2 Inleiding Transparantie, uitkomst van zorg, keuze-informatie en kwaliteit staan onverminderd hoog op ieders agenda. Er is echter een brede behoefte aan afstemming middels een overkoepelend, geïntegreerd en beheersbaar kwaliteitssysteem. Auditing lijkt hiervoor een geëigend instrument. Door de zorgvuldige registratie van zorguitkomsten en patiëntkarakteristieken wordt kwaliteit objectief inzichtelijk en komt betrouwbare sturingsinformatie beschikbaar. Met auditing is casemix-correctie (zorgzwaarte) mogelijk en analyse van het hele zorgproces. Dat biedt niet alleen transparantie naar externe partijen, maar ook gedetailleerde informatie aan de behandelaar zelf en een vergelijking met anderen (benchmarking/spiegelinformatie). Auditing geeft daarmee een intrinsieke prikkel tot kwaliteitsverbetering. In Nederland is vanuit de chirurgische beroepsverenigingen en de Dutch Colorectal Cancer Group (DCCG) de Dutch Surgical Colorectal Audit (DSCA) opgezet. Sinds 1 januari 2009 participeren alle Nederlandse klinieken aan deze uitkomst van zorg registratie voor de behandeling van darmkanker. De generieke opzet vormt de basis voor andere registraties, welke worden ondersteund door het recent opgerichte Dutch Institute for Clinical Auditing. Tijdens het jaarlijkse congres in april worden steeds de resultaten van het voorafgaande jaar gepresenteerd. Dit jaar vindt het congres op 18 en 19 april 2011 plaats in de St. Olofs Kapel van het Barbizon Hotel in Amsterdam. Op 18 april wordt in samenwerking met de DCCG een interactieve en instructieve dag georganiseerd over het lage rectum carcinoom. Nieuwe ontwikkelingen, verbetertrajecten en workshops maken deel uit van het programma. Op 19 april hebben we een bijzondere en wederom interactieve dag, waarin we iedereen uitdagen om het achterste van zijn tong te laten zien. De opening vindt plaats door de directeur-generaal curatieve zorg van VWS Drs. L.A.M. Halder, waarna niemand minder dan Atul Gawande de inleiding verzorgt. Deze ochtend wordt de discussie tussen de zaal en coryfeeën op het gebied van kwaliteit aangejaagd door voormalig international en ochtendvoorzitter Tom van t Hek. Na de lunch is het de beurt aan stakeholders en nieuwe registraties onder leiding van prof. Dr. R.A.E.M. Tollenaar en prof. Dr. P.A.M. Vierhout. Hierna worden de resultaten van de colorectale registratie over 2010 gepresenteerd onder leiding van dr. M.T.F.D Vrancken Peeters, binnen het bestuur van de Nederlandse Vereniging voor Heelkunde verantwoordelijk voor het kwaliteitsbeleid en Drs B.V.M. Crul, hoofdredacteur van Medisch Contact. De opdracht aan beiden is om de rapporteurs in deze sessie kritisch te onderhouden. Aan het einde van de dag hopen we net als vorig jaar het rapport aan de minister van VWS te kunnen aanbieden.
