% Standaardwerken blijven fluorochinolones aanraden voor empirische therapie maar toenemende R tegen chinolones: aanpassing van deze richtlijnen Données épidémiologiques : Cipro-R 24% Beaucoup de manuels d antibiothérapie continuent à conseiller les fluoroquinolones pour la thérapie empirique mais l augmentation de la résistance : modification de ces directives 5

S/5379: 63-jarige man met een artritis van de rechter knie. S/4897: garde forestier de 45 ans sans plaintes particulières. S/5379: homme de 63 ans avec une arthrite du genou droit. 9 Borrelia (2): result S/4897: IgG: 100% - IgM: 82.5% -, 4.1% +/-, 13.4% + Inter: 92.8% afwezigheid/absence, 5.8%, aanwezigheid/présence (+variant), 1.4% andere/autre S/5379: IgG: 95.6% -, 3.3% +/-, 1.1% + IgM: 83.3% -, 11.5% +/-, 5.2% + Inter: 93.5% afwezigheid/absence, 4.3%, aanwezigheid/présence (+variant), 2.2% andere/autre Borrelia (3): Euroimmun Alle niet-negatieve resultaten/tous les résultats non-négatifs = Euroimmun A significant number (75% and 65%, respectively) of participants using the EUROIMMUN Anti-Borrelia ELISA (IgM) achieved a borderline or a positive result. These results could be confirmed at EUROIMMUN when we retested the above mentioned sera. The German Society for Hygiene and Microbiology, the Robert Koch Institute and the Centres for Disease Control and Prevention (USA) recommend a twotiered procedure for the serodiagnosis of Borrelia infections. In the first diagnostic step a screening test (ELISA or IIFT) is performed. It is crucial that the screening test shows a high sensitivity to avoid the risk of not detecting a true Borrelia infection. If the screening test result is positive or borderline, it should be followed up by a specific confirmatory test. 9

10 Borrelia (4): comm. De diagnose van borreliose blijft moeilijk, omwille van de grote variabiliteit in de performantie van de beschikbare serologische testen en de geringe gevoeligheid van de rechtstreekse detectie van de pathogeen. De resultaten van de Borrelia serologie moeten altijd geïnterpreteerd worden in functie van de klinische inlichtingen. Le diagnostic de borreliose reste difficile, en raison de la grande variabilité de la performance des tests sérologiques disponibles et de la faible sensibilité de la recherche directe du pathogène. Les résultats de sérologie Borrelia doivent toujours être interprétés en fonction des renseignements cliniques. CMV (1) S/4173 Een vrouw op vruchtbare leeftijd raadpleegt haar arts voor een griepaal syndroom met koorts, spierpijnen en algemeen gevoel van onwelzijn. Het staal werd afgenomen één maand na de start van de klinische symptomen. Femme en âge de procréation, ayant consulté pour un syndrome grippal accompagné de myalgies, de fièvre te de malaise généralisé. L échantillon soumis au Q.C. a été prélevé un mois après le début des manifestations cliniques. 11 CMV (2) S/4173 Resultaten/résultats IgG: 98.8% - (2 pos res: stalen verwisseld & verkeerd vakje aangekruist / échantillons inversés & coché mauvaise case) IgM: 100% - Inter: 97.6% Negatieve CMV serologie/sérologie négative pour CMV CMV (3) S/4898 Eén week nadien raadpleegt de echtgenote van de boven vermelde patiënte zijn huisarts met dezelfde klachten. Une semaine après, le mari de la patiente mentionnée cidessus, consulte son généraliste avec les mêmes symptômes. 12 CMV (4) S/4173 Resultaten/résultats IgG: 98.2% + (2 neg res: stalen verwisseld & IgG-M verwisseld / échantillons inversés & IgG- M inversés ) IgM: 97.6% - (1 +/- res; 2 pos res (1: cfr IgG); 2 volgens techniek/suivant technique) Inter: 96.4% Serologie suggestief voor een vroeger doorgemaakte CMV infectie/sérologie suggestive d une infection ancienne à CMV CMV (5): comm. De diagnose = serologisch, gebaseerd op een geobjectiveerde seroconversie of op een significante toename in titer van geassocieerde IgG-antilichamen. De anti-cmv IgG aviditeitstest meest betrouwbare serologische procedure om primo-infectie uit te sluiten (in geval van hoge aviditeit). Le diagnostic = sérologie, basé sur une séroconversion objectivée ou une croissance significative du titre des anticorps IgG. Le test d avidité anti-cmv IgG procédure sérologique la plus fiable pour exclure une infection primaire (en cas d avidité élevée). 12

13 EBV (1) S/11067 Een student van 18 jaar heeft last van rillingen, lichte koorts, hoofdpijn, gebrek aan eetlust en uitgesproken vermoeidheid. Hij meldt zich bij de huisarts waar een bloedafname verricht wordt Un étudiant de 18 ans se plaint de frissons, d une faible fièvre, de mal de tête, d un manque d appétit, et d une forte fatigue. Interpretatie negatieve EBV serologie enkel op basis van heterofiele antilichamen = niet mogelijk Immuuncompetente personen 3 serologische parameters voldoende voor interpretatie van de EBV serologie: VCA IgM, VCA IgG en EBNA-1 IgG Interprétation sérologie négative pour EBV en ne se basant que sur la détermination d anticorps hétérophiles = pas possible Personnes immunocompétentes 3 paramètres sérologiques suffisent à l interprétation de la sérologie EBV: VCA IgM, VCA IgG et EBNA-1 IgG. 14

15 Rubella (1) IS/11068: Een jonge dame, die niet gevaccineerd werd in haar jeugd, meldt zich bij de arts met rash en koorts Une jeune femme, qui n a pas été vaccinée dans sa jeunesse, se présente chez son médecin avec un rash et de la fièvre pare labos pairs/onpare labos impairs Rubella (2) Pare labos pairs Verwachte resultaten/ résultats attendus: IgG +, IgM - Immuniteit/Immunité Resultaten - IgG : 100 % + - IgM: 94.0% -, 2.4% +/-, 3.6% + - (+/-, + = Immulite: results were affected by heterophilic interference) - Inter: 87.6% Immuniteit/Immunité 15

Uitsluiten van recente infectie = onmogelijk indien enkel IgG uitgevoerd zonder IgM. Recente infectie bevestigen : a) confirmeren positieve IgM met andere methode; b) het aantonen van evolutieve serologie ovv significante titerstijging; c) het aantonen van de afwezigheid van hoog aviede antistoffen/anti-e2 IgG s Impossible d exclure une infection récente si exécution IgG sans IgM. Confirmer une infection récente a) la confirmation de la positivité des IgM par une autre méthode b) démontrer une sérologie évolutive sous forme d une augmentation significative c) démontrer la présence d anticorps à avidité élevée/igg anti-e2 16

17 Brucella (1) IS/7727 Koorts van onbekende oorsprong bij een landbouwer met uitgebreide veestapel. Fièvre d origine inconnue chez un fermier avec un grand cheptel. Verwachte resultaten/ résultats attendus: Afwezigheid van antistoffen/absence d anticorps Brucella (2) Resultaten: Tot AS/Ac: 91.7% - IgG: 94.3% - IgM: 75% - (3/4) Interpretatie/interprétation: 91.7% Afwezigheid van antistoffen/absence d anticorps Igv twijfelachtige serologie na 2 weken 2e staal of serum naar referentiecentrum sturen. En cas de réaction douteuse nouvelle prise de sang après une quinzaine de jours ou envoyer le sérum au CNR 17

18 C. pneumonie (1) IS/8687 Een 45-jarige man zonder onderliggende pathologie met aanhoudende hoest, algemene malaise, heesheid en koorts gedurende laatste 10 dagen Un homme de 45 ans sans pathologie sous-jacente souffre d une toux continue, d un malaise général, d éraillement et de fièvre durant les 10 derniers jours. Verwachte resultaten/ résultats attendus: IgG, IgM, IgA: - C. pneumonie (2) Resultaten: Tot AS/Ac, IgM, IgA: 100% - IgG: 98.8% - Interpretatie/interprétation: 83.9% Serologisch profiel niet suggestief voor recente/aanwezige infectie met C. pneumoniae/profil sérologique pas suggestif d une infection récente/active par C. pneumoniae 11.8% Serologie is niet geschikt voor de diagnostiek van recente/aanwezige infectie met C. pneumoniae/la sérologie n est pas adaptée pour le diagnostic d une infection récente/active par C. pneumoniae 3.2% 2 e afname noodzakelijk/2 e prélèvement nécessaire 18

19 Infectieuze serologie /sérologie infectieuse C. pneumonie (3): comm.(1) Primo infecties: IgM pas na 2-3 weken en IgG na 6-8 weken Reïnfecties: IgM kan volledig ontbreken en IgG binnen 1-2 weken Infection aiguë: IgM après 2-3 semaines et IgG après 6-8 semaines Réinfection: la production des IgM peut manquer complètement et les IgG apparaissent dans 1-2 semaines Infectieuze serologie /sérologie infectieuse C. pneumonie (4): comm.(2) Moleculaire technieken tonen aan dat C. pneumoniae voor veel lager percentage van acute luchtweginfecties verantwoordelijk is dan algemeen werd aangenomen op basis van serologische studies (< 1% van respiratoire infecties). Les techniques moléculaires ont prouvé que C. pneumoniae est responsable d un pourcentage plus bas d infections respiratoires aiguës qu on le supposait sur base des études sérologiques (< 1% d infections respiratoires) 19
