5 Patiëntgestuurde zorg is zorg die zich op de eerste plaats laat sturen door de zorgbehoefte van de patiënt, zijn/haar zorgvraag, verwachtingen en voorkeuren.

7 To err is human: building a safer Health System The report indicated that as many as to people die in hospitals each year as the result of medical errors. Even using the lower estimate, this would make medical errors the eighth leading cause of death in this country higher than motor vehicle accidents (43.458), breast cancer (42.297), or AIDS (16.516). About people per year are estimated to die from medication errors alone about 16 percent more deaths than the number attributable to workrelated injuries. IOM, November 1999

33 Error in medicine Leape LL. The correct action was taken 99,0 percent of the time, translating to 1,7 errors per day. One out of five of these errors was serious and/or potentially fatal. If performance levels even substantially better than those found in the ICU (for example 99.9%, a 10-fold reduction in errors) were applied to the airline and banking industries, it would still equate to two dangerous landings per day at O Hare International Airport and checks deducted from the wrong account per hour. About a third of patients rated as "appropriate" candidates for percutaneous transluminal coronary angioplasty (PTCA) or coronaryartery bypass grafting (CABG) instead were treated medically, and these medically treated patients had worse outcomes than the patients treated in accordance with the appropriateness criteria. As compared with patients who had indications for which CABG was rated appropriate and who received the procedure, patients with indications for which CABG was considered appropriate and who were instead treated medically had four times the risk of death or nonfatal myocardial infarction and three times the risk of angina one year later. 53 Patiëntgestuurde zorg is niet minder een zaak van bedrijfscultuur dan van bedrijfsstructuur. Patiëntgestuurde zorg moet vóór alles haar inbedding en motivering vinden in de geest van de professionals en van eenieder die, binnen en buiten de ziekenhuizen, met enige sturende verantwoordelijkheid in de zorg actief is.
