4 Summary Summary In the era of modern combination antiretroviral therapy (cart) the population of HIV-infected individuals is ageing. Along with increasing longevity, the risk for agerelated disease within this population increases. Studies have suggested that the risk for several age-related conditions may be higher in HIV-infected individuals compared to HIV-uninfected individuals of similar age. Age-related disease in HIV-infected patients is multifactorial: exposure to untreated HIV infection, inflammation and immune activation, toxicity of antiretroviral drugs, behavioural factors, and genetics may all contribute. However, the extent of the contribution of each of these factors to individual conditions is unclear. In this thesis a variety of age-related conditions are examined within the AGE h IV Cohort, a cohort of ageing HIV-infected individuals, predominantly on long-term cart with suppressed viraemia, and HIV-uninfected controls of similar age and socio-demographic background. Due to the inclusion of a highly comparable control group, the AGE h IV Cohort Study allows us to better distinguish the contribution of HIV infection and ART exposure from socio-demographic and behavioural factors. The collection of detailed information on HIV-disease history, ART exposure and behaviour, as well as on clinical conditions, organ function, markers of body composition and inflammation provides insight into the relative contribution of each of these factors concerning the pathophysiology underlying each of the individual age-related conditions which we studied. Osteoporosis and osteopenia are common in HIV-infected populations and associated with an increased risk of fracture. Inflammation associated with HIV viral replication and exposure to certain antiretroviral drugs are among the hypothesized contributing factors. In chapter two a cross-sectional comparison of bone mineral density (BMD) was performed. We found a higher prevalence of both osteoporosis and osteopenia in HIV-infected individuals compared with HIV-uninfected controls, thereby confirming previous studies. However, lower BMD observed in the HIV-infected group as a whole could be explained by a higher lifetime exposure to cigarette smoking and a lower average body weight, which are both known traditional risk factors for low BMD. The relation between HIV infection, body weight, and BMD is complex; low body weight may in part be a consequence of HIV-disease related weight loss. Those individuals with a history of severe HIV disease, which may be accompanied by weight loss and sarcopenia, were more likely to have a low BMD. We found a strikingly low BMD in younger men who have sex with men, regardless of HIV-status, which may be due to as yet unidentified behavioural factors. A 175

5 Summary HIV-infected individuals may be at increased risk for developing chronic kidney disease (CKD). Certain antiretroviral drugs are known for their nephrotoxic effects, particularly tenofovir disoproxil fumarate (TDF) which is associated with increased risk for proximal renal tubular dysfunction (PRTD) and CKD. In chapter three we examined the prevalence and progression of renal disease in HIV-infected cohort participants, many of whom were currently or historically exposed to TDF, and HIV-uninfected cohort participants. HIV-infected individuals exhibited a higher prevalence of PRTD and of reduced renal function and albuminuria, indicative of CKD. In addition, HIV-infected individuals on stable cart demonstrated greater progression of renal function decline and albuminuria during a follow-up of up to 4 years. To a large extent this may be explained by the high frequency of TDF use in the studied population. Exposure to atazanavir and historic immunodeficiency were associated with progression of renal function decline; these factors may contribute to CKD in HIV-infected patients. Treated HIV infected individuals are at increased risk of cardiovascular disease (CVD); ongoing inflammation and immune activation are each thought to contribute. In chapter four we studied aortic stiffness, assessed by aortic pulse wave velocity, as a pre-clinical marker of CVD. We found a higher aortic stiffness in HIV-infected compared to uninfected individuals. However, the difference in aortic stiffness was relatively small and could largely be explained by a higher lifetime exposure to cigarette smoking in the HIV-infected study participants. The subgroup of HIV-infected individuals with a history of advanced immunodeficiency had a higher aortic stiffness; this was most pronounced in those individuals with a nadir CD4 count below 100 cells/mm 3. Higher levels of high-sensitivity C-reactive protein (hscrp) were independently associated with a higher aortic stiffness in HIV-infected as well as uninfected individuals and attenuated the association between the nadir CD4 count and aortic stiffness. This suggests that ongoing inflammation in the subgroup of HIV-infected individuals with a history of advanced immunodeficiency may be involved in aortic stiffening. HIV infection is thought to accelerate liver fibrosis in individuals coinfected with viral hepatitis, possibly mediated by immune activation. Whether HIV infection in the absence of viral hepatitis coinfection also contributes to liver fibrosis is unclear. In chapter five we assessed the Fibrosis-4 (FIB-4) score in HIV-infected and uninfected study participants without chronic active hepatitis B or C virus infection. We found higher FIB-4 levels in the HIV-infected group, indicative of more liver fibrosis, though the difference was very small. Furthermore, the proportion of participants with FIB-4 scores indicative of advanced liver fibrosis was low in either group. Factors associated with higher FIB-4 scores in HIV-infected individuals included prior exposure to didanosine and current and historic immunodeficiency. Higher levels of soluble (s)cd163, a marker of monocyte 176

6 Summary activation, were associated with higher FIB-4 scores in HIV-infected individuals but not uninfected individuals. Similarly, within a subset of virologically suppressed HIVinfected individuals and HIV-uninfected controls, activated CD8 + T-lymphocytes and regulatory T-lymphocytes were associated with higher FIB-4 scores in HIV-infected but not uninfected individuals. These results suggest a role for immune activation in the pathophysiology of liver fibrosis in HIV monoinfection. Several studies have shown a higher prevalence of frailty a state of vulnerability to adverse outcomes, associated with ageing in HIV-infected populations. Inflammation is thought to contribute to the pathophysiology of frailty in the general population, and may also mediate the increased risk of frailty in HIV-infected individuals. In chapter six we reported on the assessments of the frailty phenotype in HIV-infected and uninfected study participants. Treated HIV infection was independently associated with a higher likelihood of frailty and the intermediate state prefrailty. Levels of biomarkers of inflammation were not independently associated with (pre)frailty. Instead, our findings suggest that particularly those with a history of severe HIV-related weight loss, potentially in the context of an HIV-associated wasting syndrome, may be at increased risk of (pre) frailty. In addition, a higher waist-to-hip ratio was identified as a risk factor for prefrailty and frailty. This suggests that abdominal obesity, and in HIV-infected individuals also peripheral weight loss due to lipoatrophy, may also contribute to the onset of frailty. Cigarette smoking is common in HIV-infected populations and importantly contributes to the development of age-related disease. Some studies have suggested that the risk associated with cigarette smoking may be greater in HIV-infected individuals compared with uninfected individuals and that cigarette smoking and HIV infection may have synergistic effects on the immune system. In chapter seven we explored whether the association between cigarette smoking and levels of soluble markers of inflammation (hscrp), immune activation (soluble (s)cd14 and scd163) and coagulation (D-dimer) was different in HIV-infected compared with uninfected individuals. Our results suggest a significant contribution of cigarette smoking to inflammation and coagulation activation observed in treated HIV-infected individuals as indicated by independent associations with higher levels of hscrp, scd14, and D-dimer. We did however not find evidence of a differential effect of cigarette smoking at the level of the immune system in HIV-infected compared to uninfected individuals. A Throughout the research presented in this thesis, we have shown a higher prevalence of a variety of age-related conditions in treated HIV-infected individuals compared to uninfected controls from a similar behavioural and socio-demographic background. 7 Summary ated, be explained by factors unrelated to HIV infection and/or cart exposure. Many traditional and modifiable risk factors, such as cigarette smoking, hypertension, and dyslipidaemia are prevalent in HIV-infected populations; appropriately addressing them will help to decrease the burden of age-related comorbidity. Furthermore, the risk for age-related conditions is not evenly distributed among the HIV-infected population; those having experienced more advanced prior HIV disease, accompanied by immunodeficiency and/or weight loss, appear to be at particularly increased risk. Ongoing efforts to diagnose and treat HIV-infected patients early and the continued development of increasingly better tolerated antiretroviral drugs, may also significantly decrease the risk for age-related comorbidity in the HIV-infected population. 178

8 Nederlandse samenvatting NEDERLANDSE Samenvatting de beschikbaarheid van combinatie antiretrovirale therapie (cart) heeft geleid tot een toegenomen levensverwachting van hiv-patiënten. Als gevolg hiervan stijgt de gemiddelde leeftijd van de hiv-populatie; dit gaat gepaard met een toenemend risico op ouderdomsgerelateerde comorbiditeit. Er zijn aanwijzingen dat hiv- patiënten een hoger risico hebben op bepaalde ouderdomsgerelateerde aandoeningen dan leeftijdsgenoten zonder hiv. Ouderdomsgerelateerde ziekte bij hiv-patiënten is complex; blootstelling aan replicerend hiv, chronische ontsteking en immuunactivatie, toxiciteit van antiretrovirale middelen, gedragsmatige risicofactoren en genetische predispositie zijn allen mogelijk bijdragende factoren. De mate waarin elk van deze factoren bijdraagt aan de individuele aandoeningen is echter niet geheel duidelijk. In dit proefschrift bespreken wij de resultaten van studies uitgevoerd binnen de AGE h IV Cohort Studie. Wij hebben de prevalentie (het vóórkomen), de progressie en determinanten van verschillende ouderdomsgerelateerde aandoeningen bestudeerd. Het AGE h IV Cohort volgt ouder wordende hiv-patiënten van middelbare en hogere leeftijd en leeftijdsgenoten met een vergelijkbare socio-demografische achtergrond zonder hiv. Het grootste deel van de deelnemende hiv-patiënten wordt al langere tijd effectief behandeld met cart. De inclusie van een zeer vergelijkbare hiv-negatieve controlegroep maakt de AGE h IV Cohort Studie zeer geschikt om de verschillende potentiële determinanten van ouderdomsgerelateerde comorbiditeit te kunnen onderscheiden. Wij hebben van alle cohortdeelnemers uitgebreide informatie verzameld over doorgemaakte hiv-gerelateerde ziekten, blootstelling aan cart en (risico)gedrag. Ook hebben wij gedetailleerde gegevens betreffende klinische aandoeningen, orgaan (dis)functie, markers van lichaamsbouw en ontstekingswaarden verzameld. Hierdoor konden wij meer inzicht krijgen in de relatieve bijdrage van elk van deze factoren in de pathofysiologie van de bestudeerde ouderdomsgerelateerde aandoeningen. Osteoporose en osteopenia komen veel voor in hiv-geïnfecteerde populaties en verhogen het risico op botbreuken. Het vrijelijk repliceren van hiv, wat aanleiding kan geven tot een chronische systemische ontstekingsreactie, en blootstelling aan bepaalde antiretrovirale middelen dragen hieraan mogelijk bij. In hoofdstuk 2 beschrijven we een cross-sectionele vergelijking van botmineraaldichtheid (BMD) in onze cohort deelnemers. Zowel osteoporose als osteopenie kwamen vaker voor in hiv-positieve vergeleken met hiv-negatieve studiedeelnemers; deze resultaten komen overeen met bevindingen van eerdere studies. De lagere BMD in de hiv-positieve groep als geheel kon worden verklaard door een gemiddeld hogere blootstelling aan sigaretten (pakjaren) en een gemiddeld lager lichaamsgewicht. De relatie tussen hiv-infectie, lichaamsgewicht en A 179

9 Nederlandse samenvatting BMD is ingewikkeld; een laag lichaamsgewicht zou deels een gevolg kunnen zijn van doorgemaakte hiv-gerelateerde ziekte. Hiv-patiënten die in het verleden ernstige (complicaties van) hiv-gerelateerde ziekte hadden doorgemaakt, vaak gepaard gaand met gewichtsverlies, hadden vaker een lage BMD. Verder vonden we een opvallende lage BMD in relatief jonge homo- en biseksuele mannen, onafhankelijk van hun hiv-status; dit is mogelijk het gevolg van ongemeten gedragsmatige factoren. Mensen met hiv hebben mogelijk een verhoogd risico op chronische nierziekten. Een aantal antiretrovirale middelen heeft nefrotoxische effecten, waarbij met name het veelgebruikte middel tenofovir disoproxil fumarate (TDF) is geassocieerd met een verhoogd risico op disfunctie van de proximale renale tubulus (PRTD) en chronische nierziekten. In hoofdstuk 3 vergeleken we de prevalentie en de progressie van nierziekte in hivpositieve cohortstudiedeelnemers, van wie een groot deel is blootgesteld aan TDF, en hiv-negatieve controles. Hiv-positieven vertoonden vaker aanwijzingen voor PRTD en hadden vaker een afgenomen nierfunctie en albuminurie, beiden aanwijzingen voor chronische nierziekte. Gedurende een periode van maximaal 4 jaar was de kans op snelle afname van de nierfunctie en toenemende albuminurie groter bij hiv-positieven op een stabiel cart regime dan bij hiv-negatieve controles. Waarschijnlijk kan dit grotendeels verklaard worden doordat een groot deel van de hiv-positieve studiedeelnemers TDF gebruikt of heeft gebruikt. We vonden ook een associatie tussen zowel het gebruik van atazanavir als historische immuundeficientie en een snelle afname van de nierfunctie; deze factoren dragen derhalve mogelijk ook bij aan chronische nierziekte in hiv-patiënten. Het risico op het ontwikkelen van hart- en vaatziekten (HVZ) is verhoogd bij mensen met hiv; ontstekingsprocessen en activatie van het immuunsysteem dragen waarschijnlijk bij aan dit verhoogde risico. In hoofdstuk 4 onderzochten we aortavaatstijfheid door het meten van de aorta polsgolfsnelheid. Hiv-positieven hadden een gemiddeld hogere aortavaatstijfheid dan hiv-negatieven, een aanwijzing voor een verhoogd risico op HVZ. Het absolute verschil in vaatstijfheid was echter klein en kon grotendeels verklaard worden door een groter aantal pakjaren sigaretten in de hiv-positieve groep. De subgroep binnen de hiv-populatie die in het verleden ernstige immuundeficiëntie had doorgemaakt had een hogere aortavaatstijfheid; dit was het meest duidelijk in diegenen met een laagst historisch (nadir) CD4 getal van minder dan 100 cellen/mm 3. Een hogere bloedwaarde van de ontstekingsmarker high-sensitivity C-reactive protein (hscrp) was onafhankelijk geassocieerd met een hogere aortavaatstijfheid, zowel in de hiv-positieve als in de hiv-negatieve groep. Door het toevoegen van hscrp aan het multivariabele model, werd de associatie tussen de nadir CD4 en aortavaatstijfheid 180

10 Nederlandse samenvatting minder sterk; mogelijk dragen derhalve voortdurende ontstekingsprocessen bij aan de verhoogde aortavaatstijfheid in diegenen met een laag nadir CD4 getal. Hiv-infectie kan verbindweefseling (fibrose) van de lever versnellen in mensen met virale hepatitis, een proces waarbij mogelijk immuunactivatie een rol speelt. Of hiv-infectie ook in mensen zonder virale hepatitis infectie de kans op leverfibrose verhoogt is onduidelijk. In hoofdstuk 5 bestudeerden we de Fibrosis-4 (FIB-4) score in hiv-positieve en hiv-negatieve studiedeelnemers zonder actieve of doorgemaakte chronische hepatitis B of C virus infectie. De gemiddelde FIB-4 score in de hiv-positieve groep was hoger een aanwijzing voor meer lever fibrose, maar het verschil tussen de groepen was klein. Daarnaast was slechts in een klein percentage van hiv-positieve en hiv-negatieve deelnemers de FIB-4 score suggestief voor gevorderde lever fibrose. Zowel historische blootstelling aan het vroeger veelgebruikte antiretrovirale middel didanosine, als huidige en in het verleden doorgemaakte immuundeficiëntie waren geassocieerd met hogere FIB-4 scores. Een hogere waarde van soluble CD163 (scd163), een maat voor monocyten activatie, was onafhankelijk geassocieerd met een hogere FIB-4 score in hiv-positieve, maar niet in hiv-negatieve studiedeelnemers. Daarnaast vonden we een associatie tussen het percentages geactiveerde CD8 T-lymfocyten en regulatoire T-lymfocyten en een hogere FIB-4 score in hiv-positieve maar niet in hiv-negatieve mensen. Deze associaties suggereren dat immuunactivatie ook een rol speelt in de pathofysiologie van leverfibrose bij mensen met een hiv-infectie, maar zonder chronische virale hepatitis. Uit de resultaten van verschillende studies blijkt een hogere prevalentie van frailty een toestand van verhoogde kwetsbaarheid voor nadelige gezondheidsuitkomsten, geassocieerd met veroudering in hiv-positieve populaties. Chronische ontstekingsprocessen dragen mogelijk bij aan de pathofysiologie van frailty in de algemene bevolking, en spelen wellicht ook een mediërende rol in het verhoogde risico op frailty in mensen met hiv. In hoofdstuk 6 onderzochten wij de prevalentie van het frailty-fenotype in hiv-positieve en hiv-negatieve studiedeelnemers. Behandelde hiv-infectie was onafhankelijk geassocieerd met een grotere kans op het hebben van frailty en het voorstadium prefrailty. Wij vonden geen associaties tussen verschillende onderzochte maten van ontsteking en (pre)frailty. Zowel een lager historisch als een huidig lager lichaamsgewicht, het eerste mogelijk een gevolg van gewichtsverlies door hiv-ziekte en het hiv-geassocieerde wasting-syndroom, waren beiden onafhankelijk geassocieerd met een hogere kans op (pre)frailty. Daarnaast was een hogere taille-heup verhouding een onafhankelijke determinant van prefrailty en frailty. Dit suggereert dat abdominale obesitas, en in hiv-geïnfecteerden mogelijk ook perifeer vetverlies als gevolg van lipoatrofie, zouden kunnen bijdragen aan het ontstaan van frailty. A 181

11 Nederlandse samenvatting Het roken van sigaretten komt veel voor in hiv-positieve populaties en draagt in belangrijke mate bij aan het ontstaan van ouderdomsgerelateerde ziekten. Sommige studies suggereren dat het roken van sigaretten een groter risico met zich mee brengt bij mensen met hiv dan mensen zonder hiv, en dat de effecten van sigaretten en hiv synergistisch inwerken op het immuunsysteem. In hoofdstuk 7 onderzochten wij of de relatie tussen het roken van sigaretten en diverse maten van ontsteking (hscrp), immuunactivatie (soluble (s)cd14 en scd163) en stolling (D-dimer) verschilde tussen mensen met en zonder hiv. Onze resultaten suggereren dat roken in belangrijke mate bijdraagt aan ontsteking en stollingsactivatie; het roken van sigaretten was onafhankelijk geassocieerd met hogere waarden van hscrp, scd14 en D-dimer. We vonden echter geen aanwijzingen voor een sterker effect van het roken van sigaretten op het immuunsysteem in hiv-positieve dan in hiv-negatieve mensen. In de onderzoeken gepresenteerd in dit proefschrift hebben wij laten zien dat een aantal uiteenlopende ouderdomsgerelateerde aandoeningen vaker voorkomt bij hiv-patiënten in vergelijking met hiv-negatieven controles met een vergelijkbare gedragsmatige en socio-demografische achtergrond. De hogere prevalentie van deze aandoeningen kon voor een groot deel en in grotere mate dan voorheen gedacht werd, verklaard worden door factoren anders dan het hebben van een hiv-infectie en/of blootstelling aan cart. Verschillende traditionele en modificeerbare risicofactoren, zoals het roken van sigaretten, hypertensie en dyslipidemie, komen vaker voor in hiv-geïnfecteerde populaties; een gerichte aanpak van deze factoren draagt bij aan preventie en behandeling van ouderdomsgerelateerde comorbiditeit. Tevens vonden wij dat het risico op ouderdomsgerelateerde aandoeningen niet gelijk is voor alle hiv-geïnfecteerden; met name diegenen die ernstiger hiv-gerelateerde ziekten hebben doorgemaakt, gepaard gaande met ernstiger immuundeficiëntie en/of gewichtsverlies, lopen een hoger risico. Beleid gericht op een snelle diagnose en behandeling van mensen die hiv-geïnfecteerd zijn, evenals de voortdurende ontwikkeling van neiuwe antiretrovirale middelen met minder bijwerkingen, zouden ook het risico op leeftijdsgerelateerde comorbiditeit in de hiv-geïnfecteerde populatie in belangrijke mate kunnen helpen verminderen. 182

26 Biography biography Katherine Kooij was born in 1984 in Alkmaar, The Netherlands. After graduating from the Murmellius Gymnasium in Alkmaar, she moved to Amsterdam to study Medicine at the University of Amsterdam in During her studies she developed an interest in infectious diseases and public health. She was active in the International Federation of Medical Student s Associations Amsterdam. Intrigued by the scientific method and its application in the medical sciences, she pursued a minor in philosophy of science and ethics. Specifically drawn to patterns of health and disease in the developing world, Katherine began to study the Russian language at the University of Amsterdam; she later spent her research internship at the D.I. Ivanovsky Institute of Virology in Moscow, studying the epidemiology of HIV and hepatitis C coinfection in the Russian Federation. In the final year of her medical studies, she followed an elective rotation in Tropical Medicine at St. Luke s hospital in Malosa, Malawi. After graduating as a Medical Doctor in 2010, she worked as a resident of Internal Medicine in the Flevoziekenhuis in Almere, and as a resident in Psychiatry at Rivierduinen in Oegstgeest. Katherine began her Doctoral studies in 2012 at the Amsterdam Institute for Global Health and Development. She participated in coordinating the AGE h IV Cohort Study and investigating a range of age-related comorbidity in ageing patients with HIV. Following her PhD Katherine intends to continue her contributions in the domain of public health and to conduct research in the field of epidemiology with an emphasis on the interaction between clinical research and public health.
