Nederlandse Vereniging voor Cardiologie Nieuwsbrief Start NVVC! Connect Donderdag 26 april was het zover. Het landelijke project NVVC Connect werd afgetrapt in Alkmaar in het AZ stadion. NVVC Connect richt zich als eerste op de zorg voor acuut infarct. Met NVVC Connect wordt een structuur neergezet die door de korte lijnen snelle slagen kan maken. De uitrol is per regio. Elke regio stelt zijn eigen verbeterdoelen op en gaat ermee aan de slag. Deze maand start Rotterdam Rijnmond. Alle ziekenhuizen in die regio doen mee, evenals de huisartsen en ambulancevoorzieningen. De NVVC biedt ondersteuning voor het project door volop en veel aandacht te vragen voor het thema middels campagnes en door het bieden van veel informatie en benchmarkgegevens voor deelnemers. Alle deelnemers kunnen zich spiegelen in NVVC Connect en zien zo direct waar verbetermogelijkheden zitten. NVVC Connect maakt de zorg beter! Meer informatie kijk op www.nvvcconnect.nl. NVVC! Challenge 2012 Na de succesvolle Dom-Eiffel estafetteloop van afgelopen jaar, wordt dit jaar een Utrecht- München estafetteloop georganiseerd: de NVVC! Challenge 2012. Net als vorig jaar is dit voorafgaand aan het European Society of Cardiology (ESC) congres, welke plaatsvindt van 25-29 augustus in München. In 3 dagen wordt de 810 kilometer tussen Utrecht en München hardlopend afgelegd. Het sponsorgeld dat hiermee opgehaald wordt, zal naar het project NVVC! Connect gaan. NVVC! Hardlopen van Utrecht naar München Op 22 augustus zullen teams, samengesteld vanuit diverse cardiologische centra in Nederland, beginnen aan de estafetteloop van Utrecht naar München. Inmiddels hebben we drie teams en één subteam bij elkaar. De totale afstand die elke loper aflegt is 45 kilometer (6 stukken van 7,5 kilometer). De wisselpunten liggen om de 15 kilometer. Er wordt gestart met twee personen die afwisselend lopen en fietsen/navigeren. Ieder subteam start 6 keer en legt per beurt een afstand van 45 kilometer af. Iedere deelnemende hardloper loopt dus in totaal iets meer dan de afstand van een marathon. Sponsoring Sponsoring gaat per subteam. Ieder subteam zal, voor de start minimaal 2500 aan sponsorgeld moeten ophalen zodat de kosten voor dat subteam dekkend zijn. Natuurlijk is het streven naar een zo hoog mogelijk sponsorbedrag! Cardio Vasculair onderwijs Instituut Programma CVOI CAG Symposium Vrijdag 15 juni 2012, Het coronair angiogram speelt een centrale rol bij de diagnostiek van een groot deel van de cardiologische patiënten. De grote belangstelling en de enthousiaste reacties op de in Venlo georganiseerde CAG symposia hebben de organisatoren in samenwerking met het CVOI doen besluiten dit symposium jaarlijks op alternerende locaties in Nederland te organiseren; teneinde alle cardiologen in opleiding in de gelegenheid te stellen tenminste eenmaal in hun opleiding aan dit symposium deel te nemen. Voor dit jaar staat Utrecht als locatie op het programma. Het doel van dit symposium is om alle aspecten van het coronair angiogram als diagnosticum, met hulp van een keur van sprekers aan bod te laten komen. Het programma heeft vele leermomenten voor cardiologen en cardiologen in opleiding. Tijdens het symposium zal achtergrondinformatie en relevante literatuur worden verstrekt. De deelnemers kunnen de opgedane kennis toetsen door na het symposium elektronisch enkele meerkeuzevragen te beantwoorden waarbij de diverse programmaonderdelen van de gehele dag aan bod komen. J.G. Houweling, VUMC Amsterdam One of the aims of GENCOR is to promote research to improve the quality of diagnosis and treatment of familial heart diseases. One of the projects linked to GENCOR is the ICIN research project Arrhythmogenic Right Ventricular Dysplasia/Cardiomyopathy (Arrhythmogenic Cardiomyopathy). Arrhythmogenic Right Ventricular Dysplasia/Cardiomyopathy (ARVD/C) is considered a hereditary desmosomal disease with primarily right ventricular involvement. However, the left ventricle is affected in many cases and at the molecular level both ventricles are involved in a similar way. These observations were in favour of changing ARVD/C to Arrythmogenic Cardiomyopathy (AC) as preferred terminology. Since sudden cardiac death due to malignant ventricular arrhythmias can be the presenting symptom of disease, early diagnosis and risk stratification are essential and the ultimate goal of the research project. Secondly, assessment of genotype-phenotype correlations is pivotal in the project. This is illustrated by the observation that compared to relatives of index patients without mutations, mutation-carrying relatives have 1) a six-fold risk of AC diagnosis, 2) markedly increased risk of arrhythmias, and 3) earlier onset of AC signs and symptoms. 1 In young relatives, age <25 years, sudden death occurred exclusively in (PKP2) mutation carriers (Figure 1). SCD below the age of 25 years occurred exclusively in (PKP2) mutation carriers. In order to study early diagnostics and genotype-phenotype correlations in AC, patients fulfilling the diagnostic Task Force criteria defined in 2010 have been collected nationwide. 2 Together, the number of index patients included has grown from 115 in 2008, to 176 in 2011. Pathogenic mutations, mainly truncating PKP2 mutations, were identified in 126/176 (72%) index patients (109 desmosomal mutation carriers, 17 phospholamban (PLN) mutation carriers). A small minority had mutations in DSG2, DSC2, and DSP. Multiple desmosomal mutations were found in only 2% of patients. Besides associated desmosomal gene mutations, a substantial contribution of the non-desmosomal PLN mutation to AC phenotype and diagnosis was observed. AC was diagnosed in 82 family members from a total of 479 from 116 different families. Thus, in total 176+82=258 patients fulfilled the Task Force criteria from 2010 for AC diagnosis. Since 177 family members had a pathogenic mutation, the total number of mutation carriers was 303 individuals. In the future, more patients should be included to have more relatives per DNA mutation to identify specific mutationrelated phenotypic differences and differences within a family by genetic modifiers and environmental factors. Recently, the merging of the ICIN database on AC with the Johns Hopkins database has started for substantial enhancement of the number of AC relatives. A large group of relatives per mutation is critically important to estimate the phenotypic contribution of at least some specific mutations. In addition, in the future more data is needed about mutations encoding non-desmosomal proteins and their precise relation to intercalated disk remodeling. The substantial contribution of the non-desmosomal PLN mutation in the Dutch AC patient population is very promising. References 1. This month the investigators of the Emphasis-HF study published a subanalysis showing reduction of incident AF (a pre-specified secondary endpoint) in the group receiving eplerenone versus the placebo group (respectively 2.7% vs. 4.5%, HR 0.58 [1.35-0.96]). 1 In addition AF patients receiving eplerenone also had significantly less risk of cardiovascular death or hospitalization for worsening heart failure (the primary endpoint, HR 0.60 [0.46-0.79]). Although the study population included in Emphasis-HF differs from the RACE 3 study population (Emphasis-HF patients have more severe and long-standing heart failure and hence more extensive atrial remodeling), these results are encouraging for RACE 3. A recent, smaller study amongst paroxysmal AF patients with preserved left ventricular function showed less AF recurrences in patients receiving spironolactone, during only 12 months of follow-up. 2 However no data on cardiovascular morbidity and mortality have been published from this study. Especially the combination of upstream therapy classes in RACE 3 could improve rhythm control as well as cardiovascular morbidity and mortality. Soon there will be an amendment of the RACE 3 protocol that will prolong the follow-up period to a total of 5 years, to investigate the effects of upstream therapy on rhythm control and outcome on the long term. In addition there will be minor changes in the inclusion criteria. You will receive additional information when the amendment is accepted by the research ethics committee. The purpose of this registry is to obtain observational data on mothers with structural heart disease, their complication during pregnancy and delivery and the maternal and fetal outcome. First analysis has been done on 1321 patients. Maternal mortality was 1%, fetal mortality was 4%. This is 9 x higher than expected. Number patients hospitalized 338 (26%), of which 132 (10%) because of heart failure. Caesarean section rate was 41%.
