4 Introduction The subject of occupational diseases is always relevant: a colleague with a burnout receives compensation; Poles come to the Netherlands to perform cheap labor and are quietly sent home after an accident or injury. Dutch employees experience more and more aggression and violence in the workplace. Of course, this image is not complete until we also look at the positive developments. Many branches of industry have improved their working conditions through occupational health agreements. Various companies have taken measures to counter the increasing violence and aggression. All sorts of initiatives have been launched to disseminate and utilize information about occupational health more effectively, such as the Arboportaal (Occupational Health Portal) that the government plans to start in The NCvB wishes to contribute to prevention and quality care in relation to occupational diseases and to the distribution of information on this subject. To provide quality healthcare, in November 2005 we launched the People and Work Outpatient Clinic, in which the NCvB works together with various clinical departments of the Amsterdam Academic Medical Center (AMC) and the Medical Center of the Vrije Universiteit of Amsterdam (VUmc). To be able to continue to contribute to prevention and information dissemination, the NCvB evaluated the registration of occupational diseases, as well as its role as an information center. We came to the conclusion that a number of matters should be organized differently and more effectively. The NCvB has been registering occupational diseases for 10 years now, creating many preventive impulses. However, we would like to improve the link between registration and prevention, and this requires improving the reliability of the data. We would also like to be able to track down new risks quickly and provide information about them to employers, employees and occupational health service professionals. This means that we will have to reform and improve our set of tools for monitoring and identifying occupational diseases in The NCvB believes that a supervisory organization is necessary to identify new risks and complex occupational health problems and to develop and implement new preventive techniques, with close connections to the field. Such an organization is also necessary for the NCvB to perform its monitoring duties. This supervisory organization could be set up by forming thematic expert panels. The NCvB and the Coronel Institute have already initiated the organization of such panels, for example the Occupationrelated Allergies Working Group, the Expertise Center for Hearing and Work, the Infectious Diseases and Work Working Group, the Repro and Work Working Group, and the Psychology and Work Initiative. The NCvB has high ambitions. In the current report, you can read how we have realized our ambitions in 2006, as well as what our plans and initiatives are for The theme of this annual report is: the NCvB as information network. The diverse information channels and the various methods for information dissemination will be illustrated through a case study. 5 Inleiding Het onderwerp beroepsziekten blijft actueel: een medewerker met burnout krijgt een schadeclaim toegewezen, Polen werken als tweederangs arbeiders in ons land en worden na een ongeval of met een beroepsziekte stilletjes weggevoerd. Werknemers in Nederland ervaren steeds meer agressie en geweld in hun werksituatie. Natuurlijk is dit beeld niet compleet als we niet ook de goede ontwikkelingen weergeven. In diverse branches zijn door de arboconvenanten de werkomstandigheden verbeterd. In veel bedrijven wordt actie ondernomen tegen toenemende agressie en toenemend geweld. Er zijn allerlei initiatieven om kennis over arbeid en gezondheid beter te verspreiden en te benutten, bijvoorbeeld het Arbo portaal dat de overheid in 2007 zal lanceren. In Nederland is een arbokennisinfrastructuur in ontwikkeling. Allerlei meer of minder succesvolle experimenten ten aanzien van een kennisinfrastructuur zijn de afgelopen jaren uitgevoerd en de verschillende actoren (overheid, universiteiten, beroepsgroepen, kennisinstituten, sociale partners, arbodiensten) zijn op zoek naar hun positie in de zich ontwikkelende arbokennisinfrastructuur. De overheid (ministerie van Sociale Zaken en Werkgelegenheid, SZW) neemt zelf een regierol in het Arboportaal, maar tegelijkertijd is de leidende gedachte in het huidige beleid dat sociale partners een belangrijke verantwoordelijkheid hebben. Het NCvB is van mening dat een topstructuur nodig is voor het signaleren van nieuwe risico s, complexe arboproblematiek en de ontwikkeling en implementatie van nieuwe preventieve technieken, met een goede verbinding naar het veld. Voor het NCvB is een dergelijke structuur ook essentieel voor de uitvoering van de signaleringstaak. Deze topstructuur zal vorm krijgen door het inrichten van thematische expertgroepen. Aanzetten voor dergelijke expertgroepen bestaan al bij het NCvB en het Coronel Instituut (zoals de Werkgroep Allergische Beroepsziekten, het Expertisecentrum Gehoor en Arbeid, de Werkgroep Infectieziekten en Arbeid, de Werkgroep Repro en Arbeid en het Initiatief Psychisch en Werk). NCvB 2006 Het NCvB wil een bijdrage leveren aan preventie van en goede zorg voor beroepsziekten en de verspreiding van kennis over beroepsziekten. Voor de goede zorg hebben wij in november 2005 de Polikliniek Mens en Arbeid gelanceerd, waarin het NCvB samenwerkt met diverse klinische afdelingen van het AMC en het VUmc. Om een bijdrage te kunnen blijven leveren aan preventie en kennisverspreiding heeft het NCvB in 2006 de registratie van beroepsziekten en zijn rol als kenniscentrum geëvalueerd. De conclusie is dat een aantal zaken anders en beter moeten. Het NCvB registreert inmiddels 10 jaar beroepsziekten en dat heeft veel preventieve impulsen gegeven. We willen echter de registratie nog meer verbinden met preventie en dat vraagt ook om verbetering van de betrouwbaarheid van de gegevens. Ook willen we nieuwe risico s snel op het spoor kunnen komen en informatie hierover verspreiden naar werkgevers, werknemers en arboprofessionals. Dat betekent dat we ons instrumentarium voor monitoring en signalering van beroepsziekten in 2007 gaan hervormen en verbeteren. De ambities van het NCvB zijn hoog. In het huidige verslag kunt u lezen hoe wij onze ambities gerealiseerd hebben in Ook leest u over onze plannen en initiatieven voor Het thema van dit jaarverslag is het NCvB als kennisnetwerk. Middels een casus worden de diverse kennisstromen en de verschillende manieren van kennisoverdracht inzichtelijk gemaakt. 6 Notification and Registration In order to form an accurate picture of the incidence and dissemination of occupational diseases in the Netherlands, the NCvB carries out a number of registrations. The most extensive of these registrations is the National Registration of Occupational Diseases. As this registration does not provide all of the necessary information, there are also a number of supplementary surveillance projects. These surveillance projects provide specific information about work-related skin and lung diseases. An important problem in occupational disease registration is that due to problems such as underreporting no representative data on occupational diseases in the Netherlands can be obtained. At the request of the Ministry of Social Affairs and Employment, we therefore began work in 2006 on drawing up an advisory note to improve occupational disease monitoring. In the context of this advisory program, a number of interested parties, such as government, employers and employees organizations and occupational health service professionals, were interviewed concerning the data necessary to draw up preventive policy regarding occupational diseases. Subsequently, a number of national and international experts in the field of registration were asked how this information should be gathered and registered. The researchers then consulted the relevant literature to obtain additional information about occupational disease registration. In early 2007, this report resulted in an advisory note that will be discussed and then presented to the Ministry of Social Affairs and Employment. The results will also influence the NCvB s registration systems in For instance, in 2007 we will start to create a new registration system for new risks of occupational diseases. National Notification and Registration System Occupational health services and physicians are required to report (suspected) occupational diseases. Occupational health services have been subject to this requirement since 1999; since July 2005 this has also applied to occupational physicians. This notification is required by the Occupational Health and Safety Act. The most important goal of the National Registration System is to collect information in order to formulate sound policies for the prevention of occupational diseases. In 2001, it was determined that within 5 years, the NCvB should receive around 95% of all occupational disease notifications electronically. This goal was met in Only 5% of the notifications are still sent via paper mail. A large portion of the notifications in 2006 were sent directly by Arbouw. Further analysis shows that during the past three years, fewer physicians sent notifications, while the average number of notifications per physician has remained stable. Table 2 illustrates this decline in notifications. The notifications sent by Arbouw were not included, as the reporting physician is not known in these cases. This question allows the reporting physician to indicate the seriousness of the occupational diseases reported. This information provides more insight into the consequences of the occupational diseases reported.

7 Melding en registratie NCvB 2006 Om een goed beeld te kunnen krijgen van het voorkomen en de spreiding van beroepsziekten in Nederland wordt er binnen het NCvB een aantal registraties gevoerd. De meest omvangrijke registratie is de Nationale registratie van Beroepsziekten. Omdat uit deze registratie niet alle benodigde informatie verkregen kan worden lopen er ook nog een aantal peilstations. Uit deze peilstations wordt specifieke informatie gehaald over beroepsgebonden huiden longaandoeningen. Een belangrijk probleem binnen de registratie van beroepsziekten is dat er door onder andere onderrapportage geen representatieve gegevens over beroepsziekten in Nederland verkregen kunnen worden. Daarom is in 2006 in opdracht van het ministerie van SZW begonnen met het opstellen van een advies om de monitoring van beroepsziekten te verbeteren. In het kader van dit adviestraject is Meldingen direct via bedrijfsartsen aan het NCvB Notification by occupational physicians directly to the NCvB Meldingen via Arbouw Notification via Arbouw Totaal Total aan een aantal stakeholders (overheid, werkgevers-, werknemersorganisaties en arboprofessionals) middels een uitgebreid interview gevraagd naar de gegevens die nodig zijn voor preventief beleid van beroepsziekten. Vervolgens is aan een aantal nationale en internationale experts op het gebied van registraties voorgelegd hoe die informatie dan verzameld en geregistreerd moet worden. In een literatuuronderzoek is vervolgens gekeken naar aanvullende informatie over de registratie van bepaalde beroepsziekten. Dit rapport resulteerde begin 2007 in een advies dat zal worden besproken en voorgelegd aan het ministerie van SZW. De resultaten ervan zullen in 2007 invloed hebben op de registratiesystemen van het NCvB. Zo zal in 2007 begonnen worden met het opstellen van een registratiesysteem om nieuwe risico s die kunnen leiden tot beroepsziekten te kunnen registreren. Nationale melding en registratie van beroepsziekten Het melden van (vermoede) beroepsziekten is zowel voor arbodiensten als voor bedrijfsartsen verplicht. Voor arbodiensten geldt deze verplichting al vanaf Sinds juli 2005 zijn ook individuele bedrijfsartsen verplicht tot melden. Deze meldingsplicht is geregeld in de Arbowet. Belangrijkste doel van de Nationale registratie is informatie verzamelen om tot goede beleidsinformatie ten behoeve van de preventie van beroepsziekten te komen. In 2001 is de doelstelling geformuleerd dat binnen 5 jaar ongeveer 95% van alle beroepsziektemeldingen elektronisch binnen moet komen bij het NCvB. In 2006 komt 95% elektronisch binnen, terwijl nog maar 5% via papier binnenkomt. Een belangrijk gedeelte van de meldingen voor 2006 wordt rechtstreeks aangeleverd via Arbouw. Uit verdere analyses blijkt dat het de afgelopen 3 jaar de trend

The notifications that do mention the seriousness of the disease show that most of the diseases reported result in employee sick leave of 1 to 3 months. European Occupational Diseases Statistics (EODS) Each year, the NCvB provides data regarding occupational diseases in the Netherlands to the Eurostat project European Occupational Diseases Statistics (EODS). The goal of this project is to harmonize data on occupational disease registrations in Europe in order to produce statistics that are comparable at the European level. In 2006, the NCvB received a subsidy from Eurostat to further improve the quality of the registration of occupational diseases. As there was no data available on the seriousness of the occupational diseases reported in the Netherlands, we decided to add a question regarding the seriousness of the disease to the electronic notification form. With this information, occupational disease notifications in Europe can be compared more easily. Per 1 January 2008, another code list will be used for economic activity in the various European statistics. In order to maintain the comparability of the statistics, the NCvB will also adopt the new coding per 1 January We will submit a subsidy application for this to Eurostat in Registration of Occupational Diseases and Industrial Accidents on the Antilles, Aruba and in Suriname In October 2004, we began registering industrial accidents and occupational diseases in the Netherlands Antilles, Aruba and Suriname at the request of occupational physicians there. Approximately 31 physicians participated in this project. Each year, the Municipal Health Department of Curaçao analyses the notifications received by the NCvB. The results of this analysis were then presented to all participants during a local continuing education meeting in In 2006, we submitted an application to USONA for a follow-up project for notification and registration of occupational diseases. The project s goals are: Creating awareness of the importance of prevention of work-related diseases and accidents among the target groups (employers, employees, insurance companies, government) Organizing a test structure for effective interventions in the case of work-related complaints and disorders Promoting a safe and healthy workplace Tabel 2 Het aantal meldende artsen per jaar* Table 2 Number of reporting physicians per year* Work-related Dermatosis Surveillance In 2000 we set up the Work-related Dermatosis Surveillance (ADS) surveillance project in co-operation with the NECOD. As work-related skin diseases rarely cause absenteeism among employees, these diseases are not often reported to occupational physicians. We therefore decided to have dermatologists register all work-related skin diseases. A group of 25 Dutch dermatologists have been reporting work-

9 geweest is dat er steeds minder artsen zijn gaan melden, terwijl het gemiddeld aantal meldingen per arts ongeveer gelijk blijft. In tabel 2 is deze daling duidelijk te zien. Omdat van de beroepsziektemeldingen van Arbouw de arts die de melding gedaan heeft niet bekend is, zijn deze meldingen hier buiten beschouwing gelaten. In 2006 is er aan het elektronisch meldingsformulier beroepsziekten een extra vraag toegevoegd. Met deze vraag kan de ernst van de gemelde beroepsziekten aangegeven worden door de meldende bedrijfsarts. Deze informatie geeft meer inzicht in de gevolgen van de gemelde beroepsziekten. Het is voor de meldingen die via Arbouw binnenkomen niet mogelijk om een uitspraak te doen over de ernst van de gemelde aandoening. Van de meldingen waarbij wel informatie is over de ernst blijkt dat het grootste gedeelte van de aandoeningen leidt tot een verzuim van 1 tot 3 maanden. European Occupational Diseases Statistics Het NCvB levert elk jaar gegevens over beroepsziekten in Nederland voor het Eurostat project European Occupational Diseases Statistics (EODS). Het doel van dit project is het harmoniseren van Aantal meldende artsen Number of reporting physicians gegevens over beroepsziekteregistraties in Europa om zo op Europees niveau vergelijkbare statistieken te krijgen. In 2006 heeft het NCvB subsidie gekregen vanuit Eurostat om de kwaliteit van registratie van beroepsziekten verder te verbeteren. Omdat er in Nederland nog geen informatie voorhanden was over de ernst van de gemelde beroepsziekten is gekozen om een extra vraag over de ernst van de gemelde aandoening toe te voegen aan het elektronische meldingsformulier. Met deze informatie kunnen de beroepsziektemeldingen binnen Europa beter met elkaar vergeleken worden. Per 1 januari 2008 wordt er binnen de verschillende Europese statistieken overgegaan op een andere codeerlijst voor de economische activiteit. Om de vergelijkbaarheid van de statistieken te behouden zal ook het NCvB per 1 januari 2008 overgaan op de nieuwe codering. In 2007 zal daarvoor een subsidieverzoek worden ingediend bij Eurostat. Registratie van beroepsziekten en bedrijfsongevallen op de Antillen, Aruba en in Suriname In oktober 2004 is op verzoek van een aantal bedrijfsartsen van de Antillen, Aruba en Suriname een registratie gestart van bedrijfsongevallen en beroepsziekten. Ongeveer 31 artsen nemen aan dit project deel. De GGD op Curaçao analyseert jaarlijks de bij het NCvB binnengekomen meldingen. De resultaten van deze analyse zijn in 2006 teruggekoppeld aan alle deelnemers gedurende een nascholingsbijeenkomst ter plekke. Doel van het project is: Bewustwording van het belang van preventie van ziekte en ongevallen door het werk bij de doelgroepen (werkgevers, werknemers, sociale verzekeringsinstanties, overheid) Opzetten van een proefstructuur voor effectieve interventies bij werkgerelateerde klachten en aandoeningen Bevorderen van veilig en gezond werken NCvB 2006

Voor nader onderzoek naar de diagnose en de oorzaak van de klachten stuurt hij patiënt door naar de Polikliniek Mens en Arbeid. 12 related skin diseases for the past six years. In 2002, this surveillance project was expanded by adding a question about whether the reported skin disease resulted in absenteeism. The table below shows that approximately 60% of the reported cases did not lead to employee absenteeism. As not all notifications for 2006 have arrived, the definitive figures for 2006 will be published in next year s annual report. For more information about this surveillance project, please visit Work-related Respiratory Diseases Surveillance Project A few years ago, the NCvB set up a surveillance project for work-related respiratory diseases together with the Netherlands Expertise Center for Occupational Respiratory Disorders. As part of this project, approximately 30 lung and allergy specialists send notifications of work-related respiratory diseases to the NCvB. Each month, every lung/allergy specialist submits a chart with the notifications for that month. National and European Guidelines Registration guidelines should offer the occupational physician a foundation on which to base his/her considerations of whether the patient s complaint may or may not be a (suspected) occupational disease. They are predominantly important in the case of diseases that may also be caused by factors outside of work and for which the degree to which they were caused by work-related factors is difficult to establish. Some well-known examples are musculoskeletal and posture disorders, mental illnesses, cardio-vascular diseases and chronic obstructive pulmonary disease (COPD). A registration guideline should in fact offer a decision tree which will enable an occupational physician to come to a decision about the nature, seriousness and duration of the exposure and establish the degree to which the complaint is workrelated. This decision tree should be backed up by a document indicating the relevant literature used in composing the decision tree and explaining the rationale behind it. Over the past few years, several documents and guidelines have been created in this way to allow occupational physicians to determine the degree to which neck, arm, shoulder and back complaints and depression are work-related. In 2006, documents relating to the disorders listed below were in the process of being drawn up. The format of these information notices served 12

13 ArbeidsDermatosen Surveillance In 2000 is samen met het NECOD het peilstation ArbeidsDermatosen Surveillance (ADS) opgezet. Omdat er bij arbeidsgebonden huidaandoeningen in veel gevallen geen sprake is van verzuim komen werknemers met deze aandoeningen minder vaak bij de bedrijfsarts terecht. Daarom is besloten om de registratie van arbeidsgebonden huidaandoeningen ook uit te laten voeren door dermatologen. Een groep van ongeveer 25 dermatologen verspreid over heel Nederland meldt al 6 jaar beroepsgerelateerde huidaandoeningen. In 2002 is aan dit peilstation de vraag toegevoegd of de werknemer met de gemelde huidaandoening verzuimt. De onderstaande tabel geeft duidelijk aan dat er bij ongeveer 60% van de meldingen geen sprake is, of geweest is, van verzuim. Omdat nog niet alle meldingen van 2006 zijn binnengekomen kunnen de cijfers voor 2006 pas in het volgende jaarverslag gepresenteerd worden. Verzuim 2002 % 2003 % 2004 % 2005 % Totaal % Sickness absence Total nee no ja yes n.v.t. n / a Peilstation Arbeidsgerelateerde Longaandoeningen Samen met het Kenniscentrum Opgelucht Werken heeft het NCvB enkele jaren terug een peilstation voor arbeidsgerelateerde longaandoeningen opgezet. In dit peilstation melden ongeveer 30 longartsen en allergologen arbeidsgerelateerde longaandoeningen aan het NCvB. Elke arts/allergoloog stuurt elke maand een kaart terug met daarop de meldingen. Nationale en Europese richtlijnen Registratierichtlijnen moeten de bedrijfsarts houvast bieden bij zijn afweging om te concluderen of er al dan niet sprake is van een (vermoede) beroepsziekte. Zij zijn vooral van belang bij aandoeningen die zowel door factoren in als buiten het werk kunnen worden veroorzaakt en daardoor het wegen van de bijdrage van belasting in het werk op problemen kan stuiten. Bekende voorbeelden hiervan zijn aandoeningen van het houding- en bewegingsapparaat, psychische aandoeningen, hart- en vaataandoeningen en chronische luchtwegobstructie (COPD). Een registratierichtlijn moet dan in feite vooral een beslisboom aanreiken, die de bedrijfsarts dwingt tot uitspraken over aard, niveau en duur van de blootstelling en uitmondt in een conclusie over de mate van arbeidsgerelateerdheid. Achter deze beslisboom bevindt zich een document waarin op basis van een literatuurstudie het bewijs is samengevat dat tot het opstellen van de beslisboom heeft geleid. Op deze wijze zijn de afgelopen jaren documenten en richtlijnen gemaakt voor het vaststellen van de arbeidsgerelateerdheid van klachten van nek, armen en schouders, rugklachten en depressie. In 2006 is gewerkt aan de realisatie van documenten over onderstaande aandoeningen. De verwachting is dat zij in de loop van 2007 gereedkomen en dat de richtlijnen hierna geïmplementeerd kunnen worden. Deze information notices hebben een format dat als basis heeft gediend voor de opzet van de registratierichtlijnen. Het zijn

They are concise documents that are written after review of the academic literature, but in which the chosen formulations are the result of occasionally extensive discussions on the different guidelines used by the various member states in the context of their compensation systems for occupational diseases. Aside from documents relating to specific exposures (such as toxic materials, noise, radiation), documents have also been drawn up or revised that concern the determination of the extent to which certain diseases may be work-related (asthma, skin diseases, cancer, infectious diseases). European Forum The European Forum for insurance against occupational diseases and industrial accidents. The European Forum Occupational Diseases working group completed a study into asbestosrelated diseases in Europe in This report compares the statistics for mesothelioma, lung cancer and asbestosis from various European countries and describes the different social measures these countries have introduced with respect to these diseases. It also includes a chapter on the prognosis of the number of cases. The report was presented to representatives of European institutions in Brussels on 5 September. In October, the entire European Forum met in Florence, where they discussed various (public) campaigns aimed at preventing occupational diseases. The NCvB (G. van der Laan) has the official status of Observer in the European Forum, but actively participates in the Occupational Diseases working group. ( Onderrapportage psychische beroepsziekten Er bestaat geen overeenstemming over de criteria voor het melden van overspannenheid als beroepsziekte. Dit bleek uit een onderzoek bij 103 bedrijfsartsen die vijf gevallen voorgelegd kregen waarin verschillende accenten waren aangebracht. Hoewel bij vier belastende werkfactoren in het geding waren kwam 30% van de bedrijfsartsen bij geen enkele casus tot de diagnose beroepsziekte. De beschreven afwegingen in hun beoordelingen liet een zeer divers beeld zien, waarbij de aanwezigheid van privéfactoren het meest genoemde argument was om te concluderen dat er niet sprake was van een beroepsziekte. De onderzoekers pleiten voor de ontwikkeling van een checklist risicofactoren als onderdeel van de nieuw te maken registratierichtlijn overspanning (stressgerelateerde stoornis). De Vos en Nieuwenhuijsen, TBV 2006; 14(10): Underreporting of work-related mental illnesses There is as yet no agreement on the criteria for reporting burnout as an occupational disease. This appeared from a survey held among 103 occupational physicians who were presented with five cases displaying different accents. Although in four cases work-related factors were involved, 30% of the occupational physicians did not diagnose a single case as an occupational disease. The written explanations of their diagnoses indicated very diverse considerations, in which the presence of personal factors was the argument most used to conclude that the complaint was not an occupational disease. The researchers advise developing a checklist for risk factors as part of the new guidelines for burnout (stress-related illnesses). De Vos and Nieuwenhuijsen, TBV 2006; 14(10):

De werkgroep Beroepsziekten van het Europees Forum heeft in 2006 een studie over asbestgerelateerde ziekten in Europa afgesloten. Hierin worden de statistieken over mesothelioom, longkanker en asbestose uit verschillende Europese landen vergeleken en worden de verschillende sociale arrangementen rond deze aandoeningen beschreven. Ook is een hoofdstuk gewijd aan de prognose van het aantal gevallen. De studie is gepresenteerd aan vertegenwoordigers van Europese instellingen op 5 september in Brussel. In oktober vond in Florence een bijeenkomst van het gehele Europees Forum plaats. Hier werden verschillende (publieks-)campagnes ter preventie van beroepsziekten besproken. Het NCvB (G. van der Laan) heeft binnen het Europees Forum de status van Observer, maar participeert actief in de werkgroep Beroepsziekten. ( NCvB

16 Dissemination of Knowledge The goal of the NCvB is to contribute to improving the quality of prevention, (early) diagnosis, treatment and supervision of occupational diseases and work-related complaints. In this regard, one of the core tasks is the collecting, developing, dissemination and application of knowledge about occupational diseases and work-related complaints. To actually prevent occupational diseases and work-related complaints, it is necessary that this information reaches the shop floor the employers and employees and the occupational health service professionals who support them in ensuring good working conditions and preventing absenteeism and incapacity for work. The NCvB s knowledge products include: the annual Alert Report on Occupational Diseases, the Annual Report, information on the website the newsletter, reports to notifying dermatologists and lung specialists, the semi-annual Heijerman lectures, articles in academic publications, continuing education and training courses for occupational and insurance company physicians, and the answering of individual questions that reach us via our Helpdesk ( per year). This chapter discusses a number of activities relating to gathering and disseminating knowledge. Aside from the more well-known activities, we have also developed new initiatives. In order to gain insight into the needs of the NCvB s new target groups, such as employers and employees, we have initiated the project Research into the Demand for Branch-Oriented Dissemination of Knowledge in Together with the Coronel Institute of Occupational Health, representatives from professional organizations for occupational physician and preventionists, we have developed plans to reinforce the knowledge infrastructure, including a project plan for a website that is to provide occupational health service professionals with access to relevant information; the Arbokennisnet (Occupational Health Knowledge Network). We are also in touch with the Ministry of Social Affairs and Employment regarding contributions by the NCvB and others to the proposed Arboportaal. As regards continuing education for occupational physicians, the NCvB has developed and organized a new workshop concerning the reporting of occupational diseases. After establishing there was sufficient interest, the NCvB has created a supplementary training course regarding the new guideline of the NVAB (Netherlands Association for Occupational and Company Medicine) for contact dermatitis. This course will be made available in Research into the Demand for Branch-Oriented Dissemination of Knowledge In 2006, the NCvB performed a study into the demand for branch-oriented dissemination of knowledge. Until this time, the NCvB was mainly geared towards disseminating knowledge among occupational health service professionals, especially among occupational physicians and occupational health services; however, within the field of occupational health the responsibility for preventing work-related complaints is increasingly being placed on employers and employees. This will often be dealt with by the various branches, in which process the branch organizations play an important part, as shown by the Occupational Health (plus) agreements and the development of the Occupational Health catalogs. 16

17 Kennisverspreiding NCvB Het NCvB wil een bijdrage leveren aan de kwaliteit van de preventie, (vroeg)diagnostiek, behandeling en begeleiding van beroepsziekten en arbeidsgebonden aandoeningen. Daarbij is één van de kerntaken het verzamelen, ontwikkelen, verspreiden en (doen) toepassen van kennis over beroepsziekten en werkgebonden aandoeningen. Om beroepsziekten en arbeidsgebonden aandoeningen daadwerkelijk te voorkomen is het nodig dat die kennis terecht komt op de werkvloer bij werkgevers en werknemers en bij de arboprofessionals die hen ondersteunen in de zorg voor goede arbeidsomstandigheden en het voorkomen en beperken van verzuim en arbeidsongeschiktheid. Kennisproducten van het NCvB zijn onder meer het jaarlijkse Signaleringsrapport Beroepsziekten, het jaarverslag, informatie op de website de nieuwsbrief, rapportages aan meldende dermatologen en longartsen, de halfjaarlijkse Heijermanslezingen, publicaties in vaktijdschriften, na- en bijscholing van bedrijfs- en verzekeringsartsen en beantwoording van individuele helpdeskvragen ( per jaar). In dit hoofdstuk wordt een aantal activiteiten op het gebied van kennisverzameling en kennisverspreiding besproken. Naast de bekende activiteiten werden ook nieuwe initiatieven genomen. Zo werd om inzicht te krijgen in de behoeften bij voor het NCvB nieuwe doelgroepen als werkgevers en werknemers in 2006 het project behoeftepeiling branchegerichte kennisverspreiding uitgevoerd. In samenwerking met het Coronel Instituut voor Arbeid en Gezondheid en vertegenwoordigers van de beroepsverenigingen van bedrijfsartsen en preventisten is gewerkt aan plannen voor het versterken van de kennisinfrastructuur, waarbij onder meer een projectplan is geschreven voor de opzet van een website die arboprofessionals toegang biedt tot relevante kennis, het Arbokennisnet. Ook zijn er contacten met het ministerie van SZW voor bijdragen van het NCvB en anderen aan het nieuw op te zetten Arboportaal. Met betrekking tot nascholing van bedrijfsartsen is een nieuwe workshop over het melden van beroepsziekten ontwikkeld en uitgevoerd. Ook is na peiling van eventuele interesse daarvoor een verdiepende training ontwikkeld rondom de nieuwe NVAB-richtlijn Contacteczeem. Deze training zal in 2007 voor het eerst worden gegeven. Project behoeftepeiling branchegerichte kennisverspreiding In 2006 is door het NCvB onderzoek gedaan naar de behoefte aan branchegerichte kennisverspreiding. Tot nu toe richtte het NCvB zich bij de kennisverspreiding vooral op de arboprofessionals, vooral de bedrijfsartsen en arbodiensten, maar binnen de arbodienstverlening komt de verantwoordelijkheid voor het voorkomen van arbeidsgerelateerde klachten nadrukkelijker dan tevoren te liggen bij werkgevers en werknemers. Vaak zal dit door branches worden opgepakt, waarbij brancheorganisaties een belangrijke rol spelen, zoals bleek bij de Arbo(plus)convenanten en bij het inrichten van de Arbocatalogi. Ook bij bedrijfsartsen bestaat belangstelling voor een branchegerichte aanpak, getuige enkele initiatieven vanuit de beroepsvereniging NVAB. Uit onderzoek van onder meer AStri blijkt dat werkgevers en werknemers goed te bereiken zijn via hun brancheorganisaties. Deze worden gezien als betrouwbare bron van informatie over Arbo- en verzuimbeleid. Branches lijken dus een geschikte doelgroep voor kennisverspreiding vanuit het NCvB, zeker wanneer het mogelijk is aan te sluiten bij bestaande arbo-initiatieven.

20 Occupational physicians have also expressed interest in a branch-oriented approach, as illustrated by a few initiatives by the NVAB. Research from AStri, among others, shows that employers and employees can be reached effectively through their branch organizations. They are seen as reliable sources of information about occupational health and absenteeism policies. Branches therefore seem to be suitable target groups for knowledge dissemination by the NCvB, especially when it is possible to link in with existing occupational health initiatives. 30 Key individuals from the 10 branches concerned were asked to attend an invitational conference to exchange thoughts about the demand for knowledge and specific knowledge products regarding occupational diseases and work-related complaints. The invitational conference provided the following perspectives on the branches needs: Branches desire to be provided with information about developments concerning new technologies, materials and work methods quickly. The People and Work Outpatient Clinic, at which facility individuals can be examined, is considered to be important. Complex problems can be investigated effectively in this manner. The NCvB can improve awareness about occupational diseases in specific sectors by providing information about occupational diseases that are common in those sectors. The branch representatives accord the NCvB an important role in supporting occupational physicians in identifying and diagnosing occupational diseases. The identification and registration of work-related complaints is insufficient in many sectors. Based on the available literature and comparisons with current registrations, the NCvB can indicate which sectors and professions may be subject to underdetection and shortages of preventive measures. This can be seen as registration of the blank spaces. Publications 2006 Alert Report on Occupational Diseases At the request of the Ministry of Social Affairs and Employment, the NCvB draws up an annual Alert Report on Occupational Diseases each year. This report provides an overview of the incidence and dissemination of occupational diseases in the Netherlands. It also reports on new scientific and social developments concerning the prevention of, and rehabilitation after, occupational diseases. Each Alert Report concentrates on a number of warnings alerts on occupational diseases, intended for policymakers and professionals (see the overview on page 22). Newsletters The NCvB publishes a newsletter each quarter. The newsletters contain articles about current developments and reviews of interesting new publications. Planned lectures, workshops and newly published guidelines are posted in the Announcements section. In the Case Studies section three or four case studies are usually presented. 20
