2 Voorwoord terug naar inhoudsopgave Sinds 2006 bestaat de na- en bijscholingscommissie van de NVLKNF. De taak van de na- en bijscholingscommissie is het organiseren van na- en bijscholingen voor de Nederlandse Vereniging Laboranten Klinische Neurofysiologie (NVLKNF). Deze scholingen zijn vooral bedoeld voor Laboranten Klinische Neurofysiologie en Laboranten KNF in opleiding. Deskundigheidsbevordering voor laboranten vraagt om permanente na- en bijscholingen. Wij streven ernaar om u dit op een of andere wijze jaarlijks aan te bieden. Het doel van de commissie is om een uitgebreid pakket aan vakinhoudelijke na- en bijscholingen aan te bieden, welke in belangrijke mate voorziet in een landelijke en regionale behoefte. Bij na- en bijscholingen zullen de deelnemers een syllabus ontvangen, een certificaat ontvangen en worden geregistreerd als de na- en bijscholing volledig is gevolgd en aan de financiële verplichtingen is voldaan. De docenten zullen per na- en bijscholing worden gevraagd. 3 Inleiding terug naar inhoudsopgave Na al enige jaren na- en bijscholingen georganiseerd te hebben, denken we ook dit jaar weer een leuk programma voor u opgesteld te hebben. We hebben hierbij geprobeerd zoveel mogelijk afwisselende aspecten binnen ons vak gebied te belichten, waarbij we rekening gehouden hebben met de items die spelen binnen ons vak of waar veel vraag naar gedaan is. Wij hopen dat deze dag aan uw verwachtingen voldoet; dit voor u als bezoeker of cursist en hopen dat u met net zoveel plezier aan deze dag terug zult denken als dat wij deze dag georganiseerd te hebben. Wij wensen u een hele leerzame en prettige nascholingsdag toe en wij hopen dat u deze syllabus nog vaak zult gebruiken op uw eigen KNF afdeling. 19 Epileptische aanvallen: classificatie, semiologie en interpretatie Dr. W. Van Emde Boas, epileptoloog Stichting Epilepsie Instellingen Nederland Epilepsie is geen eenduidige hersenaandoening maar een uiting van een verstoorde hersenfunctie welke sterk uiteenlopende oorzaken kan hebben en welke zich op zeer uiteenlopende wijze kan uiten. De verschillende vormen van epileptische aanvallen vormen daarbij het primaire kenmerk van de aandoening. Traditioneel worden deze onderscheiden in een aantal vrij archetypische aanvalsvormen: absences, grote aanvallen, schudaanvallen, vreemde aanvallen, etc. In de loop van de 20 e eeuw zijn deze vormen beter beschreven en ondergebracht in opeenvolgende classificaties van epileptische aanvallen, de laatste in In deze laatste aanvalsclassificatie worden de aanvallen verdeeld in twee hoofdcategorieën: gegeneraliseerde aanvallen en partiële aanvallen. Ook voor de onderliggende aandoeningen (epilepsieën en epilepsiesyndromen) werden classificaties opgesteld, de laatste in Hierin worden de aandoeningen gecategoriseerd naar plaats (gegeneraliseerd of lokalisatie gebonden) en naar oorzaak (idiopathisch of symptomatisch met een tevens een tussencategorie: cryptogeen). Hoewel gebaseerd op klinische kennis (observatie, neuroanatomie, neurofysiologie, neuropathologie, EEG, beeldvorming en in toenemende mate ook neurogenetica) zijn beide classificaties toch in hoge mate conceptueel. Vooral door de opkomst van langdurige EEG en video registraties voor een betere diagnostiek van patiënten met epilepsie en voor een eventuele chirurgische behandeling van therapie resistente lokalisatiegebonden epilepsie is in de afgelopen 30 jaar echter duidelijk geworden dat de concepten welke aan de classificaties van 1981 en 1989 ten grondslag liggen niet langer houdbaar zijn. Met name de simpele dichotomieën partieel versus gegeneraliseerd, lokalisatiegebonden versus gegeneraliseerd en idiopathisch versus symptomatisch bleken niet langer houdbaar en voor met name de aanval classificatie werd herhaaldelijk gepleit voor een nieuwe indeling waarbij de aanvalssemiologie, het geheel van aanvalsverschijnselen en symptomen, al dan niet in samenhang met het EEG, het belangrijkste criterium diende te vormen. Het blijkt namelijk dat een nauwkeurige observatie van de aanvalsverschijnselen een redelijk betrouwbaar inzicht kan bieden in de plaats van aanvalsbegin en de bij de aanval betrokken neuronale netwerken en hersenstructuren en soms zelf een suggestie kan bieden over de onderliggende pathologie. Hoewel een nieuwe classificatie op dit moment nog niet beschikbaar is zijn er wel duidelijke suggesties gedaan over hoe in de komende tijd naar epilepsie en epileptische aanvallen dient te worden gekeken, met meer aandacht voor de onderliggende etiologie, genetica en neuropathologie en voor de specifieke neuronale netwerken welke bij de aanvallen zijn betrokken. Naast het nog steeds onvolprezen EEG zal kennis van en aandacht voor (een nauwkeurige analyse van) de aanvalsemiologie daarbij een niet meer weg te denken rol gaan spelen. In the present international classification of epileptic seizures complex partial seizures officially still are classified as a largely uniform category, subdivided according to some clinical features, yet independent of anatomical area of origin. Descrip!ions of focal motor seizures as archetypal partial seizures are found, however, already in the earliest medical texts available and the anatomo-clinical relation between such seizures and the frontal lobes was perceived as early as in the beginning of the 19th century. Hughlings Jackson developed the clinical concept of frontal lobe motor seizures as opposed to temporal lobe psychomotor seizures and Penfield and co-workers at the Montreal Neurological Institute provided further proof of the role of different discrete areas of cerebral cortex in various epileptic conditions. Following the introduction of interictal electroencephalography (EEG) and electrocorticography (ECoG), Penfield and Jasper extended these studies to the role of epileptogenic lesions and the electrophysiological mechanisms, implicated in partial seizures, thus greatly enhancing the understanding of the functional anatomy of the brain. The development of Stereo-EEG (SEEG), the recording of intracerebral EEG through stereo tactically introduced indwelling electrodes in various areas of the brain, and the study of ictal spatio-temporal scalp-eeg or SEEG patterns in relation to the clinical manifestations during spontaneous or chemically induced seizures further improved the recognition of anatomoclinical features, associated with seizure onsets in specific anatomical areas. Prompted by the re-emerging interest in surgical treatment for medically intractable partial seizures with its need for precise and reliable localising criteria, the 1989 proposal for the classification of the epilepsy s and epileptic syndromes first reflected the current approach of categorising partial epilepsy s as to their documented or presumed anatomical localisation. That such an anatomical subdivision is possible follows from the wealth of clinical semeiological and EEG data, obtained during the last two decades, following the widespread introduction of telemetric and computerised long term intensive EEG and video monitoring and the correlation of these data to the much improved neuroanatomical data, obtained by MRI, PET and SPECT or to surgical outcome data. The only definite proof of the localisation of the epileptogenic zone can be obtained by first documenting seizure onset in that area by intracranial recording and subsequently rendering the patient seizure free by the resection of that area. A fairly reliable estimate of the anatomical origin of most partial seizures is possible however by a careful analysis of the patient s history, the interictal EEG, available imaging studies and, most importantly, of the ictal EEG findings and the clinical signs and symptoms of the seizures. For the present purpose this text will focus predominantly on the clinical features of temporal - and extratemporal lobe partial seizures, with some attention to interictal and ictal EEG. Main emphasis will be given to temporal lobe seizures and frontal lobe seizures which, together, present the majority of the partial seizure types, encountered in daily clinical practice. Epidemiology The exact prevalence of partial seizures remains somewhat controversial because of discrepancies between studies in Western Countries and studies from developing countries bias due to patient selection, notably in surgically oriented programs and because of the often difficult differential diagnosis between some primary generalised epileptic syndromes and Na- en bijscholing 8 april 2010 pagina 20 van 95

21 frontal lobe partial epilepsy. Moreover, patients with diffuse or multifocal brain disorders, thus suffering from a symptomatic generalised epilepsy often have their partial seizures misclassified as generalised. In general some 40 60% of patients with epilepsy, including approximately. 50 % of the paediatric patients, are estimated to have partial seizures, many with complex partial seizures as their predominant seizure type. Estimates of prevalence of partial seizures, sub-classified as to their lobe of origin are even further biased by patient selection but at least in adult patients the overrepresentation of temporal lobe seizures appears real, not just due to their overrepresentation in surgical series but due to the inherent epileptogenic propensity of the mesial limbic structures which subsequently makes Mesial Temporal Lobe Epilepsy (MTLE) the epilepsy syndrome that is most amenable to surgical intervention. In adult surgical series temporal lobe seizures represent approximately 70% of the patient material; the remaining 30% of extratemporal seizures mostly involve the frontal lobe with only 10% originating from the parietal or occipital regions. This, however, holds not true for paediatric surgical series which include proportionally more extratemporal lesional cases. Reliable studies, concerning the prevalence of frontal lobe epilepsy specifically in childhood, however, are not available General characteristics of temporal lobe versus extratemporal lobe partial seizures Even without direct observation or Video-EEG documentation a careful clinical history, taken from the patient and his relatives, will yield a number of characteristics which may already give a first hint concerning the most likely anatomical origin of the seizures. Temporal lobe seizures tend to occur occasionally, none to several per month, and predominantly during the day. Usually they are preceded by an aura, followed by a gradual onset of the seizure, with arrest or continuation of ongoing activity and impairment of consciousness which may last for 2-3 minutes and often is followed by continued confusion and disorientation for at least another 1-3 minutes. Motor phenomena occur relatively late and remain relatively discrete with the exception of pronounced unilateral dystonic posturing. Automatism s are common but relatively quiet and mostly limited to oro-alimentary automatism s and minor semi-purposeful repetitive movements of the distal limbs. Occasional secondary generalisation may be noted but evolution to generalised status epilepticus is rare. Frequently, in patients with MTLE, this seizure pattern will be associated with a positive family history for epilepsy or febrile seizures and a seizure history, beginning with early childhood febrile convulsions, onset of spontaneous seizures in the course of the first decade, then a seizure free period of several years followed by recurrence of complex partial seizures which subsequently prove refractory to medical treatment at the end of the second decade. If further associated with neuropsychological evidence of functional temporal lobe impairment, MRI evidence of unilateral hippocampal atrophy and with certain highly characteristic interictal and ictal EEG patterns over the ipsilateral antero-temporo-basal region, a firm diagnosis of unilateral MTLE appears warranted and the patient should be referred for surgical treatment (anterior and mesial temporal lobe resection) rather than be treated by yet another gamut of old and new anticonvulsive drugs. Frontal lobe seizures, which represent the majority of all extratemporal seizures, exhibit a varied spectrum of signs and symptoms, yet as a category often can be recognised by a number of typical clinical characteristics. Their frequency often is high with multiple, sometimes up to tens of seizures per day or notably and in some patients exclusively- during the night. 22 frequent seizures alternating with weeks to months of (almost) seizure freedom. Auras are less common than in temporal lobe seizures and onset and offset of the seizures is more abrupt, even when limited to arrest of activity and impairment of consciousness, and often rather explosive. In most types of frontal lobe seizures motor activity is the predominant feature, either as focal, unilateral or bilateral tonic or clonic activity, pronounced asymmetric tonic posturing or as sudden, bizarre and uncontrollable major motor automatism s of the proximal limbs and the axial muscles, sexually oriented motor movements and often violent vocalisations which together may easily be mistaken for non-epileptic psychogenic behaviour. The seizures tend to be brief, often less than 1 minute, and postictal recovery is quick, unless in seizures with secondary generalisation or in cases with focal-, secondary generalised- or nonconvulsive status epilepticus, all of which relatively common in frontal lobe epilepsy. Occipital lobe seizures are uncommon and most of their clinical features will resemble either temporal lobe or frontal lobe semeiology, due to multiple propagation patterns, sequentially involving temporal and/or frontal lobe structures. Seizures that have their onset in the occipital cortex below the Fissura Calcerina tend to propagate toward the temporal lobe while those originating above the fissure more often will spread towards the central and frontal regions. These differences are not absolute however; moreover, due to the intricate connectivity within the occipital area itself seizure activity may move up- or downwards before further propagation occurs. Different seizure types thus can be seen even in individual subjects, suggesting multifocal epilepsy. Early signs such as prominent forced blinking, eye flutter, the subjective sensation of eye movement or objective tonic or clonic eye deviation, elementary visual hallucinations or initial ictal blindness all suggest an occipital seizure origin, notably if associated with occipital structural lesions, identified by MRI. Formed visual hallucinations are more usually associated with the posterior temporal lobe but may occur following seizure propagation from occipital to temporal lobe structures, preceding more typical temporal lobe signs and symptoms. Seizure propagation from occipital to frontal lobe areas usually results in primary motor- or secondary motor area (SMA) type of seizures. Parietal seizures, too, are considered to be rare but their prevalence is hard to establish since there are few if any reliable clinical signs or symptoms indicating seizure origin in the postcentral parietal region. Lateralised paresthesias, which according to Penfield and Jasper were typical for parietal seizures, are in fact only found in less than half of the patients. Lateralised pain in the extremity or the abdomen has been described as an early but rare symptom in parietal lobe seizures. Other subjective features include complex somato sensory sensations (limb movement, somatopagnosia, apraxia), opercular sensations (gustatory hallucinations, choking) and autonomic sensations (nausea, vertigo, sexual sensations) but none of these are specific. Objective signs in parietal lobe seizures practically always reflect seizure propagation to the frontal or temporal lobe and usually the presence of a structural or functional abnormality, identified by MRI or SPECT, is necessary to recognise probable parietal lobe seizure onset. Temporal Lobe Complex Partial Seizures Temporal lobe seizures may remain limited to a subjective aura or may evolve into a true complex seizure. The most common sequence of symptoms is behavioural arrest with or without motionless stare, usually reflecting impairment of consciousness, followed by oro-alimentary automatism s, reactive or semi-purposeful hand automatism s, looking around, some whole body movements and a gradual shift into a postictal state with some confusion. Other common signs are contralateral dystonic posturing of the upper extremity and ipsilateral non- Na- en bijscholing 8 april 2010 pagina 22 van 95

23 forced head turning. Although this sequence in the majority of patients will be preceded by a viscero-sensory, experiential or other aura this may not be remembered by the patient. The typical rising epigastric sensation is in fact found in less than 50% of all temporal lobe seizures and other autonomic signs and symptoms (pallor, flushing, sweating, nausea, pupillary dilatation, gustatory sensations and tachycardia) are often seen instead. Less typical patterns are common, however, since temporal lobe seizures can show highly varying clinical patterns, mostly amongst patients but even within a single patient, depending on the exact area of seizure onset, the extent of the epileptogenic zone, the underlying pathology and many other factors. Seizure semeiology may be extremely poor, limited to behavioural arrest, which unless marked by the patient or detected by the computer may go by unnoticed. On the other hand, consciousness and responsiveness may be retained, even in seizures with otherwise quite pronounced clinical signs, including the typical automatism s, probably as the result of limited and predominantly unilateral seizure spread. Ictal fear may be reflected in a facial expressio!, restless behaviour and vocal automatism s Pronounced motor activity, however, notably when tonic or clonic, usually reflects either seizure propagation to extratemporal areas or, if present early on in the seizure, an extratemporal seizure onset. Impairment of consciousness, by definition, is one of the hallmark signs of these seizures but is often difficult to assess; as noted above it may be lacking and the fact that automatism s often are reactive, albeit inappropriate, to environmental objects or stimuli suggests that impairment of consciousness in temporal lobe seizures reflects a gradual rather than an all or nothing phenomenon. Postictal confusion may be prolonged and associated with other signs, including early postictal nose wiping, postictal cough postictal speech impairment, paresis, neglect phenomena and/or amnesia for the seizure. In infants and young children the typical semeiology of temporal lobe seizures may be different from that in adults, certainly below the age of two years. The International Classification of Epileptic Syndromes subdivides temporal lobe epilepsy in mesial temporal lobe epilepsy and lateral or neocortical temporal lobe epilepsy. Whereas the clinical and electrographic characteristics of mesiolimbic epilepsy have been well described, the syndrome (or syndromes) of neocortical temporal epilepsy remain(s) relatively unexplored and controversial. Some clinical features (i.e.: lack of typical aura, less pronounced oro-alimentary automatim s, early auditory or vertiginous sensations, early and pronounced motor signs, pronounced ictal retching and vomiting in case of seizures from the non-dominant temporal lobe) have been associated with seizure onsets in the posterior lateral and/or basal temporal neocortex and associated EEG patterns have been reported as more widespread, somewhat slower and more irregular than the typical anterior-mesial EEG pattern. A reliable differentiation between mesiolimbic vs. neocortical temporal lobe seizures on clinical (or even EEG) criteria alone remains difficult, however, and usually requires complimentary investigations, including MRI, PET,.Wada test or even intracranial seizure recordings. Frontal Lobe Seizures The Commission on Classification and Terminology of the International League against Epilepsy recognises seven sub-types of frontal lobe epilepsy but this compartmentalisation of frontal lobe seizures into clinical syndromes, associated with specific anatomical regions of the frontal lobes remains controversial. 24 epilepsy syndromes is often difficult due to the functional anatomy of the frontal lobes with its extensive inter neuronal network and high propensity for extremely fast, widespread and bilaterally synchronous spread of epileptic activity to and from ipsi- and contra lateral frontal and extra frontal structures. Moreover, apparently frontal lobe seizures may well have their origin outside the frontal lobe. Primary motor cortex seizures, which are thought to arise from the primary motor area (Brodmann's area 4) represent the most common and most archetypal expressio! of frontal lobe epilepsy. The clinical symptoms reflect the motor Homunculus, with the caveat that primary motor cortex activation may result from intracortical propagation from other brain areas. The clinical manifestations of these seizures consist of isolated or repeated myoclonic jerks or clonic movements in a limited number of muscles, depending on site and extent of the cortical area involved. Onset in the Rolandic area produces clonic movements in the contralateral extremities, in the arm more than in the leg, with or without Jacksonian march and with or without secondary generalisation. Seizures in the lower prerolandic area result in speech arrest or vocalisations, dysphasia, tonic clonic facial movements and swallowing. Seizures involving the paracentral lobule affect predominantly the legs, including the ipsilateral foot as well as the contralateral leg. Auras are rare and, unless the seizure activity spreads to adjacent areas, consciousness is unimpaired and no automatisms occur. Secondary generalisation may follow a symmetrical or, more often, strongly asymmetrical pattern. Seizure duration varies but, if no generalisation occurs, usually does not extend 1-2 minutes. Postictally, a transient paresis or paralysis of the maximally involved contralateral muscles is common. Supplementary motor seizures originate in the supplementary motor area (SMA, Brodmann s area VI). Typical SMA seizures are characterised by brief asymmetric tonic posturing with tonic abduction and external rotation of the contralateral shoulder and arm and adversative movement of the head and eyes, suggestive of "looking at what is happening with that arm", a more or less explicit extension of the ipsilateral arm (fencing position) and asymmetrical tonic extension of the legs. Some seizures are preceded by a sensory ("tingling", "pulling" or "heavy") sensation. Less typical tonic unilateral but more often bilateral posturing movement patterns are common however. Speech arrest may occur, but vocalisations (crying, moaning, shouting or intelligible forced speech) are common. A few clonic movements may be seen towards the end of the seizure or secondary generalisation may occur, usually preceded by a contralateral adversative movement of head and eyes. Consciousness is usually retained, but may be difficult to assess, due to the overall brevity (10-40 seconds) of these seizures and the sometimes loss of speech. Postictally some fleeting distal paresis may be noticed. Dorsolateral seizures, emanating from the dorsolateral frontal cortex start with a forced tonic or clonic contralateral adversative head and eye turning, eyes usually first, without immediate pronounced tonic posturing but quickly followed by partial clonic or tonic-clonic motor activity in the contralateral face and arm more than the leg, speech arrest, aphasia and/or behavioural arrest. Auras are common, notably cephalic or contralateral somatosensory sensations. If not distracted by major motor activity the patients may be aware of visual illusions or hallucinations like a diffuse change in general visual ambience, obsessive forced thinking, possibly associated with left frontal lesions, or of forced actions with an intentional character but without the intense emotional content, associated with temporal lobe seizures. Consciousness is usually impaired, distinguishing this pattern from SMA type version. Spread to or from other areas is associated with atypical- or pseudo-absence type seizures or with Na- en bijscholing 8 april 2010 pagina 24 van 95

25 early bilateral motor activity, complex posturing, automatism s or secondary tonic clonic generalisation which may hamper differentiation with seizures originating elsewhere. Opercular (peri-sylvian, insular) seizures originate from the opercular insular or perisylvian region including the posterior part of the inferior (F3) frontal gyrus. They are characterised by pronounced oro-alimentary signs (mastication, salivation and swallowing), laryngeal symptoms (laryngo-spasm, speech arrest) and focal clonic jerking in the contralateral or ipsilateral oro-facial muscles, usually without impairment of consciousness. Auras may resemble temporal lobe epigastric sensations but usually involve the full extent of the gastrointestinal and/or urogenital tract, including gustatory sensations or hallucinations and sexual feelings. Other autonomic phenomena involving thermoregulation, respiration or the cardiovascular systems are common as is an aura of fear. Sensory numbness of the face, tongue or both hands may reflect involvement of the second sensory area. Ictal or postictal aphasia, dysarthria or anarthria suggests involvement of the dominant hemisphere. Anterior frontal and frontopolar seizures from the anterior frontal and medial intermediate frontal areas often remain limited to atypical- or frontal lobe absences, brief episodes with arrest of speech and/or behaviour and impairment of consciousness, either with opening of the eyes and staring or with some adversative or contraversive head and eye movements and occasionally some simple gestural automatism s without lateralising value. Other features include autonomic signs and axial tonic or clonic movements, falls and/or secondary tonicclonic generalisation. Seizure propagation to more posterior areas may produce confounding signs and symptoms. Non-convulsive status epilepticus or confusional state with spike and wave discharges, if not associated with other (motor) signs, probably involves predominantly the anterior frontal region. Cingulate seizures from the anterior cingulate gyrus (Brodmann s areas 24 and 32) are complex partial seizures with complex motor automatism s at onset, frequent vegetative signs and changes in mood and affect. The latter may present as an intense feeling or expressio! of fright, horror or fear, including defensive or aggressive vocalisations or shouting. The automatic gesticulate behaviour typically includes rather bizarre and sometimes semipurposeful complex bicycling, swimming, boxing or kicking movements of all extremities and sexually oriented movements such as pelvic trusting, obscene gestures or genital manipulation. Vegetative signs include cardiovascular and respiratory changes, digestive symptoms and urinary incontinence. Consciousness often is not completely lost and the combination of some retained contact with the surroundings, the predominant emotional symptoms, the bizarre and aggressive motor symptoms and the sexual behaviour may easily result in a misdiagnosis of hysteria and/or admittance to a psychiatric ward. Orbitofrontal seizures are presumed to originate from the subfrontal cortex overlying the orbital region. According to the ILAE definition these are complex partial seizures with initial motor or gestural automatism s associated with olfactory illusions or hallucinations and autonomic signs. There remains controversy, however, concerning the differentiation between orbitofrontal and cingulate seizures. Some authors consider the elaborate motor automatism s, associated with cingulate seizures, as orbitofrontal. However, seizures, strictly limited to the orbitofrontal region may remain asymptomatic or limited to olfactory, gustatory or gastrointestinal symptoms without predominant motor activity, suggesting that the more elaborate behavioural changes and automatism s, associated with orbitofrontal seizures represents seizure propagation to the anterior cingulate- or the operculo-insular-amygdalar area. 26 In temporal lobe seizures the more overt clinical signs rarely become evident before the seizure has spread to involve widespread areas of the mesial and lateral temporal lobe and the frontal lobe, ipsilateral to the side of seizure onset or bilaterally. Accordingly, the localising and lateralising value of individual clinical signs and symptoms in these seizures should be interpreted cautiously since to a large extent they reflect seizure propagation to the ictal symptomatogenic area rather than the initial activity at the site of seizure onset. Moreover, the differential diagnosis between temporal lobe seizures and frontal lobe seizures is complicated because of the high connectivity between the opercular-insular region, the frontoorbital cortex, the anterior cingulum and the mesiolimbic (amygdala and hippocampus) and anterior temporal structures, due to which frontal lobe seizures may have their origin in the anterior temporal lobe or vice versa. Nevertheless, if interpreted within the context of all available clinical data and analysed in their temporal relation and overall gestalt a number of well defined ictal and postictal clinical signs and symptoms will permit to localise and even lateralise the site of origin of many temporal lobe seizures with a reasonable degree of reliability. In frontal lobe seizures, too, most clinical signs may reflect seizure propagation from adjacent areas within or outside the ipsi- or contralateral frontal lobes proper. In general, therefore, most anatomically distinct frontal lobe seizures are hard to distinguish and often even the distinction between complex partial seizures of frontal lobe versus those of parietal-, occipital- or temporal lobe origin remains difficult. Only focal motor seizures have a highly reliable localising and even lateralising value, but even these may have their true origin in more anterior frontal or more posterior parietal or peristriate occipital cortical areas. SMA seizures, too, usually can reliably be identified although the typical asymmetrical posturing ( fencing ) may be lacking or could be confounded with the contralateral dystonic posturing of temporal lobe seizures. Seizure onset in the SMA as such is rare, however, and in many patients the ictal signs reflect seizure activity propagated from adjacent or distant areas. Strictly on their own, therefore, few if any individual signs or symptoms are unequivocally reliable for localisation or lateralisation of the epileptogenic zone in temporal or extratemporal lobe epilepsy. Only the combination of all clinical data, including medical history, physical examination, neuroimaging, interictal EEG and usually- the actual documentation of ictal signs and symptoms and ictal EEG will permit a reasonable estimate of the localisation of the epileptogenic zone in a majority of patients with localisation related epilepsy s and for definite localisation invasive ictal recordings may yet be required. Temporal and extratemporal lobe seizures in children The data, presented in the previous sections by and large represent the situation in adult patients and cannot automatically be assumed valid in a paediatric patient population. Notably in very young children with lesional and often catastrophic seizure syndromes the seizure semeiology is usually rather non specific, often suggestive of generalised, albeit asymmetrical, epilepsy and these children may present with infantile spasms or Lennox-Gastaut like multiple seizure types despite clear evidence of localised epileptogenic lesions. Accordingly, for presurgical evaluation in the very young age group, the monitoring of ictal EEG and video may be less important and more emphasis should be given to the interictal EEG and to structural (MRI) and functional (PET, SPECT) neuroimaging. Even in the somewhat older age group the semeiology of frontal lobe seizures remains slightly different from the adult group, with more evidence of absence-like behavioral arrest, spasms and / or (asymmetrical) tonic-tonic clonic motor features and less hypermotor seizures or seizures with complex motor automatisms. With advancing age, however, and notably in the second decade of life the ictal semeiology of temporal, frontal and other extratemporal seizures in children becomes more and more identical to the adult seizure patterns. 27 Interictal and ictal EEG in temporal and frontal lobe epilepsy Regular scalp recorded EEG remains the most sensitive and specific study for the diagnosis of epilepsy but suffers from some intrinsic shortcomings in regard to its localising value. To increase sensitivity for temporal lobe epilepsy and seizures at least the true temporal electrodes F9-F10 and preferably a full line of inferior electrodes should be added to the regular international system and for ictal recordings sphenoidal electrodes are still commonly used, despite continuing controversy concerning their value. For frontal lobe or other extratemporal lobe epilepsy s the application of fronto-orbital-, system- or other closely spaced additional electrodes may improve EEG sensitivity. Typical interictal EEG findings of anterior-mesial temporal lobe epilepsy consist of state independent unilateral intermittent rhythmic theta and delta activity (TIRDA) over the anterior temporal region, present during 50% or more of the time, and of predominantly (80-90%) unilateral temporal spikes or sharp waves, maximal at the sphenoidal and/or true temporal electrodes during wake or REM-sleep. Both are highly correlated to hippocampal sclerosis and anterior-mesial temporal ictal onset on that side, even when during NREM sleep there is evidence of active bitemporal spiking. More posterior or diffuse semi-continuous or intermittent polymorphic delta is more suggestive of (extratemporal) neocortical or lesional epilepsy. In frontal lobe epilepsy the localising value of non-invasive interictal EEG often remains limited. Focal spikes may be present in some patients with localised epileptogenic lesions, but in many patients epileptic discharges will be found over large areas of both frontal lobes, sometimes with a contralateral maximum or suggestive generalised epilepsy, due to volume conduction, the often large extent of frontal lobe epileptogenic zones and to the propensity to secondary bilateral synchrony of frontal lobe discharges, notably in patients with intrahemispheric seizure origin. In other patients interictal paroxysmal activity may be lacking, even during sleep or remain limited to sharp waves over the vertex. Ictal recordings with non-invasive EEG should always be interpreted with care, notably in temporal lobe epilepsy, since intracranial studies have shown that ictal activity often remains restricted to hippocampal and/or amygdalar structures for tens of seconds and only is reflected in the scalp-sphenoidal EEG following propagation to basal and lateral structures and often only after the appearance of clinical signs. Nevertheless a thorough analysis of the whole electrographic episode, including the morphology and temporal evolution of pre-ictal and postictal phenomena often will help to define the seizure onset zone. In temporal lobe seizures some subtle initial changes, slowing or suppression of ongoing activity, may be observed before the emergence of unequivocal ictal electrographic patterns, most commonly a high voltage blunt or sharp 5-7 Hz. theta rhythm over one temporal region, maximal on the sphenoidal and/or anterior/inferior temporal electrodes, followed by a gradual evolution, usually slowing of the ipsilateral activity, activation of the contralateral temporal region, propagation of seizure activity to other areas, both ipsi- and contralateral and occasionally secondary generalisation. This highly typical delayed focal onset pattern, if seen as the first ictal rhythm within 30 seconds of the electrographic onset of the seizure, is predictive of an ipsilateral anteriormesial temporal seizure onset in up to 82% of the cases. The predictive value is better in patients with this delayed focal onset pattern consistently in all recorded seizures than in patients with a mixture of focal and non-focal or non-localising seizures (94% vs. 63%). It may be falsely lateralising, though, and, although more commonly associated with temporal lobe seizures, it may also be seen in extratemporal cases. In the remainder of the temporal lobe patients a vast variety of localised, lateralised, bilateral or diffuse patterns are observed, usually with irregular or (semi) rhythmic polymorphic delta and slow theta activity but occasionally including faster rhythms or even spikes or spike and waves. A slow initial pattern of polymorphic 2-4 Hz activity, notably when equipotential or Na- en bijscholing 8 april 2010 pagina 27 van 95

28 maximal on the posterior temporal electrode but not over the antero-temporal region, may suggest neocortical seizure onsets. Postictal slowing or spiking may provide additional information but the reliability of postictal changes for localisation or even lateralisation appears limited and mostly adjunctive, if consistent with other findings. For frontal lobe epilepsy the interictal EEG often remains more enigmatic, showing either nothing or nonspecific abnormalities only or, on the contrary, showing frequent discharges which, due to their bilateral synchronous distribution, suggest a generalised epilepsy and result in erroneous classification and treatment. Notably in children the differential diagnosis between idiopathic or cryptogenic generalised epilepsy and cryptogenic frontal lobe epilepsy often remains extremely difficult, based on the history and the interictal EEG alone. Ictal recordings thus may be required but in frontal lobe seizures non-invasive ictal recordings are often hampered by fast bilateral involvement or are rendered illegible by massive muscle artefacts. Diffuse electrodecremental ictal patterns may reflect focal rather than generalised seizure onsets and late or even postictal sharp slow waves ( Zeta waves ) over the prefrontal region or the vertex may be the only EEG evidence of orbitofrontal seizures or seizures from the interhemispheric pericentral region. While non-invasive ictal patterns, combined with video-data of the seizure semeiology and other clinical data, will usually permit a reliable differential diagnosis between most temporal lobe versus extratemporal lobe seizures, precise localisation and lateralisation of the latter category, for purpose of surgical treatment, will usually require additional studies, including intracerebral or subdural grid recordings. 48 De behandeling van kinderen met epilepsie met antiepileptica terug naar inhoudsopgave Professor Onno van Nieuwenhuizen Prof. dr. Onno van Nieuwenhuizen Afdeling Kinderneurologie, Wilhelmina Kinderziekenhuis, Utrecht Inleiding: Het kind met epileptische aanvallen is een uitdaging voor de behandelaar. Epileptische aanvallen zijn onverwacht en schokkend; heel veel ouders die voor t eerst met een aanval bij hun kind worden geconfronteerd denken dat het kind letterlijk in doodsnood verkeert en vrezen een volgende aanval (1). De behandelaar kan niet beter doen dan direct met antiepileptica aan te vangen. Hij heeft daarbij de wetenschap dat het gebruik van antiepileptica bij patiënten met epilepsie bij 70% leidt tot aanvalsvrijheid. Maar rechtvaardigt dit een direct begin? Wanneer moet men met antiepileptica beginnen? In een recent verschenen review (2) wordt deze vraag goed beantwoord. Beginnen na een enkele aanval is niet noodzakelijk; tóch beginnen (druk van ongeruste ouders) zou bij 30-70% van de kinderen overbodig zijn. Men kan dus wachten; op een volgende aanval. Twee spontane epileptische aanvallen vormen de basis voor de diagnose epilepsie en de indicatie voor behandeling. Dit geldt ook indien een (ongeprovoceerde) status epilepticus het eerste symptoom van epilepsie is. Omdat een eerste status bij 32-51% van de kinderen door een tweede kan worden gevolgd wordt in deze gevallen vaak wel voor start van therapie gekozen. Terughoudendheid geldt ook voor de goedaardige focale kinderepilepsie, zoals de Rolandische epilepsie. Er zijn echter ook kinderen waar men direct, na het stellen van de diagnose, wil beginnen met behandelen. Dit spreekt vanzelf bij epilepsievormen met een hoge lijdensdruk, zoals het Syndroom van West (salaamkrampen). Dit geldt ook voor epilepsie met aanvallen met een hoog risico op letsel (valaanvallen bij het Lennox-Gastaut Syndroom) of epilepsie met cognitieve belemmering, zoals de absence epilepsie. Men kan deze lijn volgend een onderverdeling naar ernst van de epilepsie in 4 groepen maken (3) en zich dan opnieuw de vraag stellen: beginnen?. Men onderscheidt: de excellente prognosegroep (benigne partiële epilepsie bij kinderen, zoals de Rolandische epilepsie en aanvallen die alleen op bepaalde triggers ontstaan): behandeling vaak niet nodig. Vervolgens goed : childhood absence epilepsy, idiopathische epilepsie met tonisch-clonische aanvallen en sommige partiële epilepsieën: behandeling vaak wel nodig, maar ook dikwijls effectief. De groep onzeker : juveniele myoclonus epilepsie en het merendeel van de partiële epilepsieën: behandeling nodig, recidieven bij onttrekken van medicatie frequent. En tenslotte: slecht : de epileptische encefalopathieën (Syndroom van West, van Lennox-Gastaut, van Dravet, epilepsie in het kader van een neurometabole aandoening): de respons op de medicamenteuze behandeling is zeer gering.. Bij de afweging al dan niet met behandelen te beginnen is uiteindelijk ook het risico op (fatale) ongelukken veroorzaakt door de epileptische aanvallen een factor. Verdrinking komt bij kinderen met epilepsie maal meer voor (4). Na- en bijscholing 8 april 2010 pagina 48 van 95

49 Welk antiepilepticum? Op dit moment is het onmogelijk om van de individuele patiënt een profiel te maken dat leidt tot de keuze van het antiepilepticum waar deze optimaal op zal reageren. In veel gevallen komt men niet verder dan trial and error ; men beperkt zich tot de meest gebruikte antiepileptica met een breed werkingsgebied, zoals valproaat en carbamazepine. Zijn beide middelen niet effectief dan dient allereerst de diagnose epilepsie te worden heroverwogen. Epilepsie is een moeilijk te stellen diagnose en zelfs bij patiënten die al jarenlang bekend zijn met epilepsie en waarbij bij de behandelaar geen enkele twijfel bestaat over de juistheid van de diagnose, blijkt een flink percentage (30%) bij herbeoordeling geen epilepsie te hebben (5). Na het heroverwegen van de diagnose dient ook een paradoxale reactie op het antiepilepticum te worden overwogen. Dit is met name bekend van carbamazepine en difantoïne. Carbamazepine kan een toename van aanvallen geven bij kinderen met absence epilepsie, atypische absence epilepsie, myoclonische aanvallen en astatische aanvallen bij het Lennox-Gastaut Syndroom en juveniele myoclonus epilepsie. Difantoïne kan hetzelfde veroorzaken bij kinderen met absence epilepsie en juveniele myoclonus epilepsie (6). Is het alleen maar trial and error of zijn er toch onderbouwde keuzes mogelijk? De classificatie van epilepsiesyndromen beïnvloedt de keuze van de medicatie; het is derhalve belangrijk over voldoende kennis van de verschillende epilepsie syndromen op de kinderleeftijd te beschikken. Het Syndroom van West bijvoorbeeld dient direct na diagnose met ACTH (adrenocorticotroop hormoon) te worden behandeld (7). Aangezien het syndroom met progressieve ontwikkelingsstagnatie en -regressie gepaard gaat is een snelle diagnose van belang; kennis van semiologie (salaamkrampen in alle varianten) en het EEG (typische hypsarrhythmie en alle hypsarrhythmie-like varianten) is dus noodzakelijk. Overigens: de onderverdeling tussen partiële en gegeneraliseerde epilepsie en de daarmee samenhangende voorkeur voor bepaalde antiepileptica is in de praktijk zonder hulp van een aanvalsregistratie in een tertiair centra niet altijd eenvoudig te maken. Bij uitblijven van effect: doorgaan met zoeken? De behandelaar heeft een keuze uit tientallen antiepileptica. Moeten deze allemaal worden geprobeerd? Met andere woorden: heeft doorzoeken zin? In de veel geciteerde publicatie van Kwan en Brodie (8) wordt gesteld dat doorzoeken na het falen van twee eerste keus medicamenten (b.v. valproaat en carbamazepine) zinloos is. Recent zijn echter publicaties verschenen waarin wordt aangegeven dat er nog heel wat te winnen is bij het doorgaan met zoeken naar een voor de individuele patiënt effectief antiepilepticum. Luciano en Shorvon (9) beschrijven dat door toepassing van de meest recente antiepileptica volledige aanvalsvrijheid met een duur van tenminste 1 jaar kan worden bereikt bij 16% van een groep volwassen patiënten die als onbehandelbaar bekend stonden. De stelling na 2 antiepileptica zonder resultaat is men onbehandelbaar is dus niet vol te houden. Maar; doorzoeken zonder oog te hebben voor andere behandelingsmogelijkheden (chirurgie, ketogeen dieet, nervus vagus stimulatie) kan sterk in het nadeel van de patiënt uitpakken. In de work-up van geschikte kandidaten voor epilepsiechirurgie is het gemiddeld dokters-delay in ons land 6 jaar. Indien men aanneemt dat men niet meer dan 1 jaar nodig heeft om de eerste twee antiepileptica uit te proberen, dan kan vijf jaar eerder worden nagegaan of chirurgie een optie is. Oud of nieuw In de laatste twintig jaar zijn 10 nieuwe antiepileptica ter beschikking gekomen. Men kan aldus een onderverdeling in oude en nieuwe antiepileptica maken. Het is niet ongebruikelijk in de geneeskunde om te kiezen voor nieuw ; men loopt niet graag het risico achter te raken bij nieuwe ontwikkelingen. Oud zijn valium, carbamazepine, phenobarbital, phenytoine en valproaat; nieuw zijn felbamaat, gabapentine, lamotrigine, levetiracetam, Na- en bijscholing 8 april 2010 pagina 49 van 95

50 oxcarbazepine, pregabaline, tiagabine, topiramaat, vigabatrine en zonisamide. Het staat niet vast dat de nieuwe effectiever (meer aanvalsreductie) zijn dan de oude (10,11). Wel is het gebruik (met name het instellen) eenvoudiger. Voor zover bekend lijken de bijwerkingen van de nieuwe minder te zijn dan die van de oude, maar hierbij dient te worden opgemerkt dat bijwerkingen soms pas na jaren duidelijk worden (de concentrische gezichtsveldbeperking bij vigabatrine werd pas 6 jaar na introductie bekend (12). In Europa kozen ervaren behandelaars van epilepsie op de kinderleeftijd in 2007 merendeels voor oud (13). Zij kozen valproaat als eerste keus bij symptomatische myoclonus epilepsie en generaliseerde tonisch-clonische aanvallen, bij complex-partiële aanvallen, bij het Lennox-Gastaut Syndroom, bij Rolandische epilepsie, bij childhood en juvenile absence epilepsy en bij juveniele myoclonus epilepsie. Carbamazepine ( oud ) en oxcarbazepine ( nieuw ) werden als eerste keus geselecteerd voor complex-partiële aanvallen. Nieuw werd gekozen bij de behandeling van het Syndroom van West (vigabatrine, wanneer het syndroom voorkwam in aanwezigheid van tubereuze sclerose) en bij de behandeling van childhood en juvenile absence epilepsy (lamotrigine). Compliance Met name bij adolescenten speelt therapietrouw, compliance een belangrijke rol. Kyngäs (14) vermeldt dat 30% van de adolescenten met epilepsie zich niet houdt aan het doktersvoorschrift (bij andere chronische ziekten op de adolescentenleeftijd is dit percentage gunstiger: b.v. 6% bij diabetes, 16% bij juveniele arthritis en 17% bij asthma). Bij een tegenvallend therapieresultaat bij een adolescent moet altijd aan gebrek aan compliance worden gedacht. Bijwerkingen Uitgangspunt van de behandeling van epilepsie is het doen verdwijnen of tenminste verminderen van epileptische aanvallen. Echter, bij het gebruik van antiepileptica kunnen bijwerkingen optreden. Het is kinderen met epilepsie niet altijd gegeven (leeftijd, verstandelijke beperking) om zelf melding te maken van bijwerkingen (15). Op dit moment ontbreekt een betrouwbaar, gevalideerd instrument om bijwerkingen bij kinderen op te sporen. Veel aandacht voor het aanwezig zijn van deze is dus geboden. In conclusie: Epileptische aanvallen bij kinderen hebben een groot impact op de omgeving. Toch is het lang niet altijd nodig direct te gaan behandelen. Ook na het uitblijven van effect van de eerste pogingen heeft doorzoeken zin, mits men een open oog heeft voor andere behandelingsmodaliteiten (chirurgie). De nieuwe antiepileptica zijn niet effectiever dan de oude. Met name bij niet sprekende kinderen (leeftijd, verstandelijke beperking) dient men alert te zijn op bijwerkingen.
