Introduction: Lifestyle-education should not only be teached, but has to get accepted in practise. People with a high-risk for chronical diseases who aware positive effects of exercising, improve their compliance and their self efficacy. Physical exercise under professional supervision intergrated in interactive education stimulates community discussion on topics and traditions. Strengthened physical condition combined with an education program decreases risks on obesitas, hypertension and bad regulated diabetes mellitus. Design and methods: Observational pilot in a low-literated population with high risk for hypertension, obesity, and diabetes. According to the statistics there was a high prevalence of obesitas, diabetes and hypertension, there was no individual selection on physical caracteristics. Included 30 women, excluded two women because of their bad condition. Weekly program with presence-registration during 20 weeks. Program of 2 hours, 45 minutes exercising and education, pause, and 45 minutes discussion and roleplay on healthliteracy and cultural topics. Use of professional validated materials of Diabetesfonds [1], NISB [2] and NIGZ [3] integrated with exercising makes prevention work at all levels in a low-literated population. Results: Micro: knowledge of human body, invreased self efficacy, better use of healthcaresystem, better physical condition, reduced risk for obesity, hypertension, diabetes or bad regulation in diabetes. Meso: shift in living tradition in family and environment. Macro: Gain of health in common, increased open communication, threats like obesity, hypertension, and diabetes. Conclusions: Although in this population this pilot is difficult to evaluate on personal effects, it seems effective on daily behaviour and social values. It is shown a better understanding in causality between behaviour and health, better upcome on prescribed medical checks, individual initiatives for physical exercising. Larger pilots are necessary. The paradigm is to construct this as a lasting program in low-literated population as a base for lifestyle-intervention.
