Doelgroep Het gebruik van de Nortonschaal richt zich op alle patiëntengroepen. Het meetinstrument werd reeds binnen een verscheidenheid van patiëntenpopulaties gevalideerd (Pancorbo-Hidalgo, Garcia-Fernandez, Lopez-Medina, & Alvarez-Nieto, 2006). Beschrijving De Nortonschaal is samengesteld uit vijf items, met name de algemene lichamelijke toestand, de geestelijke toestand, activiteit, mobiliteit en incontinentie. Het scoren van de verschillende items gebeurt op een vierpuntenschaal. De totaalscore zal bijgevolg variëren tussen 5 en 20. Als afkappunt hanteert men de grens van 14. Ingeval een score lager dan 14 dient men de patiënt als risicopatiënt te beschouwen (Defloor et al., 2004). De auteurs (Norton, McLaren & Exton-Smith, 1962) gaan er van uit dat de ontwikkeling van decubitusletsels gegarandeerd is indien de score lager ligt dan 12.
2 In latere publicaties stelt de auteur (Norton, 1987; Norton, 1996) dat een afkappunt van 15 of 16 wenselijker zou zijn. Dit heeft tot gevolg dat de sensitiviteit van het meetinstrument een aanvaardbaar resultaat oplevert. Hiertegenover staat dat (te)veel patiënten als risicopatiënt geïdentificeerd worden waardoor talrijke onnodige middelen ingezet worden. Tevens benadrukt de auteur dat een item zoals voeding ontbreekt in de Nortonschaal. Betrouwbaarheid Betrouwbaarheid van de Nortonschaal is zelden bestudeerd. In de review van Pancorbo-Hidalgo et al. (2006) werden slechts twee studies terug gevonden, de interrater reliability bedroeg r = 0.93 en r = In het onderzoek van Defloor & Grypdonck werd een waarde van r = 0.97 (p < ) gerapporteerd (Defloor & Grypdonck, 2005). Validiteit De validiteit van de Nortonschaal werd in mindere mate bestudeerd dan de Bradenschaal. De review van Pancorbo-Hidalgo et al. (2006) vond zes studies terug die hieromtrent handelen. Volgende intervallen werden weergegeven: sensitiviteit: 16-81%, specificiteit: 31-94%, positive predictive value: %, negative predictive value: %, area under the curve: %, odds ratio: 2.16 (95% CI = ). Meer recente onderzoeken die niet opgenomen zijn in de review en waarvan de waarden buiten de hierboven gerapporteerde intervallen vallen, zijn ondermeer Kwong et al. (2005) en Jalali & Rezaie (2005). In de eerste studie bedroeg de sensitiviteit 89%. Het tweede onderzoek rapporteerde volgende waarden: een sensitiviteit van 49%, een specificiteit van 100%, een positive predictive value van 100% en een negative predictive value van 52%. Gould, Goldstone, Kelly & Gammon (2004) hebben de concurrent validity van de Nortonschaal nagegaan via een simulatie waarin het oordeel van een expertenpanel vergeleken werd met risicobeoordelingen door verpleegkundigen aan de hand van de Nortonschaal. De beoordelingen van de foto s met casestudie-informatie door experten gold als gouden standaard. De overeenkomst bedroeg slechts 4.6%. Uit de hierboven beschreven resultaten kan worden geconcludeerd dat de Bradenschaal uitgebreider onderzocht werd en wat beter scoort dan de Nortonschaal. Dit wordt ook bevestigd in het onderzoek van Vanderwee (2006). Gebruiksvriendelijkheid De Nortonschaal werd geëvalueerd door 30 verpleegkundigen. Omwille van het dagelijks gebruik van dit meetinstrument binnen het ziekenhuis, vond de bevraging onmiddellijk plaats zonder een voorafgaande proefperiode van 3 weken. De schaal is echter duidelijk voor 87% van de respondenten. Vier op de vijf verpleegkundige zijn van mening dat de schaal ook op te maken is in afwezigheid
3 van de patiënt. De totaalscore is eenvoudig te berekenen (100% van de respondenten). De gemiddelde afnameduur bedraagt 3 tot 5 minuten. Opmerkingen Decubitusrisicoschalen, zoals de Nortonschaal en de Bradenschaal, voorspellen het risico op decubitus slecht (Defloor et al., 2004; Schoonhoven, 2003; Vanderwee, 2006). Veel patiënten zullen ten onrechte als risicopatiënt worden gecatalogeerd en heel wat patiënten zullen ten onrechte niet als risicopatiënt worden aangeduid. De predictieve validiteit van risicoschalen moet in vraag worden gesteld. Als effectieve preventieve maatregelen genomen worden bij echte risicopatiënten, zal het aantal nieuwe decubitusletsels verminderen en hierdoor daalt de sensitiviteit en specificiteit van de risicoschaal. Een decubituspreventiebeleid kan dus niet uitsluitend op een risicoschaal gebaseerd zijn. Risicoschalen dienen gecombineerd te worden met de klinische blik van verpleegkundigen, gebaseerd op jarenlange ervaring en gezond verstand (Defloor et al., 2004). Een mogelijk alternatief is het uitstellen van preventieve maatregelen tot op het moment dat niet-wegdrukbare roodheid optreedt. Dit blijkt geen effect uit te oefenen op de incidentie van het aantal decubitusletsels, niettegenstaande een significant kleiner aantal patiënten preventieve maatregelen nodig heeft (Vanderwee, 2006).
