Vergelijking MMPI-2 met 5 SAMENVATTING De Minnesota Multiphasic Personality Inventory 2 (MMPI-2) Restructured Clinical (RC) scales vormen de kern van een verkorte versie van de MMPI-2 (Tellegen et al., 2003) welke begin 2011 in Nederland zal verschijnen, genaamd de MMPI-2-Restructured Form (MMPI-2-RF) (Ben-Porath & Tellegen, 2008). In dit onderzoek wordt een (cross)valideringsonderzoek verricht naar de constructvaliditeit van de Nederlandse RCschalen. Hierin worden de klinische schalen uit de MMPI-2 vergeleken met de RC-schalen en de Hogere Orde Schalen uit de MMPI-2-RF voor classificatie van As-1 pathologie in termen van Diagnostic and Statistical Manual of Mental Disorders IV (4th. Ed. [DSM-IV]; American Psychiatric Association, 1994). De algemene hypothese van dit onderzoek is dat RC-schalen uit de MMPI-2-RF duidelijkere verbanden met As-I pathologie laten zien dan de klinische schalen uit de MMPI-2. De hypothesen zullen worden getoetst aan de hand van de construct-, predictieve - en incrementele validiteit. De deelnemers zijn 94 ambulante psychiatrische patiënten tussen de 18 en 60 jaar binnen de tweedelijns GGZ. Alle deelnemers hebben een persoonlijkheidsonderzoek ondergaan waarbij zowel de klinische schalen van de MMPI-2, de RC schalen uit de MMP-2-RF, als de Structured Clinical Interview for DSM-IV Disorders (SCID-I; First, Spitzer, Gibbon & Williams, 1997) gelijktijdig en éénmalig zijn afgenomen. In dit onderzoek zijn van de MMPI-2 de volgende klinische schalen gebruikt: Hypochondrie, Depressie, Psychopathische deviatie, Psychastenie en Hypomanie. De Restructured Clinical Scales uit de MMPI-2-RF die zijn gebruikt betreffen: Demoralisatie, Somatische Klachten, Lage Positieve Emoties, Antisociaal Gedrag, Dysfunctionele Negatieve Emoties en Hypomane Activatie. In tegenstelling tot bovengenoemde hypothese is de constructvaliditeit van de RC-schalen van MMPI-2-RF in dit onderzoek nagenoeg gelijk aan de klinische schalen van MMPI-2 voor het meten van As-I pathologie. Voor wat betreft de predictieve
Vergelijking MMPI-2 met 6 validiteit van de RC-schalen die internalizing symptomatology meten, is deze ongeveer gelijk aan die van de corresponderende klinische schalen. De RC-schalen die externalizing behaviors meten, bezitten een betere predictieve validiteit dan hun corresponderende klinische schalen. De RC-schalen die depressieve stoornissen en alcohol en middelenafhankelijkheid meten hebben incrementele validiteit ten opzichte van de klinische schalen. Voor het meten van externalizing behaviors kan worden gesteld dat de voorspellende waarde van de RCschalen van MMPI-2-RF groter is dan de voorspellende waarde van de corresponderende klinische schalen van MMPI-2. Voor het meten van de aanwezigheid van een depressieve stoornis heeft RC-schaal Low Positive Emotions toegevoegde waarde ten opzichte van klinische schaal Depressie en tenslotte heeft RC-schaal Antisocial Behavior incrementele validiteit ten opzichte van klinische schaal Psychopatische Deviatie voor het meten van alcohol- en middelenafhankelijkheid. Het werken met dimensionale modellen laat in deze studie slechts een beperkte toegevoegde waarde zien ten opzichte van categoriaal classificeren van As-I pathologie door middel van de RC-Schalen van de MMPI-2-RF. Dit onderzoek besluit met de aanbeveling voor de clinicus om de MMPI-2-RF als nuttige aanvulling te gebruiken naast de klinische schalen voor de classificatie van As-I pathologie. Vervolgonderzoek is noodzakelijk om de construct en incrementele validiteit van de MMPI- 2-RF nader te onderzoeken.
Vergelijking MMPI-2 met 7 SUMMARY The Minnesota Multiphasic Personality Inventory 2 (MMPI-2) Restructured Clinical (RC)-scales form the center of a shortened version of the MMPI-2 (Tellegen et al., 2003) which will be published in a Dutch version at the beginning of 2011, the MMPI-2- Restructured Form (MMPI-2-RF) (Ben-Porath & Tellegen, 2008). This study investigates the construct validity of the Dutch RC-scales based on a (cross)validation. The clinical scales from the MMPI-2 will be compared to the RC-scales and Higher Order Scales from the MMPI-2-RF for the classification of Axis-I pathology in terms of Diagnostic and Statistical Manual of Mental Disorders IV (4th. Ed. [DSM-IV]; American Psychiatric Association, 1994). The expectation is that the RC - and Higher Order scales from the MMPI-2-RF show better constructs with Axis-I pathology in terms of Diagnostic and Statistical Manual of Mental Disorders IV in comparison to the standard clinical scales of the MMPI-2 based on the construct-, predictive- and incremental validity. The participants ( n = 94) are all adult patients between 18 and 60 years of age whom were seen at the outpatient psychiatric hospital in s-hertogenbosch. All participants completed a personality inventory once and at the same time, wich contained the Clinical Scales of the MMPI-2, the RC-scales of MMPI-2-RF as well as the Structured Clinical Interview for DSM-IV Disorders (SCID-I; First, Spitzer, Gibbon & Williams, 1997). The clinical scales from the MMPI-2 which were used are: Hypochondriasis, Depression, Psychopathic deviate, Psychasthenia and Hypomania. The Restructured Clinical Scales from the MMPI-2-RF which were studied are: Demoralisation, Somatic Complaints, Low Positive Emotions, Antisocial Behavior, Dysfunctional Negative Emotions and Hypomanic Activation. The construct validity of the RC-scales of the MMPI-2- RF is almost equal compared to the standard clinical scales of the MMPI-2 for the classification of the studied Axis-I disorders. Vergelijking MMPI-2 met 8 measuring internalizing symptomatology corresponds in this study almost equally with clinical scales, while the RC-scales measuring externalizing behaviors shows a better predicitve validity than their corresponding clinical scales. The RC-scales added incrementally to the prediction of depressive disorders and substance abuse dependancy. When measuring externalizing behaviors it appears that the predictive validity of the RCscales is better than then the predicitive validity of the corrensponding clinical scales. Furthermore, it seems that to measure the presence of a depressive disorder the RC-scale Low Positive Emotions show incremental validity compared to the clinical scale Depression. Also the RC-scale Antisocial Behavior has incremental validity in comparison to the clinical scale Psychopatic Deviation for measuring substance abuse. Classification of Axis-I pathology based on dimensional models shows in this study moderate added value compared to categorical classification. This study concludes that for clinicians the MMPI-2-RF forms a useful addition for the classification of the Axis-I pathology. Further studies are necessary to examine the construct- and incremental validity of the MMPI-2-RF.
