3 Summary Pancreatic cancer has one of the worst prognosis with an overall mean 5-year survival of 7%. The only curative treatment remains surgery, but this treatment is often hampered by a difficult anatomical tumor location relative to the surrounding major blood vessels. The addition of adjuvant (chemo) radiation treatment may be beneficial and may increase the chance on a complete surgical resection. A major geometric uncertainty during radiation delivery is the respiratory-induced motion. This intrafractional motion results in a blurring of the dose distribution with respect to the tumor. The resulting dose distribution can have a reduced target coverage or higher dose values to surrounding organs at risk (OARs). To ensure sufficient target coverage, often a motion encompassing technique is employed in the form of an internal target volume (ITV). When using an ITV, the planning target volume (PTV) is increased so that the tumor is constantly irradiated and this results in large volumes that receive a high dose. Other, more sophisticated, techniques to account for the respiratory-induced tumor motion could also be used, and these techniques might only minimally increase the PTV while maintaining sufficient target coverage. Chapter 2 describes our retrospective study in which we measured the respiratory-induced pancreatic tumor motion in a group of 18 patients. We measured the motion during the single pretreatment 4DCT scan that was used to create the ITV and we also measured the motion during each of the daily cone-beam CT (CBCT) scans for each patient. Each patient was implanted with 2 4 intratumoral gold fiducials, which were placed using an endoscopic ultrasound-guided procedure. These fiducials were visible on the 4DCT and CBCT images and could be used as surrogates for the tumor position. The tumor motion on 4DCT was obtained by using image registration between the respiratory phase scans of the 4DCT, based on the fiducials. To obtain the tumor motion during the CBCT acquisition we retrospectively extracted the respiratory signal from the CBCT projection images, sorted the projection images into respiratory bins and made respiratory correlated CBCT reconstructions. For each patient we compared the motion during the single pre-treatment 4DCT scans to the distribution of motion magnitudes obtained from the daily CBCTs. In 13 out of 18 patients we observed a significant difference between the motion magnitude during the 4DCT and the mean motion magnitude during the daily CBCTs. In addition, we observed substantial interfractional motion magnitude variations. The overall pre-treatment motion magnitude was significantly larger than the daily motion magnitude. So, the single pre-treatment 4DCT is not representative for daily tumor motion. In chapter 3 we show that using a mid-ventilation approach instead of an ITV to account for the respiratory-induced tumor motion resulted in smaller PTVs and therefore less dose to the surrounding OARs. When using mid-ventilation, the respiratory-induced motion is regarded as random motion around the mid-ventilation tumor position (i.e. the mean tumor position during respiration). This random motion can then be incorporated in the margin recipe and this results in a target volume expansion that is less than when the full motion magnitude is used as safety margin. The mid-ventilation approach was developed for lung tumors and there, due to the broader beam penumbra, the reduction in PTV can be substantial. In the upper-abdominal region the gain was smaller, but we showed a significant PTV reduction and concurrent reduction of dose to the OARs while retaining good target coverage. 164

4 Summary Chapter 4 describes the use of a probabilistic treatment planning approach to account for the respiratory-induced tumor motion during treatment delivery. This technique requires a modification of the treatment plan optimization process that allows the user to prospectively incorporate the expected respiratory-induced tumor motion in the treatment plan. A probability density function describing the tumor motion can be given as input to the treatment planning system. We showed that this approach resulted in treatment plans that were robust, in terms of target coverage, against clinically expected variations of the respiratory amplitude. For a group of 18 patients we showed that when the respiratory motion amplitude ranged from 5 50 mm (in the inferior-superior direction only) the probabilistic treatment planning approach resulted in significantly steeper dose gradients around the PTV when compared to the use of an ITV. We also investigated the potential clinical benefit of the introduced probabilistic approach. This was done by determining the dosimetric differences for the duodenum when comparing the probabilistic approach to the use of an ITV when using the clinical 3D respiratory motion amplitudes observed in the 4DCT of these 18 patients. We observed that probabilistic planning only showed a limited clinical benefit and the dosimetric differences were only clinically relevant for sufficiently large respiratory amplitudes. Both mid-ventilation and probabilistic planning are still dependent on the respiratory-induced motion magnitude measured on the single pre-treatment 4DCT. Since we showed that the 4DCT is not representative for daily tumor motion, motion reduction might be the better option. A approach to eliminate the respiratory-induced motion would be to use breath-holding. However, in chapter 5 we showed in a group of 12 pancreatic cancer patients that there are substantial residual uncertainties in tumor position during and between consecutive breath-holds. All included patients were asked to perform three 30-second inhalation breath-holds, at a preset lung volume, after three treatment fractions. Two patients were unable to perform the breath-holds and quit the study before participating in any measurements, we obtained a total of 76 successful breathhold measurements. During breath-holding we measured the airflow through the mouth using a spirometer and we made lateral fluoroscopic movies to visualize the intratumoral fiducials during breath-holding from which we could derive the tumor position during breath-holding. We observed substantial tumor drifts during breath-holding, predominantly in the superior direction, without registering any airflow. We also observed considerable tumor position variations between consecutive breath-holds. Both observed uncertainties would have to be accounted for when using breath-holding during radiotherapy or else the treatment efficacy could be reduced. The breath-holding protocol that was used in the patient study was adapted from the breast cancer treatment protocol from our department and was thus not yet optimized for tumor position reproducibility. In chapter 6, we investigated the effect of breath-holding at different lung volumes on the organ motion during breath-holding. We included 16 healthy subjects who were asked to perform second inhalation breath-holds (six with fully inflated lungs and six with a lung volume of 70%) and second exhalation breath-holds (six with fully deflated lungs and six with a lung volume of 30%). All breath-holds were performed while the subject was placed within a 3T-MRI scanner and we used a fast sequence (one image every 0.6 s) to visualize and quantify the 165

5 Summary organ (pancreatic head and diaphragm) motion. We observed significantly smaller organ motion magnitudes during the exhalation breath-holds compared with the inhalation breath-holds. We also found that the motion was most pronounced at the start of the breath-hold (i.e. during the first 10 s of breath-holding). The velocity of the organ motion was significantly larger during these first 10 s compared with the remainder of the breath-hold and this was the case for all four lung volumes. Delaying the treatment or imaging in order to exclude the start of the breath-hold could potentially improve the tumor position accuracy. However, in this study, we did not take the inter-breath-hold tumor position variation into account. Respiratory-induced pancreatic tumor motion is still an unresolved problem in radiotherapy. Using a mid-ventilation or probabilistic planning technique reduces the target volume and thus reduces the dose to surrounding healthy tissues. Since the daily tumor motion cannot accurately be predicted by a single pre-treatment measurement, motion reduction techniques such as breathholding might be more beneficial. However, we observed substantial residual tumor motion during breath-holding and considerable position variations between consecutive breath-holds. If breathholding is used we recommend to use exhalation breath-holding since this resulted in the least organ motion. 166

9 Samenvatting Patiënten met alvleesklierkanker hebben een zeer slechte prognose met een gemiddelde vijfjaarsoverleving van 7%. De enige behandeling die een kans geeft op volledige genezing is chirurgie. De potentiële tumor resectie wordt bemoeilijkt wanneer de tumor dicht bij de grote bloedvaten ligt die zich bij de alvleesklier bevinden of wanneer de tumor ingegroeid is in deze bloedvaten. Het toevoegen van radio(chemo)therapie voor deze patiënten kan van grote toegevoegde waarde zijn aangezien het de kans op een succesvolle resectie kan vergroten. Een grote onzekerheid tijdens de radiotherapie behandeling komt door de ademhalingsbeweging van de tumor aangezien het de geplande dosis distributie ten opzichte van de tumor kan verslechteren. Hierdoor kan de effectiviteit van de behandeling verminderen. Een manier om dit te voorkomen is door de het bestralingsgebied te vergroten zodat de tumor gedurende de gehele ademhalingscyclus toch bestraald wordt, het resulterende de doelgebied wordt ook wel het internal target volume of ITV genoemd. Deze betrekkelijk conservatieve techniek zou vervangen moeten worden door meer verfijnde technieken. Hoofdstuk 2 beschrijft onze retrospectieve studie waarin we de grootte van de ademhalingsbeweging van de alvleeskliertumor hebben gemeten in een groep van 18 patiënten. We hebben voor iedere patiënt de beweging gemeten tijdens de 4DCT scan die gemaakt is ten behoefte van het opstellen van het behandelplan en tijdens de dagelijkse cone-beam CT (CBCT) scans die gemaakt zijn vlak voor de dagelijkse behandelingen. De tumor was niet altijd zichtbaar op de dagelijkse scans, maar alle geïncludeerde patiënten hadden kleine goudmarkers in de tumor die wel zichtbaar waren en die gebruikt konden worden als surrogaat voor de tumorpositie. Het doel van de studie was om te onderzoeken of de beweging die gebruikt wordt om het ITV te maken wel representatief is voor de dagelijkse tumor beweging. De tumorbeweging tijdens de 4DCT scan werd verkregen door de positie van de markers op de verschillende CT scans behorende bij de verschillende ademhalingsfases (inademing tot en met uitademing) te vergelijken. De dagelijkse tumorbeweging tijdens de CBCT scans werd op een vergelijkbare manier verkregen, maar hier moesten we wel eerst handmatig de CBCT projectiebeelden onderverdelen in de verschillende ademhalingsfases. We vonden een significant verschil tussen de beweging tijdens de 4DCT en tijdens de dagelijkse CBCT scans in 13 van de 18 patiënten en in de meeste gevallen was de beweging tijdens de 4DCT groter. Ook vonden we grote verschillen tussen de dagelijkse grootte van de ademhalingsbeweging. De beweging die gebruikt wordt om grootte van het ITV te bepalen is dus niet representatief voor de rest van de behandeling. In hoofdstuk 3 laten we zien dat het gebruik van een mid-ventilatie techniek resulteert in significant kleinere doelgebieden, wat weer resulteert in minder dosis op de omliggende gezonde weefsels. Bij mid-ventilatie wordt de ademhalingsbeweging beschouwd als een willekeurige beweging rondom de gemiddelde tumorpositie. De beweging kan vervolgens meegenomen worden in het veiligheidsmarge recept en dit resulteert vervolgens in een doelgebied dat kleiner is dan wanneer het ITV gebruikt wordt. De mid-ventilatie techniek was oorspronkelijk ontwikkeld voor longtumoren waar door de brede penumbra, typisch voor longweefsel, dit resulteerde in substantiële reducties van het doelgebied. In het abdomen was de winst van de mid-ventilatie techniek minder substantieel, maar leidde nog wel degelijk tot kleinere doelgebieden en minder dosis in de omliggende gezonde weefsels. 170

10 Samenvatting Hoofdstuk 4 beschrijft het gebruik van een probabilistische planningstechniek welke gebruikt kan worden om de te verwachten ademhalingsbeweging mee te nemen in het optimalisatieproces van het bestralingsplan. De ademhalingsbeweging moet worden beschreven door middel van een functie die de waarschijnlijkheidsdichtheid per tumorpositie geeft en deze functie wordt vervolgens gebruikt in de optimalisatie van het bestralingsplan. In deze studie tonen we aan dat deze techniek klinisch acceptabele plannen oplevert die robuust zijn tegen de dagelijkse variatie in de ademhalingsbeweging. We hebben dit laten zien voor een groep van 18 patiënten en ademhalingsbewegingen van 5 50 mm (alleen in de craniaal-caudale richting). De resulterende bestralingsplannen hadden een significant steilere dosis gradiënt rondom het doelgebied vergeleken met wanneer het ITV gebruikt werd. Om het klinische voordeel van deze probabilistische techniek te onderzoeken hebben we voor dezelfde 18 patiënten probabilistische bestralingsplannen gemaakt gebaseerd op de ademhalingsbeweging die gemeten was tijdens de 4DCT van de patiënt. Vervolgens hebben we en aantal dosimetrische waardes voor het duodenum van deze plannen vergeleken met wanneer een ITV gebruikt zou zijn. Het klinische voordeel van de probabilistische planningstechniek was betrekkelijk klein en was sterk afhankelijk van de grootte van de ademhalingsbeweging, pas als de tumor bewegingsamplitude groot genoeg was werd de probabilistische methode relevant. Zowel de mid-ventilatie als de probabilistische techniek zijn afhankelijk van de ademhalingsbeweging die verkregen wordt uit de 4DCT. Aangezien we hebben aangetoond dat deze 4DCT niet representatief is voor de dagelijkse tumor beweging is het misschien beter om de beweging te onderdrukken dan het simpelweg mee te nemen in het bestralingsplan. De ademhalingsbeweging kan onderdrukt worden door de patiënt te vragen zijn of haar adem vast te houden tijdens de bestraling. Over het algemeen wordt er aangenomen dat tijdens het vasthouden van de adem alle organen vrijwel stilstaan. Hoofdstuk 5 beschrijft de resultaten van onze studie waarin we een groep van 12 alvleesklierkankerpatiënten gevraagd hebben hun adem voor 30 seconden vast te houden. Zij deden dit met een longvolume van ongeveer 70% van hun respectievelijke totale longcapaciteit. Elke patiënt werd gevraagd dit drie keer te doen per meting en op drie verschillende dagen. Twee patiënten waren niet in staat om hun adem vast te houden en stopten met de studie en dit resulteerde in 76 succesvolle pogingen om de adem vast te houden door de andere 10 patiënten. Tijdens het vasthouden van de adem was de neus afgesloten en werd de eventuele ontsnapte lucht door de mond gemeten, ook werden laterale fluoroscopie (ook wel doorlichting genoemd) opnames gemaakt. Deze opnames konden vervolgens, met behulp van de goudmarkers in de tumor, gebruikt worden om de tumorpositie tijdens het vasthouden van de adem te bepalen. We observeerden dat in bijna alle gevallen de tumor continue in beweging was, deze beweging was vooral zichtbaar als een drift in de richting van het diafragma en kon niet verklaard worden door het ontsnappen van lucht. Ook was er een grote variatie in de beginpositie van de tumor wanneer de adem meerdere malen achter elkaar vastgehouden werd. Wanneer de adem wordt vastgehouden tijdens de behandeling moet er dus rekening gehouden worden met de gevonden onzekerheden in tumorpositie. Het studieprotocol dat gebruikt was voor de patiëntenstudie was afgeleid van het protocol dat gebruikt wordt om borstkankerpatiënten hun adem te laten vasthouden tijdens de bestraling. Dit 171

11 Samenvatting protocol was dus nog niet geoptimaliseerd voor een zo reproduceerbaar mogelijke tumorpositie. In hoofdstuk 6 wordt een studie beschreven waarin we het effect van het vasthouden van de adem met verschillende longvolumes op de orgaanbeweging onderzochten. We vroegen 16 gezonde proefpersonen om 12 keer 60 seconden lang hun adem vast te houden na een inademing (6 keer met volledig volle longen en 6 keer met een longvolume van ongeveer 70%). Ook vroegen we de proefpersonen om 12 keer 60 seconden lang hun adem vast te houden na een uitademing (6 keer met volledig lege longen en 6 keer met een longvolume van ongeveer 30%). De proefpersoon hield de adem vast terwijl hij of zij in een 3T-MRI scanner lag. We visualiseerden de beweging van de alvleesklier en van het diafragma door elke 0.6 seconde een MRI beeld te maken. We observeerden significant minder beweging gedurende het vasthouden van de adem na een uitademing dan na een inademing. Ook constateerden we dat de beweging meer geprononceerd was tijdens de eerste 10 seconden van het vasthouden van de adem, ongeacht het longvolume. De ademhalingsbeweging van alvleeskliertumoren is nog steeds een onopgelost probleem binnen de radiotherapie en zorgt dus voor een onzekerheid in de tumorpositie tijdens de behandeling. Door gebruik te maken van een mid-ventilatie techniek of van een probabilistische planningstechniek kan het doelgebied verkleind worden en de dosis op de omliggende weefsels worden verminderd. Aangezien we hebben aangetoond dat de ademhalingsbeweging tijdens de 4DCT niet representatief is voor de beweging gedurende de rest van de behandeling, is het reduceren van de beweging waarschijnlijk een betere optie. Hiervoor kan de patiënt gevraagd worden om de adem vast te houden. Als deze techniek wordt toegepast moet er wel rekening gehouden worden met de tumorbeweging tijdens het vasthouden van de adem en om dit te minimaliseren raden wij aan om de adem vast te houden na een uitademing. 172
