4 Curriculum Vitae Moniek van de Luijtgaarden werd op maandag 16 april 1984 geboren in Roosendaal. Na het behalen van haar Atheneum diploma op het Gertrudiscollege begon ze in 2002 met de studie Biomedische Wetenschappen aan de Radboud Universiteit Nijmegen. Voor de Master Epidemiologie volgde ze een tweetal stages; allereerst op de afdeling Intensive Care van het Universitair Medisch Centrum in Nijmegen en vervolgens bij het Integraal Kankercentrum Oost, te Nijmegen. In juni 2008 begon Moniek aan haar promotietraject bij de registratie van de European Renal Association European Dialysis and Transplant Association (ERA-EDTA). Deze registratie maakt onderdeel uit van de afdeling Klinische Informatiekunde in het Academisch Medisch Centrum (AMC) te Amsterdam. Naast het onderzoek bestond de functie voor een gedeelte uit datamanagement voor de ERA-EDTA Registratie en het assisteren bij epidemiologie cursussen. Het promotieonderzoek zoals beschreven staat in dit proefschrift is uitgevoerd onder leiding van prof. dr. Ameen Abu- Hanna, dr. Kitty J. Jager en dr. ir. Marlies Noordzij. Sinds februari 2012 is Moniek naast haar baan als promovenda ook aangesteld als internationale coördinator van de EQUAL Study. Deze functie zal zij na haar promotie blijven vervullen en op die manier betrokken blijven bij de ERA-EDTA Registratie. Curriculum Vitae Moniek van de Luijtgaarden was born in Roosendaal on Monday April 16th, After graduating secondary school (Atheneum) at the Gertrudiscollege in Roosendaal, she started to study Biomedical Sciences at the Radboud University Nijmegen in During the Master s phase of her study she completed two research internships, at first the department of Intensive Care Medicine at the University Medical Center Nijmegen and subsequently at the Comprehensive Cancer Centre in Nijmegen. In June 2008, Moniek started her PhD at the registry of the European Renal Association European Dialysis and Transplant Association (ERA-EDTA). This registry is part of the department of Medical Informatics at the Academic Medical Center (AMC) in Amsterdam. Besides performing research, her work consisted of data management for the ERA-EDTA Registry and providing assistance with epidemiology courses. The PhD project as described in this thesis has been conducted under supervision of prof. dr. Ameen Abu-Hanna, dr. Kitty J. Jager en dr. ir. Marlies Noordzij. Since February 2012, Moniek holds a position as international coordinator for the EQUAL Study in addition to her position as PhD student. Within this function as post-doctoral researcher she will remain active within the ERA-EDTA Registry. 159
