1 Praktische-opdracht door Een scholier 3215 woorden 22 januari ,7 20 keer beoordeeld Vak Biologie NEDERLANDS Inleiding De wereld van de biotechnologie is de laatste jaren sterk verbeterd. Er zijn tegenwoordig een heleboel mogelijkheden op dit gebied. Xenotransplantatie van dier naar mens is zo n nieuw ontwikkelde techniek. Toch is niet alles wat mogelijk is, meteen acceptabel. Onderzoekers moeten rekening houden met ethische kwesties. Deze brochure biedt een eerste kennismaking met xenotransplantatie. Algemeen Xenotransplantatie is het transplanteren van levende cellen, weefsels of organen van het ene dier naar het andere dier, of naar de mens. Xenos betekent vreemd in het Grieks en xenotransplantatie is dus letterlijk de transplantatie van vreemde organen. Het transplanteren van een varkenshart in een aap is zo n voorbeeld. Uiteindelijk wil men in staat zijn dierlijk materiaal naar de mens te transplanteren. De ontwikkeling van xenotransplantatie Sinds orgaantransplantaties van mens naar mens (allotransplantatie) al een jaar of twintig met succes wordt uitgevoerd, is er een veel te grote vraag naar donororganen. Deze zijn echter niet voldoende beschikbaar. Daarom zijn wetenschappers bezig met een onderzoek naar xenotransplantatie, om te kijken of het misschien mogelijk is dierlijke organen te gebruiken ter vervanging van menselijke organen. Aangezien dierlijke organen in principe onbeperkt voor handen zijn, zou dit waarschijnlijk een goed alternatief voor menselijke organen kunnen zijn. Met xenotransplantatie wordt al sinds begin jaren 50 geëxperimenteerd, maar de resultaten zijn tot nu toe niet bijzonder succesvol. Daarom is xenotransplantatie nu nog in volle ontwikkeling. Het afstotingsprobleem Het probleem bij xenotransplantatie is dat het dierenorgaan door het lichaam van de ontvanger wordt afgestoten. Die afstoting komt door ons afweersysteem (immuunsysteem), dat het nieuwe orgaan als een vreemde indringer herkent en vervolgens antistoffen produceert zodat het nieuwe orgaan wordt beschadigd. Dit probleem komt behalve bij xenotransplantatie ook bij allotransplantatie voor. Bij deze laatste heeft men het probleem van afstoting inmiddels enigszins onder controle door bepaalde medicijnen (afstotingsremmers). Deze medicijnen werken niet goed bij xenotransplantatie omdat de afstoting van dierlijke organen anders verloopt dan de afstoting van menselijke organen. Wetenschappers doen hier onderzoek naar om het afstotingsprobleem bij xenotransplantatie vervolgens gericht tegen te Pagina 1 van 8

2 kunnen gaan. Met behulp van genetisch manipulatie wordt wel geprobeerd dierenorganen menselijker te maken, maar dan nog bestaat er kans dat het geïmplanteerde orgaan wordt afgestoten en moet de patiënt toch nog gebruik maken van medicijnen. Varkens als brondier Het dier waarvan het transplantaat afkomstig is wordt het brondier genoemd. Bij het onderzoek heeft men eerst gekeken naar de aap als brondier, aangezien de aap qua genetische opmaak het dichtst bij de mens staat, maar de organen bleken al gauw te worden afgestoten door het menselijk lichaam. Bovendien is de kans op overdracht van virussen naar de mens erg groot. Ook zijn apen moeilijk te fokken en duurt het lang voor de apen groot genoeg zijn. (De organen moeten namelijk groot genoeg zijn om in een volwassen mens goed te functioneren.) Men heeft zich toegelegd op het onderzoek naar varkensorganen, omdat varkens makkelijk te fokken zijn en snel tot gewenste grootte groeien. Bovendien kunnen ze in een steriele omgeving worden gefokt om zo de meeste virussen buiten te sluiten. Varkensorganen bleken tevens een kleinere kans te hebben te worden uitgestoten door het menselijk lichaam. Wij beperken ons daarom in deze brochure tot het varken als brondier voor xenotransplantatie. De vier fases van xenotransplantatie Fase 1 Omdat een varkensorgaan veel verschilt van een menselijk orgaan moet het menselijker worden gemaakt om zo het afstotingsgevaar te verkleinen. Dit gebeurt in deze fase door middel van genetisch manipulatie: het erfelijk materiaal van de varkens wordt kunstmatig veranderd. Dit kan bijvoorbeeld worden gedaan door een nieuw gen in een bevruchte eicel te spuiten. Fase 2 Omdat deze genetisch gemanipuleerde varkens niet meteen geschikt zijn voor xenotransplantatie, moet er eerst een aantal keren mee gefokt worden om een stabiele groep varkens te krijgen die wel geschikt zijn. Fase 3 De varkens moeten in een steriele laboratoriumachtige ruimte worden gehouden, om zo de kans op bacteriën en virussen te verkleinen. Fase 4 In deze laatste fase worden de benodigde organen uit het varken gehaald en naar het ziekenhuis gebracht om vervolgens in een patiënt te worden getransplanteerd. Alternatieven en oplossingen voor het orgaantekort In principe is het het beste als xenotransplantatie gewoonweg niet toegepast hoeft te worden. Een deel van de orgaantransplantaties dat nodig is komt namelijk door te veel roken en vet eten. Als de ongezonde leefstijl van tegenwoordig verandert, zullen onder andere een aantal hart- en levertransplantaties niet meer nodig zijn. Voorkomen is immers beter dan genezen. Mensen kunnen door middel van campagnes worden gestimuleerd donor te worden. Ook door het leeftijdscriterium van donororganen te laten vervallen en alleen de kwaliteit van het orgaan te beoordelen kan het donoraanbod worden vergroot. Kunstorganen kunnen dienen als alternatief. Hierbij onderscheiden we kunstorganen die buiten het Pagina 2 van 8

3 lichaam werken en organen daadwerkelijk in het lichaam worden geïmplanteerd. Het nierdialyseapparaat is een kunstorgaan dat buiten het lichaam wordt gebruikt en wordt al lang met succes gebruikt. Nierpatiënten die hiervan gebruik maken moeten echter wel zeer regelmatig naar het ziekenhuis. De ontwikkeling van een mechanisch kunsthart om in het lichaam te plaatsen is al heel ver. Toch heeft dit ook weer zijn risico s: er kunnen bloedstolsels ontstaan en het kunststof is misschien niet duurzaam genoeg. Bovendien is er bij beide vormen van kunstorganen een grote kans op infecties. Een ander alternatief is Tissue Engineering. Met deze techniek probeert men in het laboratorium de benodigde organen te kweken met cellen van mensen of, nog beter, van de patiënt zelf. Dit is nu nog in ontwikkeling want van het kweken van cellen naar het produceren van een compleet orgaan is nog wel een hele stap. Onderzoekers menen dat als ze van embryonale stamcellen (cellen die kunnen uitgroeien tot elk gewenst celtype) gebruik mogen maken, het onderzoek naar Tissue Engineering veel sneller zou verlopen en er veel meer mogelijkheden zouden zijn. Het probleem hierbij is echter dat het gebruik van stamcellen grote ethische bezwaren oplevert omdat ze afkomstig zijn van beginnend leven, embryo s. De positieve gevolgen en voordelen van xenotransplantatie Omdat er tegenwoordig een groot tekort is aan donororganen en weefsels staan er veel patiënten op de wachtlijst voor een transplantatie. Voor velen van hen (circa 10%) komt een donororgaan te laat en sterft de patiënt. Xenotransplantatie biedt dus een goede oplossing om dat tekort tegen te gaan en daarmee dus mensenlevens te redden. Xenotransplantatie heeft het voordeel dat men niet afhankelijk is van de donor. Er is namelijk in principe elk moment een varkensorgaan beschikbaar. Bij allotransplantatie is dit niet het geval, daarbij is het van belang dat er op het juiste moment een orgaan beschikbaar komt. Door xenotransplantatie kunnen patiënten dus elk moment geholpen worden. Nierpatiënten die nu nog meerdere keren per week naar het ziekenhuis moeten om hun bloed te laten zuiveren van afvalstoffen en zouten, kunnen door deze nieuwe techniek een varkensnier krijgen. Zij zullen daardoor niet meer telkens naar het ziekenhuis hoeven en dus een aangenamer en beter leven kunnen leiden. De negatieve gevolgen en nadelen van xenotransplantatie Er moeten zeer veel varkens worden opgeofferd voor zowel langdurig onderzoek als de transplantaties zelf. Hun omgeving wordt daarbij aangepast en ze komen zo te leven in steriele hokken, zonder stro, buitenlucht en daglicht. Daarbij komt nog dat ze genetisch gemanipuleerd worden. De patiënt moet voor de rest van zijn leven medicijnen slikken om het afweersysteem lam te leggen, zodat het varkensorgaan niet wordt afgestoten. Hierdoor wordt de patiënt zeer vatbaar voor andere ziekten, zodat hij zelfs aan een kleine verkoudheid kan bezwijken. Het risico dat het varkensorgaan, ondanks de genetische manipulatie, toch nog wordt afgestoten blijft bestaan. Een aantal zaken zijn nog niet helemaal duidelijk. Varkens leven gemiddeld vijftien jaar. Gaat een varkensorgaan in een menselijk lichaam langer mee, of moet de patiënt om de zoveel jaar weer geopereerd worden? Ook is niet duidelijk of en hoe zo n geïmplanteerd orgaan de menselijke persoonlijkheid beïnvloedt. Het is ook maar de vraag of een orgaan van een dier ooit zo zal functioneren als een menselijk orgaan. Pagina 3 van 8

4 Ook al worden de varkens in een steriele omgeving gefokt, de kans dat onbekende ziektekiemen een infectie in de patiënt veroorzaken is niet volledig uit te sluiten. Bovendien bestaan er virussen die niet op deze manier weg zijn te houden omdat deze zich in het DNA van alle varkens schuilhoudt (zogenaamde endogene retrovirussen). Voor varkens zijn ze niet gevaarlijk. Hoe ze zich in mensen gedragen is nog niet bekend, maar men vreest voor het ontstaan van een gemakkelijk overdraagbare ziekte, die pas jaren na infectie ziekteverschijnselen geeft en zich daardoor ongemerkt heeft kunnen verspreiden. Virussen die oorspronkelijk alleen in varkens leefden, kunnen via xenotransplantatie dus een bedreiging voor de samenleving vormen. Omdat het risico bestaat dat onbekende ziekten zich verspreiden door xenotransplantatie, moet de patiënt enige dagen na de operatie in afzondering leven. Na deze tijd moet hij blijvend worden gecontroleerd op infectieziekten. Directe relaties van deze patiënt zouden zich misschien moeten laten controleren op ziekteverschijnselen. Onderzoeken op dit gebied kosten een hoop geld. Goed om te weten Leef gezond! Iedereen kan immers plotseling voor het probleem van een transplantatie komen te staan. Een gezonde levenswijze kan het risico beperken. In 1998 heeft iedere burger boven de 18 jaar een formulier van de overheid ontvangen waarop ingevuld kon worden of hij/zij wel of niet donor wil zijn van organen. There are many possibilities at this realm nowadays. Xenotransplantation from animal to men is such a new developed technical science. Yet not everything, which is possible, is acceptable. 5 Xenotransplantation is the transplantation of living cells, tissue or organs from one animal to the other, or to men. Xenos means strange in Greek and so xenotransplantation means the transplantation of strange organs literally. The transplantation of a pig heart into a monkey is an example. At the end one wants to be able to transplant animal material into men. The development of xenotransplantation Since organ transplantations from men to men (all transplantation) are done successfully the last twenty years, there is a far too big request of organs. These are however not enough available. Therefore scientists examine xenotransplantation to see whether it s possible or not that animal organs can be used as a replacement for human organs. Since animal organs are in principle unlimited available, this may be a good alternative for human organs. Even since the beginning of the 50 there is experimented with xenotransplantation, the results though are still not very successful. That is why xenotransplantation is still developing now. The repulsion problem A problem with xenotransplantation is that the body of the receiver repulses the animal organ. This repulsion is caused by our defence system, which recognizes the new organ as an intruder and then starts producing antimaterial immediately so that the new organ is damaged. This problem not only happens by xenotransplantation but also by allotransplantation. The repulsion by allotransplantation is now somewhat under control thanks to some medicines called repulsion breakers. These medicines do not work by xenotransplantation because the repulsion of animal organs is different from repulsions of human organs. Scientists are making researches into the repulsion of animal organs so that they can fight it directionally. The animal organs are made more human with genetic manipulation, but then still there is a chance that the organ is repulsed and the patient has to take medicines as well. Pigs as source-animals The animal from which the donor-organ is used for transplantation is called source-animal. At the research monkeys were considered in the first place as source-animals because these resemble men most in their genetic-material. But the human body very quick repulsed the organs. Moreover, there is a great chance of transfer of viruses to men. Monkeys are furthermore difficult to breed and it takes al long time till they are the desired size. Scientists apply themselves to the research of pig organs for xenotransplantation, because they are easy to breed and grow to the desired size very fast. Moreover, they can be breed in a sterile surrounding to shut out most viruses. The human body in general less repulses pig organs. We confine ourselves in this brochure to pigs as source-animals. The four stages of xenotransplantation Stage 1 Because a pig organ differences a human organ a lot, it has to be made more human to reduce the risk of repulsion. This is done in this stage by genetic manipulation: the hereditary material of the pigs is artificial changed. This can be done for example by spouting a new gene into a fructified eicel. Stage 2 Because these genetic manipulated pigs are not immediately suitable for xenotransplantation, Pagina 5 van 8

6 they have to be bred a few times to obtain a group pigs which is suitable for xenotransplantation. Stage 3 The pigs have to be kept in a sterile laboratory surrounding to decrease the risk of viruses. Stage 4 In this final stage the required organs are taken out of the pig and brought to the hospital to be transplanted into the patients body. Alternatives and solutions for the organ shortage In principle it is best if xenotransplantation simple not has to be used. A part of all the transplantations that are necessary are caused by too much smoking and too much fat food. If the unhealthy way of living changes, a few heart- and livertranplantations will not be necessary anymore. Preventing is indeed better than healing. Men can be stimulated to become a donor by campaigns from the government. Also by taking away the age-criterion for donor organs and only judging the quality of the organ, the donor offer can be enlarged. Artificial organs can be used as an alternative. These can be distinguished in organs that are used outside the body and organs that are implanted in the body. The kidney dialyse-set is an artificial organ that is successfully used outside the body. Kidney patients who use this set though have to go to the hospital very often. The development of a mechanical artificial heart that can replace a real one is very far. But there are still risks: there can arise blood coagulations and this artificial heart may be not very durable. Both kinds of artificial organs furthermore have a high risk of infection. Another alternative is Tissue Engineering. With these technique is tried to bring up necessary organs in a laboratory from human cells or, even better, from the patient himself. This is still in development because from the couch grass of cells to really producing a complete organ is a huge move. Scientists think that if they were allowed to use embryonic stem cells (cells that can grow to every desired celtype), the research into Tissue Engineering would go so much faster and that there would be many more opportunities. The problem herewith however is that the use of these cells bring up ethical protests because they come from starting life, embryos. The positive consequences and advantages of xenotransplantation Because there is a great lack of donor organs and tissue nowadays, there are many patients on the waiting list for transplantation. For many of them (about 10%) a donor organ comes too late and the patient passes away. Xenotransplantation therefore is a great solution to save many lives. Xenotransplantation gives us the benefit that one is not depending on the donor. There is in principle a pig organ available every moment. This is not the case by allotransplantation, because in that case it is important that there is a donor organ available at that very moment. Patients can be helped every moment through xenotransplantation. Kidney patients, who have to return to the hospital several times a week to deprive their blood of refused matters and salts, can have a pig kidney now. By that they don t have to go to the hospital anymore and so the can live a better and more ordinary life. The negative consequences and disadvantages of xenotransplantation Many pigs have to be sacrificed for both a research at a great length as the transplantations. Their Pagina 6 van 8

7 surroundings are accommodated and so they come to live in sterile sties, without straw, fresh air and sunlight. They are furthermore genetic manipulated. The patient has to take medicines for the rest of his life to stop the defence system from doing its work, so that the new pig organ will not be repulsed. In that way the patient becomes impressible for other diseases. He can die of a simple cold. The risk that the pig organ will be repulsed anyway, despite the fact that it has been genetic manipulated, still exists. Some things are still not very clear yet. Pigs live fifteen years averagely. Does a pig organ go along longer in a human body, or has the patient be operated again within some years? It s also not clear if and how the pig organ influences the human personality. It is still an open question if an animal organ will ever function the same as a human organ. Although the pigs are bred in a sterile surrounding, the chance that an unknown disease-germ an infection in the patients body creates cannot fully be shut out. Moreover, there are viruses that cannot be excluded this way because they hide themselves in the DNA of every pig. These viruses are called endogen retroviruses. They are not dangerous for pigs, but in what way they conduct themselves in the human body is not clear yet. One is afraid nevertheless for the beginning of an easy transferable disease that brings symptoms just years after infection so that it can be spread easily without being perceived. Viruses that used to live only in pigs can be a serious threat for the society by xenotransplantation. Because there is a risk that unknown diseases are spread through xenotransplantation, the patient has to live in isolation just after the operation. After that he will have to be checked on infection diseases forever. Close relations also may have to be checked on symptoms. Researches at this realm cost a lot of money. Good to know Live healthy! Everybody can suddenly stand for a problem that includes transplantation. A healthy way of living can restrict the risk. Every citizen above 18 years has received a form from the government in 1998 on which could be filled in whether he/she wants to be a donor or not.
