3 Chapter 7 Summary Neuroblastoma (NB) is the most common extracranial solid tumor in children under five years of age. NB tumors may arise in different locations of the sympathetic nervous system and the adrenal glands are the most common primary site with around 30 % prevalence. NB is also very heterogeneous in symptoms and outcome. The survival rate of high-risk NB patients is below 50 %. In addition, the conventional therapy leads to severe side effects. Therefore, great efforts have been made to find and validate novel molecular targets for NB therapy. The main objective of this thesis is to explore doublecortin-like kinase (DCLK1) as a therapeutic target for NB. For this purpose, we investigated the expression levels of doublecortin-like (DCL) and DCLK-long, two microtubuleassociated proteins (MAPs) encoded by the DCLK1 gene, in human NBs. Furthermore, we performed several in vitro and in vivo studies to investigate the effect of DCL/DCLK-long loss-of-function in NBs. In chapter 2 we reviewed molecular targets that have been proposed for NB therapy. We discussed some targets that are under investigation in the clinic and, in particular, those that have been proposed but that have not reached the phase of clinical investigation yet.. Some of the targets that play a role in at least one of the tumorigenic processes (proliferation, anti-apoptosis, angiogenesis and/or metastasis) were described in detail, particularly DCLK1. DCL/DCLK-long are mainly expressed in neuroblasts and are involved in microtubule stabilization, retrograde transport of the glucocorticoid receptor, neuroblast proliferation and migration. We detected that DCL and DCLK-long are overexpressed in human NBs and in gliomas compared to other type of cancer and normal tissue (chapter 3). In this chapter, we investigated the effect of DCL/DCLK-long knockdown in mouse and human NB cells as well. Gene-expression profiling in NB cells with DCL/DCLKlong knockdown revealed that pro-apoptotic processes were induced. Using several assays, including time-lapse imaging, caspase-3 activation and fluorescence-activated cell sorting we confirmed that silencing DCL and DCLK-long induces apoptosis in NB cells. In chapter 4 we tested the hypothesis that knockdown of the microtubulestabilizing DCL and DCLK-long sensitizes NB cells to the microtubule-destabilizing vinca alkaloids (VAs). Our results showed a strong synergistic apoptotic effect, indicating that a combined treatment requires lower doses of VAs to induce NB cell death. Our data suggest that a future NB therapy combining DCL/DCLK-long knockdown and low doses of VA may result in less side effects compared to apply higher doses of VA only. However, future in vivo studies need to be carried out to validate the potential therapeutic significance of our findings. The synergistic apoptotic effect might result from the independent and distinct action of VAs and DCL/DCLK-long knockdown on mitotic spindles and mitochondria activity (see also chapter 5). A potential interaction between DCL and mitochondria activity is described in chapter 5. In human NBs we identified a high correlation between DCL and mitochondrial-related genes (chapter 3). In chapter 5 we described experiments showing colocalization of DCL with mitochondria. Moreover, induction of DCL knockdown results in less cytochrome c oxidase activity and less ATP synthesis. 202

4 Summary/ Samenvatting We found that the C-terminal Serine/Proline-rich domain and the second microtubule-binding domain are crucial for the regulation of cytochrome c oxidase activity and ATP synthesis by DCL. Also, we showed that the decreased energy availability due to DCL silencing resulted in less NB cell proliferation. Previous studies have shown that silencing DCL results in mitotic spindle disruption, leading to cell-cycle arrest and in this thesis we demonstrate that the decrease in NB proliferation may also be due to alterations in mitochondria activity. In vivo studies using xenograft NB tumors in immunosuppressed mice demonstrated that DCL expression plays a role in NB proliferation (chapter 5). A significant delay in NB tumor growth was observed after DCL knockdown. These results also indicate that targeting DCL alone is not sufficient to fully inhibit tumor growth and that combined approaches need to be considered. In chapter 6 I discussed our findings and I proposed some explanations for the induction of apoptosis in NB after silencing DCL and DCLK-long. Future perspectives for the present study are also discussed. Overall, the data presented in this thesis suggest that MAPs encoded by the DCLK1 gene play an important role in NB growth. DCL and DCLK-long loss-offunction results in alterations in mitochondrial activity, reduced energy production, decrease in proliferation and induction of apoptotic cell death. Therefore, the general conclusion from the data described in this thesis is that DCLK1 is a potential target for NB therapy

6 Summary/ Samenvatting Samenvatting Het neuroblastoom (NB) is de meest voorkomende buiten de schedel gelegen vaste tumor bij kinderen in de leeftijd onder de vijf jaar. NB tumoren kunnen ontstaan op verschillende plaatsen in het sympatische zenuwstelsel en de bijnieren zijn de meest voorkomende primaire locaties met een prevalentie van circa 30 %. NB is zeer heterogeen voor wat betreft symptomen en uitkomst. De overlevingskans van patiënten met hoog-risico NB bedraagt minder dan 50 %. Daarnaast leidt conventionele therapie tot ernstige bijwerkingen. Er zijn daarom grote inspanningen geleverd om nieuwe moleculaire targets voor NB-therapie te vinden en te valideren. Het hoofddoel van dit proefschift is het onderzoeken van doublecortin-like kinase (DCLK1) als therapeutisch target voor NB. Hiertoe hebben we de expressieniveaus van doublecortin-like (DCL) en DCLK-long, twee microtubuligeassocieerde eiwitten (MAPs) die gecodeerd worden door het DCLK1 gen, in humane NBs onderzocht. Verder hebben we verschillende in vitro- en in vivostudies uitgevoerd om het effect te onderzoeken van verlies-van-functie van DCL/DCLK-long in NBs. Hoofdstuk 2 betreft een overzicht van moleculaire targets die zijn beschreven in de wetenschappelijke literatuur voor NB-therapie. Enkele targets zijn reeds in de klinische onderzoeksfase en met name een aantal die zijn voorgesteld maar de klinische fase nog niet hebben bereikt worden behandeld. Sommige van de targets die een rol spelen bij tenminste één van de tumorigene processen (proliferatie, anti-apoptose, angiogenese en/of uitzaaiing) zijn gedetailleerd beschreven, met name DCLK1. DCL/DCLK-long komen hoofdzakelijk tot expressie in neuroblasten en zijn betrokken bij stabilisatie van microtubuli, retrograde transport van de glucocorticoïde receptor, neuroblast proliferatie en migratie. We hebben gevonden dat DCL en DCLK-long veel hoger tot expressie worden gebracht in humane NBs en in glioma in vergelijking met andere vormen van kanker en met normaal weefsel (hoofdstuk 3). In dit hoofdstuk onderzochten we het effect van verlaging van DCL/DCLK-long-expressie op muizen en humane NB cellen. Genexpressieprofielen in NB-cellen met DCL/DCLK-long-verlaging suggereerden dat apoptotische processen worden geïnduceerd. Met behulp van verschillende technieken waaronder time-lapse imaging, caspase-3-activatie en fluorescentie-geactiveerde celsortering, hebben we bevestigd dat apoptose geïnduceerd wordt door uitschakeling van DCL en DCLK-long in NB-cellen. In hoofdstuk 4 toetsten we de hypothese dat knockdown van de microtubuli-stabiliserende DCL en DCLK-long ervoor zorgt dat NB-cellen gevoelig worden voor de microtubuli-destabiliserende vinca-alkaloïden (VAs). Onze resultaten toonden een sterk synergistisch apoptotisch effect, wat erop duidt dat er bij een combinatiebehandeling lagere doses VAs nodig zijn om NB-celsterfte te induceren, hetgeen mogelijk minder bijwerkingen suggereert. Echter, meer in vivoonderzoek is nodig voor validatie van de potentiële therapeutische betekenis van onze bevindingen. Dat het synergistisch apoptotische effect het gevolg is van de verschillende werkingen van VAs en DCL/DCLK-long knockdown op mitotische spoeldraden en mitochondriële activiteit (zie hoofdstuk 5), biedt nieuwe aanknopingpunten voor verder onderzoek

7 Chapter 7 Het onderzoek naar de koppeling tussen DCL en mitochondriële activiteit wordt beschreven in hoofdstuk 5. In humane NBs hebben we een sterke correlatie vastgesteld tussen DCL expressie en mitochondriaal-gerelateerde genen (hoofdstuk 3). In hoofdstuk 5 zagen we dat DCL co-localiseert met mitochondriën. Inductie van DCL-knockdown leidt tot een verminderde activiteit van cytochroom-coxidase en verminderde synthese van ATP. We vonden dat het DCL C-terminale serine/proline-rijke domein en het tweede microtubuli-bindingsdomein cruciaal zijn voor de regulering van cytochroom-c-oxidase-activiteit en ATP-synthese door DCL. Bovendien toonden we aan dat de verlaagde energiebeschikbaarheid als gevolg van het uitschakelen van DCL resulteerde in minder NB-celproliferatie. Voorgaand onderzoek heeft aangetoond dat het uitschakelen van DCL leidt tot verstoring van mitotische spoeldraden met celcyclus-arrest als gevolg, en in dit proefschrift tonen we aan dat de verlaging in NB-proliferatie ook het gevolg is van veranderingen in mitochondriële activiteit. In vivo-studies waarbij gebruik werd gemaakt van getransplanteerde NBtumoren in immunodeficiënte muizen lieten zien dat DCL-expressie een rol speelt bij NB-proliferatie (hoofdstuk 5). Een significante vertraging in NB-tumorgroei werd waargenomen wanneer verlaging van DCL expressie werd geïnduceerd. Deze resultaten wijzen er ook op dat het onderdrukken van DCL expressie alleen niet voldoende is om tumorgroei in zijn geheel te stoppen en dat combinatietherapiën dienen te worden overwogen. In hoofdstuk 6 bediscussieerde ik onze bevindingen en opperde ik een aantal verklaringen voor de inductie van apoptose in NB na het uitschakelen van DCL en DCLK-long. Een toekomstvisie met betrekking tot het huidige onderzoek werd ook beschreven. Samenvattend suggereren de in dit proefschrift beschreven gegevens dat MAPs gecodeerd door het DCLK1-gen een belangrijke rol spelen bij NB-groei. Verlies-van-functie van DCL en DCLK-long leidt tot veranderingen in mitochondriële activiteit, verminderde energieproductie, vermindering van proliferatie en inductie van apoptotische celsterfte. De conclusie van mijn onderzoek is daarom dat DCLK1 een potentiële target is voor NB-therapie. 206
