6 4 Van de voorzitter From the Chairman Beste Interplast vrienden, Dear Interplast friends, In dit eerste jaaroverzicht ziet u het bewijs dat Stichting Interplast Holland nog zeer actief is en nieuwe uitdagingen beslist niet uit de weg gaat. Het komend jaar hopen we op weer een uitgebreidere samenwerking, ook met nieuwe organisaties, binnen Nederland en in onze gastlanden. Door geregeld te bezoeken blijft u direct op de hoogte van ons wel en wee. Het afgelopen jaar is er een record aantal teamuitzendingen geweest: negen! Nieuwe landen ditmaal waren Zanzibar en Burundi. Vanaf pagina 7 leest u verslagen van wat onze teams waar allemaal hebben gedaan. In Oeganda was de Burns Awareness Week weer een groot succes. Ditmaal was de aandacht gevestigd op de sloppenwijken rondom Kampala, waar de meeste patiënten vandaan komen. Lees op pagina 26 ook over de verdere plannen die wij daar hebben, samen met de Nederlandse Brandwonden Stichting. This first annual review provides you with clear evidence that Stichting Interplast Holland is still extremely active and is avidly taking on new challenges. In the year ahead we are again hoping to cooperate even more, with new organisations as well, both in The Netherlands and in our host countries. You can keep in touch with the ups and downs of our work by regularly visiting our website Last year saw a record number of team missions: nine, to be precise. New countries this time were Zanzibar and Burundi. You can read the reports of what our teams have done and where from page 7 onwards. In Uganda the Burns Awareness Week was again a big success. This time attention focused on the slums around Kampala where the majority of our patients come from. Read on page 26 about our further plans there in partnership with the Dutch Burns Foundation. Na zijn documentaire over Guinee-Bissau maakte Joris Kreugel ook een documentaire over The Uganda Burns and Plastic Surgery Institute. Een project dat geheel in ontwikkelingstermen zeker sustainable te noemen is. Dit centrum functioneert nu volledig met lokaal personeel. Joris Kreugel has made a documentary about the Uganda Burns and Plastic Surgery Institute after his documentary about Guinea-Bissau. In development terminology the Burns Centre is a good example of a definitely sustainable project and now functions entirely with local staff. Helaas kregen we ook een trieste boodschap te verwerken. Father Albert, bestuursvoorzitter van de UBPSI en van Interplast Oeganda, kwam om het leven bij een autoongeluk. U leest in het In Memoriam meer over deze bijzondere man. Els Gerritsen stopt bij de verpleegkundige commissie. Zij zette zij zich jarenlang fantastisch voor Interplast in, samen met vele andere vrijwilligers. Een klein team van vrijwilligers volgt haar op. Tot slot zeker ook namens onze patiëntjes een oprecht dankwoord aan iedereen die heeft bijgedragen aan het succes van Interplast Holland. Blijft u ons vooral steunen! Els Gerritsen will be leaving the nursing committee. For years she has done fantastic work for Interplast together with many other volunteers. A small team of volunteers will take over from her. 7 5 Inleiding Introduction Stichting Interplast Holland is een ideële organisatie die zich richt op reconstructieve chirurgie voor kinderen en (jong) volwassenen in ontwikkelingslanden. Jaarlijks wordt een aantal teams van ervaren Nederlandse plastisch en reconstructief chirurgen, anesthesiologen en operatieen anesthesieassistenten uitgezonden. De artsen en assistenten, die ter plekke worden bijgestaan door lokale artsen en verpleegkundigen, zetten zich hiervoor kosteloos tijdens hun vakanties in. Zij bieden hulp door: het uitvoeren van kosteloze reconstructieve operaties bij kinderen en (jong) volwassenen met lip- en gehemeltespleten (schisis), brandwondverminkingen, tumoren en andere aangeboren en/of verworven (bijvoorbeeld oorlogsslachtoffers) afwijkingen het opleiden van lokaal medisch personeel in de reconstructieve chirurgie, anesthesiologie en verpleegkundige technieken te helpen bij het opzetten van brandwondafdelingen en afdelingen voor reconstructieve chirurgie binnen de bestaande ziekenhuizen Om duurzame resultaten te bereiken, wordt intensief samengewerkt met lokale ziekenhuizen, artsen, verpleegkundigen, universiteiten en hulporganisaties. Om de lokale ziekenhuizen zoveel mogelijk te ontlasten worden alle materialen die nodig zijn voor circa operaties meegenomen vanuit Nederland. Interplast is een internationale organisatie die in 1969 in de Verenigde Staten ontstond. In 1990 werd Stichting Interplast Holland opgericht. In vele andere landen is Interplast inmiddels vertegenwoordigd. Interplast Holland is als stichting volledig onafhankelijk van haar zusterorganisaties. Interplast Holland richt zich momenteel voornamelijk op Jemen, Nigeria, Rwanda, Oeganda en Guinee-Bissau. Naar deze landen vinden jaarlijks missies plaats. Stichting (Foundation) Interplast Holland is a charitable organisation that performs reconstructive surgery for children, young people and adults in developing countries. Each year the organisation sends teams of experienced plastic and reconstructive surgeons, anaesthesiologists and operating theatre assistants on a number of missions to developing countries. The doctors and assistants, who are assisted by local doctors and nurses, travel during their holidays and provide their services free of charge. Their assistance extends to: performing reconstructive operations free of charge for children and adults with cleft lips or palates, mutilations caused by burns, tumours and other congenital defects training local medical personnel in reconstructive surgery, anaesthesia and nursing techniques helping to set up burns and reconstructive surgery units in hospitals To secure lasting results Interplast Holland works intensively with local hospitals, doctors, nurses, universities and NGOs. In order not to burden local hospitals unduly, all materials required to perform operations are flown in from The Netherlands. Interplast is an international organization which was founded in the United States in Stichting Interplast Holland was founded in By now Interplast is represented in many other countries. As a foundation Interplast Holland is fully independent from its fellow Interplast organizations. They are regarded as sister organizations. Stichting Interplast Holland currently concentrates its activities in Yemen, Nigeria, Rwanda, Uganda and Guinea-Bissau. Missions to these countries take place every year. Interplast heeft in 2009 ook twee nieuwe landen bezocht, Burundi en Zanzibar (2 x). In het verleden werden teams uitgezonden naar Indonesië, Vietnam, Pakistan, India, Bhutan, Tibet en Libanon. Two new locations for Interplast in 2009 were Burundi and Zanzibar (two visits). In the past teams have also been sent to Indonesia, Vietnam, Pakistan, India, Bhutan, Tibet and Lebanon.

8 6 Inleiding Teamuitzendingen In 2009 vonden acht reguliere Interplast missies plaats: twee naar Nigeria, één naar Guinee-Bissau, één naar Jemen, twee naar Zanzibar, één naar Burundi en één naar Rwanda. De geplande missie naar Noord-Oeganda in samenwerking met het Trust Fund for Victims van het International Criminal Court werd uitgesteld tot juni Missions Eight regular missions took place in 2009: two to Nigeria, one to Guinea-Bissau, one to Yemen, two to Zanzibar, one to Burundi and one to Rwanda. The planned mission to North Uganda in partnership with the Trust Fund for Victims of the International Criminal Court was postponed until June Een negende missie had een speciaal karakter, operatieassistente en docente Greta Hesseling verbleef aan het eind van het jaar 6 weken in Jemen. Zij bracht de plastisch chirurgen van het Al-Thawra General Teaching Hospital in Sana a de eerste technieken van de Microchirugie bij. Na 5 weken voegden plastisch chirurgen Rein Zeeman en Hay Winters zich bij Greta in Jemen. Zij opereerden samen met de lokale chirurgen enkele vrije lappen. A ninth mission was of a special kind. Operating assistant and teacher Greta Hesseling stayed for six weeks in Yemen at the end of the year to teach the initial techniques of microsurgery to the plastic surgeons of the Al-Thawra General Teaching Hospital in Sana a. After five weeks Greta was joined by Rein Zeeman and Hay Winters who operated some free tissue transfers together with the local surgeons. Uganda Burns and Plastic Surgery Institute (UBPSI) Op 26 november was het precies 5 jaar geleden dat de eerste afdeling van het Brandwondencentrum in het Mulago University Hospital te Kampala geopend werd door First Lady Mrs. Janet Museveni. Reden voor de officemanager van Interplast Holland om nu zelf ook eens een kijkje te gaan nemen in Oeganda. De National Burns Awareness Week in november bood daar de ideale gelegenheid voor. BNN verslaggever en cameraman Joris Kreugel reisde mee naar Oeganda en maakte een documentaire over het functioneren van het centrum. Uganda Burns and Plastic Surgery Institute (UBPSI) 26 November was the fifth anniversary of the opening of the first ward of the Burns Centre in Mulago University Hospital in Kampala by the First Lady Mrs Janet Museveni. A good enough reason for the office manager of Interplast Holland herself to go and take a look at what happens in Uganda. The National Burns Awareness Week in November was a perfect opportunity. BNN reporter and cameraman Joris Kreugel travelled along too and made a documentary of the work of the centre in Uganda. Johannes Mooij. Land in opbouw Guinee-Bissau, een jarenlang door burgeroorlogen geteisterd land, is pas sinds kort weer een beetje in opbouw. Het land leeft voornamelijk van de landbouw van onder andere rijst en van de export van cashewnoten. Het wordt beschouwd als doorvoerland voor drugs. Hulp van buitenaf komt nog maar moeizaam op gang. Nu zijn de leefomstandigheden voor de bevolking op het platteland uiterst primitief en lopen kinderen door open kookvuurtjes gevaar om brandwonden op te lopen. De kennis (en de middelen) voor een juiste behandeling van brandwonden ontbreekt, waardoor veel contracturen ont staan. Er is wel een kleine brandwondenafdeling op het zieken huis terrein, maar van een behandeling is nauwelijks sprake. In zijn verslag schrijft Rein Zeeman: Je ziet de contracturen letterlijk voor je ogen ontstaan, huid transplantaties kunnen ze hier niet. Hier is nog heel veel op te leiden! After the first exploratory mission to this poverty-stricken West African country in 2008, a second mission followed in February The mission was again well prepared by the local organising committee: consul Jan van Maanen and his wife Will, and Professor Johannes Mooij. Country under construction Guinea-Bissau is a country that was ravaged by civil war for years and has only recently begun to show some signs of recovery. Agriculture is the main source of income and crops grown include rice and cashew nuts for export. Guinea-Bissau is also known as a transit country for drugs. Foreign aid penetrates only with great difficulty. The population in rural areas live in extremely primitive conditions and children are in constant danger of being burned since cooking is all done on open fires. Knowledge and resources to treat burns properly are lacking so that contractures are prevalent. There is a small burns department in the hospital grounds but treatment is barely the word. In his report Rein Zeeman writes: You literally see the contractures happening before your very eyes. They cannot do skin transplants here. A great deal of training is needed!

10 8 Guinee-Bissau Burn contracture, before and after surgery Een goede voorselectie Weken van tevoren stuurde de honorair-consul Jan van Maanen al foto s van mogelijke patiëntjes door. Zo kon op afstand een eerste selectie gemaakt worden door teamleider en plastisch chirurg Rein Zeeman. De dag na aankomst van het team werd direct begonnen met het screenen van de patiënten en in totaal werden (door 1 plastisch chirurg) 62 operaties verricht, waarvan meer dan de helft bestond uit het opheffen van brandwondcontracturen. Onder begeleiding van Rein Zeeman opereerde chirurge Maimuna Baldé ook enkele schisispatiëntjes. Naast schisis- zijn hier ook meer dan Good preselection Weeks beforehand, the honorary consul Jan van Maanen passed on photographs of potential patients. That meant that a preliminary selection could be made in advance by team leader and plastic surgeon Rein Zeeman. The day after the team arrived the members immediately began screening the patients and altogether 62 operations were performed by a single plastic surgeon, more than half of them involving burn contractures. De Duitse organisatie Hilfsaction Noma e.v. wil er zelfs een speciaal nomaziekenhuis neerzetten! a debilitating disease that damages the tissues of the mouth and cheeks. The German organisation Hilfsaction Noma e.v. is actually thinking about building a special noma hospital here. Een documentaire De eerste 6 dagen van het werk van Interplast Holland in Guinee-Bissau zijn op film vastgelegd door Joris Kreugel. Joris is verslaggever bij BNN radio 1 en heeft zijn eigen mediabedrijfje (Joris Media). De documentaire zal begin 2010 ook op de televisie in Bissau vertoond worden. Via plastisch chirurg (en Interplast vrijwilliger sinds jaar en dag) Johan Klasens werd in 2008 contact gelegd met het ministerie van Gezondheid in Zanzibar. En al snel werd Interplast uitgenodigd om te komen werken in het Mnazi Mmoja Hospital (430 bedden) in Zanzibar-Stad. Uitstekende voorbereiding De Nederlandse arts Boudewijn Peters woont en werkt op Zanzibar, als Senior Health Advisor voor de Deense ontwikkelingsorganisatie Danida. Dankzij een uit stekende samenwerking met Boudewijn én een goede voor bereiding van het ziekenhuis zelf, kon een eerste team begin 2009 worden uitgezonden. In het ziekenhuis zelf lagen de voorbereidingen en organisatie in handen van Dr. Ramadhan ( Dr. Rama ), die de touwtjes strak in handen had. Zanzibar is an island off the east coast of Africa and is part of Tanzania. Johan Klasens, plastic surgeon and long-term Interplast volunteer, got in touch with the Health ministry in Zanzibar in Very soon Interplast was invited to come and work in the Mnazi Mmoja Hospital with 430 beds in Zanzibar City. Excellent preparation It so happens that the Dutch doctor Boudewijn Peters works in Zanzibar as Senior Health Advisor for the Danish development organisation Danida. Thanks to excellent cooperation with Boudewijn and sound preparation by the hospital itself, it was possible to send a first team in early Preparation and organisation in the hospital itself was in the hands of Dr Ramadhan or Dr Rama for short, who had everything well under control.

13 Zanzibar 11 Childrens ward Niet alles kon geopereerd worden Men had van tevoren ruim 40 patiënten geselecteerd, waarvan helaas direct een deel moest afvallen (veel orthopedische problematiek, bijvoorbeeld klompvoetjes). Verder was er zoveel vraag naar hypospadie-operaties, dat een aantal patiëntjes niet geopereerd kon worden Er was gewoon onvoldoende materiaal meegenomen of voorradig. In meer dan de helft van de gevallen betrof het trouwens geen hypospadieën (aangeboren afwijkingen aan de plasbuis), maar fistels en defecten door complicaties na circumcisies. Not all the patients could be operated Forty patients had been selected in advance but unfor tunately some of them had to be turned down right from the start since a lot of the cases involved orthopaedic problems like clubfoot, which is not a medical field that plastic surgeons handle. And then there was such a demand for hypospadias operations to remedy a congenital defect in which the urethra opens inferior to its normal location, that a number of young patients could not be operated as planned. Not enough materials had been brought or were in stock. More than half of the cases involved not hypospadias, but fistulas and defects arising from complications from circumcisions. 14 12 Zanzibar maar veel gelukkig wel Er werd geopereerd op twee van de vier operatiekamers die het ziekenhuis rijk is. Naast de hypospadie-operaties werden vooral schisis-patiënten en patiënten met brandwond contracturen geopereerd. Doordat zich elke dag weer nieuwe patiënten aandienden, kwam het aantal uiteindelijk uit op 42. Daarnaast werden 22 verbandwisselingen onder narcose uitgevoerd. but there was lots we could do Operations were carried out in two of the hospital s four ope rating theatres. Patients with cleft lip and /or palates or those with burns contractures were the main ones operated on besides the hypospadias cases. Every day new patients turned up so that in the end 42 operations were performed. In addition 22 dressing changes were carried out under anaesthetic. Enthousiasme Al die twee weken dat het team er was, bleef het lokale OK-personeel behulpzaam, aandachtig en enthousiast. En zoals sowieso blijkt uit alle verslagen: dit was nou echt een leuke en vruchtbare eerste missie, die zeker voor herhaling vatbaar is! De tweede missie Het vorige Interplast-bezoek aan Zanzibar in februari had nuttige gegevens opgeleverd en zo kon het oktober-team zich al vroeg gaan voorbereiden. Gebleken was bijvoorbeeld een grote vraag naar behandeling van kinderen met klompvoetjes, dus werd besloten er nu wél enkele te behandelen. Dit Interplast-team was toevallig twee gipsmeesters rijk, dus dankzij Jan en Jolanda kon het allemaal prima! Bovendien kwamen deze extra vaardigheden en know-how goed van pas na de operaties bij brandwondcontracturen, zoals ook uit Reinier Schoorls verslag blijkt (deze is te lezen op Enthusiasm The local operating theatre personnel were helpful, attentive and enthusiastic during the whole two weeks of the team s stay. And as is apparent from all the reports, this first mission was fruitful and fine and certainly worth repeating! The second mission The previous Interplast visit to Zanzibar in February had produced useful information so that the October team was able to prepare early. For example, it became apparent that there was a big demand for clubfoot operations, so it was decided this time that some of these cases would be treated. The Interplast team this time happened to have two plaster cast experts, so thanks to Jan and Jolanda these operations were no problem at all. Moreover, the extra skills and know-how came in very handy after the operations on burn contractures, as you can also read in Reinier Schoorls report on the website at Plaster of Paris masters Jolanda and Jan at work Zanzibar

15 Zanzibar 13 Dressing change Child with cleft lip Goede voorbereiding Wederom had Dr. Rama de zaken uitstekend voorbereid: hij wist dat dit team ook klompvoetjes zou behandelen en dus waren bij de eerste screening ook kinderen daarmee aanwezig. Verder werden er weer veel jongetjes gezien met urethrale fistels (veelal door verkeerd uitgevoerde circumcisie) en kinderen met brandwondcontracturen. Zo vol waren de dagen hier, dat Dr. Rama na enkele dagen moest melden dat alle bedden bezet waren! Gelukkig ging het met sommige patiëntjes zo goed, dat zij snel naar huis konden en men weer door kon gaan met opereren. Well prepared Again Dr Rama had made excellent preparations. He knew that this team was going to be doing clubfoot operations and children with this disorder were present at the first screening. Again there were a lot of young boys with urethral fistulas (often caused by badly performed circumcisions) and children with burns contractures. The days were so full that it wasn t long before Dr Rama had to report that all the beds were occupied. Fortunately some patients made such excellent progress that they were able to go home quickly, leaving more room for other patients to be operated on. Uiteindelijk zijn er in totaal 63 operaties verricht en vonden er 23 verbandwisselingen/ gipsbehandelingen plaats. Voor een volgend team staan al zeker twaalf patiëntjes op de lijst. The recording of this visit was broadcast in the evening on local television and this in turn generated an influx of new patients in the subsequent days. Altogether 63 operations were carried out as well as 23 dressing changes and plaster cast treatments. There are already twelve patients on the waiting list for the next team. Daar op het Jos Plateau ligt Mangu en daar bevindt zich nu het COCIN Hospital & Rehabilitation Centre (een voormalig ziekenhuis van de Nederlandse Leprastichting). Steun van de ambassade Direct vanaf 2007 is de Nederlandse ambassade in Abuja betrokken geweest bij het Interplast-project in Mangu. Onder meer met hun steun is in 2009 een begin gemaakt met de bouw van een nieuwe verpleegafdeling, voor 50 bedden. Since 2007 a team mainly comprising plastic surgeons from the north of The Netherlands has been travelling to the north of Nigeria twice every year. There on the Jos Plateau lies Mangu, the location of the COCIN Hospital & Rehabilitation Centre, a hospital that formerly belonged to the Nederlandse Leprastichting or Netherlands Leprosy Relief (NLR). Support from the embassy From the outset in 2007 the Netherlands Embassy in Abuja has been involved in the Interplast project in Mangu. Thanks in part to their support a start was made in 2009 on building a new nursing ward with fifty beds.

17 Nigeria 15 Cooking place Een missie in maart en een in oktober Tijdens de eerste missie in maart konden 75 patiënten geopereerd worden, vooral patiënten met brandwonden en contracturen. In oktober werd niet alleen geopereerd (86 patiënten), maar gaven de Interplast-artsen en -verpleegkundigen ook voorlichting over brandwondenzorg. Men is nu bezig een brandwondenafdeling op te zetten! Ook werden de plannen om een nieuwe operatiekamer te bouwen verder uitgewerkt. Materiaal wel drie jaar onderweg Tot grote vreugde van het oktober team was de container met medische apparatuur eindelijk gearriveerd, na een reis van wel drie jaar! Zoals Eddy Verschuren ook in zijn verslag schrijft, had dit wel zijn tol geëist: het team is twee avonden bezig geweest met de hydrauliek van de OK-tafel en stoel. Uiteindelijk functioneerde alles weer. A mission in March and October During the first mission in March the team operated on 75 patients mainly cases of burns and contractures. In October, besides 86 operations, Interplast doctors and nurses briefed local staff on caring for burns patients. Now the locals are in the throes of setting up a burns ward. Plans to build a new operating theatre were worked out in further detail. Materials on their way for three years To the great joy of the October team, the container with medical equipment finally arrived after a journey lasting three years. As Eddy Verschuren writes in his report, the time it took had taken its toll. The team spent two evenings trying to get the hydraulic mechanism of the operating theatre table and chair to function properly. In the end everything was working. Zichtbare vooruitgang Hoe dan ook verbeteren de omstandigheden in het ziekenhuis per jaar zichtbaar: niet alleen wat betreft materialen, maar ook qua scholing van het lokale personeel. 19 Jemen 17 Het is 12 jaar geleden dat Interplast Holland voor het eerst in Jemen werkte. Sindsdien zijn veel resultaten geboekt in het opleidingsziekenhuis in Sana a, het Al-Thawra Modern General Teaching Hospital, waar de plastische chirurgie is geïntegreerd in het opleidingstraject. De drie vaste plastisch chirurgen hebben er dit jaar een eigen afdeling gekregen. Interplast Holland went to Yemen to work for the first time twelve years ago. Since then successful results have been achieved in the teaching hospital in Sana a, the Al-Thawra Modern General Teaching Hospital where plastic surgery is integrated in the training programme. The three permanent plastic surgeons were given their own department this year. Spannende missie Na alle onrust in Jemen en enkele ontvoeringen in de maanden voor vertrek was deze missie extra spannend. Maar Interplast heeft al jaren goed contact met de Nederlandse Ambassade in Sana a en men houdt daar de veiligheidssituatie goed in de gaten. Ook was extra bewaking geregeld, om niets aan het toeval over te laten. Op 17 april vertrok Rein Zeeman met zijn team naar Jemen. Voor plastisch chirurge Esther Mesters en het anesthesieteam Marjolijn Beuning en Frank Haklander was dit de eerste keer! Tense mission This mission was additionally tense because of the unrest in Yemen and a number of kidnappings in the months prior to departure. But for years now Interplast has been in close touch with the Netherlands Embassy in Sana a and the staff there keep a keen eye on the security situation. Additional surveillance had also been organised so as to leave nothing to chance. Rein Zeeman with his team left for Yemen on 17 April. This was the first time to Yemen for plastic surgeon Esther Mesters and the team of anaesthetists Marjolijn Beuning and Frank Haklander. Greta Hesseling in her shop Yemen

Starting in Sana a As usual the team started in the Al-Thawra Modern General Teaching Hospital, where the surgeons led by Dr Al-Ammari had saved the complicated patients for the team. By operating on these patients with the Interplast surgeons the local surgeons can also learn from each mission. daarna in Ibb Na 5 dagen vertrok het team met de collega s uit Sana a naar het Mother and Child Hospital in Ibb. Daar kon men op 4 operatiekamers tegelijk werken! Operatieassistente Greta Hesseling legde vele kilometers af om iedereen telkens van instrumenten te blijven voorzien. Na een radio-interview waren er ineens meer dan 400 patiënten en in totaal zouden er 600 geregistreerd worden! Uiteindelijk zijn maar liefst 97 patiënten geopereerd en met de operaties in Sana a kwam het totaal op 121: een record! In Ibb is voorlopig nog heel veel te doen en de gouverneur aldaar verzocht het team dan ook om snel terug te komen. Grote kans dat ook Esther, Marjolijn en Frank dan weer graag meegaan, naar dit prachtige en gastvrije land. and finishing in Ibb After five days the teams and local colleagues from Sana a went on to the Mother and Child Hospital in Ibb. There the team was able to work in four operating theatres simultaneously. Operating assistant Greta Hesseling clocked up numerous kilometres travelling from one theatre to the next keeping everyone supplied with instruments. A radio interview on the spot suddenly produced more than 400 patients and altogether it was said that 600 had been registered. In the end no less than 97 patients operated which brought the total to a record number of 121 including the Sana a operations. Much still has to be done in Ibb and the local governor asked the team to come back soon. There s a big chance that Esther, Marjolijn and Frank will join the team again on the next mission to this wonderful, hospitable country. 21 Jemen 19 Cursus microchirurgie in Jemen Tekst: Greta Hesseling Microsurgery course in Yemen In de Isala Klinieken geef ik al lang les: aan de staf van het OK-personeel geef ik plastische chirurgie, en aan de artsen-in-opleiding de basisprincipes van de microchirurgie. Toch was het een uitdaging om in Al Thawra Modern General Teaching Hospital in Jemen vijf weken lang een cursus microchirurgie te gaan geven. I ve been teaching for a long time in the Isala Clinics, I ve handled plastic surgery with the operating theatre staff and taught the basic principles of microsurgery to young doctors in training but it was a real challenge to give a five week course on micro surgery at the Al Thawra Modern General Teaching Hospital in Yemen. Niet veel tijd tegelijk, toch veel nuttige uren Met Dr. Ammari aldaar kwam ik overeen om vier studenten op te leiden. Meer was ook niet mogelijk: er waren maar 3 microscopen en ook de meegebrachte instrumenten waren slechts toereikend voor drie studenten tegelijkertijd. Bovendien moesten die studenten nu eenmaal nachtelijke taken doen, waren operaties reeds ingeroosterd en afspraken al gemaakt. Toch kon ik elke dag praktische oefeningen met alle studenten doen en zodra iedereen tijd had, deden we de lessen microchirurgie. In totaal hebben we 25 uur geoefend met de microscopen en 20 uur aan colleges besteed. Plus de vele besproken onderwerpen tijdens het werk onder de microscopen! Inhoud van de lessen Alle studenten kregen een kopie van boeken over microchirurgie. Het meest gebruikte is dat van Dr. Acland, over microvasculaire operaties, het bevat alles wat je nodig hebt voordat je gaat oefenen. Andere lessen kwamen vanuit mijn eigen ervaring en gingen bijvoorbeeld over: hoe de condities voor de patiënt in een operatiezaal verbeteren, allerlei tips die werk makkelijker maken, de diverse hecht technieken, de verschillende anastomoses (vaat verbindingen) en het prepareren van bloedvaten. Uiteraard bespraken we mogelijke problemen en oplossingen daar voor, zoals: wat kan fout gaan tijdens het hechten van de anastomose, en wat te doen als klemmen verwijderd zijn en de circulatie niet op gang komt. Goed niveau: verdiend certificaat Ook dankzij lezingen over soorten vrije lappen (in de laatste cursusweek gegeven door Dr. Winters en Dr. Zeeman) ontwikkelden drie van mijn studenten goede technieken en deden ze veel kennis over microchirurgie op. (Eén student kreeg helaas de technieken onder de microscoop niet onder de knie.) Dus na afloop ontvingen deze drie chirurgen (Dr. Ahmed Al-Ammari, Dr. Mohammed Al-Amri en Dr. Abdel Fatach) een certificaat voor deze eerste microchirurgische cursus in Jemen, georganiseerd en gegeven door leden van Interplast Holland. Hopelijk kunnen nog meer plastische chirurgen onze cursussen volgen! Not much time together simultaneously, but still many useful hours I agreed with Dr Ammari that we would train four students. It was impossible to train more because there were only three microscopes and the instruments that I had taken to Yemen were only sufficient for three students simultaneously. What s more the students also had night duty, were scheduled to assist at operations and be present at appointments that had already been made. In spite of this I was able to do practical exercises every day and, as soon as everyone had time, to give lessons in microsurgery as well. All together the students attended 25 hours of practicals using the microscope and 20 hours of lectures, quite apart from the time put in discussing the many subjects that came up while working under the microscopes. Lesson content All the students were given a copy of books about microsurgery. The most current is the one by Dr Acland on microvascular operations, which contains everything you need to know before you start practising. Other lessons were based on my own experience and dealt with subjects like improving the conditions for patients in an operating theatre and all kinds of tips that make work easier, the various suture techniques, the different anastomoses and preparing the veins and arteries. Obviously the possible problems and solutions were discussed, such as what can go wrong when suturing the anastomosis, and what to do if the clamps have been removed and circulation fails to resume. Good standard: certificate deserved Also thanks to lectures on types of free flaps (given in the last weeks of the course by Dr Winters and Dr Zeeman), three of my students developed good techniques and gained a lot of expertise in microsurgery. (One student unfortunately failed to master the techniques under the microscope.) Having completed the training the three surgeons (Dr Ahmed Al-Ammari, Dr Mohammed Al-Amri and Dr Abdel Fatach) were awarded a certificate for this first microsurgery course in Yemen organised and given by members of Interplast Holland. Het ligt in de Grote Merenregio van Midden-Afrika, zonder directe verbinding met zee. Door de burgeroorlog van 1994 tot 2005 is het bovendien een van de armste landen ter wereld. Medische zorg is maar voor een enkeling toegankelijk. Op een bevolking van zo n 8 miljoen zijn er 200 artsen. Burundi is one of Africa s most densely populated countries and also Africa s smallest. It is located in the Great Lakes region of Central Africa with no direct link to the sea. As a result of the civil war from 1994 to 2005 the country is one of the poorest in the world. Medical care is the preserve of the very few. There are 200 doctors in Burundi for a population of around 8 million. Verkennende missie Tijdens De Andere Markt van 2008 in Katwijk werd Interplast benaderd door de voorzitter van Izere (Burundi Diaspora in Nederland) André Nkeshimana; of Interplast ook interesse had om in Burundi een project op te zetten? Dit leidde na een jaar van samenwerken en voorbereiden tot een verkennende missie in juni Teamleider Rein Zeeman had zijn team zorgvuldig samengesteld, met Interplast-vrijwilligers van het eerste uur (de meesten met bijna 20 jaar ervaring met werken in Afrika). Reconstructieve chirurgie onbekend Tot verbazing van de Interplasters was er net een Amerikaans team van 18 man sterk in Burundi geweest! Voor dit team uit Bermuda ( projectchangebermuda. org/) had men grote tenten buiten op het ziekenhuisterrein in Bujumbura gezet Exploratory mission During De Andere Markt or The Other Market in 2008 in the Dutch village of Katwijk Interplast was approached by the chairman of Izere, an organisation representing the Burundi diaspora in The Netherlands, André Nkeshimana. André wondered whether Interplast was interested in setting up a project in Burundi as well. The result was an exploratory mission in June 2009 after a year of cooperating and making preparations. Team leader Rein Zeeman had carefully assembled his team made up of volunteers who had been with Interplast from the very beginning, the majority having almost twenty years experience of working in Africa. Reconstructive surgery is unknown To the surprise of the Interplast team an 18-man strong team of American medics had just visited Burundi. Large tents had been erected in the hospital grounds in Bujumbura for the US

23 Burundi 21 The team at their campsite en die waren nu uitstekend te gebruiken voor de grote hoeveelheid patiënten (en hun familie) met schisis (lipen/of gehemeltespleet). Zelfs de Minister van Gezondheid wist niet dat er zoveel schisis in zijn land voorkomt! Feit is dat plastisch reconstructieve chirurgie in Burundi tot voor kort volkomen onbekend was en dat veel volwassenen daar met een lipspleet een leven moeten zien op te bouwen. En als je er anders uitziet, valt dat in Afrika niet mee Met de bus De eerste week werd gewerkt in de hoofdstad Bujumbura, waar voornamelijk schisis-patiënten werden geopereerd. Er kon prima op twee tafels gewerkt worden. Het was de bedoeling om de tweede week in Gitega te werken, dus werd op maandag alles gepakt en daarheen gereden. Helaas werkte de anesthesieapparatuur er niet naar behoren en was het dus onverantwoord anesthesie te geven. Zo kon er niet geopereerd worden en dat terwijl er al honderden patiënten stonden te wachten op de komst van het team. Maar er kwam een oplossing: er werd overlegd met het ziekenhuis in Bujumbura of daar weer geopereerd kon worden. Er werd een bus gehuurd op het lokale bus station, die 33 schisis-patiënten naar Bujumbura vervoerde! group from Bermuda ( and they proved to be excellent accommodation for the huge numbers of patients, and their families, who had cleft lips and/ or palates. Even the Minister of Health was unaware that there were so many cases of cleft lips and palates in his country. In fact until recently plastic reconstructive surgery was totally unknown in Burundi and many adults with this defect had to manage to build a life for themselves as best they could. And in Africa, if you look different that s no easy matter With the bus The first week in the capital of Bujumbura was mainly devoted to patients with cleft lips and/or palates and all went well operating on two operating tables. The plan was to work the second week in Gitega so on Monday everything was packed and driven to Gitega. Unfortunately the anaesthetic equipment there was not working properly which would have made it irresponsible to administer anaesthetic. It was consequently impossible to operate even though there were hundreds of patients who had been waiting for the team s arrival. A solution was found however. The hospital in Bujumbura was consulted to see if it was possible to continue operating there. A bus was hired at the local bus station and 33 patients with cleft lips and/or palates were transported to Bujumbura. Burundi

Dit gaf hoop om tijdens een volgende missie meer aan het overdragen van kennis te kunnen doen dan nu. Passing on knowledge At the end of the second week in Bujumbura more and more doctors and nurses came to look and assist. This was a hopeful sign that in a subsequent mission more can be done to pass on knowledge than was possible this time. Uiteindelijk zijn in totaal 111 patiënten geopereerd, waarvan 86 met schisis. Het was een mooi gezicht om de geopereerde mensen op het ziekenhuisterrein naar zichzelf te zien kijken in spiegeltjes! In the end 111 patients were operated on, 86 of them with cleft lips and/or palates. It was a wonderful sight to see the operated patients looking at themselves in mirrors in the hospital grounds. Hierdoor zijn tijdens Interplast-bezoeken niet veel artsen beschikbaar om samenwerkend met het team al doende zelf het vak te leren. Maar elke beschikbare arts is er toch weer één, in dit Land van Duizend Heuvels, waar een Interplastteam eind 2009 in twee Rwandese ziekenhuizen goed werk heeft verricht. As a result during Interplast visits there are very few doctors available to assist and learn while working together with the team. But every one that is available is better than none in this Country of the Thousand Hills, where in late 2009 an Interplast team carried out good work in the two Rwandan hospitals. Leemte in organisatie gelukkig opgevuld De organisatie van deze Rwanda-missie leek begin 2009 onzeker, door het vertrek van Dr. Arjan Hogewoning, initiatief nemer en medeorganisator van de twee voorgaande missies. Gelukkig ontmoette onze officemanager Annemarie Maas begin 2009 de Rwandese Minister van Gezondheid, Dr. Richard Sezibera. Kort na die ontmoeting op de ambassade van Rwanda in Den Haag zegde de minister zijn medewerking voor de lokale organisatie toe. Interplast is een groot voorstander van samenwerking met lokale overheden. Gap in the organisation luckily filled The organisation of the Rwanda mission seemed uncertain in early 2009 as a result of the departure of Dr Arjan Hogewoning, the initiator and co-organiser of the two previous missions. Fortunately our office manager Annemarie Maas had met the Rwandan Health Minister, Dr Richard Sezibera, in early Shortly after that meeting at the embassy of Rwanda in The Hague, the minister committed himself to supporting the local organisation. Interplast is very much in favour of cooperating with local authorities.
