1 Juni Nr. 4. Van de Voorzitter: Beste leden, beste collega s, In deze vanuit het bestuur weer een beknopte update van de zaken die spelen rond de beroepsvereniging. Marianne Wijers-Hille is Koninklijk onderscheiden : Marianne Wijers, erelid van de NeSECC, werd tijdens haar afscheidsreceptie verrast met de uitreiking van een Koninklijke onderscheiding door de loco burgemeester van Rotterdam. Hieronder volgt het persbericht dat ons bereikte naar aanleiding van deze bijzondere gebeurtenis : Op woensdag 3 mei 2017 is Mevr. Marianne J. Wijers-Hille benoemd tot Officier in de Orde van Oranje-Nassau. Deze plechtige gebeurtenis heeft plaatsgevonden tijdens haar afscheidsreceptie ter gelegenheid van haar pensionering in Restaurant 'De Tuin van de Vier Windstreken' aan de Kralingse Plas te Rotterdam. Mevr. Wijers-Hille heeft als hoofd van de afdeling Extracorporale Circulatie (ECC), een onderafdeling van de afdeling Cardio -Thoracale Chirurgie van het Erasmus Medisch Centrum te Rotterdam, een belangrijke bijdrage geleverd aan de ontwikkeling van het beroep klinisch perfusionist. Een klinisch perfusionist houdt zich bezig met de kennis en toepassing van de kunstmatige circulatie van bloed. Van deze technologie wordt gebruik gemaakt bij openhartchirurgie, door het gebruik van de hart-longmachine. Ook wordt van deze technologie gebruik gemaakt in de oncologie, bij de behandeling van tumoren in geïsoleerde organen en bij ernstige longaandoeningen, waarbij door een infectie of intoxicatie de longen tijdelijk onvoldoende functioneren. Daarnaast zijn er nog diverse andere toepassingen waarbij extracorporale behandelingen van bloed een belangrijke rol spelen in geneeskundige c.q. chirurgische behandelingen.marianne was in 1976 één van de oprichters van de beroepsvereniging in haar vakgebied, de Nederlandse Sociëteit voor Extra-Corporale Circulatie (NeSECC). Zij heeft in deze vereniging 12 jaar gefunctioneerd als secretaris binnen het bestuur. Daarnaast was zij in 1989 mede oprichter van de internationale stichting Foundation European Congress on Extracorporeal Circulation Technology (FECECT). In deze stichting functioneert zij eveneens als secretaris binnen het bestuur. Zij heeft binnen deze stichting, welke een tweejaarlijks platform organiseert voor internationale kennisuitwisseling binnen haar vakgebied, eveneens een belangrijke bindende rol gespeeld tussen de vertegenwoordigers van diverse buitenlandse beroepsverenigingen en als editor van de congress proceedings. Naast haar werkzaamheden in de diverse besturen in verschillende functies, heeft Mevr. Wijers ook een zeer belangrijke bijdrage geleverd aan de opleiding van perfusionisten binnen en buiten haar afdeling en het stimuleren van wetenschappelijk onderzoek in haar vakgebied. We kunnen trots zijn op een erelid binnen de NeSECC met een dergelijk Koninklijke benoeming. Update NVIC Richtlijn ECLS : In juni 2016 heeft werkgroep ECLS van de NVIC unilateraal de NVIC Richtlijn ECLS gepubliceerd. Gedurende het najaar heeft het bestuur van de NeSECC aansluiting gezocht bij de Inspectie en bij de NVT om een brede alliantie te mobiliseren om deze richtlijn op korte termijn van een update te kunnen voorzien die recht doet zowel aan de therapie, als aan de positie van de klinisch perfusionist daarbinnen. Op 3 juli aanstaande is een breed overleg tussen NVT, NeSECC en NVIC gepland waarop dit besproken wordt. De NeSECC steekt hierbij in op het nog dit jaar uitbrengen van een aangepaste Richtlijn ECLS, waarbij met name de multidisciplinaire approach van de therapie recht gedaan wordt.

2 Visitatiecommissie : Al in eerdere nieuwsbrieven is gemeld dat binnen het platform kwaliteit op verzoek van het bestuur een werkgroep onder leiding van Jacoline Steinfelder actief is met het opstellen van visitatie documenten. Het gaat hier om een huishoudelijk reglement voor de commissie, een visitatie document en een visitatie formulier. Deze documenten bevinden zich inmiddels in een afrondende fase. Doel is eind dit jaar de documenten definitief vastgesteld te hebben, waarna in eerste aanleg een aantal pilot-visitaties zullen volgen. De voorzitter van het platform kwaliteit (Roel Hendrickx) roept hierbij erkend klinisch perfusionisten op zich aan te melden als visiteur. Doel is 8 Erkend Klinisch Perfusionisten die ten minste 3 jaar geregistreerd zijn in deze commissie plaats te laten nemen. De leden krijgen een professionele cursus aangeboden hoe een visitatie uitgevoerd behoort te worden alvorens zij daadwerkelijk het veld ingestuurd worden. Nadere informatie volgt uiteraard, maar als je geïnteresseerd bent in het deelnemen aan deze commissie, wordt je van harte uitgenodigd om je aan te melden via bestuurnesecc@gmail.com. Inmiddels zijn er reeds vijf aanmeldingen binnen, dus. meld je aan voordat het te laat is :-) Algemene Leden Vergadering 2017 : Zonder vooruit te willen lopen op de notulen van de ALV zoals die op de wetenschappelijke dag in Leeuwarden heeft plaatsgevonden, wil ik jullie in ieder geval op de hoogte brengen van het feit dat Huub Eijkelenboom gekozen is als penningmeester-elect. Huub behartigt als schaduw van onze schatbewaarder Peter Schouten het komend jaar tesamen met Peter de financiële zaken van de vereniging. Tijdens de ALV van 2018 verwachten we dat Huub voldoende ingewerkt zal zijn om het stokje op een goede manier van Peter over te kunnen nemen. Tot dat moment van overdracht is Peter Schouten de enige officiële penningmeester van de NeSECC. Voor de functie van vice-voorzitter had zich naast Roel Hendrickxs tevens Korrie Wierda als kandidaat aangemeld. Uiteindelijk is Roel, als huidige vice-voorzitter, door de leden herkozen voor een volgende termijn. Het bestuur wil met name Korrie heel hartelijk bedanken voor haar enthousiaste kandidatuur, en haar getoonde interesse en commitment richting de beroepsvereniging. Belangrijk voor de NeSECC is dat de statuten en huishoudelijk reglementen weer van een update voorzien zijn. De leden hebben de documenten versie 2017 tijdens de ALV goedgekeurd. De stukken zijn op de NeSECC website terug te vinden. Wetenschappelijke dagen : Op 12 mei is door de organisatoren van het Medisch Centrum Leeuwarden een zeer succesvolle wetenschappelijke dag georganiseerd. De opkomst was met bijna 90 deelnemers bijzonder hoog. De ochtend werd gevuld met presentaties van pas afgestudeerde klinisch perfusionisten. De kwaliteit van het gepresenteerde onderzoek was hoog, en voor een volle zaal ging Jori Tip - Leenders er met haar presentatie getiteld the influence of oxygen delivery during cardiopulmonary bypass on delirium in CABG patients er met de First Time Presenter Award van 500 euro vandoor. Het wetenschappelijk middagprogramma met als thema Hypothermie en verdrinking was zeer boeiend en onderhoudend. De najaars wetenschappelijke dag zal op zaterdag 4 november aanstaande worden georganiseerd tezamen met onze Belgische collega s. Als locatie voor deze eerste gezamenlijke wetenschappelijke dag heeft het Nederlandse kamp de eerste slag gewonnen : Breda wordt the place to be. Het Amphia Ziekenhuis zal dus als gastheer optreden, en onze collega's aldaar hebben er zin in. De komende maand wordt in gezamenlijk overleg het programma vastgesteld. 4 The treatment of accidental hypothermia and drowning with the aid of extracorporeal circulation/ecmo Kees Visser; Perfusionist, Ronald Schepp; Cardio-anaesthetist, Heart centre Friesland, The Netherlands Introduction The Heart centre Friesland is the youngest cardiac surgery facility in the Netherlands, starting in January It is located in the northern part of the country serving inhabitants. The environment is very water rich, with a lot of recreational activities on the water, in summer and winter time. Seven patients were presented to our centre in the last two years. Four of the patients severely hypothermic, three with mild hypothermia. Six of the patients were submerged and had aspirated large volumes of water. Results Three of the deep hypothermic patients where discharged without any morbidity, the last one was weaned from bypass, but suffered from cerebral damage due to insufficient resuscitation. The mild hypothermic patients all could be weaned from bypass, but predictably suffered severe cerebral damage. One of them became a non beating donor patient after five days of ECMO. Several cannulation techniques were used, depending on logistic possibilities and the indication, cardiac or pulmonary support. In the cardiac group we used three times a heart lung machine and two times mobile device with femoral canulation. For the pulmonary insufficiency patients the cardiohelp with veno-venous canulation was used. Bypass time ranged from 2 to 125 hours, depending on the recovery of the pulmonary function. The following describes a case report of one of our patients. Case report and techniques This case report is about a 31-year-old woman with severe hypothermia, who was presented at the OR of the Medical Centre Leeuwarden on December 10, She was found in her car under the ice, and was submersed for about 45 minutes, before the Mobile Medical Team arrived. After intubation, resuscitation was immediately started and the patient was transported to the hospital. At the moment of arrival, her rectal temperature was 21 C and her peripheral temperature 14 C. Fulminant pulmonary oedema was present, and the patient was in her 12 th week of pregnancy. The Cardiohelp system was used to restore circulation, using a femoral arterial-venous canulation. Circulation was restored with a cardiac index of 2,4. The arterial blood temperature was maintained at 21 C. This to convert the patient biochemically from a severe Ph-stat situation to an alpha-stat acid base regulation. Only after this, the patient was carefully rewarmed, while correcting the metabolic part of the acidosis. Rewarming to 34 C rectal temperature took about 7 hours. The patient was not rewarmed further, to facilitate the resuscitation protocol on the intensive care. After defibrillation and rewarming, the patient could not be weaned because of severe ARDS due to aspiration. The second day of support, the pulmonary function recovered, but stunning of the myocardium forced us to continue circulatory support. Finally, after 60 hours of support, the patient could be weaned from bypass. Unfortunately, a compartment syndrome occurred because of thrombus on the arterial canula. Outcome The baby was lost on bypass. For two days, the patient showed a severe neurological status, but starting the third day, she slowly recovered and walked, shortly after Christmas, by herself to the general nursing ward. At this moment, she has a complete neurological recovery and can perform her work a lawyer without any problem. Conclusion Although the amount of patients is not very high, the fact that these patients are most of the time completely healthy and young before the incident rectifies a protocollised approach. Regionally, it is of the utmost importance to train and inform the rescue services. For a good outcome the primary decision and a good logistical chain are very important. Intramural, a proper medical protocol is necessary, for the complexity of these procedures is too high to allow improvisation. Especially deep hypothermic patients are not doomed, even in the presence of drowning. The medical team should be prepared and motivated to continue the treatment with cardiac and/or pulmonary support for a longer period after the initial resuscitation.

6 The influence of oxygen delivery during cardiopulmonary bypass on the incidence of delirium in CABG patients Jori Leenders Introduction Postoperative delirium is the most common neurological complication of cardiac surgery. Hypoxia during bypass has shown to increase the risk of postoperative delirium. The possibility to continuously calculate oxygen delivery (DO 2 ) offers an adequate approximation of the oxygen status in a patient. Therefore, this study investigates the role of oxygen delivery during cardiopulmonary bypass in the incidence of postoperative delirium. Methods Three hundred fifty-seven adult patients that underwent CABG surgery under normothermic conditions were included in this retrospective study. The lowest DO 2 value on bypass, the accumulated time under the critical DO 2 level and the area under the curve were determined for each patient. Results The lowest DO 2 significantly differed comparing the group of patients with postoperative delirium to the group without. After establishing a critical threshold value of 310ml/min/m 2, significant differences were found for all DO 2 variables. However, multivariate analysis demonstrated that only age, pre-existing cognitive impairment, kidney dysfunction and cross-clamp time remained as independent risk factors. The results also indicated that patients of older age were more sensitive to a declined DO 2. Conclusion A low DO 2 during cardiopulmonary bypass is significantly associated with the incidence of postoperative delirium in CABG patients. However, a role of DO 2 as an independent predictor of delirium could not be proven. Winnaar first time presenters award!!

7 The effect of different cardiopulmonary support strategies during bilateral lung transplantation A retrospective evaluation of early outcome measures and mortality Daphne Mulders UMCG In the case of end-stage pulmonary disease, lung transplantation is still the only available treatment option. The procedure of bilateral lung transplantation can be performed with several strategies of cardiopulmonary support, namely; off-pump, cardiopulmonary bypass (CPB), emergent conversions from off-pump to CPB (conversion) and more recently with extracorporeal life support (ECLS). This study aimed to retrospectively evaluate the effect of these support strategies on early outcome measures, such as primary graft dysfunction(pgd) and mortality. To achieve this goal, 221 patients who underwent a bilateral lung transplantation in the UMCG between July 2015 and June 2016 were included in this study. The patients in the off-pump group (n=132) had significantly lower PGD scores at 0, 24, 48 and 72 hours after the procedure than the patients in the CPB (n=46) and conversion (n=31) groups. The ECLS group (n=12) had the lowest PGD scores at 24, 48 and 72 hours. Both the 30- day and 1 year mortality rates were highest in the conversion group, followed first by the CPB group and then the off-pump group. All ECLS patients were still alive after 1 year. With regard to duration of mechanical ventilation, ICU stay, hospital stay, post-operative ECLS, number of bleeding related rethoracotomies and transfusion of red blood cells, the offpump group had the best results, followed first by the ECLS group, then the CPB group and last the conversion group. In conclusion, off-pump procedures have the best results and should remain the gold standard for most patients. Conversions should preferably be prevented, but they are hard to predict and probably inevitable. ECLS appears to be a good alternative for CPB, but adaptations in the CPB circuits and procedure might reduce the negative effects of CPB in certain cases where its use is more desired.
