1 Doelmatigheid Enhanced Recovery After Surgery (ERAS) Paul van Gils, Anita Suijkerbuijk, Ardine de Wit, Matthijs van den Berg RIVM, Centrum voor Voeding, Preventie en Zorg Inleiding Om antwoord te kunnen geven op de vraag: Is toepassing van een enhanced recovery after surgery (ERAS) programma doelmatig bij laparoscopische resectie of open resectie van een colorectaal carcinoom? is literatuuronderzoek uitgevoerd. In deze rapportage wordt de zoekstrategie beschreven en worden de bevindingen samengevat. We sluiten af met een conclusie op basis van het beschikbare bewijs over de kosteneffectiviteit van ERAS bij darmkankeroperaties. In bijlagen 1 tot en met 3 zijn de zoekstrategie, kwaliteitsbeoordeling van de gevonden studies en de bewijstabel weergegeven. Methode De databases van Medline, van het Centre for Reviews and Dissemination, van de Cochrane Collaboration, en van Embase-SciSearch zijn onderzocht. De zoekstrategie is opgenomen in bijlage 1. Er is gezocht naar Engelstalige publicaties verschenen van 2007 tot en met november De Medline search leverde 6 publicaties op, de Embase-SciSearch search 17 publicaties, het Centre for Reviews and Dissemination 2 publicaties en de Cochrane géén. De titels en abstracts zijn door twee onderzoekers beoordeeld. Selectiecriteria hierbij waren of het economische evaluaties betrof, dat wil zeggen studies waarin 2 of meer interventies met elkaar vergeleken worden wat betreft de effecten en de kosten, of het om ERAS ging en om dikke darm kanker. Op basis van deze criteria is 1 economische evaluatie geïncludeerd (Sammour et al., 2010). Deze studie is door 2 onderzoekers beoordeeld op basis van de Consensus Health Economic Criteria (CHEC-list) (Evers et al., 2005a; Evers et al., 2005b). Het resultaat van deze kwaliteitsbeoordeling is opgenomen in bijlage 2.

2 Resultaat De economische evaluatie van Sammour et al. is een studie die is gedaan vanuit het perspectief van de gezondheidszorg (Sammour, 2010). De follow-up duur bedroeg 30 dagen na de operatie. De ingreep betrof een electieve colonresectie waarin ERAS vergeleken werd met een standaard behandeling. De klinische analyse was gebaseerd op een prospectieve cohortstudie met een historische controlegroep. De studiegroep bestond uit patiënten die in aanmerking kwamen voor electieve colon chirurgie tussen december 2005 en maart Inclusiecriterium voor deelname aan het ERAS-programma was een electieve colon resectie bij patiënten ouder dan 15 jaar. Eclusiecriteria waren patiënten die een stoma vereisten, ASA (American Society of Aneasthesiologists) IV, significante cognitieve beperking, onvermogen tot het spreken van Engels en patiënten die informed consent weigerden. De patiënten in de controlegroep werden geïdentificeerd door middel van een search in de ziekenhuisdatabase van september 2004 tot september De controle patiënten werden individueel gematched met die in de studiegroep op basis van de operatie die werd ondergaan, body mass inde, ASA -score en Cr-POSSUM score (Colorectal Physiological and Operative Severity Score for enumeration of Morality). Patiënten in de controlegroep kregen conventionele, niet gestructureerde zorg. Data waren beschikbaar voor 50 patiënten in elke groep. Conform de resultaten uit een eerdere studie in het centrum (Zargar-Shoshtari et al., 2008) was er een significante reductie van het postoperatieve ziekenhuisverblijf. Er was een significant verschil in complicaties. Het aantal heropnames verschilde niet significant (6 in ERAS vs. 7 in controlegroep). Mortaliteit was niet meegenomen als uitkomstmaat. Zie verder bijlage 3a. De kosten opgenomen in de economische evaluatie betroffen programmaontwikkeling, voorlichtingsmateriaal ziekenhuisverblijf, medicatie, verzorging, complicaties, heropnames en poliklinische kosten. De kosten van huisartsenzorg werden niet meegenomen. Er werd géén sensitiviteitsanalyse uitgevoerd en het prijsjaar werd niet genoemd. De kosten werden helder uitgesplitst in eenheden van zorggebruik, kosten per unit zorggebruik en totale kosten van zorggebruik, hetgeen de transparantie vergroot. De totale kosten en de per patiënt kosten waren lager in de ERAS groep dan in de controlegroep. De resultaten werden niet gepresenteerd in een incrementele kosteneffectiviteitsratio, het quotiënt van het verschil in kosten en het verschil in effecten tussen de groepen. Net als de effecten werden de kosten gedisaggregeerd gepresenteerd als in een kosten-consequentieanalyse. De per patiënt kosten van het ERAS programma waren NZ$ ,35 en van de controlegroep NZ$ ,74,

3 gemiddeld dus een besparing van bijna NZ$ 6900 per patiënt. Grootste verschillen werden gevonden in de kosten voor complicaties en intraoperatief- en postoperatief vloeistofgebruik. Zie bijlage 3 voor de verdere informatie over deze economische evaluatie. Aanvulling vanuit de RCT s Onder de gevonden RCT s (zie effectiviteit) bevonden zich 2 studies die ook kosten hebben opgenomen. Dit waren echter geen volledige economische evaluaties. Ren et al., meldden dat de postoperatieve kosten per patiënt in de ERAS groep $548,40 (±$223,10) waren en $804,00 (±$288,70) in de controlegroep (p<0,001) (Ren et al., 2012). Er was géén significant verschil in de preoperatieve en chirurgische kosten. De Nederlandse studie van Vlug et al. gaf de ziekenhuiskosten (in Euro), afzonderlijk weer voor de universitaire ziekenhuizen en opleidingsziekenhuizen (Vlug et al., 2011). Laprascopie en Fast Track Universitair ( ) Opleiding 5768 ( ) Open en Fast Track ( ) 5497 ( ) Laprascopie en standaard ( ) 6228 ( ) Open en standaard ( ) 5650 ( ) P 0,56 0,41 Conclusie De auteurs van de economische evaluatie concluderen dat het ERAS programma bij electieve chirurgie op de middellange termijn kosteneffectief is en dat de ontwikkelkosten meer dan gecompenseerd worden door verminderd gebruik van middelen in de postoperatieve periode. Echter, omdat dit slechts één studie is, die ook nog een aantal forse beperkingen kent, moet voorzichtigheid worden betracht bij de interpretatie van de conclusies. Drie belangrijke beperkingen zijn: 1) De basis van de economische evaluatie is een observationele effectstudie, en geen RCT. 2) Er is geen gevoeligheidsanalyse uitgevoerd. 3) Er werd géén incrementele kosteneffectiviteitsratio gepresenteerd. Kortom, er zijn te weinig economische evaluaties om een goed onderbouwde conclusie over de kosteneffectiviteit van ERAS te trekken. De twee RCT s waarin kosten zijn opgenomen zijn, geven ook geen eenduidig beeld.
