2 Inhoud Wat is Capacity Building? Wat zijn laag prevalentie landen? Wat is de situatie in Nederland? Waar is behoefte aan in Nederland? Voorbeelden van wat er gebeurt. Definitie capacity building (UNDP 1991) long-term continual process of development that involves all stakeholders; including ministries, local authorities, non-governmental organizations, professionals, community members, academics and more. Capacity building uses a country's human, scientific, technological, organizational, and institutional and resource capabilities. The goal of capacity building is to tackle problems related to policy and methods of development, while considering the potential, limits and needs of the people of the country concerned.. [1]

12 Samenvattend De incidentie van tuberculose daalt snel en wordt grotendeels bepaald door immigratie uit hoog prevalente landen Grote verschillen in het voorkomen van tuberculose tussen stad en platteland. De tbc-screening bij binnenkomst in Nederland en vervolgscreening in de eerste jaren na binnenkomst heeft weinig impact op de incidentie onder immigranten Relatieve toename van extrapulmonale tuberculose, (bacteriologische) diagnostiek is vaak lastig Relatieve toename van multiresistente vormen van tuberculose, snelle diagnostiek is van groot belang Risicogroepen voor tuberculose wonen verspreid over Nederland Behoud van expertise staat onder druk bij dalende aantallen patiënten en verrichtingen Waar is behoefte aan? 15 Leidraad multiresistente tuberculose Operational research: Diagnostiek van LTBI: IGRA of tuberculinetest? Methode: Cohort van 3352 personen bij GGD en getest met 2-step: THT gevolgd door IGRA Resultaten: IGRA negatief bij 61% van de personen met LTBI gebaseerd op THT IGRA positivief bij 13% van de personen met LTBI gebaseerd op THT Conclusie: Meer dan 60% van preventieve behandelingen voor latente tbc-infectie op basis van de positieve uitslag van de tuberculinetest zijn onnodig (uitgaande van een hoge voorspellende waarde van de negatieve uitslag van de IGRA in laag-prevalente settings). De meerkosten van de 2-step test strategie worden grotendeels gecompenseerd door besparingen op de kosten van preventieve behandeling.
