3 Chapter 10 Summary and conclusions 144 Summary The aim of this thesis was to develop, evaluate and implement technological advances in MDCT in clinical practice. Additionally dose assessment was performed to obtain more insight into the risk of radiation effects by using MDCT. Chapter 1 provides general background information on the improvements made in CT which made new applications possible such as imaging of cochlear implants, imaging of the coronary arteries and CT fluoroscopy. Chapter 2 describes the evaluation of an automated phase selection algorithm and comparison to a manual phase selection method for assessment of global left ventricular function in CT coronary angiography investigations. With the automated tool an adequate assessment can be performed of the global left ventricular function. The automated tool can reduce the time needed for handson post processing and reduces reconstructions and data transfer by 80%. Chapter 3 evaluates three phase selection methods on axial and MPR image quality of clinical coronary angiography investigations. The evaluated methods were: 1 using a single predefined phase, 2 selecting the best phase with the least motion throughout the cardiac cycle manually by a visual inspection, 3 automated selection of the optimal phase for image reconstruction. For patients with low heart rates ( 65 bpm), no statistically significant differences in image quality were found for the three phase selection methods. For patients with high heart rates (> 65 bpm), the best image quality was found with the automated and manual phase selection methods. The practical benefits of the automated compared with the manual phase selection method are its ease of use, the fact that it is operator-independent, and the potential time saving. Chapter 4 accurately assessed the radiation exposure associated with the cardiac CT acquisition protocol after developing an algorithm for Monte Carlo dosimetry adapted to the characteristics of the Aquilion 64 CT scanner. It showed that an underestimation of effective dose by 43% or 53% occurs in coronary CTA according when an E / DLP conversion factor is used. The effective radiation dose for the retrospective gated acquisition of cardiac CT angiography was 19 msv. Chapter 5 revealed that relatively high effective patient doses can occur during CT fluoroscopy using MDCT. However, this finding did not result in acute concerns with regard to the radiation protection of workers and patients because estimated effective patient and occupational dose were below the threshold level for serious deterministic effects for patients and below generally accepted dose limits for workers. Complex pathology can be treated safely with CT fluoroscopy using MDCT; however, the few cases of relatively high doses

4 Summary and conclusions, Samenvatting en conclusies, Publications, Presentations, Curriculum Vitae should serve as a warning that further optimization of radiation protection of both patients and workers should be pursued. Chapter 6 showed that the developed low dose simulator for CT images corresponded well with actual low dose CT images. The mean difference in noise over all comparisons was 5.0% for the cylindrical test object and 3.3% for the anthropomorphic phantom. Also the noise power spectrum showed overall comparable shape and intensity, meaning that noise in actual and simulated lower dose images will affect visual appearance and diagnosis in a similar way. The major practical benefit of the algorithm is that it can be retrospectively applied to CT to optimise acquisition protocols in order to reduce the radiation dose. The software has become available for researchers to perform clinical studies in order to investigate the possibility of dose reduction for clinical CT protocols. Chapter 7 describes a metal artefact correction method which is performed in original raw data. We developed a segmentation method that is capable of segmenting metal structures in original raw CT data. The method is based on the use of Bayesian techniques and application of a Markov random field model. After the metal is segmented, an interpolation method is used in combination with scaling of the metal signal, to replace the metal signal by a more appropriate signal intensity. In a phantom and in five patients with metal implants, the segmentation method resulted in improved image quality. In phantom images, the technique showed reduced standard deviation close to the standard deviation for the case where metal was not inserted. In the patient study a significant improvement in image quality was found by a radiologist. Chapter 8 extends the developments described in chapter 7 with the development of a new metal artefact reduction technique and also a new evaluation technique for quantitative measurements in clinical images. Two techniques were implemented: one generic method with corrections in Radon transformed images, and another a more sophisticated method based on a multidetector row CT (MDCT) forward projection that matches the specific CT scanner, and subsequent corrections in the original raw projections. This new method which is performed in the original raw projections, proved to be superior to methods based on artificial raw data created from CT images. Quantitative clinical measurements have shown improvements in image quality by a decrease of metal artifacts in clinical images. This decrease of artifacts improved the clinical value of the CT scan for radiologists. Chapter 9 describes the evaluation of the visualization and accuracy of measurements of electrode positions of a cochlear implant for acquisitions with 64 detector row CT scanners of 4 major CT systems. Evaluation was performed in a human cadaver temporal bone and in a polymethylmethacrylate (PMMA) phantom both with a cochlear implant. Despite a small difference in spatial resolution, the assessment of the human cadaver scans showed a very wide range in image quality among the 4 scanners. The main reason for this finding was that the available range of HUs is very limited on some systems. Because the 145

5 Chapter densities of the evaluated structures differ considerably, a large window width and window level is needed to accurately visualize the electrode contacts. Also quantitative assessment of electrode contact positions, varied between scanners. It seems plausible that software adjustments allowing for a wider range of HUs will be needed in two scanners to improve image quality for a possible higher diagnostic value. Conclusions Equipment for medical imaging, like computed tomography scanners, becomes more sophisticated and provides better image quality. The clinical implementation of new technology should be done with care. Effective assessment of new imaging technology has been achieved in cooperation between radiologists and physicists. The research contributed to the optimal post processing of cardiac CT scans (optimal phase selection chapters 2 and 3), and the assessment of the visualization of cochlear implants scanned with four CT scanners (chapter 9). With the rapidly growing use of computed tomography scanners, the concern about detrimental effects of the associated radiation exposure increased as well. Radiation exposure was evaluated for two procedures. Accurate assessment of patient dose for cardiac CT revealed that most of the published values underestimate effective dose by about 50% (chapter 4). In CT fluoroscopy procedures the radiation exposure to medical staff and patients, although rather high, was judged as acceptable (chapter 5). Scientific experiments to support the optimization of CT acquisition protocols with regard to image quality and patient dose are often difficult to carry out in a clinical setting. Such studies can now be performed in cooperation with radiologists by using a mathematical simulator that was developed to generate CT studies at lower doses (chapter 6). Artifacts in CT studies that result from metal implants can deteriorate image quality to an unacceptable poor quality. Several solutions for metal artefact reduction were developed, and validated. Substantial improvement of image quality was achieved by the algorithms that were developed for this purpose (chapters 7 and 8).

Bovendien zijn effecten van röntgenstraling door het gebruik van MDCT te onderzocht. Hoofdstuk 1 geeft algemene informatie over de technologische ontwikkelingen op het gebied van CT waardoor specifieke toepassingen mogelijk zijn geworden zoals het afbeelden van een cochleair implantaat en coronaire vaten of CT geleide doorlichting. Hoofdstuk 2 beschrijft de evaluatie van een algoritme dat automatisch een fase selectie uitvoert voor de globale linker ventrikel functie met CT angiografie van coronaire vaten. Het algoritme is vergeleken met de handmatige methode voor fase selectie. Met het algoritme voor automatische fase selectie kan de globale linker ventrikel functie adequaat worden bepaald. Het algoritme kan de tijd die nodig is voor de functie meting sterk verminderen en daarnaast worden de reconstructies en data overdracht met 80% verminderd. Hoofdstuk 3 evalueert drie methodes voor fase selectie van klinische coronaire angiografie onderzoeken met behulp van axiale en MPR beelden. De drie methodes zijn: 1 het gebruik van een vooraf gedefinieerde fase, 2 het selecteren van de fase met de minste beweging handmatig door visuele beoordeling en 3 automatische selectie van de fase met de minste beweging. Bij patiënten met een lage hartslag ( 65 slagen per minuut) was er geen statistisch significant verschil gevonden tussen de drie methodes. De beste beeldkwaliteit bij patiënten met een hoge hartslag (> 65 slagen per minuut) was gevonden met de handmatige en automatische fase selectie. De praktische voordelen van het gebruik van het algoritme voor automatische fase selectie zijn het gemak, het feit dat het onafhankelijk is van de gebruiker en de potentiële tijdsbesparing. Hoofdstuk 4 geeft een nauwkeurige evaluatie van de blootstelling aan straling bij een CT van het hart. Hiervoor werd een algoritme ontwikkeld met alle karakteristieken van de Aquilion 64 CT scanner voor Monte Carlo dosimetrie. Hieruit bleek dat wanneer een E / DLP conversie factor wordt gebruikt voor het schatten van de effectieve dosis bij een CTA van de coronaire vaten, er een onderschatting van 43% tot 53% optreedt. De effectieve dosis van een retrospectieve gated CT acquisitie van het hart was 19 msv. Hoofdstuk 5 geeft aan dat een relatief hoge effectieve dosis voor de patiënt kan voorkomen bij CT geleide doorlichting. De hoge dosisniveaus hebben daarentegen niet geleid tot acute zorgen voor de stralingshygiëne van personeel of patiënten omdat de effectieve dosis onder het niveau is waarbij ernstige deterministische effecten bij patiënten optreden of onder de geldende 147

7 Chapter dosislimieten voor personeel. Met CT geleide doorlichting kan complexe pathologie veilig worden behandeld maar de gevallen waarbij hoge dosissen zijn waargenomen dienen als waarschuwing zodat er naar verdere optimalisaties van stralingshygiëne voor personeel en patiënten kan worden gestreefd. Hoofdstuk 6 laat zien dat de ontwikkelde lage dosis simulator voor CT beelden een goede gelijkenis heeft met echte beelden. Het gemiddelde verschil over alle metingen in ruis was 5.0% voor het cilindrische test object en 3.3% voor het antropomorf fantoom. Het power spectrum van de ruis liet ook goede gelijkenissen zien in de vorm en intensiteit. Dit wil zeggen dat de ruis in echte en gesimuleerde beelden in dezelfde verschijningsvorm is waar te nemen. Het grote voordeel van dit algoritme is dat het retrospectief is toe te passen op CT studies om de acquisitie protocollen te verbeteren en de stralingsdosis te verlagen. De software is beschikbaar voor onderzoekers om klinische studies te verrichting met als doel een potentiele dosis reductie te behalen voor klinische CT protocollen. Hoofdstuk 7 beschrijft een methode voor de correctie van metaal artefacten die wordt toegepast op de originele ruwe CT data. Wij hebben een segmentatie methode ontwikkeld die in staat is om metaal structuren in de ruwe data te segmenteren. De methode is gebaseerd op het gebruik van Bayesiaanse techniek in combinatie met het Markov random field model. Nadat het metaal is gesegmenteerd wordt een correctie uitgevoerd door een interpolatie methode toe te passen in combinatie met een schaling van het originele metaal signaal. Deze correctie techniek resulteerde in een verbeterde kwaliteit van CT beelden bij fantoom beelden en bij vijf patiënten met metalen protheses. In fantoom beelden met metaal is er na correctie een standaard deviatie gemeten die dicht in de buurt lag van de standaard deviatie bij fantoom beelden zonder metaal. In patiënten beelden is door een radioloog een significante verbetering gevonden. Hoofdstuk 8 gaat verder met de ontwikkeling van software voor metaal artefacten reductie zoals beschreven in hoofdstuk 7. In dit hoofdstuk wordt naast een nieuw methode ook een nieuw evaluatie techniek voor kwantitatieve metingen in klinische beelden besproken. Er zijn twee technieken geïmplementeerd voor metaal artefacten reductie: een generieke methode met correcties in Radon getransformeerde beelden en een meer geavanceerde methode die gebaseerd is op het gebruik van een forward projector die dezelfde geometrische kenmerken heeft als de specifieke CT systeem en gebaseerd op correcties in de originele ruwe data. De nieuwe methode, gebaseerd op correcties in de ruwe data, resulteerde in betere beeldkwaliteit dan de methodes gebaseerd op artificiële ruwe data die uit CT beelden worden gecreëerd. Kwantitatieve klinische metingen lieten een verbeterde beeldkwaliteit zien door een vermindering van metaal artefacten. De afname van metaal artefacten zorgde voor een verbetering in de diagnostische waarde van de CT scan voor de radiologen. Hoofdstuk 9 beschrijft de visualisatie en nauwkeurigheid van metingen van elektrode posities van een cochleair implantaat. Hiervoor werden acquisities

Evaluaties zijn gedaan in het slaapbeen van een menselijk kadaver en in een polymethylmethacrylaat fantoom die beiden een cochleair implantaat bevatte. Ondanks dat er kleine verschillen in spatiële resolutie zijn gevonden liet de evaluatie met het menselijk kadaver zien dat er een grote variatie is in beeldkwaliteit tussen de vier verschillende systemen. Dit verschil werd hoofdzakelijk veroorzaakt doordat het bereik van het aantal HUs erg beperkt is op sommige systemen. Omdat de pixel waarden van de structuren erg veel van elkaar afwijken is een brede venster breedte en niveau nodig om nauwkeurig de elektrode contacten in beeld te brengen. Daarnaast zijn er ook grote verschillen gevonden in de kwantitatieve metingen van de electrode contact posities. Het is erg waarschijnlijk dat aanpassingen in de software van de CT scanner nodig is op twee CT scanners zodat een groter bereik van HUs kan worden gerealiseerd. Dat zal de beeldkwaliteit verbeteren zodat de diagnostische kwaliteit ook mogelijk kan toenemen. 149 Conclusies Apparatuur voor medische beeldvorming zoals computer tomografie is de laatste tijd meer geavanceerd geworden met als resultaat een betere beeldkwaliteit. De klinische implementatie van deze nieuwe ontwikkelingen moet zorgvuldig worden gedaan. In dit proefschrift zijn nieuwe technieken voor medische beeldvorming besproken die effectief zijn geïmplementeerd in samenwerking met radiologen en fysici. Het onderzoek droeg bij aan de implementatie van postprocessing van CT van het hart (optimale fase selectie hoofdstuk 2 en 3) en de beoordeling van de visualisatie van een cochleair implantaat dat was gescand op vier verschillende CT systemen (hoofdstuk 9). Door de toename van het gebruik van computer tomografie scanners zijn de zorgen over de effecten van röntgenstraling ook toegenomen. De blootstelling aan straling is geëvalueerd in twee studies. Nauwkeurige analyse van de stralingsdosis bij een CT van het hart laat zijn dat de meeste publicaties de effectieve dosis met ongeveer 50% onderschat (hoofdstuk 4). In CT geleide doorlichting zijn hoge dosissen gevonden voor personeel en patiënten maar deze werden beoordeeld als acceptabel (hoofdstuk 5). Wetenschappelijke studies om CT acquisitie protocollen te optimaliseren met betrekking tot beeldkwaliteit en patiënten dosis zijn vaak moeilijk uit te voeren. Dergelijke studies kunnen nu worden uitgevoerd in samenwerking met radiologen met behulp van een mathematische simulator die was ontwikkeld om CT beelden met een lagere dosis te simuleren (hoofdstuk 6). Metalen objecten kunnen CT beelden ernstig verstoren doordat artefacten ontstaan waardoor de kwaliteit van de CT beelden niet acceptabel is voor een goede diagnose. Er zijn verschillende oplossingen voor een reductie van metaal artefacten ontwikkeld en gevalideerd. Met de ontwikkelde technieken zijn er aanzienlijke verbeteringen in beeldkwaliteit gerealiseerd (hoofdstuk 7 en 8).

In 2000 behaalde hij zijn HAVO diploma aan het Segbroek College Den Haag. In dat jaar begon hij de studie Technische Natuurkunde aan de Technische Hogeschool Rijswijk. In 2004 studeerde hij vervolgens af met als afstudeerrichting toegepaste natuurkunde en kwam na een stageperiode van 5 maanden in dienst bij het Leids Universitair Medisch Centrum op de afdeling Radiologie. Hier begon hij in 2007 zijn promotieonderzoek onder begeleiding van Dr. J Geleijns en Prof. A. de Roos. Dit onderzoek richtte zich op technologische ontwikkelingen op het gebied van Computer Tomografie. De resultaten van dit onderzoek zijn beschreven in dit proefschrift. 153
