1 Online request frm fr requesting articles, bks and / r theses: Erasmus MC - emplyees New accunt : Register Medical Library website: On the right handed side, chse Quicklink 6. Online requestfrm articles / bks / theses. On the right upper side, chse Register t make a new accunt. Tick the bullet t state whether yu are an emplyee f the Erasmus MC, Student f the Erasmus MC (r EUR) r an Individual. 1

2 1. Emplyee Erasmus MC Beware: each time yu fill in a field, the frm jumps t the tp. Fill in yur micrsectin number. This will be yur username. Fill in yur Erasmus MC address. Beware: yu can nly fill in an Erasmus MC address. Give a passwrd. Minimal length is 4 characters. Cnfirm yur passwrd by filling it in again. Click n Registrer. 2

3 In yur mailbx yu will receive an cntaining a link. Click n the link t activate yur accunt. It is pssible the is in yur Spam bx. Click n the link. Yu will return t the frm. Lg in using yur micrsectin number (yur Username) and passwrd. Click n Lg in. 3

4 Befre yu can send a request yu have t fill in the tabs Delivery address, Budgetnumber and Invice address. Tab Delivery address The tab Delivery address is autmatically pened. Fill in yur Last name, Initials and seks. Chse yur department frm the drp dwn menu. If yur department is nt mentined in the drp dwn menu, then yu can add it. Fill in yur department name and click n the buttn Update. 4

5 Fill in yur rm number (r the rm where the mail is sent t). Fill in yur phne number (mbile r intern number). Fill in a different mail address, if yu want t receive the articles at a different mail address than the Erasmus MC address. Click n Save. Tab Budget number Click n the tab Budget number t add a budget number t yur accunt. If yu d nt want t use a budget number, g the tab Invice address. Click n Add new budget number. Fill in yur budget number (4 digits). If the number nly has 3 digits, add 0 (zer s) in frnt f it i.e. 123 will becme

6 Click n Search budget manager. Chse the right budget manager frm the drp dwn menu Yu will see the data which is knwn t us. If the budget number is unknwn, yu can fill in the data f the budget manager. Click n Save. An is sent t the budget manager. This states that yu want t request using that budget number. If the budget manager apprves, he / she can click n the link in the . Yu will see the fllwing text: 6

7 Yu will receive an , when the budget manager has authrized yu. If after sme time yu still have nt been authrized, click n the buttn Authrisatin request, t resend the t the budget manager. In the tab Budget manager, chse yur added budget number. Select frm the drp dwn menu Select a budgetnumber, the budget manager. Click n the buttn: Authrisatin request. A new is sent. Only when yu are authrized can yu sent a request! Tab Invice address If yu cannt use a budget number t request articles / bks, yu have t fill in the tab Invice address. Beware: internal addresses, r the address f the Erasmus MC is nt valid here. Fill in the fllwing fields: Yur name (Cntact persn) Institute (hgstwaarschijnlijk Erasmus MC) Male f female Department Address, pstal cde, city and cuntry (yur hme address). Telephne number (mbile, hme r internal number Erasmus MC) Click n Save. Yu have finished registering. Yu can sent requests. 7
