5 2. Diagnostiek Bij diagnose van een kwaadaardige Hoofd-Halstumor is een volledig klinisch onderzoek van de bovenste luchtweg aangewezen. Aanvullend ondergaan alle patiënten een onderzoek in narcose bestaande uit een endoscopie van de bovenste luchtweg met inbegrip van een tracheoscopie en een starre oesofagoscopie van de bovenste slokdarm. Dit moet toelaten de exacte tumorlokalisatie in kaart te brengen, representatief materiaal te bekomen voor histologische diagnostiek en operabiliteit na te gaan. Voor classificatie van de T-categorie wordt naast het onderzoek in narcose ook minstens een CT-scan van de hals zonder en met contrast uitgevoerd. Een MRI-scan van de hals zonder en met contrast wordt toegevoegd wanneer de primaire tumor zich in de mondholte, oropharynx, parotis, nasopharynx of de neus/paranasale sinussen bevindt. Voor de N-categorie wordt naast palpatie van de hals gebruik gemaakt van de beste en beschikbare beeldvormende technieken. Cytologisch onderzoek (al dan niet onder geleide van echografie) van verdachte lymfeklieren kan aanvullend worden uitgevoerd. Voor de M-categorie wordt een CT-scan van thorax en mediastinum met contrast gevraagd. Indicatie voor bijkomende PET-CT zijn alle tumoren in stadium III en IV. Algemeen wordt voorzien in een routine bloedafname met hematologisch en scheikundig nazicht. TSH en FT4 worden bij gevraagd. 3. Voorbereiding Elke tanddragende patiënt wordt voor behandeling naar de tandarts gestuurd, ongeacht de uiteindelijke behandelkeuze. Bij voorkeur worden de patiënten naar de tandarts gestuurd van het tandheelkundig instituut in poli 8. Extracties worden zo snel mogelijk uitgevoerd. Bij een heelkundige ingreep ten laatste bij die gelegenheid. De patiënt wordt zo vroeg mogelijk in het traject voorgesteld aan de verpleegkundig oncologisch consulent die het uithangbord is van de psycho-sociale omkadering die patiënt kan worden aangeboden (verpleegkundig consulent, diëtiste, psychologe, sociaal verpleegkundige). 4. Algemeenheden 4.1. Aanpak van de hals Indien de primaire tumor heelkundig wordt behandeld wordt bij N+ ook een halsevidement uitgevoerd. Unilateraal bij cn1-2b of N3, bilateraal bij cn2c of een primaire tumor op de middenlijn. Voor primaire tumoren van lip en mondholte die cn0 zijn wordt een supraomohyoidaal halsevidement uitgevoerd. Unilateraal voor gelateraliseerde letsels, bilateraal voor middenlijn letsels. Bij residuele ziekte in de hals na radiochemotherapie wordt het halsevidement beperkt tot de aangetaste halsklierregio met de aanliggende halsklierregio( s). Bij een primaire tumor in de nasopharynx wordt nooit een halsevidement uitgevoerd Radiotherapie Standaard wordt gebruik gemaakt van intensiteitsgemoduleerde radiotherapie of een vergelijkbare techniek (zoals bvb. boogtherapie) met het oog op een optimale dosisverdeling zonder onintentionele onder- of overdoseringen en met maximaal uitsparen van risico-organen waar dat mogelijk is. Afhankelijk van de indicatie wordt met standaard fractionatie (2 Gy per dag / 5 dagen per week) of lichte hypofractionatie gewerkt. Oncologisch Handboek UZ Gent Hoofd- & Halstumoren
6 4.3. Chemotherapie Bij toepassen van radiochemotherapie wordt standaard gezien gebruik gemaakt van cisplatinum. Om de toxiciteit te kunnen controleren wordt hierbij gebruik gemaakt van een wekelijks schema IV, tijdens het volledige verloop van de radiotherapie. Alternatief voor patienten met contraindicatie voor platinum is cetuximab, anti EGFR, eveneens wekelijks IV. Zelden wordt carboplatinum toegepast als alternatief Palliatie Palliatie kan bestaan uit antalgische radiotherapie, chemotherapie (cfr hieronder), best supportive care of een combinatie van voorgaande Gemetastaseerde patiënten In geval van oligometastase moet overwogen worden deze lokaal te behandelen met heelkunde of radiotherapie. Bij gemetastaseerde ziekte komen patienten in aanmerking voor systemische therapie zijnde combinatie van cisplatinum, 5-fluoro-uracil en cetuximab IV. In tweede en derde lijn zijn taxanen en metotrexaat mogelijk. 5. Regio overzicht 5.1. Opvolging 6.1. Algemene opmerkingen Hoofd-halstumoren zijn in principe zeer toegankelijk. De basis van de opvolging zal dan ook routine klinisch NKO-onderzoek zijn. Wanneer nodig moet het onmiddellijk aangevuld zijn met flexibele fibroscopie van de bovenste luchtwegen. CT-scan of MRI-scan zullen in principe slechts gevraagd worden bij klinisch vermoeden van recidief of bij klachten van de patiënt die niet direct te verklaren zijn. PET-CT kan dan ook worden toegevoegd. Bij verdachte letsels wordt steeds een biopsie uitgevoerd. Voor sinonasale carcinomen wordt een referentie beeldonderzoek voorgesteld 4 maanden na het einde van de behandeling. Verdere beeldvorming vanaf dan om de 12 maanden. Op tandheelkundig vlak moet de nodige aandacht worden besteed aan de verzorging van jet restgebit. Een 6- maandelijks bezoek aan de tandarts wordt aangeraden Tijdschema routine opvolging Eerste gemeenschappelijke opvolging 1 maand na het einde van de behandeling. 1 ste jaar om de 3 maanden. 2 de jaar om de 4 maanden. 3 de 5 de jaar om de 6 maanden. Voorbij het 5 de jaar om de 12 maanden Jaarlijkse onderzoeken Er is geen evidentie dat een routine nazicht door middel van beeldvorming een positief effect heeft op de overleving van de patiënt.
