13 Over hormoontherapie Die hormonen zijn heel goed, maar nu niet zo geschikt. Dat is wel jammer, want het is wel makkelijk die hormonen. Daar neemt u een tabletje van en daar heeft u weinig last van. En zo is het met elk medicijn, dus paracetamol heeft ook veel bijwerkingen, maar bij de meeste mensen niet.

23 Communicating uncertainty - Evidence-based probabilities often guide treatment decision making - Probabilities are intrinsically imperfect - Aleatory uncertainty: unpredictability of future events - Epistemic uncertainty: limited reliability (imprecision) and accuracy of estimates (limited applicability to individual)

24 Research questions 1) Do clinicians convey epistemic and/or aleatory uncertainty? 2) Do patients perceive the uncertainty associated with probabilities, and if so, how? Clinical context: Adjuvant chemotherapy and/or endocrine therapy for early-stage breast cancer

25 Results: Disclosure of uncertainty Uncertainty disclosed in 97 (49%) consultations 84 references to aleatory uncertainty [there are] no guarantees. So you either get the disease back or you don t, right. I can t tell just by looking at you whether you are one of the 70 lucky ones or one of the 30 unlucky ones. 23 references to epistemic uncertainty Imprecision of probabilities: Of course there always is a margin associated with such statistics. Applicability to specific (subgroups of) patients: look, these are averages It s a large database, and we can t comment on the individual... the only thing we can do is look at averages. And of course, there are always exceptions on both sides.

29 Results: Quotes from In-depth interviews Patient answers often contained logical contradictions what is precise? I don t know whether they can say it so precisely, that it s not greater than 10% [recurrence risk]. But I do think that if it had been 11% she would ve told me. Yes I m confident about that, so it is exact. on average it could also be higher. if you say that on average you have 10% chance of getting it again I don t think in terms of averages, because then it could also be 5 or 15. I want to think about the 10% In truth I cling on to it [prognostic estimates], although I do realize that everyone is unique and you never know [how things will turn out]. Still, I attach great importance to them [prognostic estimates].

30 Conclusions Consultations Uncertainty conveyed in half of consultations, most often aleatory Oncologists' references to epistemic uncertainty were vague Patient interviews Methodologically hard to ask patients about uncertainty Patients need specific probe about uncertainty Aleatory uncertainty is type of uncertainty that most patients perceive and seem comfortable discussing Logical contradictions were common Guidance on whether, and if so, how to best communicate uncertainty urgently needed
