3 COPD & Mobility woning Introductie OTIB, het Opleidings- en ontwikkelingsfonds voor het Technisch InstallatieBedrijf, wil bewerkstelligen dat er voldoende en goed opgeleide medewerkers in de branche werkzaam zijn en blijven. De vergrijzing in de samenleving raakt ook de installatiebranche. OTIB zet daarom concrete proefwoningen op voor het ontwerpen, inrichten en uitproberen van installatietechnologische voorzieningen. Deze voorzieningen maken het voor ouderen en gehandicapten mogelijk langer zelfstandig te blijven wonen. De eerste woning is medio 2007 opgeleverd en heeft als beoogd bewoners vitale 55+ers en mensen met een lichte visuele handicap. De woning heeft als thema Comfort en Veiligheid. OTIB wil nu starten met de realisatie van de volgende woning. Deze woning is bedoeld voor mensen met COPD (longaandoeningen) en/of een beperkte mobiliteit. Programma van Eisen COPD (longaandoeningen) & mobility Het programma van Eisen (PvE) voor de tweede woning is opgesteld met behulp van experts op het gebied van COPD en op het gebied van mobility in samenwerking met experts uit het installatieveld. De wensen en de oplossingen zijn tijdens brainstormsessies met experts vanuit de bewoners/patiënten en vanuit de verzorging verzameld. Door de installateurs is, tijdens een volgende bijeenkomst, hierop gereageerd. De wensen van de bewoners met de reacties van de installateurs zijn gebundeld en tijdens een gezamenlijke sessie besproken. Hieruit is het definitieve programma van eisen gevolgd, voor de bouwkundige en de installatietechnische voorzieningen maar ook voor de inrichting. Programma van Eisen voor de tweede woning van TTN! Uitgangspunten voor het woningontwerp Voordat de ontwerpeisen gespecificeerd zijn, is vastgesteld wat OTIB voor deze tweede proefwoning ongeveer voor ogen staat: het type woning, een afbakening van de doelgroep(en), uitgangspunten voor inrichting en ontwerp en de keuzes ten aanzien van de aanpassingen en (technische) voorzieningen. De ontwerpeisen en wensen zijn toegespitst op deze uitgangspunten en afbakening. De tweede proefwoning is: een betaalbare nieuwbouwwoning; gelijkvloers; bedoeld voor zelfstandige bewoning; door twee personen; met COPD- of Mobility gerelateerde klachten. Uit het oogpunt van levensloopbestendig bouwen is het aantrekkelijk als de woning ook geschikt is voor bewoning door een gezin met één kind. 5 Bewoners In de tweede proefwoning worden twee thema s uitgewerkt: Mobility en COPD. De proefwoning moet geschikt (en aantrekkelijk) zijn voor rolstoel-gebruikers en voor COPDpatiënten. Elk van deze twee doelgroepen is nader afgebakend om de ontwerpeisen voor de woning vast te kunnen stellen. De doelgroep van het thema Mobility bestaat in principe uit bewoners met bewegingsbeperkingen. Dat kan variëren van mensen die een deel van de tijd een rollator gebruiken, via mensen die vooral buitenshuis een rolstoel of scootmobiel gebruiken, via mensen die (ook binnenshuis) een groot deel van de tijd in een (eenvoudige) rolstoel zitten, tot mensen met ernstiger bewegingsbeperkingen die zich voortbewegen in een complexe, elektrische rolstoel met daarop verschillende voorzieningen. Voor de proefwoning gaan wij uit van bewoners in mobiliteitsklasse C: de bewoner is meestal rolstoel-gebonden, kan een korte afstand lopen, zichzelf ondersteunen en kort staan en is deels van hulp door zorgverleners afhankelijk. De doelgroep van het thema COPD bestaat in principe uit bewoners die COPD-patiënt zijn. Dat is een erg ruim begrip: er is een grote variatie in de aard en ernst van de klachten en beperkingen en een grote variatie in de doelgroep: van jong tot oud en van (voormalige) rokers tot bouwvakkers. Het meest relevant voor de proefwoning zijn COPD-patiënten die tamelijk ernstige klachten hebben, duidelijk beperkt zijn in hun energie en mogelijk van extra zuurstof gebruik moeten maken. De ontwerpeisen laten zich niet lezen als een overzicht van alle eisen waar een echte woning voor een rolstoelgebruiker en een COPD-patiënt aan moet voldoen. Het zijn de ontwerpeisen voor de proef- en demonstratiewoning, die bedoeld is voor het opleiden van installateurs en het tonen van mogelijkheden. Dat bepaalt de invulling van de proefwoning en ook de eisen die in het overzicht opgenomen zijn. De woning Algemene eisen en relevante literatuur naar aanleiding van het bovenstaande zijn onder meer: - de woning is realistisch en voldoet aan het Bouwbesluit; - de woninginstallaties voldoen aan ISSO publicatie 77; - de woning is geschikt voor een bewoner met COPD; o GGD-richtlijn Gezonde Woningbouw hfdst ¾; - de woning is bruikbaar voor een rolstoeler; o Handboek Toegankelijkheid; o Handboek Woonkeur. Door expertgroepen is een aantal elementen/wensen genoemd. De belangrijkste wensen staan hieronder per ruimte beschreven. Persons with COPD ask for a living environment in which they can perform independently and free of symptoms. Such a vital living environment should be characterised by an excellent indoor environmental quality, the ability of its occupants to perform daily activities, and support a healthy lifestyle. Increased vitality of older adults with COPD is expected to delay their demand for care, which will ease the strain on the health care system. Keywords: COPD, Indoor environment, Vitality. Category: Architecture for healthy environment, Economic and social costs of IAQ related health effects and benefits of good IAQ. 1 Introduction Chronic Obstructive Pulmonary Disease ranks as the 4 th leading cause of death worldwide [1] and is expected to increase rapidly in the coming 20 years due to changes in size and ageing of the population and changes in smoking behaviour [4]. In the Netherlands, the costs of COPD care are approximately 280 million euros per year (in 2000). It is estimated the costs will rise with 300% to 1100 million euros in 2025! [5]. Because of the extensive disability involved with the disease, and the future burden due to the ageing society, COPD care should be prioritised. Increased vitality of older adults with COPD is expected to delay their (aggravated) demand for care and continuation of independent living. This will ease the strain on the health care system (professional as well as informal care), and reduce the expenses on COPD care. The delay of the demand for professional care and stimulation of ageing-in-place are needed, since a future shortage of professional caregivers is foreseen, due to the ageing society. A strain on informal caregivers should be prevented. In this position paper we express our view on the vital living environment for people with COPD and the importance of an excellent indoor air quality. 2 Chronic Obstructive Pulmonary Disease COPD [6], which includes chronic bronchitis and emphysema, is a slowly progressive disease of the airways that is characterised by a reduced pulmonary capacity and increased metabolic rate. The degenerative process in the lungs of people with COPD leads to serious disability, which hinders normal functional performance of daily activities. The most important risk factor for COPD is tobacco smoking [6]. Other risk factors are passive smoking (exposure to environmental tobacco smoke), occupational exposure to respiratory irritants, indoor and outdoor air pollution, and genetic disorders. In addition to respiratory complaints persons with COPD contend with cardiac impairment, skeletal muscle dysfunction, osteoporosis, or weight loss [7]. COPD is positioned among the most disabling diseases. Functional limitations that are connected with COPD are a decreased mobility and a restriction in daily activities, such as body care, household management, eating, recreation and communication [8]. The degenerative process in the lungs of COPD patients hinders normal functional performance in daily activities, often ending up with physical deconditioning and occupational disability. The decline of physical condition ensues with serious consequences, such as anxiety, depression and social isolation [8-10]. Psychosocial problems, however, also influence functional performance [10,11], so a vicious cycle ensues. In addition to functional limitations due to COPD, older people, which are a vast majority among this group of patients, cope with the impairments and disabilities that are a result of the normal ageing process, such as impairments in hearing, vision, the neuromusculoskeletal system, and cognition [12]. 3 COPD management COPD management programs mainly focus on the disease related efforts by optimisation of pulmonary status by medication, as well as stimulation of smoking cessation, exercise, and appropriate nutrition, taking into account the individualised PvE 2 e wonin. The goals of current programs are to prevent disease progression, relief symptoms, improve exercise tolerance, improve health status, prevent and treat complications, prevent and treat exacerbations and reduce mortality [13]. The latest self-management programs for respiratory rehabilitation also focus on effective behaviour, based on sufficient knowledge about COPD, adequate coping behaviour, medical compliance, and attention to changes in the severity of the disease [13]. These self-management programs meet the demand of the patients for practical information about their disease and provides an improved possibility to manage the disease according to one s own interpretation. Environmental control, however, is not an important issue in current COPD management programs. The living environment of the patient is hardly taken into account, although poor indoor environmental quality shows adverse health effects and can even lead to social isolation, e.g. when people start avoiding exposure to environmental irritants. 4 Towards a vital living environment Persons with COPD ask for a living environment in which they can perform independently and free of symptoms. This vital living environment should offer resistance to a continuing increase of dysfunctions and structural abnormalities of lung functionality (impairments), restrictions in basic physical and mental actions (functional limitations), and difficulties doing activities of daily life (disabilities), influencing both physical, mental and social wellbeing of people with COPD. Intra-individual and extra-individual impact factors influencing the health outcomes are presented in the disablement process model (Fig. 1). Positive changes of the impact factors are expected to lead to an improved physical independence of the focus group. The factor air pollution exposure is positioned in the early phases of the disablement process model. Improvement of the indoor air quality will increase the lung functionality, followed by physical performance and the potential for physical independence. Excellent 68 PvE 2 e wonin. 70 indoor air quality is expected to result in fewer exacerbations leading to a gradual decline of the condition of the patient, and a delay of the demand for care regarding functional limitations and disability. However, the use of medical care, medication and pulmonary rehabilitation should also be considered. Later phases of the disablement process model show the importance of lifestyle and psychosocial factors regarding physical performance, and the importance of human, environmental and technological assistance regarding physical independence. 4.1 Indoor air quality Previous research in indoor air sciences has shown a positive effect of improved indoor air quality on people s health [14]. Exposure to pollutants in or around the dwelling, such as tobacco smoke, suspended dust, fog, cold air, odour of paint, frying and baking odours, and perfume, can trigger shortterm respiratory symptoms among persons with COPD. Exposure to low concentrations of outdoor air pollutants also showed adverse health effects [15-18]. In addition to infections, these irritants are the most important factors for triggering exacerbation. The air quality indoors is influenced by the lifestyle, such as smoking habits, and level of functional performance, influencing the ability to clean and maintain the residence, of the individuals with COPD or their relatives. In the Netherlands, coaching towards a healthy indoor environment by respiratory nurses is available for people with COPD. The effectiveness the indoor environmental control programs, however, is currently low [3,19]. 4.2 Design considerations Two approaches are available for realising a vital living environment for people with COPD; (i) architecture solutions, and (ii) technological solutions [20]. Still, the basis of the design of this vital environment for people with COPD is an excellent indoor air quality (Fig. 2). Architecture solutions, including interior design, supporting an excellent indoor air quality focus on source control, for example by using the use of lowemitting building materials and furnishings. Since COPD is leading to decreased mobility, the architecture of the vital living environment should account for the adaptability of the dwelling to future needs regarding the decreasing physical performance of the focus group. Layout and interior of the vital living environment enable optimal functional physical performance as assistive technologies for movement assistance or oxygen therapy come into use. Moreover, building layout en interior are designed to prevent falls, which is of major importance for people with COPD that frequently cope with osteoporosis. Technological solutions to improve indoor air quality contain ventilation supplies and intelligent sampling and control mechanisms. The greatest challenge, however, is to maintain the excellent indoor air quality [3,19]. Technology might offer possibilities for long-term management of the indoor environment. Solutions from information and communication technology (ICT) can be used to stimulate a healthy lifestyle and functional performance within minimal contact smoking cessation programs, stimulation of exercise, medical compliance or teleconsultancy by, e.g., a physiotherapist, dietician or community nurse. Home health monitoring enables early recognition of exacerbations and swift follow-up and medical treatment [21]. Moreover, information and communication technologies can be applied to stimulate mental and social well-being, using devices that give comfort to people living alone, enhance social networks, or provide cognitive stimulation and distraction. A vital living environment cannot be created by modifications of the home environment alone, since people also spend part of their time outdoors. Supportive urban planning and an increased attention to accessibility might be beneficial to the functional performance of the people with COPD. 6 Conclusions Excellent indoor air quality is the basis of the vital environment for people with COPD, since this is expected to improve the vitality of individuals with COPD. Moreover, attention should be given to mental and social well-being. The vital living environment can be realised by both architecture and technological solutions. However, research is needed to understand in what degree the vitality of persons with COPD will improve due to environmental adaptations. Fig. 2.
