2 SUMMARY SUMMARY Introduction Uveal melanoma is the most common primary malignant intraocular tumor, and the estimated annual incidence is six per million cases in the Western world. Primary uveal melanoma can be treated by enucleation or eye-preserving methods but there is still no effective therapy for uveal melanoma metastases. Up to 50% of the patients who undergo enucleation die from metastases within 10 to 15 years after diagnosis of choroidal and/or ciliary body melanoma. In order to improve survival, it is essential to prevent metastases by identifying patients who are at increased risk and to treat uveal melanoma metastases more effectively. To establish this, it is necessary to develop methods detecting metastases in an early stage and to determine reliable prognostic parameters. This thesis describes the results of genomic and proteomic analyses of uveal melanoma, aiming to define prognostic markers and/or profiles to categorize uveal melanoma and to identify the biological mechanism of metastatic disease of uveal melanomas. Genomics: analysis of 'single' genes in uveal melanoma genesis. In the first part of this thesis, analysis was performed in uveal melanoma on genes that are known to play a role in cutaneous melanoma, since both tumors share the same embryonic origin. Nine uveal melanoma cell lines were examined using cytogenetic analysis supplemented with FISH to search for deletions or translocations affecting the PTEN gene region at 10q23 (Chapter 2). No PTEN mutations were found in these uveal melanoma cell lines suggesting that PTEN mutations or deletions probably do not play a role in the aetiology of this tumor. In Chapter 3, we performed mutation analysis and expression analysis in uveal melanoma cell lines and tumor samples on members of the MAPK pathway. Although we demonstrated that uveal melanoma cells rarely harbor BRAF or RAS mutations, we did find constitutive activation of the MAPK pathway in all specimen. Therefore, we conclude that uveal melanomas are similar to cutaneous melanomas in that constitutive activation of the MAPK pathway appears necessary for tumor pathogenesis, but that uveal melanomas use a different mechanism for activation of this pathway. Genomics: analysis of 'multiple' genes in uveal melanoma genesis. In the second part of this thesis we used macroarray (Chapters 4 and 5) and further on microarray methods (Chapters 6 and 7) to gain more insight in the gene expression profiles of uveal melanomas. Expression profiling of a uveal melanoma cell line model, i.e. a cell line derived from a primary uveal melanoma and from two of its metastases, was effective in recognizing expression differences that could be validated in primary tumors and in revealing a possible mechanism of gene repression (Chapter 4). An important finding was the 5-fold decreased expression of TIMP3, a molecule involved in extracellular matrix degradation, in the metastatic cell lines. We demonstrate in the cell lines that TIMP3 expression is regulated by methylation. These observations were confirmed in primary uveal melanoma and suggest a role for TIMP3 in uveal melanoma development. With the same technique used in the former chapter, we analysed in Chapter 5 the gene expression profiles in 15 cell lines, derived from 154

3 SUMMARY primary, metastases and normal choroidal melanocytes. Four candidate tumor markers, Laminin Receptor 1, Endothelin 2, Von Hippel Lindau Binding protein 1 and Cullin 2, have been selected from genes that were differentially expressed in the uveal melanoma cell lines compared to the normal uveal melanocytes. Validation of the four genes on primary melanoma samples resulted in two distinct subgroups with specific expression profiles, which could be indicative of different disease processes. In Chapter 6 we used microarray expression profiling combined with phenotypic characterization of uveal melanoma and melanocyte cell lines to define a gene signature associated with cellular invasion. This analysis gave a large panel of transcripts significantly differentially expressed between samples with high or low invasive phenotype. A majority of the genes in this list have not been reported before in uveal melanoma. Of particular interest, the p21-activated kinases PAK1 and PAK7 were elevated in the highly invasive group. Because PAK1 has been shown to play a central role in regulating cell motility and invasiveness, we sought to determine whether PAK1 may be involved in invasiveness of uveal melanoma. Our data indicate that elevated levels of PAK1 are indeed associated with invasive potential in uveal melanoma, with knock-down of PAK1 resulting in up to a 5-fold decrease in invasion using the MICS assay. Thus PAK1 represents an attractive target for development of rational therapies to treat this disease and prevent its metastatic spread. Differences in invasiveness between uveal melanomas are likely to be one of the most important factors for their differences in clinical outcome. In light of the observation that Wnt5a expression is associated with invasive potential of cutaneous melanoma, we sought to determine in Chapter 7 if activation of the Wnt signalling pathway, through increased expression of Wnt5a, β-catenin and MMP7, contributed to an invasive phenotype in uveal melanoma. Both Wnt5a and MMP7 blocking antibodies reduced the invasiveness of uveal melanoma cells. Furthermore, the proportion of cells immunoreactive for Wnt5a, β-catenin and MMP7 was higher in primary uveal melanomas from patients with shorter survival. This differences were statistically significant for all three members. Proteomics and uveal melanoma In the last part of this thesis the results of proteomic analysis are described. Due to the fact that protein alterations may occur that are not reflected in changes at the RNA level, we assessed protein expression by using two-dimensional gel electrophoresis and we subsequently identified a subset of proteins with mass spectrometry (Chapter 8). A few proteins have been described before in the literature as potential tumor markers, but most proteins of the list have not been established in uveal melanoma development. Proteomic analysis can thus provide us with more profound insight in the multifactorial mechanisms of uveal melanoma progression. 155

4 SUMMARY Concluding remarks Finally, in Chapter 9 the results of this thesis are discussed in line of the aim and suggestions for future research are given. The contents of this thesis have given us increased genetic and molecular knowledge of uveal melanoma and will provide better opportunities to identify patients at risk for metastasis in the near future. Eventually, insight in the complex mechanisms of metastasis development should allow accurate and specific therapeutic interventions, before uveal melanoma becomes a deadly tumor. 156

5 SAMENVATTING SAMENVATTING Inleiding Het oogmelanoom (uveamelanoom) is de meest voorkomende tumor in het oog bij volwassenen. Circa zes op de miljoen volwassenen in de westerse samenleving krijgen per jaar deze intraoculaire tumor. Het oogmelanoom kan worden behandeld door het oog in zijn geheel te verwijderen (enucleatie) of door oogsparende methoden, maar desondanks is er nog steeds geen effectieve therapie ontwikkeld om de uitzaaiingen (metastasen) van deze tumor te behandelen. Ongeveer 50% van de patiënten waarbij als behandeling het oog geheel verwijderd wordt komt binnen 10 tot 15 jaar na het stellen van de diagnose oogmelanoom te overlijden. Om de overleving van mensen met een oogmelanoom te verbeteren is het van belang patiënten te identificeren met een verhoogd risico op uitzaaiingen. Daarnaast is het van belang de uitzaaiingen effectief te behandelen. Om dit doel te bereiken, moeten er methoden ontwikkeld worden om in een vroeg stadium de uitzaaiingen te kunnen detecteren en moeten er betrouwbare voorspellende parameters bepaald worden. Dit proefschrift beschrijft de resultaten van fundamenteel genetisch onderzoek (genomics) en eiwit analyses (proteomics) in uveamelanomen, met als doel het bepalen van deze voorspellende parameters waardoor tevens meer inzicht verkregen wordt in het biologisch mechanisme achter het metastaseringspatroon van het oogmelanoom. Genomics: analyse van enkele genen in oogmelanoom ontwikkeling In het eerste gedeelte van dit proefschrift zijn genen bestudeerd die reeds een bekende rol spelen in de ontwikkeling van huidmelanomen. Huidmelanomen delen dezelfde embryologische oorsprong als oogmelanomen. Negen verschillende cellijnen gekweekt uit oogmelanoom werden door middel van cytogenetische onderzoeks technieken aangevuld met de Fluorescent in Situ Hybridisation techniek, bekeken op het voorkomen van genetische veranderingen (deleties of translocaties) in het PTEN gen, dat gelokaliseerd is op het tiende chromosoom (10q23) (Hoofdstuk 2). In de cellijnen werden geen veranderingen in PTEN gevonden, hetgeen inhoudt dat het PTEN gen waarschijnlijk geen rol speelt in de ontstaanswijze van het oogmelanoom. In Hoofdstuk 3 is vervolgens naar de veranderingen en expressie van genen in de MAPK route gekeken in tumoren en in verschillende cellijnen gekweekt uit oogmelanomen. Ondanks dat we nauwelijks afwijkingen (mutaties) vonden in de BRAF en de RAS genen, bleek dat de MAPK route in alle tumoren geactiveerd was. We kunnen daarom concluderen dat zowel bij huidmelanomen als bij oogmelanomen de MAPK route is geactiveerd, maar dat het bij het oogmelanoom gebaseerd is op een ander onderliggend mechanisme en niet veroorzaakt wordt door mutaties zoals bij het huidmelanoom. Genomics: analyse van meerdere genen in oogmelanoom ontwikkeling In het tweede deel van dit proefschrift hebben we gebruikt gemaakt van de macroarray techniek (hoofdstukken 4 en 5) en later de microarray techniek (hoofdstukken 6 en 7) om meer inzicht te krijgen in de simultane expressie van zeer veel genen in oogmelanomen. Genexpressie analyse van een oogmelanoom cellijn model, i.e. een cellijn afkomstig van een 157

6 SAMENVATTING tumor en twee cellijnen afkomstig van twee verschillende metastasen van diezelfde tumor, toonde genexpressie verschillen aan (Hoofdstuk 4), onder andere een evidente vermindering van de expressie van het gen TIMP3, een molecuul betrokken in de afbraak van de extracellulaire matrix waardoor cellen aan elkaar verbonden zijn. We konden in de cellijnen aantonen dat de expressie van het TIMP3 gen gereguleerd wordt door methylatie. Het bevestigen van deze bevindingen in ongekweekt (primair) tumor materiaal, suggereert dat TIMP3 een rol speelt in de ontwikkeling van oogmelanomen. Middels dezelfde techniek zijn in Hoofdstuk 5, 15 verschillende cellijnen, afkomstig van primaire tumoren, metastasen en gezonde cellen uit het oog (melanocyten), geanalyseerd. Vier kandidaat tumor markers zijn geselecteerd uit genen die verschillend tot expressie kwamen tussen oogmelanomen en de normale melanocyten; Laminin Receptor 1, Endothelin 2, Von Hippel Lindau Binding protein 1 en Cullin 2. Vervolgexperimenten toonden aan dat op grond van deze vier tumor markers de tumoren verdeeld konden worden in twee groepen met specifieke genexpressie profielen voor deze markers, dat mogelijk gerelateerd kan worden aan een verschillend beloop van het ziekteproces. Om genen te kunnen identificeren die gerelateerd zijn aan uitzaaiend vermogen, zijn in Hoofdstuk 6 genexpressie profielen van oogmelanomen en gezonde melanocyten gecombineerd met eigenschappen gerelateerd aan verplaatsing van de cel (invasie). Deze analyse leverde een groot panel van genen op die verschillend tot expressie kwamen tussen tumoren met een hoog en laag invasief vermogen. Het merendeel van de genen uit deze lijst is nog niet eerder gerapporteerd in relatie tot oogmelanomen. Van speciaal belang waren twee genen, de p21-geactiveerde kinasen PAK1 and PAK7, die verhoogde expressie vertoonde in de groep met hoog invasief vermogen. Omdat van PAK1 eerder is aangetoond dat dit gen een centrale rol speelt in het verplaatsen en invasief vermogen van cellen, is dit ook bepaald voor het oogmelanoom. De rol van PAK1 in invasie kon inderdaad worden aangetoond door middel van experimenten waarin het PAK1 werd uitgeschakeld en waardoor een 5 maal zo grote afname in invasief vermogen werd waargenomen. PAK1 fungeert dus als een aantrekkelijk aangrijpingspunt voor de ontwikkeling van therapeutische doeleinden om deze ziekte te behandelen en om uitzaaiingen te voorkomen. Omdat het verschil in invasief vermogen een van de meest belangrijke factoren is die onderscheid maakt in de klinische presentatie van het oogmelanoom, is in Hoofdstuk 7 wnt5a onderzocht, een gen dat te maken heeft met het invasief vermogen van het huidmelanoom. Het bleek dat verhoogde activiteit van genen en gerelateerde genen uit de Wnt signaal route (Wnt5a, β-catenin en MMP7) bijdroegen aan een sterker invasief gedrag van oogmelanoom cellen. Remming van zowel Wnt5a als MMP7 verminderden het invasief vermogen van de oogmelanoom cellen. Bovendien was het percentage cellen met verhoogd Wnt5a, β-catenin en MMP7 hoger in oogmelanomen van patiënten met een kortere overleving. Proteomics en oogmelanomen In het laatste gedeelte van dit proefschrift worden de resultaten van eiwit analyses (proteomics) beschreven. Vanwege het feit dat eiwit veranderingen kunnen optreden die niet terug te vinden zijn op genexpressie niveau, hebben we eiwit expressie vergeleken in een cellijn afkomstig van een oogmelanoom met cellijnen afkomstig van uitzaaiingen van 158

7 SAMENVATTING dezelfde tumor. (Hoofdstuk 8). Naast eiwitten die al eerder beschreven zijn in relatie tot oogmelanoom, werden nieuwe eiwitten geïdentificeerd in relatie tot uitzaaiing. Naast genomics geeft proteomics onderzoek meer inzicht in de complexe mechanismen van oogmelanoom progressie. Conclusie In Hoofdstuk 9 worden tenslotte de resultaten in het kader van het gestelde doel van dit proefschrift bediscussieerd en worden er suggesties gegeven voor onderzoek in de nabije toekomst. Concluderend kunnen we stellen dat de inhoud van dit proefschrift de genetische en moleculaire kennis op het gebied van oogmelanomen heeft uitgebreid en perspectieven biedt om in de nabije toekomst oogmelanoom patiënten met een verhoogd uitzaaiingsrisico beter te kunnen identificeren. Het door dit proefschrift verworven inzicht in de complexe mechanismen van het ontwikkelen van uitzaaiingen biedt tevens perspectief voor nieuwe therapeutische mogelijkheden in een stadium voordat het oogmelanoom een dodelijke tumor wordt. 159

15 CURRICULUM VITAE Wieke Zuidervaart was born on the 16th of December 1973 in Delft. In 1992 she passed her VWO (Gymnasium) exam at the Stedelijke Scholengemeenschap Hugo Grotius in Delft. After highschool, she spent a year in France, including six months as Au Pair in the capital Paris. In 1993, she started her Medicine study at the Erasmus Medical University of Rotterdam, where she became Master of Science in 1997 and obtained her medical degree in March She worked as a medical student at the department of Obstetrics of the Sint Franciscus Hospital in Rotterdam and in 1998 she participated in a research project on FISH analysis in uveal melanoma for three months at the Royal University Hospital of Liverpool. She also passed the propedeutical exam in psychology at the University of Leiden in Already in 1995, Wieke became enthusiastic for research and, under the supervision of Dr. Gre Luyten, she started a project on evaluation of intraocular lenses. In the following years she also participated in the uveal melanoma research group of Drs. Annelies de Klein, Gre Luyten and Nicole Naus. In June 2000, she performed clinical work as a resident (AGNIO) at the department of Ophthalmology (Leiden University Medical Center) for seven months (Prof. CC Sterk). Thereafter, Wieke started her PhD project on uveal melanoma (Prof. Jan Keunen, Drs. Martine Jager and Nelleke Gruis). During her PhD she collaborated with different departments, at first with the department of Human Genetics (Head: Prof. dr Rune Frants), then continuing with the department of Dermatology of the LUMC (Head: Prof. dr Rein Willemze). For her last projects, she spent in total five months at the Queensland Institute of Medical Research in Brisbane, Australia, under supervision of Drs. Nick Hayward and Sandra Pavey. Since July 2003 she works as a resident in training for Ophthalmology at the LUMC (Head: Prof. CC Sterk). 167
