4 2 Van de Voorzitter From the Chairman Vanuit Stichting Interplast Holland is er in 2007 weer veel in gang gezet! Rwanda en Guinee-Bissau zijn toegevoegd aan het rijtje landen waar we projecten opzetten. Ghana echter laten we los omdat er genoeg andere organisaties zijn die daar werk verrichten. Once again Stichting Interplast Holland got a lot of things moving in 2007! Rwanda and Guinea-Bissau have been added to the list of countries where we have set up projects. But we ve withdrawn from Ghana because there are plenty of other organisations doing good work there. In Oeganda is het Brandwondencentrum nu compleet met een intensive care unit met 8 bedden, een brandwondenafdeling met 20 bedden, een dieetkeuken, een operatiekamer, een trainingslokaal en een kleine bibliotheek. De afdeling functioneert en is geïntegreerd in het Mulago University Hospital. In Jemen zijn de drie chirurgen bezig een eigen afdeling reconstructieve chirurgie op te zetten. We hopen dat de Nederlandse ambassade in dit land het initiatief gaat ondersteunen en het mede mogelijk maakt dat artsen en verpleegkundigen vanuit Jemen naar Nederland kunnen komen voor verdere opleiding en training. Er werd in 2007 ook weer een missie naar Soemba, Indonesië uitgevoerd waarbij behalve veel patiëntjes met schisis (lip- kaak en gehemeltespleet) ook vijftig patiënten met struma (vergrote schildklier) werden geopereerd. In samenwerking met het Trust Fund for Victims (of war) van het International Criminal Court (ICC) werd een speciale korte trip gemaakt naar Gulu, Noord-Oeganda om slachtoffers van de oorlog met de rebellen van de LRA te opereren. En na bijna twee jaar in Oeganda te hebben gebivakkeerd, is de voorzitter van Interplast Holland weer helemaal terug! Hij zal zich het komende jaar intensief met Interplast bezig houden, maar ook op zoek gaan naar een opvolger! Ondertussen is er in 2008 ook al weer heel wat werk verricht. De website van de stichting wordt vernieuwd zodat de site optimaal kan worden gebruikt als communicatiemiddel. Kijk daarom regelmatig op Zoals gebruikelijk sluit ik mijn voorwoord af met een blijk van waardering. Waardering voor alle vrijwilligers die, op welke manier dan ook, in 2007 aan Interplast hebben bijgedragen. In een soms harde en ruwe wereld voelt het goed dit werk te mogen doen! Dat wij als Interplast iets voor onze patiëntjes kunnen betekenen, is weer te danken aan al die mensen die in ons werk geloven. Dankzij onze sponsors toveren we bij honderden kinderen en hun ouders uiteindelijk een lach op het gezicht. Zij zijn intens dankbaar en zien een nieuwe toekomst tegemoet. In Uganda the Burns Unit is now complete with an intensive care unit of eight beds, a burns ward with 20 beds, a diet kitchen, an operating theatre, a training room and a small library. The whole is functioning and has been integrated into Mulago University Hospital. In Yemen the three surgeons are working on setting up their own department of reconstructive surgery. We hope that the Netherlands Embassy in Yemen will support this initiative and help in ensuring that doctors and nurses from Yemen can come to the Netherlands for further courses and training. Another mission to Sumba in Indonesia was also carried out in 2007 and many young patients with cleft lips and palates were operated as well as fifty goitre patients. A special short trip was made to Gulu in North Uganda in conjunction with the Trust Fund for Victims of the International Criminal Court (ICC). The team operated on the victims of the war with the rebels of the Lord s Resistance Army. Having spent almost two years in Uganda the chairman of Interplast Holland is back home! This year he s going to be closely involved with Interplast activities, but he s also looking for a successor! Meanwhile in 2008, too, a lot has already been done. The organisation s website has been renewed so that it can be used to communicate optimally with our supporters and well-wishers. Take a regular look at As usual I conclude my foreword with an expression of appreciation. Appreciation for all the volunteers who have helped Interplast in 2007 in whatever way. In a world that is sometimes rough and hard we feel privileged that we can do this work! That we as Interplast can mean something to our young patients is in turn thanks to all those people who believe in what we are doing. It is thanks to our sponsors that in the end we manage to bring a smile to the faces of hundreds of children and their parents. 5 Inleiding Introduction Stichting Interplast Holland is een ideële organisatie die zich richt op reconstructieve chirurgie voor kinderen en (jong) volwassenen in ontwikkelingslanden. Jaarlijks wordt een aantal teams van ervaren Nederlandse plastisch en reconstructief chirurgen, anesthesiologen en operatieen anesthesieassistenten uitgezonden. De artsen en assistenten, die ter plekke worden bijgestaan door lokale artsen en verpleegkundigen, zetten zich hiervoor kosteloos tijdens hun vakanties in. Zij bieden hulp door: het uitvoeren van kosteloze reconstructieve operaties bij kinderen en (jong) volwassenen met lip- en gehemeltespleten, brandwondverminkingen, tumoren en andere aangeboren afwijkingen. het opleiden van lokaal medisch personeel in de reconstructieve chirurgie, anesthesiologie en verpleegkundige technieken. te helpen bij het opzetten van brandwondenafdelingen en/of units voor reconstructieve chirurgie binnen de bestaande ziekenhuizen. Om duurzame resultaten te bereiken, wordt intensief samengewerkt met lokale ziekenhuizen, artsen, verpleegkundigen, universiteiten en hulporganisaties. Interplast is een internationale organisatie die in 1969 in de Verenigde Staten ontstond. In 1990 werd Stichting Interplast Holland opgericht. In vele andere landen is Interplast inmiddels vertegenwoordigd. Interplast Holland is als stichting onafhankelijk van haar zusterorganisaties. Stichting (Foundation) Interplast Holland is a charitable organisation that performs reconstructive surgery for children and young adults in developing countries. Each year the organisation sends teams of experienced plastic and reconstructive surgeons, anaesthesiologists and operating theatre assistants on a number of missions to developing countries. The doctors and assistants, who are assisted by local doctors and nurses, travel during their holidays and provide their services free of charge. Their assistance extends to: performing reconstructive operations free of charge for children and adults with cleft lips or palates, mutilations caused by burns, tumours and other congenital skin defects. training local medical personnel in reconstructive surgery, anaesthesia and nursing techniques. helping to set up burns and reconstructive surgery units in hospitals. To secure lasting results Interplast Holland works intensively with local hospitals, doctors, nurses, universities and NGOs. Interplast is an international organisation which was founded in the United States in Stichting Interplast Holland was founded in By now Interplast is represented in many other countries. As foundation Interplast Holland is independent from its fellow Interplast organizations. They are regarded as sister organisations. Interplast Holland richt zich momenteel voornamelijk op Jemen, Nigeria en Oeganda. Naar deze landen vinden jaarlijks missies plaats. Ook in Indonesië is Interplast Holland actief. Regelmatig worden missies naar dit land georganiseerd. In het verleden werden teams uitgezonden naar Vietnam, Pakistan, India, Bhutan, Tibet en Libanon. Ook vinden regelmatig verkennende bezoeken plaats aan nieuwe locaties, zoals in 2007 aan Rwanda. Stichting Interplast Holland currently concentrates its activities in Yemen, Nigeria and Uganda. Missions to these countries take place every year. Interplast Holland is also active in Indonesia where a mission is organized once every two or three years. In the past teams have also been sent to Vietnam, Pakistan, India, Bhutan, Tibet and Lebanon. Preparatory visits to new locations take sometimes place and in 2007 a visit was paid to Rwanda. Teamuitzendingen In 2007 vonden zes reguliere Interplast missies plaats: twee naar Nigeria, twee naar Jemen, één naar Indonesië en één naar Rwanda. Een zevende missie had een uniek karakter dankzij de samenwerking met het Trust Fund for Victims (of war) van het International Criminal Court. Werkte Interplast eerder samen met Artsen Zonder Grenzen in het onrustige noorden van Oeganda om ernstig verminkte slachtoffers van de rebellen daar te opereren, nu kwam de hulpvraag vanuit het Trust Fund. Team missions Six regular Interplast missions took place in 2007, two to Nigeria, two to Yemen, one to Indonesia and one to Rwanda. A seventh mission was a unique event in which Interplast cooperated with the Trust Fund for Victims of the International Criminal Court. Previously Interplast had worked together with the Dutch Doctors without Borders organisation in the turbulent north of Uganda, operating on seriously disfigured victims. In this case it was the Trust Fund that asked for help.

6 I n l e i d i n g Uganda Burns and Plastic Surgery Institute (UBPSI) Mijlpaal bereikt, steun blijft nodig Begin 2007 werd, na een grondige verbouwing, de tweede afdeling van het Brandwondencentrum in gebruik genomen. Met deze afdeling, Holland Ward genoemd, is het eerste gespecialiseerde brandwondencentrum van Oeganda een feit geworden! Hoewel het centrum nu volledig operationeel is, blijft steun vanuit Nederland de komende jaren hard nodig. Zeker op het gebied van scholing van de verpleegkundigen is nog veel te bereiken. Om een trainingsprogramma vorm te geven, gaat Interplast Holland in 2008 een samenwerkingsverband met de Nederlandse Brandwonden Stichting aan. Uganda Burns and Plastic Surgery Institute (UBPSI) A milestone achieved but support is still needed In early 2007, after thorough renovation, the second ward of the Burns Unit became operational. With this ward, the Holland ward as it is called, the first specialist burn centre in Uganda has come into being. Although the centre is fully operational, support from the Netherlands in the coming years is still badly needed. Much still has to be achieved especially in the field of training nurses. Interplast Holland is hoping to enter into a partnership with the Nederlandse Brandwondenstichting (Dutch Burns Injuries Foundation) in 2008 to give shape to a training programme. Visie Visionary goal Stichting Interplast Holland beoogt een wereld waarin ook gehandicapte kinderen en volwassenen in ontwikkelingslanden voldoende kansen krijgen fysiek en geestelijk volledig te participeren in een gezonde en rechtvaardige samenleving. Mangu ligt in het noorden van Nigeria op het Jos Plateau. In de periode dat de arts daar woonde en werkte, heeft hij een belangrijke bijdrage geleverd aan de bouw van een revalidatiecentrum en een school voor gehandicapten naast het ziekenhuis. In zijn Friese woonplaats Oentsjerk is de Stichting Faridpur actief die (onder meer) Cees Spronk in zijn opbouwwerk steunt. Stichting Interplast is gevraagd om dit project te ondersteunen. Zowel in januari als in oktober 2007 vertrok een team naar het Jos Plateau. In totaal zijn er 150 patiënten geopereerd. Years ago, plastic surgeon Cees Pronk worked as a leprosy doctor in Mangu. Mangu is in the north of Nigeria on the Josh plateau. While he lived there he had a major hand in the building of a convalescence centre and a school for handicapped children next to the hospital. The Faridpur Foundation in the Frisian town of Oentsjerk where Cees Spronk lives continues to support him and others in this work. Interplast was asked to join in the project. In January and October 2007 a team went to the Jos plateau where altogether they operated on 150 patients.

Nu wordt het gedragen door de Church of Christ in Nigeria. Op chirurgisch gebied gebeurt er gewoonlijk niet veel in Mangu. Dat is ook te zien aan de operatiekamers, leert de ervaring. Annet Brouwer, die in januari als OK-assistent meeging, vertelt: Wat me als eerste opviel is dat er niet netjes was opgeruimd op de operatiekamer. Overal lag stof. Kasten puilden uit van de oude instrumenten en hechtmaterialen. Dat het primitief zou zijn had ik wel verwacht. Maar van die spullen schrok ik toch wel. Nicely cleared away COCIN Hospital and Rehabilitation Centre in Mangu is a former hospital belonging to the Dutch Leprosy Foundation. It is now supported by the Church of Christ in Nigeria. Normally speaking little surgery is done in Mangu. This is all too obvious from the operating theatres. Annet Brouwer, who joined the team in January as an operating theatre assistant has this tale to tell: The first thing I noticed was that the operating theatre was a bit of a mess. There was dust everywhere. Cupboards were bulging with old instruments and surgical materials. I had expected it to be primitive but they had not cleared things away nicely. Ik begon alles op te ruimen. Kasten leeghalen, kapotte instrumenten weggooien, oorkruipers naar buiten gooien en ga zo maar door. Wat ik niet allemaal tegenkwam. Oud hechtmateriaal dat van ellende uit elkaar viel, bijvoorbeeld. Het fijne was dat het personeel van het ziekenhuis het zo leuk vond dat alles er plotseling zo netjes bij lag. Als ik nog een keer naar Mangu kan, ga ik hiervoor zeker een plan van aanpak maken. Voor Annet was Mangu haar eerste missie. Ik heb mijn ogen uitgekeken, veel gezien wat verbeterd kan worden, maar ook veel geleerd van de mensen daar. Het waren allemaal zulke hartelijke en warme mensen! I started by clearing away the mess. Old surgical suture material that fell apart when you touched it, for example. The wonderful thing was that the staff in the hospital were really thrilled that everything suddenly was so neat and tidy. If I get the chance to go to Mangu again I am certainly going to make a plan to tackle this. Mangu was Annet s first mission. My eyes popped out on stalks. I saw so much that can be improved but I also learned a lot from the people there. All of them were so welcoming and warm-hearted. Standards have to improve The first and the second mission were successful, though both teams were quite clear that much still has to be done in the hospital. In a talk with the hospital s management team we emphasized that standards had to improve, says anaesthetist Rob Steur, who was in Nigeria both in January and October. Learning moment This first mission is one long learning moment. For example I had to work in circumstances that you would never find in the Netherlands. Unlike the other shy children she had a wide smile and secretly she rather stole my heart. Interplast An organisation that offers medical help to people who need it. One last word I am thankful that I was part of the team. The mission made a deep impression on me. In Mangu alone we screened 200 patients on the first day and operated 60! Ook de komende jaren is er nog veel te doen in het noorden van Nigeria. De stroom patiënten lijkt soms onophoudelijk te zijn. Als mensen door wat voor reden dan ook niet in aanmerking voor een operatie komen, is dat soms moeilijk voor hen. Rob Steur: Dat hebben we wel gezien. Het leidde soms tot heftige taferelen. Je probeert het allemaal uit te leggen, maar toch. That applies especially to surgery and anaesthetics. It would mean that more people would come and work there and more patients could be treated. In the coming years, too, there s plenty of work to be done in the north of Nigeria. The influx of patients sometimes seems endless. If people for whatever reason cannot be operated on, this is sometimes very difficult for them to accept. Rob Steur: We have seen this happen. Sometimes fierce scenes occur. You try to explain, but it is not always easy. Transportproblemen In januari lieten sommige patiënten het echter zelf afweten. Cees Spronk schreef daarover in zijn verslag: In het weekend blijkt dat er een toenemend benzinetekort in het land is. Weinig benzinestations zijn open, er worden woekerprijzen gehanteerd en er bestaan lange wachtrijen. Hierdoor komen er in de week die volgt steeds minder patiënten. Sommigen die voor een operatie staan ingepland komen niet of veel te laat opdagen. Ondanks dat zijn onze dagen goed gevuld. Transport trouble But in January some of the patients themselves failed to turn up. Cees Pronk wrote about it in his report. At the weekend we discovered that there was a growing petrol shortage in the country. Few petrol stations were open, astronomical prices were being charged and there were long queues. The result was that in the following week fewer and fewer patients turned up. Some of them who were scheduled for an operation failed to arrive or arrived much too late. Despite that the days were extremely busy. N i g e r i a

De Stichting Faridpur doneerde per patiënt vijftig euro zodat het voor deze arme bevolkingsgroep toch nog meeviel. Tamtam Het oktober-team heeft ook gewerkt in het Yadakunya Hospital in Kano. Dit was op verzoek van de Nederlandse Stichting voor Leprabestrijding. De autoriteiten grepen het bezoek van Interplast aan om flink wat tamtam te maken. In het Yadakunya Hospital was juist een nieuwe afdeling geopend. De minister van gezondheid van Kano stond klaar om het team te ontvangen, samen met journalisten van kranten, radio en televisie. Ook de Nederlandse ambassadeur Arie van der Wiel kwam langs. Ondertussen begonnen de vrijwilligers aan het werk waarvoor ze waren gekomen: mensen helpen. Although the Interplast team operated for free and there were barely any expenses for the hospital, the hospital nevertheless charged the patients for their stay. The Faridpur Foundation donated fifty euros for each patient to make the treatment affordable for even the poorest group in the community. PR event The October team also worked in the Yadakunya Hospital in Kano. They went there at the request of the Dutch Leprosy Foundation. The authorities saw the visit as an opportunity for a PR event. The fact was that a new ward had just been opened in the Yadakunya Hospital. The Health Minister of Kano was ready and waiting to receive the team along with journalists from the local newspaper, radio and television. The Dutch ambassador Arjan van der Wiel was also present. Meanwhile the volunteers set to work on the task they had come for: helping people. Dat bleek uiteindelijk niet mee te vallen. Yadakunya is een lepraziekenhuis. Van de operatiekamer wordt doorgaans weinig gebruik gemaakt. Het team moest werken onder moeilijke omstandigheden. Toen de generator van het ziekenhuis het ook nog eens begaf en de temperatuur in de operatiekamer steeg tot boven de veertig graden Celcius, moest het team stoppen. De sterilisator deed het inmiddels ook niet meer. Toch zijn in het Yadakunya ziekenhuis nog 56 patiënten geopereerd, waarvan tien mensen met lepra die een slechte hand- of voetfunctie hadden en 27 kinderen met lipspleten. That turned out to be a little more difficult than expected. Yadakunya is a leprosy hospital. Normally speaking the operating theatre is used very little. The team had to work in difficult conditions. When, moreover, the hospital s generator failed suddenly as well and the temperature in the operating theatre rose to above 40 Celsius, the team too had to stop. Meanwhile the steriliser was not working either. Even so, 56 patients were operated on in the Yadakunya Hospital, ten of whom had leprosy and could barely use their hands or feet, and 27 children with cleft lips. N i g e r i a

Het Arabische land, dat in het uiterste zuiden van het Arabisch Schiereiland ligt, kampt met een tekort aan goed opgeleid personeel en een gebrek aan medisch materiaal en medicijnen. Artsen en verpleegkundigen verdienen in vergelijking met andere landen slecht en aanvaarden, als ze de kans krijgen, een betrekking elders. Ondanks de wetgeving die gratis gezondheidszorg voor de allerarmsten in het land garandeert, moeten patiënten wel degelijk voor hun ziektekosten betalen. Al met al een weinig rooskleurige situatie. Toch klinken er ook optimistische geluiden uit het land waarmee Interplast Holland al enige tijd een band heeft. The health care situation in Yemen is still rather difficult. The Arab country in the deep south of the Arab peninsula has a shortage of well-trained personnel and a lack of medical materials and medicines. Doctors and nurses earn little by comparison with other countries and if they get the chance they accept posts elsewhere. Despite legislation which guarantees free health care for the poorest in the country, patients nevertheless still have to pay hospital fees. Altogether a fairly dispiriting situation. Even so there are some optimistic signs in the country with which Interplast Holland has had ties for some time now.

12 10 J e m e n Beroemdheid Rein Zeeman: We hebben de afgelopen jaren geïnvesteerd in de opleiding van een aantal collega s in Jemen. Dat betaalt zich nu zeker terug. Toen we in april in Sana a waren, stonden er geen hordes patiënten met lipspleten meer op ons te wachten. Dat was een paar jaar geleden wel anders. We waren dit jaar vooral nodig voor de meer gecompliceerde zaken. Ahmed Ammari doet de schisischirurgie zo goed, dat hij in Jemen een beroemdheid is geworden. Inmiddels werken er nu drie plastisch chirurgen in het Al Thawra Modern General Teaching Hospital. Dokter Ammari is door Interplast opgeleid. Ahmed Ammari en zijn collega s hebben nog geen eigen afdeling in het ziekenhuis, maar voor de plastische chirurgie worden wel vaste operatiedagen ingeroosterd. In 1997 vloog er voor het eerst een Interplast-team naar Sana a. In vergelijking met die eerste missie zijn er grote resultaten geboekt in het opleidingsziekenhuis. Toch zijn we er nog lang niet, vindt Rein Zeeman. We moeten de opleiding nog verder verbreden. Uiteindelijk zijn we dan niet meer nodig in Jemen. In totaal werden door Interplast in april 54 operaties gedaan. Het bewijs dat we Jemen nog niet kunnen loslaten. Kennisoverdracht In april werd overigens ook het Al Gmhuriah Teaching Hospital in de havenstad Aden bezocht. Een groot verschil met het ziekenhuis in de hoofdstad. Vier jaar geleden ben ik ook in Aden geweest, vertelt Rein Zeeman. Ik zag dat er nieuwe lampen in de operatiekamer hingen en dat er een nieuwe anesthesiemachine stond maar verder was het ziekenhuis in deplorabele staat. Toch heeft het team, na een wat haperende start, hier lekker kunnen werken. De lokale artsen waren zo enthousiast dat ze ons graag terugzien. Diezelfde geluiden klonken in december in het Seiyun General Hospital, waar een hele ochtend werd ingeroosterd voor lezingen en trainingen. Dat werd door het personeel, dat in groten getale aanwezig was, enorm gewaardeerd. Celebrity doctor Rein Zeeman: In the past years we have invested in the training of a number of colleagues in Yemen. We are now seeing the fruits of this investment. When we were in Sana a in April there were no longer huge numbers of patients with cleft lips waiting for us. A few years ago the situation was quite different. This year we were needed above all for the more complicated cases. Ahmed Ammari performs cleft surgery so well that he has become a celebrity doctor in Yemen. Meanwhile three plastic surgeons are working in the Al Thawra Modern General Teaching Hospital. Doctor Ammari was trained by Interplast. Ahmed Ammari and his colleagues still have no ward of their own in the hospital but set operating theatre days are scheduled for plastic surgery. It was in 1997 that the first Interplast team flew to Sana a. By comparison with the first mission enormous progress has been made in the teaching hospital. But we still have a long way to go, says Rein Zeeman. We have to expand the training further. In the end we will no longer be needed in Yemen. Altogether 54 operations were performed by Interplast in April. Proof that we cannot relinquish Yemen yet. Transfer of knowledge In April, too, the team visited the Al Gmhuriah Teaching Hospital in the port of Aden. A big difference with the hospital in the capital. I was in Aden four years ago, says Rein Zeeman. We saw that new lamps were hanging in the operating theatre and that there was a new anaesthetic machine, but otherwise the hospital was in a deplorable state. Even so the team worked here well after a somewhat hesitant start. The local doctors were so enthusiastic that they wanted to see us come back. The same response was heard in December in the Seiyun General Hospital, where a whole morning had been scheduled for lectures and training. The staff came in huge numbers and were enormously appreciative. In december werden door Interplast in totaal 60 patiënten geopereerd. In December Interplast operated a total of 60 patients. De specialismedeskundige, die in Nederland lesgeeft aan OK-assistenten in opleiding, verbleef in 2003 enkele maanden in Jemen om daar haar vak te doceren. Dus kwam ze dit jaar in de ziekenhuizen oud-leerlingen tegen die niet zijn vergeten wat Greta Hesseling hen allemaal heeft geleerd en de Nederlandse erg hoog hebben staan. An expert who teaches operating theatre assistants in the Netherlands spent several months in Yemen in 2003 to pass on her expertise. So this year she met former students in the hospitals who had not forgotten what Greta Hesseling had taught them and how much they owed her. Vanuit de Kleine Soenda-eilanden van Indonesië was namelijk een dringende hulpvraag binnengekomen. Men had een arts nodig die strumapatiënten kon opereren (struma is een schildklierafwijking). Paula Ellen, een ok-assistente die al jaren vrijwilliger bij de organisatie is, kende een geschikte chirurg. Zo werd Robbert Vree benaderd. Hij was meteen enthousiast. Logistiek gezien, was het best een hele klus om de missie te organiseren. Soemba is een betrekkelijk geïsoleerd eiland. Maar het resultaat maakte alles goed. In totaal konden 112 patiënten (waaronder vijftig met struma) worden geholpen door het team dat keihard heeft gewerkt. The mission to the dry and mountainous island of Sumba, formally known as the Sandlewood Island, comes under the heading of one of those unique experiences. Normally the Interplast team comprises plastic surgeons, anaesthetists and their assistants. This year there was another specialist in the group. An urgent call for help came from the Lesser Sunda Islands in the Indonesian archipelago for a doctor who could operate on goitre patients (goitre is a thyroid disorder). Paula Ellen, an operating theatre assistant, who has been a volunteer with the organisation for years now, knew a suitable surgeon. Robbert Vree was approached and he was immediately enthusiastic. Logistically speaking, organising the mission proved to be quite a job. Sumba is a relatively isolated island, but the result made everything worthwhile. Altogether a total of 112 patients, including 50 with goitre, were operated on by the team who worked very hard indeed. De missie werd uitgevoerd in samenwerking met Stichting Rena (die zich expliciet richt op kinderen in Indonesië die geboren zijn met een lip- en/of gehemeltespleet) en de Stichting Yayasan Sinar Pelangi van de Nederlandse Zr. 15 I n d o n e s i ë S o e m b a 13 Overweldigend onthaal De reis was vermoeiend, zo staat te lezen in de verschillende verslagen die over deze missie zijn gemaakt. Bij de hartelijke ontvangst in het Karitas Ziekenhuis in Weetabula was iedereen dat meteen vergeten. Personeelsleden en patiënten stonden het team op te wachten. Met inheemse dans en grote spandoeken werden de Nederlanders welkom geheten. Overweldigd door het onthaal én de grote hoeveelheid patiënten, besloot het team maar meteen aan het werk te gaan. Zo n driehonderd patiënten, waaronder veel volwassenen met struma en kinderen met lip- en/of gehemeltespleet, hoopten in aanmerking te komen voor een operatie. Zoals bij elke missie kon ook hier niet iedereen worden geholpen. Vanwege de lichamelijke conditie of simpelweg omdat de werkomstandigheden daarvoor te beperkt waren. Uiteindelijk zat het operatieschema ook vol en kon er dus niemand meer bij. Tremendous reception The journey was tiring as can be gathered from the various reports on the mission. But everyone soon forgot their fatigue with the warm reception that awaited them in the Karitas Hospital in Weetabula. Staff and patients stood waiting for the team. The Dutch group was welcomed with local traditional dances and huge banners. Overwhelmed by the welcome and the huge number of patients, the team decided to set to work immediately. Around 300 patients, including many adults with goitre and children with cleft lips or palates, hoped to be eligible for an operation. As is the case with each mission not everyone can be treated. This may be because of their physical condition or simply because the working conditions are unsuitable. In the end the operating schedule was full and there was no room for more. For days on end the team operated in a single operating theatre on two tables. On the one side the schisis patients and on the other side those with goitre. The team had to improvise considerably to do their work well, certainly once the anaesthetic equipment failed. Naar schatting zijn toen een miljoen mensen in een periode van honderd dagen vermoord. Het land werkt nog steeds aan zijn wederopbouw. Richard Koch en zijn team ontdekten dat zich tegen deze onmenselijke achtergrond positieve ontwikkelingen aftekenen. Dat hebben we ondanks de moeilijke omstandigheden waarin we moesten werken, zeker kunnen ervaren in het ziekenhuis in Kigali. In het Centraal Ziekenhuis van Kigali (CHK), zo geeft de plastisch chirurg aan, werkt medisch personeel dat graag de kwaliteit van de gezondheidszorg in Rwanda wil verbeteren. De kennis van de plastische en reconstructieve chirurgie is nog zeer gebrekkig. De artsen zijn daardoor terughoudend bij de behandeling van kinderen met ernstige brandwonden. Ook schisis-patiëntjes worden niet snel geopereerd. Daarnaast kunnen de verpleegkundigen en het personeel in de operatiekamers extra scholing gebruiken. Richard Koch hoopt dat Interplast Holland een meerjarig trainingsprogramma wil ontwikkelen en aanbieden. Als het aan de Rwandese artsen ligt, wordt zo n initiatief met open armen ontvangen en breed ondersteund. The daily life in Rwanda is still marked by the genocide or Rwandan civil war that took place in It is estimated that at the time a million people were murdered in a period of one hundred days. The country is still working on recovery. Richard Koch and his team discovered that in the wake of this inhuman tragedy positive developments are beginning to show. We certainly felt this in the hospital in Kigali, despite the difficult conditions in which we had to work. The plastic surgeon says that there are medical staff working in the Central Hospital in Kigali CHK who would very much like to improve the quality of health care in Rwanda. Knowledge of plastic and reconstructive surgery is still poor. As a result doctors are reluctant to treat children with serious burns. Hardly any schisis patients are operated on. Moreover, nurses and staff in the operating theatres need extra training. Richard Koch hopes that Interplast Holland will develop and offer a multi-year training programme. If it is left to Rwandan doctors such an initiative would be welcomed with open arms and broadly supported. CHK, as one of the major hospitals in the country, is the obvious spot to set up a programme of this kind.

17 R w a n d a 15 Als één van de belangrijkste ziekenhuizen in het land, is het CHK de aangewezen plek om het programma op te zetten. Perfect Het was voor het eerst dat Interplast een team naar Rwanda stuurde. Toch was er van een verkennende missie nauwelijks sprake. De Nederlandse arts Arjan Hogewoning, die in Rwanda woont, had alles perfect voorbereid. Wat niet wil zeggen dat alles even soepel ging. Het team liep wel tegen een paar problemen op. Richard Koch: Voor het OKpersoneel in Rwanda bestaat geen specifieke opleiding. Een verpleegkundige kan zomaar aan het einde van haar opleiding te horen krijgen dat ze als chirurgieassistente wordt ingezet. Zonder dat ze ook maar iets weet van sterilisatie, instrumenten en operatietechnieken. Daar stond tegenover dat het lokale team wel erg leergierig was. Zo zijn we geleidelijk op het idee gekomen dat Interplast een meerjarenplan moet ontwikkelen voor een opleidingsprogramma. Dan worden artsen, evenals de assistenten, door Interplastteams ingewijd in de basale technieken van de plastische en reconstructieve chirurgie. We hebben vast een grote stapel boeken en instructiemateriaal achtergelaten. Ruhengeri Naast het CHK, bezocht het team ook het districtsziekenhuis in Ruhengeri. Daar viel direct op dat het ziekenhuis slecht geoutilleerd was. Een gebrek aan werkelijk alles! Perfect It was the first time that Interplast had sent a team to Rwanda. Even so this was barely a fact-finding mission. The Dutch doctor Arjan Hogewoning, who lives in Rwanda, had prepared everything perfectly. Though that is not to say that everything went smoothly. The team did come up against a number of problems. Richard Koch: There is no specific training for operating theatre personnel in Rwanda. Nurses at the end of their training may discover that they are assigned to a job as a surgical assistant without knowing anything about sterilisation, instruments and operating techniques. But on the other hand the local team was very keen to learn. And that is how we gradually came up with the idea that Interplast should develop a multi-year plan for a training programme. That would mean that doctors, as well as assistants, would be trained in the basic techniques of plastic and reconstructive surgery by Interplast teams. We left behind a huge pile of books and instructional material. Ruhengeri Besides the CHK the team also visited the Ruhengeri district hospital. They immediately noticed how badly equipped the hospital was. There was a shortage of absolutely everything. Money, infrastructure, medical knowledge in the broadest sense of the word, you name it. One last word I hope I can keep on going on missions and that I can do a lot of good. Geld, infrastructuur, medische kennis in brede zin en noem maar op. We hebben er schrijnende gevallen gezien. Zoals een brandwondenpatiëntje dat al twee jaar op bed ligt met open wonden aan de benen. Door een dwangstand van de heupen en knieën kan het kind onmogelijk lopen. Het werd het team wel duidelijk dat het kind, een meisje, niet in Rwanda geholpen kan worden. Ze moet naar het brandwondencentrum in Kampala, aldus Richard Koch. Ik hoop dat het Interplast lukt om voor dit kind extra fondsen te werven om het meisje in Oeganda te laten behandelen. Het werk in Ruhengeri verliep niet helemaal zoals het team zich dat had voorgesteld. Er was wel op onze komst gerekend, maar we kregen niet de indruk dat er echt ruimte en mankracht voor ons was vrijgemaakt. Waarschijnlijk had dit te maken met personeelsgebrek. Maar wij komen niet alleen om te opereren, we willen ook personeel opleiden. Het bestuur van Stichting Interplast Holland gaat onderzoeken of het inderdaad mogelijk is voor Rwanda een opleidingsplan te ontwikkelen. We saw devastating cases. For example a burns injury patient who has been lying in bed for two years with open wounds on her legs. Through contracture of the hips and knees the child is completely unable to walk. It was clear that the young girl could not be operated on in Rwanda. We had to get her to the Burns Unit in Kampala, said Richard Koch. I hope that Interplast will be able to raise additional funds for this child so that she can be operated on in Uganda. The work in Ruhengeri did not proceed entirely as the team had expected. Our arrival had been planned, but we did not get the impression that sufficient space and manpower had been made available for us. This was probably because of lack of staff. But we are not just there to operate, we also want to train people. The Board of the Interplast Holland Foundation is going to ascertain whether indeed it is possible to develop a training plan for Rwanda. R w a n d a

Het Verzetsleger van de Heer (LRA), met Joseph Kony aan het hoofd, heeft in de loop der jaren in de noordelijke streken van de parel van Afrika honderden mensen doelbewust en wreed verminkt. Het leger schroomt niet om de lippen, oren, neuzen en vingers van zijn slachtoffers af te snijden. Rein Zeeman kan écht iets betekenen voor de verminkte rebellenslachtoffers. Het Trust Fund for Victims, opgericht in de schoot van het International Criminal Court, verleent bijstand aan de meest kwetsbare slachtoffers van genocide, misdaden tegen de menselijkheid en oorlogsmisdaden. Dat wordt niet gedaan met geld, maar met middelen en mogelijkheden. Het was overigens niet de eerste keer dat Rein Zeeman in het Mary s Hospital in Lacor (district Gulu) stond om rebellenslachtoffers te opereren. Wel nieuw was dat de missie onder auspiciën van het Trust Fund plaatsvond. Plastic surgeon Rein Zeeman, who is the founder of the Dutch Interplast Holland Foundation, has already led dozens of missions and was approached by the Trust Fund for Victims asking whether he was prepared to operate on a number of rebel victims in the north of Uganda. The Lord s Resistance Army, the LRA, led by Joseph Koni, has deliberately and cruelly disfigured hundreds of people in the course of the years in the northern parts of the pearl of Africa. The army does not hesitate to cut off lips, ears, noses and fingers of victims. Rein Zeeman can really mean something for the disfigured victims of the rebels. The Trust Fund for Victims, founded under the auspices of the International Criminal Court, offers support to the most vulnerable victims of genocide, crimes against humanity and war crimes. The help is not just in the form of money but in the form of resources and opportunities. It was otherwise not the first time that Rein Zeeman had operated on rebel victims in St. Mary s Hospital in Lacor in the district of Gulu. What was new was that the mission now was taking place under the auspices of the Trust Fund. Screening via internet Om zoveel mogelijk te kunnen doen in het doorgaans gevaarlijke district, werd aan twee lokale organisaties (Caritas en Ayinet) gevraagd om vooraf patiënten te selecteren. Internet screening To be able to do as much as possible in what is normally a dangerous district two local organisations, Caritas and Ayinet, were asked to select patients beforehand.

20 18 O e g a n d a U B P S I Via internet werden foto s van patiënten naar het Interplastkantoor in Leiden gestuurd. Tot verbazing van Rein Zeeman zag hij de patiënten uit de pré-screening niet terug op de eerste werkdag van zijn missie. Een punt van aandacht, maar op dat moment had ik geen tijd om me daar verder druk over te maken. Photographs of patients were sent via Internet to the Interplast office in Leiden. Rein Zeeman was surprised when he failed to see the patients from the pre-screening on his first working day of the mission. I made a mental note but right then I had no time to make an issue of it. Het werd hem al snel duidelijk dat honderden mensen in het district een medische behandeling nodig hebben, maar dat lang niet iedereen ook voor de specifieke specialistische hulp van Interplast in aanmerking komt. Als ik een orthopedisch chirurg had meegenomen, had die ook veel goed werk kunnen doen. Neusreconstructie Nose reconstruction It rapidly became clear to him that hundreds of people in the district needed medical treatment but that by no means everyone was eligible for the specific specialist help that Interplast could offer. If I d taken an orthopaedic surgeon with me, he would have been able to carry out a lot of good work. Uiteindelijk opereerde Rein Zeeman dertig van de ruim vijftig patiënten die hij had onderzocht. Daarbij ging het in zeven gevallen niet om een plastische of reconstructieve operaties maar om gewone chirurgische ingrepen. Zoals het verwijderen van kogels. Marjo Aerts heeft in 2005 ook een paar maanden in het brandwondencentrum gewerkt.
