3 Instructions for use Ossur Therapeutic Shoes ENGLISH Why these shoes? These ready-made special shoes are primarily designed for diabetics with severe foot ulcers or where the shape of the foot has changed making the use of ordinary shoes impossible. They are called therapeutic shoes because they are part of the treatment in the same way as drugs and dressings. You could almost say they are part of the dressing. In most cases they are used for a limited period and later on you can go back to more normal shoes. Why do they look so different? The shoes are for cases where ordinary shoes cannot be worn. They are higher and wider at the front to allow room for inlays, bandages and, in some cases, crooked toes. There are a few different makes available but they are all designed in the same way. The differentiating feature is the appearance of the upper. There are ordinary shoes, sandals and boot models. Why are they so stiff? The sole is stiff and rounded like a clog. The aim is to prevent the foot moving around in the shoe. In an ordinary shoe, when the front part bends, the foot always moves in relation to it and this can cause chafing and pressure. Why are they so heavy? This is an optical illusion! They are actually lighter than most other shoes. Compare them with your ordinary shoes. The shoes look heavy because they are wider and higher at the front. Could I wear something different instead? Regrettably, there are not many alternatives. Plaster, as used for fractures, is one way and is used with the more serious ulcers and often after operations on the foot. Many people with therapeutic shoes have had plaster first. It sounds grim to say so, but it can be necessary to amputate toes or parts of the foot if the therapeutic shoes are not worn. Crutches and complete pressure relief, i.e. freeing the foot from any contact with the ground is another possibility, but it makes walking laborious and uncomfortable. Hand-made shoes are another possibility. These shoes take a long time to make and are expensive. Therapeutic shoes are usually only worn for a limited period and the foot often changes during the treatment, so hand-made shoes may prove unsuitable in various ways. When should they be worn? Therapeutic shoes must be worn all day long, both outdoors and in. You must never take a step without these shoes and you must not wear other shoes. The inlays must be inserted the whole time. If you have bulky ulcer dressings, the shoes may feel tight, but in that case the dressings need to be changed. Without inlays the 1

4 shoes are no good at all. The sole is hard and slippery and not shaped for your foot. Without inlays the risk of new foot ulcers is high. How do I put them on? The whole treatment is based on the idea that the foot is firmly held in the shoe. If the foot takes up the wrong position in the shoe or is able to move about, the inlay will not do its job or the foot will knock against some part of the shoe. Undo the straps sufficiently wide so it is easy to put your foot in. Place the heel of the shoe on the floor and press the foot back. Tighten the velcro straps firmly. It is especially important to tighten the second strap from the toe. The foot will then be held securely in place. If this strap is loose, the foot can easily slip forward, causing pressure on the end of your big toe. How do I look after them? Therapeutic shoes need looking after just like other shoes. If you have diabetes you will often have low sensitivity in your feet and may not notice if any stones get into the shoe. Take a look every time you put the shoes on. The inlays can be taken out and cleaned with ordinary washing-up liquid and rinsed off. The velcro straps may start coming loose over time or if they get clogged up. If so, they need replacing. The tongue may get out of position or fold over. When do the shoes need altering or replacing? If you have ulcers on your foot, the foot will change all the time. For instance, it could have been swollen the first time. Look at your foot at least once a day or ask someone to help you. If there are any signs of pressure, contact the orthopaedic workshop. Don t delay, your feet can change quickly and then the inlay will not be right. When can you stop wearing them? You need to decide this with your own doctor. When your foot improves you can often change to what we call posttreatment shoes. These look like ordinary shoes but are wider and higher at the front. They are also sold in ordinary shops, where they are called comfort shoes. If your foot shape is too difficult to accommodate, you may need a hand-made shoe. Part. Dessa färdigtillverkade specialskor används framför allt vid diabetes där man har allvarliga sår på fötterna eller där fotens form ändrat sig så att vanliga skor inte kan användas. De kallas för behandlingsskor då de ingår i behandlingen på samma sätt som mediciner och förband. Man kan säga att de utgör en del av förbandet. I de flesta fall använder man dem en begränsad tid och Du kan sedan återgå till mer normala skor. Varför är de så annorlunda? Skorna är avsedda för sådana tillfällen där vanliga skor inte kan användas. De är högre och bredare framtill för att man skall få plats med ett inlägg, förband och kanske krokiga tår. Det finns ett par olika fabrikat, men de är uppbyggda på samma sätt. Vad som skiljer är hur ovanlädret ser ut. Det finns lågsko-, sandal- och kängmodeller. Varför så stela? Sulan är stel och avrundad på samma sätt som en träsko. Skälet är att foten skall vara stilla i skon. I en vanlig sko där framändan böjer sig, rör sig foten alltid i förhållande till foten och det kan ge skav och tryck. Varför så tunga? Det är en synvilla! De är faktiskt lättare än de flesta andra skor. Jämför med Dina vanliga skor. Skorna ser tunga ut för att de är bredare och högre framtill. Vad kan man använda istället? Tyvärr är möjligheterna inte så många. Gips, som vid ett benbrott, är ett sätt och används vid allvarligare sår och ofta när man gjort operationer på foten. Många, som får behandlingsskor har haft gips först. Det låter obehagligt, men amputation av tår eller delar av foten kan bli nödvändig om man inte kan använda behandlingsskor. Kryckor och fullständig avlastning, d.v.s. att man inte alls får stödja på foten är en annan möjlighet, men det är arbetsamt och obekvämt att gå så. Handgjorda skor är ytterligare en möjlighet. Sådana skor tar rätt lång tid att göra och är dyra. Behandlingsskorna används oftast bara en begränsad tid och foten förändras ofta under behandlingstiden så det kan av flera skäl vara olämpligt med hangjorda skor. Hur skall de användas? Behandlingsskorna skall användas hela dagarna, både utomhus och inomhus. Du får inte ta ett steg utan dessa skor och Du får inte använda några andra skor. Fotbäddarna skall vara i hela tiden. Om Du har stora sårförband kan det bli trångt, men då måste förbandet ändras. Utan fotbäddarna är skorna inte alls bra. Bottensulan är hård och hal och inte alls formad efter foten. Utan fotbäddar är risken för nya fotsår stor. 3

6 Hur skall de tas på? Hela behandlingen bygger på att foten sitter stadigt i skon. Om foten kommer fel i skon eller kan röra sig, stämmer inte fotbädden eller så stöter foten emot någonstans. Öppna remmarna ordentligt så att det är lätt att få i foten. Sätt hälen på skon mot golvet och pressa foten bakåt. Dra åt kardborrebanden ordentligt. Speciellt viktigt är det att det andra bandet från tån räknat dras åt. Foten sitter då stadigt. Om det glappar åker foten gärna framåt och man kan få tryck på stortåspetsen. Hur skall de skötas? Behandlingsskorna skall skötas som andra skor. Har Du diabetes är känseln i foten ofta dålig och Du märker kanske inte om det kommer skräp i skon. Titta därför efter var gång Du tar dem på Dig. Fotbäddarna kan tas ut och rengöras med vanligt diskmedel och sköljas av. Kardborrebanden kan fästa dåligt med tiden eller om det kommer skärp i dem. De måste då bytas. Plösen kan komma ur läge eller vika sig dubbel. tiden till exempel. Titta på foten minst en gång varje dag eller be någon hjälpa Dig. Finns det tecken på tryck, kontakta ortopedverkstaden! Tveka inte att göra detta, fötterna kan ändra sig snabbt och då stämmer ju inte inläggen. När kan man sluta använda dem? Detta måste Du göra upp med Din behandlande läkare. När foten blivit bättre kan man ofta gå över till vad vi kalla efterbehandlingsskor. Dessa ser ut som vanliga skor, men är bredare och högre framtill. De säljes också i en del vanliga affärer där de kallas bekvämskor. Om foten är allför besvärlig i formen kan en handgjord sko behövas. Deze kant-en-klare speciale schoenen zijn vooral ontworpen voor diabetici met ernstige voetontstekingen, of bij wie de vorm van de voet veranderd is, waardoor het gebruik van gewone schoenen onmogelijk is geworden. Ze worden therapeutische schoenen genoemd omdat ze een deel vormen van de behandeling, net als medicijnen en verbanden. Je kunt bijna zeggen dat ze deel uitmaken van het verband. In de meeste gevallen worden ze tijdelijk gebruikt, en kunt u later teruggaan naar meer normale schoenen. Waarom zien ze er zo anders uit? De schoenen zijn juist voor gevallen waarin gewone schoenen niet gedragen kunnen worden. Ze zijn hoger en wijder aan de voorkant om ruimte te geven voor steunzolen, verbanden en, in sommige gevallen, scheve tenen. Er zijn een aantal verschillende soorten verkrijgbaar, maar ze zijn allemaal op dezelfde manier ontworpen. Wat het verschil maakt, is het uiterlijk van de bovenkant. Er zijn gewone schoenen, sandalen en laarzen. Waarom zijn ze zo stijf? De zool is stijf en afgerond als een klomp. Het doel is om te voorkomen dat de voet binnen de schoen heen en weer schuift. In een gewone schoen, als het voorste deel buigt, beweegt de voet altijd in overeenstemming hiermee, en dit kan wrijving en druk veroorzaken. Waarom zijn ze zo zwaar? Dit is een optische illusie! Ze zijn in feite lichter dan de meeste andere schoenen. Vergelijk ze met uw gewone schoenen. De schoenen zien er zwaar uit, omdat ze aan de voorkant wijder en hoger zijn. Kan ik iets anders hiervoor in de plaats dragen? Er zijn helaas niet veel alternatieven. Gips, zoals bij breuken gebruikt wordt, is één optie, die gebruikt wordt bij ernstige zweren en ook vaak na operaties aan de voet. Veel mensen die therapeutische schoenen dragen hebben eerst gips gehad. Het klinkt wreed, maar het kan noodzakelijk blijken om tenen of delen van de voet te amputeren, als geen therapeutische schoenen gedragen worden. Stokken en volledige drukverwijdering, d.w.z. de voet ontzien van alle contact met de grond, is een andere mogelijkheid, maar het maakt lopen inspannend en onplezierig. Handgemaakte schoenen zijn een andere mogelijkheid. De productie van zulke schoenen duurt lang en ze zijn duur. Therapeutische schoenen worden meestal alleen voor een korte periode gedragen, en de voet verandert vaak tijdens de behandeling, zodat handgemaakte schoenen om verschillende redenen ontoepasselijk kunnen zijn. Wanneer moeten ze gedragen worden? Therapeutische schoenen moeten de hele dag gedragen worden, zowel binnen als 5

8 buiten. U moet geen stap verzetten zonder deze schoenen en u moet geen andere schoenen dragen. De inlegzolen moeten de hele tijd in de schoenen zitten. Als u dikke verbanden voor ontstekingen heeft, kan de schoen strak aanvoelen, maar in dat geval moeten de verbanden veranderd worden. Zonder inlegzolen zijn de schoenen van geen enkel nut. De zool is hard en glad en is niet voor uw voet gevormd. Zonder inlegzolen is er een groot risico dat zich nieuwe voetontstekingen vormen. Hoe trek ik ze aan? De hele behandeling is gebaseerd op het idee dat de voet stevig in de schoen wordt vastgehouden. Als de voet een verkeerde positie in de schoen aanneemt, of kan bewegen, zal de inlegzool zijn werk niet doen, of zal de voet tegen een deel van de schoen aanstoten. Maak de banden ver genoeg los, zodat het makkelijk is om uw voet erin te doen. Plaats de hiel van de schoen op de vloer, en duw uw voet erin. Maak het klittenband stevig vast. Het is vooral belangrijk om de tweede band vanaf de teen stevig aan te trekken. Hierdoor zal de voet veilig op zijn plaats gehouden worden. Als deze band los zit, kan uw voet makkelijk naar voren schuiven, wat druk veroorzaakt op het einde van uw grote teen. Hoe onderhoud ik ze? Therapeutische schoenen moeten onderhouden worden op dezelfde manier als andere schoenen. Als u diabetes heeft, zal u vaak maar geringe gevoeligheid in uw voeten hebben, waardoor u het misschien niet merkt als er steentjes in uw schoen komen te zitten. Kijk hiernaar, elke keer dat u de schoenen aandoet. U kunt de inlegzolen eruit halen en schoonmaken met gewoon afwasmiddel, waarna u ze goed afspoelt. De klittenbanden kunnen los gaan zitten, na langdurig gebruik of als ze verstopt raken. In dat geval moeten ze vervangen worden. De tong kan scheef gaan zitten of dubbelvouwen. Wanneer moeten de schoenen veranderd of vervangen worden? Als u ontsekingen op uw voet heeft, zal deze continue van vorm veranderen. Hij kan bijvoorbeeld de eerste keer opgezwollen zijn geweest. Kijk ten minste één maal per per dag naar uw voet en vraag iemand om u te helpen. Als er tekenen van druk zijn, neem dan contact op met de orthopedische werkplaats. Stel dit niet uit, uw voeten kunnen heel snel veranderen en dan is de inlegzool niet juist meer. Wanneer kan je ophouden ze te dragen? Deze beslissing maakt u in overleg met uw eigen dokter. Als uw voet vooruit gaat, kunt u vaak overschakelen op wat we nabehandelingschoenen noemen. Die zien er uit als gewone schoenen, maar ze zijn aan de voorkant wijder en hoger. Ze worden ook in gewone winkels verkocht, waar ze comfort schoenen genoemd worden. Als de vorm van uw voet te moeilijk is om een pasmaat te vinden, heeft u misschien handgemaakte schoenen nodig.
