2 Summary Summary In this thesis red fluorescent dental plaque (RFP) and its applicability to indicate oral microbial diseases was studied, using in vitro (part one) and in vivo (part two) approaches. The in vitro part starts in chapter 2 with the assessment of red fluorescence from bacterial species, which are related to dental caries and periodontitis. The bacteria were grown anaerobically in the presence of different metalloporphyrins. Red fluorescence from bacteria was assessed using a fluorescence camera (λ ex = 405 nm). The results showed that red fluorescence from the bacteria, whether related to dental caries or periodontitis, varied in wavelength and intensity depending on the metalloporphyrins present. This suggested that nutrients in a biofilm affect red fluorescence. The effect of age, thickness, and cariogenicity of microcosm biofilm on the extent of red fluorescence was assessed in a constant depth film fermentor model using a fluorescence spectrophotometer and a fluorescence camera (chapter 3). Biofilms of variable thicknesses were grown, receiving a constant medium supply. Their red fluorescence was analysed at different time points, up to 24 days after inoculation. The influence of cariogenicity on red fluorescence was evaluated by growing biofilms, which received a constant medium supply with 3 or 8 pulses of sucrose per day. The composition of the biofilms was assessed using 454 pyrosequencing of the 16S rdna gene. From day 7 onward, the biofilms emitted increasing intensities of red fluorescence, which correlated with biofilm thickness. Biofilm fluorescence also correlated with the imposed cariogenicity. An increase in the biofilm age or an increase in the sucrose pulsing frequency led to a shift in the microbial composition. These shifts in composition were accompanied by an increase in red fluorescence. Subsequently, we assessed the real-time dynamics of red and green fluorescence during the first twelve hours of biofilm formation as described in chapter 4. In addition, the fluorescence patterns of biofilm formed from saliva of eight donors were described under simulated gingivitis and caries conditions. A microfluidic BioFlux flow system with high performance microscopy was used combined with a camera and dedicated excitation and emission filters for fluorescence imaging. Both green and red fluorescence proved to be linearly related to the total biomass of the biofilms. All biofilms displayed green and red fluorescence to some extent, with higher fluorescence intensities from biofilms grown in the presence of serum (gingivitis simulation) as compared to sucrose (cariogenic environment simulation). Fluorescence was not homogeneously distributed within the biofilms: highly fluorescent spots (both green and red) were observed. The red fluorescence from the in vitro biofilms appeared to be positively correlated to the clinical inflammatory response of the respective saliva donors. 139

3 Summary In the second part of this thesis, RFP was studied using a fluorescence camera in a clinical setting. Chapter 5 describes a cross-sectional study, which assessed the correlation between RFP scores and disclosed plaque scores (using a two-tone solution) of 48 participants, using fluorescence photographs and clinical photographs. Total and blue disclosed plaque was scored on white-light photographs. The total quantity of disclosed plaque corresponded well with clinically assessed disclosed plaque, while what at present is considered to be matured dental plaque (blue disclosed plaque) correlated weakly with RFP. Chapter 6 describes the dynamics of RFP in comparison to clinically assessed plaque and bleeding scores during a period of 14 days without oral hygiene in a cohort of 41 participants. The RFP of the anterior teeth was monitored with fluorescence photographs before the trial period (24 h plaque), during 14 days plaque accumulation and after 7 days recovery (24 h plaque). RFP was observed to be reproducible in time (twice 24 h plaque), its amount increased during 14 days plaque accumulation and returned to baseline after 7 days of recovery. Groups of low, moderate and high red fluorescent plaque formers were statistically significantly different at all times. The individual RFP responses during 14 days plaque accumulation correlated well with RFP of 24 h plaque. From day 2 of the gingivitis challenge, RFP correlated with bleeding on probing at day 14, which indicated that RFP was clinically related with the risk of a participant for developing gingival inflammation. The next two chapters discuss the composition of the oral microbiome, as assessed using Illumina sequencing of the 16S rdna gene, in relation to RFP. In the first study, as described in chapter 7, saliva samples were obtained and fluorescence photographs of the anterior teeth were taken from 70 parent-child pairs. No differences in RFP were found between children with and without caries experience. Moreover, no correlation was found between the RFP of parents and their children. In addition, no correlations were found between the presence or absence of RFP and the microbial composition of saliva. Therefore, in chapter 8 the microbial composition of overnight plaque from sites with RFP and from sites with no visible red fluorescent plaque (NFP) was assessed in 50 children. Correspondingly, the relation between the presence of RFP and the caries status in children was studied. No clinically relevant correlations were found between the presence of RFP and (early) carious lesions, which can be explained by the fact that caries is an oral disease that needs time to develop. However, clear differences were found in the composition of the phenotypically different plaque samples: compared to NFP, the RFP contained more Gram-negative, anaerobic taxa and was more diverse. In this thesis, associations were found between RFP and the age and thickness of biofilm, with clear indications that the composition of plaque affects the development of red fluorescence. In addition, the association between RFP and gingival inflammation provides new directions for future research. 140

4 Samenvatting Samenvatting Dit proefschrift beschrijft onderzoek naar rood fluorescerende tandplaque (RFP) en de mogelijkheden om dit fenomeen toe te passen als indicator voor de aanwezigheid van microbiële mondziekten. Deel één van het proefschrift beschrijft het uitgevoerde in vitro onderzoek met in hoofdstuk 2 de rode fluorescentie van verschillende bacteriën die in eerdere onderzoeken in verband zijn gebracht met cariës (tandbederf) en parodontitis (vergevorderde tandvleesontsteking). Deze bacteriën zijn anaeroob gekweekt in de aanwezigheid van verschillende metalloporfyrinen, zoals chlorofyl. De rode fluorescentie van deze bacteriën is bekeken met een fluorescentiecamera met een excitatiegolflengte van 405 nm (blauw-paars zichtbaar licht). Rode fluorescentie van de bacteriën, de golflengte en de intensiteit van de fluorescentie bleken gerelateerd aan de aan- of afwezigheid van de metalloporfyrinen. Deze resultaten suggereren dat voedingsstoffen in een biofilm de rode fluorescentie-eigenschappen van bacteriën beïnvloeden. Het effect van de leeftijd, dikte en cariogeniteit van een complexe biofilm op de hoeveelheid rode fluorescentie is onderzocht in een constant depth film fermentor model (hoofdstuk 3). Biofilms van verschillende dikten werden gekweekt met een continue toevoer van groeimedium. Rode fluorescentie van de biofilms werd gemeten met een spectrofotometer en een fluorescentiecamera op verschillende tijdstippen, tot 24 dagen na de start van het experiment. De invloed van de cariogeniteit van de biofilms op de rode fluorescentie is beoordeeld door complexe biofilms te groeien met een continue toevoer van groeimedium met daarbij 3 of 8 pulsen sacharose per dag. De microbiële samenstelling van de biofilms is bepaald met behulp van 454 pyrosequencing van het 16S rdna gen. Vanaf dag 7 fluoresceerden de biofilms in toenemende mate rood en de mate van fluorescentie correleerde met de dikte van de biofilms. De fluorescentie van de biofilms correleerde ook met de cariogeniteit van de biofilms. Een oudere biofilm of een hogere frequentie van sacharose-pulsen leidden beide tot een verschuiving in de microbiële samenstelling van de biofilms. Deze verschuivingen in de samenstelling vielen samen met een toename in rode fluorescentie. Vervolgens werd in een BioFlux flow biofilmmodel real-time de dynamiek van rode en groene fluorescentie onderzocht, zoals beschreven in hoofdstuk 4. De biofilms van 8 speekseldonoren werden gekweekt onder simulatie van gingivitis (tandvleesontsteking) en cariës omstandigheden. De biofilms werden bestudeerd met een microscoop tijdens de eerste 12 uur van biofilmvorming. Zowel de groene als de rode fluorescentie bleken lineair gecorreleerd aan de totale hoeveelheid biomassa van de biofilms. Alle biofilms fluoresceerden rood en groen, maar in verschillende mate. De fluorescentie-intensiteit was hoger voor biofilms die waren gekweekt in de aanwezigheid van serum (gingivitis-simulatie) vergeleken met sacharose (cariës- 141

5 Samenvatting simulatie). De fluorescentie in de biofilms was niet homogeen verdeeld: er waren hoog fluorescente spots (zowel groen- als rood fluorescent) te zien. De rode fluorescentie van de in vitro biofilms correleerden positief met de klinische ontstekingsrespons van de verschillende speekseldonoren, zoals beschreven in hoofdstuk 6. Het tweede deel van dit proefschrift beschrijft de RFP in de klinische situatie. Hoofdstuk 5 beschrijft een cross-sectioneel onderzoek waarbij bepaald is of er een correlatie is tussen RFP scores en aangekleurde plaque (met twee-kleuren plakverklikker). Hiervoor zijn bij 48 deelnemers fluorescentie foto s en wit licht foto s genomen. De totale hoeveelheid plaque (blauw en roze gekleurd) en de blauw gekleurde plaque is gescoord op wit licht foto s. De totale hoeveelheid gekleurde tandplaque kwam goed overeen met de klinisch bepaalde totale hoeveelheid gekleurde tandplaque. Daarentegen was de blauw gekleurde tandplaque, waarvan wordt aangenomen dat dit oude tandplaque is, slechts zwak gecorreleerd met RFP. Hoofdstuk 6 beschrijft de dynamiek van RFP in vergelijking met klinisch bepaalde plaque- en bloedingsscores gedurende 14 dagen bij 41 deelnemers die geen mondhygiëne toepasten om zo gingivitis te ontwikkelen. De RFP op de voortanden werd vastgelegd met een fluorescentiefoto, zowel vlak vóór de testperiode (24-uursplaque), tijdens de 14 dagen plaqueopbouw en een herstelperiode van 7 dagen (24-uursplaque). De hoeveelheid RFP was reproduceerbaar in de tijd, nam toe tijdens de 14 dagen plaqueopbouw en keerde terug naar het beginniveau na de herstelperiode van 7 dagen. Er werden groepen geïdentificeerd van laag, gemiddeld en hoog fluorescerende plaquevormers die verschilden op alle tijdspunten. De individuele RFP responses tijdens de 14 dagen plaqueopbouw correleerden goed met de hoeveelheid RFP van vóór de testperiode (24-uursplaque). Vanaf dag 2 van de testperiode correleerde de hoeveelheid RFP met bloeding na sonderen op dag 14. Dit suggereert dat de hoeveelheid RFP klinisch gerelateerd is aan het risico van een deelnemer op gingivitis. De hoofdstukken 7 en 8 belichten de samenstelling van het orale microbioom, bepaald met Illumina sequencing van het 16S rdna gen, in relatie tot RFP. Hoofdstuk 7 beschrijft een onderzoek waarbij speekselmonsters zijn afgenomen bij 70 ouderkind paren. Bij deze paren zijn ook fluorescentiefoto s van de voortanden gemaakt. Er werden geen verschillen gevonden in RFP tussen kinderen met en zonder eerder doorgemaakte cariës. Daarnaast is er geen correlatie gevonden tussen de RFP van de ouders en hun kinderen. Ook waren er geen correlaties tussen de aan- of afwezigheid van RFP en de microbiële samenstelling van het speeksel. Daarom is in hoofdstuk 8 bij 50 kinderen de microbiële samenstelling onderzocht van tandplaque met rode fluorescentie en van tandplaque zonder rode fluorescentie. Daarnaast is een mogelijk verband onderzocht tussen de aanwezigheid van RFP en de cariësstatus 142

6 Samenvatting van de kinderen. Er werden geen klinisch relevante correlaties gevonden tussen de aanwezigheid van RFP en (vroege) cariës. Dit kan worden verklaard doordat cariës een mondziekte is die tijd nodig heeft om te ontwikkelen voordat deze kan worden waargenomen. Er werden wel duidelijke verschillen gevonden in de microbiële samenstelling van de fenotypisch verschillende plaquemonsters: RFP bevatte meer gramnegatieve anaerobe taxa en deze plaque was ook diverser vergeleken met niet fluorescente tandplaque. In dit proefschrift zijn verbanden gevonden tussen RFP en de leeftijd en dikte van de biofilm, met duidelijke aanwijzingen dat de samenstelling van de plaque van invloed is op de ontwikkeling van rode fluorescentie. Bovendien geeft het gevonden verband tussen RFP en gingivitis nieuwe mogelijkheden voor verder onderzoek. 143
