3 Beroepsprofielen V&V2020 Zelfmanagementondersteuning: de kern van verplegen Streven is optimale kwaliteit van leven zoals de patiënt dat ervaart Zelfmanagement vraagt veel van de patiënt: het bijstellen van de levensdoelen, inzicht hebben in zijn aandoening bijvoorbeeld en vaardigheden om problemen op te lossen Daarbij is ondersteuning door verpleegkundige / VS van essentieel belang

10 Coach Perspectief Zelfmanagement is een onlosmakelijk onderdeel in het leven van iemand met een chronische aandoening. De patiënt is een expert op het gebied van omgaan met zijn aandoening en de relatie tussen mij en de patiënt is gelijkwaardig. Ik besteed vooral aandacht aan wat de patiënt nog kan en geef de patiënt een actieve rol tijdens contacten

11 Poortwachter Perspectief Volgens mij is zelfmanagement een middel om de patiënt zo zelfstandig te maken, dat deze zo min mogelijk gebruik hoeft te maken van de professionele gezondheidszorg. Ik geef advies om patiënten te helpen gezonde keuzes te maken maar ik accepteer het als patiënten de keuze maken om van behandeling af te zien.

12 Behandelaar Perspectief Therapietrouw is het voornaamste doel. Dit is de sleutel tot een goed leven met de aandoening. Ik ben actief in het geven van adviezen en oplossingen voor problemen waar de patiënt tegen aan loopt. Ik help de patiënt om goede doelen te stellen en de juiste keuzes te maken.

13 Leraar Perspectief Ik zie zelfmanagementondersteuning als middel om ervoor te zorgen dat de patiënt weet wat hij moet doen om complicaties te voorkomen. Ik geef instructie en voorlichting zodat de patiënt weet wanneer hij aan de bel moet trekken. Mijn verpleegkundige kennis is daarbij onmisbaar.

24 You only live once Nurse Erica works in a kidney dialysis centre. One of het patients is Richard, who is 20 years old and who has been on haemodialysis tratment since he was 15. His adherence to the rigid regimen seems to be OK, as his serum potassium levels and other markers are good. Still, Erica is worried about Richard. For him, being with friends and drinking some beers is an essential part of normal life. But Erica has her reasons to be worried about binge drinking. Whenever she brings up the beer subject, Richard is irritated and turns away. He tells her that he really hates it when people (like his parents) interfere with his own affairs. What should Erica do?

25 You only live once- choices A. Erica continues to convince Richard that his potential risky drinking at weekends is damaging his health and to make him aware of possible complications B. Erica accepts Richards drinking habits in the weekend; he has a right to make his own decisions. She decides to wait with informing about the risks he takes until he s open to it. C. Erica informs the nephrologist of Richards drinking and they decide to refer Richard to a social worker/ a psychologist since there is a possibility be might be depressed D. Erica invites Richard and his parents for a family meeting to talk about living with a kidney failure and to address the existing tensions.

26 Professional boundaries Nurse Dennis works with people with HIV. One of his patients, Salim, is a Somalian refugee who arrived here five years ago. Within Salim s community HIV is stigmatized. Therefore, Salim is very concerned about maintaining confidentiality and this hampers his adherence. At the same time, Salim also struggles with other problems, such as coping with trauma and finding his way in Dutch institutions. Salim does not work and his social network is very limited. When Dennis asks Salim how he s coping with his medication, Salim tells him he is about to be kicked out of his appartment due to his debts. Dennis struggles with the question whether helping patients such as Salim is really part of his professional responsibility. What should Dennis do?

27 Professional bounderies- choices A. Dennis decides it is beyond the hospital s responsibility to solve Salim s personal problems. He shows empathy but continues to talk about the medication and support Salim in being adherent. B. Dennis decides it is beyond his own professional responsibility to solve Salim s personal problems and he refers Salim to a social worker who could help him access social security, dept or trauma counselling services. C. After listening to Salim s story, Dennis decides to help him. In such a difficult circumstances you can t really say to a patient good luck and bye bye. Dennis contacts the local council and Salim s housing association D. Dennis encourages Salim to look for peer support for example in hus local parish or in the national HIV association where there is a network for refugees with HIV

28 To care or not to care Fatima works in a palliative home care team. One of her elderly patients, Mrs. Welby, is in a terminal stage of cancer and is mostly bedridden. Fatima thinks it s important that Mrs. Welby participated in her care as much as possible and therefore encourages Mrs. Welby to do little things, such as washing her face, eating by herself, sitting in a chair, etc. On the other hand, some of her colleagues in the team do not agree with her. John feels sorry for Mrs. Welby. Since she is so very ill and tires easily, it is more than justified to pamper and coddle her a little bit. John also noticed that Mrs. Welby enjoys being cared for. Fatima and John decide to discuss this case during the team meeting. What should be the outcome of this meeting?

29 To care or not to care - choices A. The team concludes that it is inevitable that individual nurses have different views on how to provide good nursing care. As long as it doesn t damage patients health, this should be accepted. B. The team decides to support Fatima s position by arguing that safeguarding a patient s dignity and autonomy as long as possible, is a core value in palliative care and thus of the team. C. The team decides that John s position should be supported. Showing sympathy and helping people lies at the heart of nursing, particularly in palliative care. D. The team decides to consult Mrs. Welby whether she likes to eat by herself or prefers to be fed by others. The team settles on accepting whatever the patient wishes.
