Helaas ontbrak in het vorige nummer de aankondiging hiervan. Hopelijk was er desondanks een goede opkomst in Amersfoort De notulen zijn vanaf half juli op te vragen bij de ledenvoorlichting KNGF Verslag congres Fuctie en participatie Mechthild van de Ven en Marieke van Wijngaarden, kinderfysiotherapeuten, doen op verslag van het NVFK congres van 28 en 29 maart jongstleden. REGIOVERTEGENWOORDIGERS voor de NVFK GEVRAAGD Communicatie tussen de leden en het bestuur is van essentieel belang voor het maken en uitvoeren van beleid. Maandelijks worden de regiovertegenwoordigers geïnformeerd over de actuele zaken binnen het bestuur, zodat zij op hun beurt de leden op de hoogte kunnen stellen. De regiovertegenwoordigers zijn de spreekbuis voor de leden en voeden het bestuur met informatie vanuit het werkveld. Dit gebeurt doorgaans per mail. Twee maal per jaar zijn er bijeenkomsten (1x een avond en 1x een dag ) waarop het bestuur en de regiovertegenwoordiging bij elkaar komt. Daarnaast is het de bedoeling dat zij een goed contact onderhouden met de RGF besturen. In de meeste regio s is er een tweede regiovertegenwoordiger, zodat taken verdeeld kunnen worden en men elkaar zonodig kan vervangen. Het bestuur van uw vereniging vindt het zorgwekkend dat momenteel vijf regio s géén regiovertegenwoordiger hebben. De leden van deze regio s kunnen daardoor onvoldoende geïnformeerd en gehoord worden. In de volgende regio s is er een vacature: Drenthe Randmeren Amstel-, Meer- en Rijnlanden Holland Zuid Hollands Midden Daarnaast hebben de regio s Kuststreek Leiden, Rotterdam e.o., Zeeland en Zuid Oost Brabant slechts één regiovertegenwoordiger. 4 Bestuursgezicht in het licht In drie opeenvolgende nummers worden de bestuursleden van de NVFK aan de lezers voorgesteld en vertellen zij iets over hun werk achter de schermen bij de NVFK. Voor dit nummer heeft de redactie Ghislaine Janssen en Jacqueline Nuysink een aantal vragen gesteld. Deze dames hebben als congrescommissieleden het nodige werk verzet om het tweedaagse lustrumcongres van 28 en 29 maart tot een succes te maken. Wat was de reden om je voor het bestuur verkiesbaar te stellen? Jaqueline: Ik was al een flink aantal jaren regiosteunpunt vertegenwoordiger van Utrecht en bestuurslid van de Werkgroep Kinderfysiotherapie Midden-Nederland. Dat laatste had ik met veel plezier gedaan. Ik was wel toe aan een nieuwe uitdaging en heb nadat ik door mijn voorgangster Meta van de Briel gevraagd was om na te denken over een bestuursfunctie eerst een half jaar meegedraaid met het bestuur om te kijken of het iets voor mij was. Ghislaine: Jacqueline benaderde mij persoonlijk met de vraag een vacature op te vullen in het bestuur. Op dat moment was de sectie PR en communicatie vacant. Voor een andere sectie zou ik niet in het bestuur zijn gegaan. Vanuit mijn nieuwe werkgebied in de eerste lijn voelde ik meer betrokkenheid bij de zaken die de NVFK voor de kinderfysiotherapie betekent. Wat is je functie in het bestuur? Jaqueline: Mijn eerste functie werd de congresorganisatie en de regiosteunpunten. Binnen het bestuur werken wij in duo s zodat je ook rechtstreeks betrokken bent bij tal van andere verenigingszaken. Voor mij waren dat de contacten met het KNGF en het CBI (College van beroepinhoudelijke verenigingen), de wetenschappelijke commissie en deels de PR. Vanaf november 2001 ben ik penningmeester, coördinator congressen en vice voorzitter, een Ghislaine Janssen 47 jaar woonachtig te Zeist Bestuurslid sinds december 2001 Jacqueline Nuysink 49 jaar woonachtig te Amersfoort bestuurslid sinds november 1999 dubbele taak dus. Gelukkig is vanaf voorjaar 2003 ons bestuur weer compleet, zodat ik een deel kan overdragen aan een van de nieuwe collega s. Ghislaine: PR en communicatie; zoals tijdschrift, website, PR stand, informatievoorziening op allerlei manieren naar de verschillende doelgroepen, leden, verwijzers, ouders, patientenverenigingen etc. (dit lukt nog niet allemaal, hoor) Ik ben als bestuurslid afgevaardigd in de Scholingscommissie en de Congrescommissie. Wat vind je een leuke kant van het besturen? Jaqueline: Ik ben betrokken bij de organisatie van ons vak en kan er zelf daadwerkelijk iets aan bijdragen. Ik zit altijd vol met ideeën en plannen, en vaak kunnen ze vanuit zo n bestuursfunctie nog uitgevoerd worden ook. Dat is toch fantastisch. Daarnaast heeft het een leuke spin off: je leert heel veel collega s kennen die vernieuwend en kritisch naar het vak kijken en je verbreedt je horizon naar onze plek in de gezondheidszorg en de maatschappij. Ghislaine: Ik ben praktisch ingesteld en organiseer graag. Ik streep ook graag dingen van mijn lijstje af: gedaan!!! Daarom past de sectie PR en communicatie bij mij. Wat is het moeilijkste van besturen? Jaqueline: Ik vind het soms lastig dat je voor werkelijke veranderingen veel geduld moet hebben. Verder moet je je voortdurend realiseren dat je als bestuurslid heel veel informatie hebt die naar de leden maar langzaam doorsijpelt. Communicatie is dan ook een belangrijk speerpunt voor het bestuur. Ik vind het jammer dat het enthousiasme van de leden voor de vereniging van de eerste jaren aan het vervlakken is nu er al zoveel bereikt is voor de kinderfysiotherapie Ghislaine: Inzicht in structuur van de vereniging bijvoorbeeld het KNGF. Hoofd en bijzaken scheiden en delegeren. Wat zijn je speerpunten binnen het bestuur? Jaqueline: Mijn rol binnen het bestuur zal veranderen van organisator naar een meer beleidsmatige. Mijn speerpunt zal naast een gezonde financiële huishouding de ontwikkeling naar een wetenschappelijke vereniging zijn vanuit een meer beleidsmatige rol Ghislaine: Communicatie naar de leden. Voor mij was het voorheen niet duidelijk wat de NVFK deed. De afstand tussen de vereniging en de leden verkleinen. De leden zijn de vereniging. Ik ben blij met de doorstart van het tijdschrift en de lancering van de website. Wat hoop je in je bestuursperiode te bereiken? Ghislaine: Ik hoop in mijn bestuursperiode meer bekendheid te geven aan ons vak bij andere gezondheidszorgwerkers, bijvoorbeeld de mogelijkheden onderzoeken om samen met CB/jeugdartsen een congres te organiseren. Ben je zelf werkzaam als kinderfysiotherapeut? Sinds wanneer? Jaqueline: Als fysiotherapeut werk ik al sinds Vanaf 1985 ben ik kinderen gaan behandelen en kindercursussen gaan doen. Mijn registratie heb ik sinds Ghislaine: Ik ben vanaf 1980 werkzaam met kinderen, eerst als oefentherapeute Cesar in astma centrum Heideheuvel. Daarna 20 jaar op de vroegbehandeling van revalidatiecentrum de Hoogstraat in Utrecht. Sinds 1 januari 1998 ben ik geregistreerd. 2 Kinderfysiotherapie juni 2003

5 Waar werk je en op wat voor werkplek? Jaqueline: Ik werk 3 dagen per week in de eerstelijn in mijn eigen prakijk. In Bussum werk ik samen met een collega kinderfysiotherapeut en een logopediste; in Amersfoort werk ik in het wijkgezondheidscentrum. Ghislaine: Ik werk sinds 3 jaar in een particuliere praktijk samen met mijn man Jan van Rijn en nog drie andere kinderfysiotherapeuten. Daarnaast geef ik geregeld les op de opleiding kinderfysiotherapie en ben ik supervisor van de studenten kinderfysiotherapie. Verder ben ik bestuurslid van de stichting nascholing Kinderfysiotherapie en ben ik cursuscoördinator. De afgelopen 3 jaar hebben wij de verplichte nascholing aangepast binnen de grenzen die het genootschap ons stelde. Wat vind je de mooiste kant van het vak? Jaqueline: Een kind leren om het zelf te kunnen! Ghislaine: Het contact tussen ouder(s)-kindtherapeut en de begeleidende rol. De zeer verschillende situaties waarin ik bij de aan huis behandeling terecht kom, vind ik het meest spannend. De grote varieteit daagt mij uit om bij ieder uniek kind en ouder in korte tijd een inschatting te maken van de situatie. ECHT contact krijgen met kind en ouders is voor mij de grootste uitdaging, zodat je hulp( hoe klein ook ) in goede aarde valt. Wat heb je zelf van het congres mee naar huis genomen? Jaqueline: Ha, een bos bloemen, een lekker flesje wijn en een heleboel werk nog dat afgerond moet worden. Nee, een serieus antwoord: Inhoudelijk vond ik het een heel sterk congres. Er zat veel in van filosofisch tot humor en van praktijk tot wetenschap. Voor elk wat wils en dat is moeilijk met zo n gediffentiëerde beroepsgroep. Persoonlijk vond ik de lezing van professor Lindstrom over zijn onderzoek naar wat kinderen belangrijk vinden in het leven een hoogtepunt. Organisatorisch heb ik er van geleerd dat je bij zo n groot project onmogelijk alles in de hand kunt houden en verantwoordelijkheden moet delen. Binnen de congrescommissie is dat uitstekend gelukt, ik draag mijn taak dan ook met een gerust hart over. Met Koningshof was dit minder geslaagd, vooral ten aanzien van de lunch zijn er zaken heel anders gelopen dan afgesproken en bedoeld. Daar moet nog flink over nagepraat worden. Ghislaine: De opmerking van diverse collega s dat ons congres zich kan meten met congressen met groot internationaal karakter. Dat beschouw ik als een compliment naar de congrescommissie. De ervaring dat een congresburo minder perfectionistisch is dan ik. Ook al besteed je veel zorg aan de organisatie er gaan altijd dingen anders dan gewenst. Ik heb weinig tijd gehad om zelf lezingen te volgen. Hoeveel tijd ben je ongeveer kwijt aan besturen? Jaqueline: De tijd die je kwijt bent hangt af van je taken en die worden natuurlijk in overleg gekozen. Zelf ben ik er vrij veel tijd aan kwijt, al zou ik liever niet van kwijt willen spreken. Rondom een congres en rondom de jaarrekening en de begroting maak ik heel wat extra uurtjes. Ik waag me maar niet aan een schatting. Omdat ik in september jl gestart ben met de met de opleiding Fysiotherapie Wetenschap trek ik me terug uit de congresorganisatie en zal wat minder tijd kunnen investeren in het bestuur. Ghislaine: Ook het werk als bestuurslid is vrijwilligerswerk. Een bestuurslid ontvangt 2 uur vergoeding voor 40 weken in het jaar. In een normale week besteed ik zeker 4 uur aan bestuurszaken dan reken ik vergaderingen en ALV, kaderdagen niet mee; denk alleen al aan de berichten in je mailbox lezen en beantwoorden, reageren op website vragen etcetera. Op hoogtijdagen, bijvoorbeeld bij de voorbereiding van een congres, zal ik maar niet bijhouden hoeveel uur erin gaat zitten. Hoe vind je het contact met de leden? Jaqueline: Wij hebben 1100 leden met zeer uiteenlopende wensen en belangen. Door het tijdschrift, de website, de regionieuwsbrieven en de ALV s proberen wij zoveel mogelijk zichtbaar te maken waar we mee bezig zijn. De communicatie vanuit de andere kant is wat stiller. Wij zijn wel op zoek naar middelen om feedback en suggesties vanuit de leden boven tafel te krijgen. Soms maak ik me wel eens bezorgd over het contact met de leden, maar dan vooral in de zin van: worden wij als bestuur niet teveel gezien als een instituut dat zaken wel even voor je regelt? Het bestuur is de NVFK niet, dat zijn wij allemaal. Natuurlijk zijn wij aanspreekbaar op beleid, maar het is wel met elkaar en voor elkaar. Dat laatste mis ik nog wel eens in reacties als je mensen vraagt iets te willen doen. Ghislaine: Er is bedroevend weinig belangstelling voor ALV. Voorheen ging ik zelf ook niet! Ook zijn de posities van de Regio Vertegenwoordigers niet allemaal ingevuld. Toch als je mensen persoonlijk vraagt voor een klus is er veel bereidheid om mee te werken en mee te denken. Ghislaine over Jaqueline: Jacqueline is een persoon met een grote hoeveelheid energie en doorzettingsvermogen. Zij heeft een helicopterview, waardoor zij hoofden bijzaken kan scheiden, goed kan analyseren en planmatig kan werken. Hierdoor zijn de congressen onder leiding van haar goed verlopen. Als penningmeester weet zij bijna altijd naar tevredenheid de karige vergoeding naar een persoon te motiveren. Jacqueline over Ghislaine Ghislaine is een zeer integer bestuurslid. Zij werkt heel zorgvuldig, met een enorme drive en een goed oog voor details. In de organisatie van het laatste congres was het heel plezierig samenwerken en was zij een grote steun voor mij. Zij is enthousiast en heeft een prettig soort humor. Ze gaat vrolijk op zaterdagavond op krukken mee een band bekijken voor het feest op het congres! Kinderfysiotherapie juni

6 lezing gehouden tijdens het congres van de NVFK op 28 maart 2003 The Nature of Paediatrics Prof. dr. J.P.M. Helders Pediatrics is concerned with the health of infants, children and adolescents, their growth and development and their opportunity to achieve full potential as adults. Professionals working with a paediatric population not only need to have an understanding of growth and development, but to understand the emergence of motor, cognitive and social-emotional competence in order to timely identify delay or deviance as well. They also need to understand the sometimes difficult interaction between biology/genetics and the environment that shape developmental processes. This century-old debate on Nature-Nurture nowadays seems to be replaced by a biopsychosocial view on development that recognises the importance of both intrinsic and extrinsic factors. P.J.M. In this Nature-Nurture controversy, scientists wonder if the behaviour of human beings is due to their Nature (or genetics) or to their Nurture (or parents/environment). In other words: why do children turn out the way they do? Although no one believes that development is controlled entirely by the genes or by the environment or parents, there are various gradations of influence that researchers assign to each end of this continuum. Of particular interest for pediatric physiotherapy is the Nature-Nurture controversy about the basics of motor development. We used to believe that motor development was largely a neuromaturational phenomenon, as described by Gesell et al. Nowadays, the Nature-Nurture controversy focuses on the development of automatic motor patterns in relation to Edelman s theory of neuronal group selection, showing that pattern selection is experience dependent. Also other theories are stressing the importance of experience and context dependency. Whether or not pediatric physiotherapists need to join the big debate, shifting the focus of their treatment from impairments to function in a context-dependent environment seems of significant importance. The dramatic advances in pediatric medicine in the past three decades have led to increased survival rates of children with serious physical illnesses or disabilities. As a consequence, attention has shifted from dealing with mortality towards coping with morbidity with an emphasis on the medical, psychological and social consequences of chronic conditions. That s an intriguing aspect regarding the Nature-Nurture debate. Theories on childhood development: a changing panorama Neuromaturational theory For long time theories on childhood development were based largely on the assumptions drawn from the work of McGraw, Gesell, Sherrington, Magnus and others. The main tenet of these neuromaturational and reflex-hierarchical theories is that changes in motor behaviour during childhood are based on a genetic script ( a genetic blueprint ). From this nature-like point of view, motor development follows predictable rules and a predictable sequence. This is reflected in the opinion how development takes its course: in a cephalo-caudal direction, from proximal to distal and following motor milestones. Research, to our knowledge, has failed to support many of the neuromaturational and reflex-hierarchical theories. The cephalo-caudal, proximal to distal and gross to fine motor development has never been confirmed. Development may appear to be like that, but other factors have been shown to account for the mastering of skills. Dr. Ester Thelen clearly demonstrated the significant influence of balance, and strength for instance in the acquisition of arm versus leg movements. More recently, Dr. Karen Adolph, a developmental psychologist and paediatric physiotherapist from New York University, published some very intriguing experiments regarding the problem solving capacity during the emergence of crawling and walking of babies, in which she clearly demonstrated the influence of environment on stability of these skills. One could say that the neuromaturational theory is a typical example of Nature-wise thinking. Dynamic Systems theory Research data and scientific concepts stemming from biomechanics, neurosciences, developmental and cognitive psychology and movement sciences have merged into the Dynamic System Theory. This theory suggests (motor) behaviour to be the end product of multiple internal and external components or subsystems. Some subsystems contributing to motor control have been identified by researchers and include sensorimotor, cognitive, mechanical and task variables. Dynamic System Theory has an important theoretical assumption in the rejection of a hierarchical view of motor development and motor control, and allowing variation as an important feature of normal development. Motor development and co-ordination are assumed to emerge from a dynamical interaction of many subsystems in a task-specific context. For this reason, adequate motor behaviour is more likely to result from working on functional tasks than from working on movement patterns just for the movement s sake alone. The central nervous system in this Nurturelike view is seen as a necessary, but not dominating component to explain motor behaviour. Co-ordination and control of movements are seen as the net result of several subsystems working dynamically together in a co-operative way. The child and the environment, all equally influencing development and motor behaviour, challenge the subsystems. 4 Kinderfysiotherapie juni 2003

7 Neuronal Group Selection Theory This theory is based on the in vivo and in vitro work of neurobiologist Edelmann. It has been put forward as a framework for normal and abnormal motor development by Dr. Hadders-Algra and others. Basic idea of this theory is that the genetic blueprint does not offer a detailed plan for motor development, but just a rough sketch. The interaction of genetics and environment ( Nature - Nurture ) leads to a dynamic regulation of neuronal networks by cellmigration and neuronal growth. Neuronal networks are active soon after they are established and are connected to peripheral effector organs. The try-out of these networks is expressed in motor behaviour that may vary considerably. In childhood development one could refer to this phase as trial and error. Based on the afferent feedback of information, the system than selects the movement s repertoires, which are the most effective and can be used in different circumstances. So, the Neuronal Group Selection Theory is highly characterised by notions as variability and selection. It may represent the perfect balance between Nature and Nurture views. Developmental diagnosis Key issue in developmental diagnosis is the capability versus performance, meaning that one not only should determine whether a child can do certain activities, but also whether it actually performs this activities. This can best be done when using functional diagnostic measurements. Measuring just impairments is only meaningful when there is a causal relationship between the diagnosed impairment and the ascertained change in the child s abilities or activities. In developmental diagnosis one thoroughly questions what kind of problems emerges, under what circumstances, in this child with these parents, in this cultural context. The actual developmental assessment, that is whether a child does do certain activities, should be performed in an activity-centre. This is a semi-structured space where with the help of toys, climbing frames and other age-specific activity-eliciting equipment or objects, most of the daily childhood activities can be simulated in a child-natural environment. Age-specific measurement-instruments guide the choice of activities that needs to be assessed. Moreover, an inventory should be used to get insight into the specific problems regarding daily childhood activities in a, for this specific child, relevant childhood context. This strategy serves two purposes. One can observe the spontaneous movement behaviours of the child, its problem-solving behaviour and secondly those behaviours can be quantified. Depending on the chosen instruments, a distinction can be made between normal and abnormal motor behaviour (its capacities are tested and its performance is quantified) or an evaluation is made of the change in (motor) development. The ultimate setting to evaluate the child s level of activities and participation is in the context of family and community. A growing number of paediatric instruments based on this view are being published. In the way we are currently thinking about developmental diagnosis and the importance of a childnatural environment, we accept the influence of environment on the expression of motor behaviour and by that are Nurtureoriented. Intervention strategies Although most of the so-called neurophysiological interventions differ in their theoretical foundations, they have their focus on impairments in common. Muscle tone, range of motion, stretching muscles to keep their anatomical length for instance, is an important issue in most methods. There is increasing doubt however whether to change motor patterns in patients with a neurological disorder. Latash and Anson in their impressive publication argue that changed motor patterns in patients with a neurological disorder should be considered not pathological, but rather adaptive to a primary disorder and may even be viewed as optimal for a given state of the system of motor production. Intervention strategies should not be directed towards restoring the motor patterns to as close as normal as possible, but rather towards resolving the functional problem. They should therefore focus primarily at restoring or, if indicated and possible, improving functional ability, respecting the typical characteristics of the disease and the disease related natural course. Moreover, treatment of movement disorders should be viewed as learning to learn ; meaning that solving motor problems by the child itself should be enhanced. Solutions made by the therapist have not only been shown to be more energy demanding, but they also don t sink in. More and more researchers are also questioning the automatic carry-over of the treatment effects into functional gain. The assumed linear relationship between impairments, functional limitation and disability is an oversimplification of reality and has not yet been shown to really exist. The current models of disability (International Classification of Functioning, Disability and Health, ICF) have changed accordingly and no longer give the impression of any linearity between the different domains of the disability process (WHO 2001). Most if not all of the intervention strategies/methods have their own assessment: for example NDT-assessment, Vojta- assessment and Ayres- assessment. They focus almost entirely on impairment, while any childhood disability significantly goes beyond the impairment domain. Assessments should come up with more information regarding the health status of the child than just the amount and localisation of impairments, like for instance functional ability, well being, parental assistance and involvement, family dynamics, and participation in community affairs. Moreover, there is little evidence to document that intervention at the impairment level is effective for children with developmental disabilities, particularly beyond the first year or two of life. Rather, it is time to let go of the concept of normalisation with respect to muscle tone, reflexes, and movement patterns, and work instead towards attainment of functional skills such as independence in mobility and communication. With independence in mobility we do not always have to focus on the ability to walk. Even if it has a great appeal to both parents, children and rehabilitation professionals and all are willing to do the utmost to reach that goal. Although most of the children with cerebral palsy who learn to walk will do so just before or during pre-school years, some do not learn to walk before the age of 14 years. Buttos et al has shown that those who learn to walk late, are the first one to loose these skills as adults. We should discuss whether this should have implications for our intervention strategies; maybe these children are more adequately treated with wheelchair training. Professionals in childhood disabilities should be aware of the functional course of specific disorders. For example, in juvenile arthritis the joint impairments may follow a natural course in normalisation of function and for this reason treatment goals might well be directed at walking ability to warrant this function for future utilisation in daily life. We believe that it is time to consider prog- Kinderfysiotherapie juni

8 nostic issues in childhood disabilities more accurately to improve counselling with parents, for planning interventions and functional goal setting. Having Nature- Nurture in mind, I would say that we used to think that almost everything could be normalised or restored to a normal function. We have learned however, that it is impossible to do so. Maybe Mother Nature is more dominant in some childhood disabilities than we used to think. Family centred care One of the most important changes in the management of children with disabilities is the different orientation of the used treatment strategies: from impairment driven and child-centred to functional based and family-orientated, with that acknowledging the importance of family - and peers environment. This not only means an adjustment of the existing (outcome) measurement instruments (from an impairment domain to multidimensional domain), but also means broadening of the total scope of our invention. In recent publications it was found that family environment predicted outcome after controlling for injury severity, and moderated the effects of traumatic brain injury in children. In another study, researchers found greater developmental losses among children of parents with minimal amounts of social support. It underlines the need for further investigation, whether improved family functioning leads to important clinical benefits for both children and families. Schoenmakers described 39 children with hemophilia who were found comparable with their healthy peers with regard to motor performance and ADL. However, a majority (79%) perceived an impact of their disease on their life associated with pain (56%) and restrictions in sports. It underlines that families with children with potentially disabling medical conditions need extensive counselling in what that medical diagnosis means for the functional status of their child. Restrictions in daily childhood activities are only than indicated when there is evidence that the child will benefit from that. Having Nurture in mind, family centred care might well turn out to be a strong modifier of functional outcome in childhood disability. Concluding remarks Every pediatric profession should reflect to true elements of pediatrics, that is: growth, development and the opportunity to achieve full potential as an adult. That means that theories and professional paradigms regarding patient care, research and education should start from the beginning of childhood leading into adulthood. Other professions like pediatric surgery, pediatrics and pediatric psychology also originated from the adult version of the profession, later on developing into an own, recognised and accepted sub-speciality. Pediatric physiotherapy has gone the same road. Originating from the adult-physiotherapy, strengthened by the outbreak of polio in the last century and the developments in the area of cerebral palsy, the focus more and more shifted to the specific pediatric aspects of treatment and research. With the changing views of developmental neurologists on childhood development and the changing focus in patient care, theoretical concepts are changing as well, leading to a specific body of knowledge in pediatric physiotherapy. The Nature-Nurture debate has had some significant impact on our profession. Outcome of that research has shaped and continues to shape our views on development, the focus of our interventions and our research activities. Although this Nature - Nurture controversy merely is a philosophical question which hard science can only answer at the sidelines, it is an interesting debate that is ongoing since ancient times and will probably do so in the times ahead. Whether or not we actively take part in this discussion, research both Nature - and Nurture oriented will continue to shape our opinions and activities. Zo kennen we de Joop den Uil lezing in Amsterdam, of de Van der Leeuw lezing in Groningen. Beide naamdragers kennen we van hun positie in de samenleving. Meestal gebeurt het instellen van zo n lezingen cyclus op initiatief van een groep geïnteresseerden of vertegenwoordigers van een bepaald vakgebied. Het is een eer de naamdrager van zo n cyclus te mogen zijn. Ook de Kinderfysiotherapie kent sinds kort een dergelijke cyclus waarvan de naamdrager Paul Helders is. De Paul Helders Lecture is tijdens het Symposium Dynamics in Child Development op 20 Mei 2000 te Utrecht geïnstalleerd door professor Dr. Beverly Bishop van de New York University at Buffalo, NY, USA. Zij deed dit op verzoek van de medewerkers van de afdeling Kinderfysiotherapie en vertegenwoordigers van de Divisie Kindergeneeskunde van het Wilhelmina Kinderziekenhuis te Utrecht. Dit initiatief werd genomen uit waardering voor het werk dat Paul deed en nog steeds doet voor de kinderfysiotherapie en de kindergeneeskunde. De instelling van de Lecture gebeurde in nauw overleg en met medewerking en ondersteuning van het Hoofdbestuur van het KNGF. De Paul Helders Lecture is bedoeld als blijk van waardering voor een kinderfysiotherapeut met aantoonbare en bewezen grote verdiensten voor het vakgebied van de kinderfysiotherapie op het gebied van wetenschap, onderwijs, onderzoek, bestuur en beheer. De Paul Helders Lecture werd voor de eerste Paul Helders feliciteert Laila de Groot keer uitgesproken door Prof. Dr. Paul Helders zelf op 20 Mei Iedere twee jaar wordt ze herhaald. In overleg met het hoofdbestuur van de KNGF en het bestuur van de NVFK heeft de afdeling kinderfysiotherapie WKZ deze aktiviteit overgedragen aan het Bestuur NVFK met dien verstande, dat men zich houdt aan de afspraak dat kandidaten die worden voorgedragen, dienen te zijn voorzien van een uitgebreid curriculum vitae en officiële toewijzing in overleg met zowel afdeling WKZ als naamgever (PH) plaatsvindt. 10 Tweede Paul Helders Lezing, Internationaal congres NVFK te Veldhoven, 29 Maart 2003 Development in Paediatric Physiotherapy Dr. Laila de Groot First of all I want to thank the organisers of the congress and Paul Helders for inviting me to give this lecture. I am proud to be given this opportunity. May be the best moment to give a talk like this, is at the end of one s career, since it gives one the possibility to look back at everything that has been achieved while one still has the capacity to talk and think about the future. This is why I want to talk about Development. In my opinion the concept of development is the main distinguishing factor between our profession and other forms of physiotherapy. This concept, development should be at the core of our expertise. Moreover I will also use it as a metaphor when talking about the future of our profession. Dr. Laila de Groot Kinderfysiotherapeut en onderzoeker faculteit der bewegingswetenschappen, Vrije Universiteit Amsterdam Development: selection and memory This Bushman s child is going to differ from his mom in many ways. The genetic makeup of the Bushman was not prepared for choosing between 20 sorts of ketchup. Slowly but surely the innate and inherited characteristics fit for life in the bush, will change or disappear. During the last decades many theories and concepts of human development have been developed and described. In my view development is a non-linear process limited by genetic, biological and factors shaped and altered by experience. In order for the system to develop exploration: selection and memory are absolute requirements, which are innate. For exploration active movement is necessary, first small movements like eye movements, haptic mouthing, and grosser movements like turning the head etc., etc. Thus a very efficient way to study development is to study the motor behaviour of an individual since it can be considered to mirror the function of the brain. So, can we than conclude that motor development is everything, is everything motor? This statement is a highly debatable, but I think that one could agree that motor behaviour enables us to communicate with the world around us. The hard part still remains: to explain how the physiological world mediates the psychological part or vice versa. When the definition of the verb to develop is looked up in Webster dictionary it gives us definitions as: to unfold, to make more effective, to grow or to strengthen, to evolve gradually. Growth is often used synonymous, indicating the need for nourishment, warmth and a typical human necessity: love. Another capacity and property essential to the system is the capacity to remember. Memory is the ability to repeat certain actions and is absolutely necessary for the system to learn. Explanations of motor development All of these descriptions are describing an active process, but leaves us in the dark about how this comes about. Many models have been mentioned: It was thought that all functions are innate versus people thinking everything was acquired, or development is considered to come about by the maturation of biological systems following an evolutionary defined pre-programmed pattern. Darwin, even without knowing anything about genetics and DNA, already described the theories of natural selection, mutations evolving from variation and habituation to the environment. And I argue that is not so completely different from Bernstein s theories today about perception and action. Although Gesell and Mac Graw may have put too much emphasis on the timetable of biological maturation of the brain, it still holds a lot of truth. Many studies which tried to enhance or accelerate motor development by a stimulating intervention have shown disappointing results. Thus there must be such a thing as a timetable; it may differ between the individual children. But there are limits: when a child does not walk free at the age of 24 months, we get concerned and the child is entitled to an examination. Since the possibility to study the unborn child in utero, by means of ultra sound techniques, we now know a lot more about early development, its continuity and timetable. We know now that already at the age of 9 weeks post conception most of the basic motor patterns are already there, eye movements are already discernable at 16 weeks, while it must be rather dark in the womb. So there is little perception, but still there is action! Neither intrinsic developmental timetables nor experience can give sufficient explanation for motor development. At every stage motor development is depending on experience and the quality of the stimulus perceived. The perceived information in turn is depending on the right period of brain organisation, the so-called critical periods in development. Of course we all know this. It is no use to try to teach a 4-month-old infant to walk, while we also know that a newborn infant walks or at least makes stepping movements when supported. Esther Thelen an American psychologist, tried to explain the fact that this phenomenon disappears at a certain age only to re-appear many months later. She argued this was due to the changing biomechanics of the infant, in this case the heavier legs. 11 The genetic make-up of the Bushman was not prepared for choosing between twenty sorts of ketchup king in The States, repeated this research by calculating the weight of the legs and the ponderal index of a group of preterm infants, but could not confirm these findings (published in Neuropediatrics (2000) 31 page 180). Esther Thelen also trained the stepping response on a treadmill and in that way could maintain the reaction, but still this does not enhance free walking. For that function far more complicated equilibrium reactions are required. This shows us that many controversies over the origin of human (motor) behaviour have not been solved as yet. Still it is exactly on this slippery path that paediatric physiotherapists can and should use their expertise. Preventive intervention is a major part of our profession, certainly so in paediatric physio. As Andras Petö said; Catch a fault before it is evident. This is very difficult since there is so much individual variability. Trial and error and variability During early infancy each movement is slightly different, each trial is a little different from the one before. By trial and error the sensory motor system is shaped and formed, but always has to be decoded to the morphological changes of the brain during maturation. This way variability in the case of normal development and stereotypy in a damaged brain can be explained. It is the task of the physiotherapist to catch this lack of variability before stereotypies become fixed. By structuring the environment, the perception of the infant will get more stimuli. This in order to create a bigger chance to create variability by using the overflow of neurons available in early development. Once the synapses or circuits are fixed, trial and error processes become limited and result in an unusual performance; be it accelerated or retarded. Until now I have only been talking about brain damage, but the therapist should also be familiar with the influence of the different (congenital) syndromes and how different diseases interact with the overall development of the child. Preterm infants get abnormal stimuli As many of you might know my main interest, for the last 20 years or so, has been the infant born prematurely. The fact that the infant is born too early makes it possible to study development outside the womb and to study the influence of the environment on the (motor) behaviour of these infants. Preterm infants are particularly vulnerable as their immature nervous system is very sensitive to abnormal stimuli, such as abnormal gravity, pain, extreme light and noise. This may result in a higher arousal level, which in turn may have lasting effects on behaviour. Furthermore preterm infants lack movement experience, sometimes due to the necessary sedation or because of the extreme hypotonia caused by their medical condition. Inappropriate or abnormal stimuli received via proprioception or by vestibulair, tactile and visual systems and their mutual integration are of great importance during this period of development. Muscle power disregulation will influence posture and postural control and at the later ages hand function and cognitive functions. These subtle deviances are often seen in preterm infants who were not obviously neurologically impaired, but more and more evidence has come available showing diffuse disturbances in cerebellum and cerebello-thalamo-cortical pathways. It is very questionable whether these aberrations can be prevented. The infant gets the wrong stimuli at the wrong period Kinderfysiotherapie juni

12 of development. Thus, even after correction for gestational age one still cannot say the corrected preterm infant is the same as an infant born fullterm. Is a paediatric physiotherapist needed? So where does this lead the physiotherapist? Our help is often asked for when the motor development of these precious infants deviates from the normal. But is it abnormal? We do not have a golden standard for the preterm that has been in the incubator for 3 weeks compared to the ones that were there for much longer periods of time. We do not know the impact of the different medical interventions. In short there is no golden standard for the profile of THE preterm infant. The question is are we dealing with the normal behaviour of the preterm infant or is it a stereotyped motor pattern that requires intervention? It should be remembered that any intervention is interference in the affairs of others and certainly is not always welcomed by the family already under stress after the preterm birth. On the other hand, preventive therapy shows the best results, because of the plasticity and adaptation of the young nervous system. Before 12 weeks corrected age it is very hard to assess pathology. At this age major transformations take place and it may be the best time to intervene. The repertoire of movement may show less variability but is not yet stereotyped. The therapist should council and coach the parents, teaching them to elicit active reactions and responses from their infant and most important: teach them how to wait for them. When than the child recovers and develops normal, the question remains: Was this the wonderful therapist s intervention or was it mother nature s own doing??? The theoretical underpinnings of therapies are weak, with little scientifically basis. It is hard to give solid scientifically evidence, since we are dealing with a nonlinear system and there are many variables influencing child development. But there is one thing we can focus on: the motor behaviour of the infant. So is a Paediatric Physiotherapist needed? The answer is a simple: yes!!! The paediatric physio is a specialist in the field of motor development, understands how the different subsystems influence each other and how deviations can lead to impairment of function. She or he has knowledge of the overlapping fields of psychology and pedagogy, has specific skills to coach the child and parents and is able to create an environment were functional tasks can be practised and learned. It is of main importance for the physio to use the right approach at the right time. The most important thing to keep in mind is the time window of development in which the therapy takes place and this need not always be the calendar age of the child but is set by the developmental timetable of the individual child. So far my success story and pep talk!!! Beren op de weg Now I want to talk about the bears on the road actually two!!!, de beren op de weg. I want to address a few of my worries concerning the way our profession is developing. There are a few things that really worry me. Don t forget treating the underlying impairment Since the publication of the new International Classification of Functioning, Disability and Health; the ICF criteria, the way of thinking about disability has changed considerably. From a medical model the thinking has changed to a problem a minority group. The process we physio s are involved in is now no longer called re-habilitation, but habilitation. Our congress is called: Function and Participation! In my opinion this way of thinking is OK but should be looked at with great caution. It may be a good idea to get a uniform classification and coding needed for research and useful for political planning of services, but it does not do much good for the individual patient. Therapeutic approaches are no longer directed toward restoring motor patterns as normal as possible, but often-distorted adaptive motor patterns are considered as an optimal solution to the underlying pathology. With modern neuroscientific evidence in mind this is a slippery path to walk on. Motor behaviour is the mirror of the brain, but cannot be looked at without the body, as Edelman teaches us. And I hope you will forgive me to put the main emphasis of this talk on development concept, but I think this holds true for all disease that affects the motor system. The brain and motor behaviour is shaped by trial and error, by experience. When movement becomes restricted it is bound to affect the brain and the muscular skeletal system. Motor learning techniques have demonstrated that special constructed environments assist the brain to adapt beyond the environment where the task is normally learned. It is the expertise of the therapist to find a variety of contexts where a specific task is able to form adaptations and in this way variability of function. I argue that when the impaired patient should be left to himself, he might be able to find the best functional way, this will lead to stereotyped rigid movement patterns and in time to less effective energy use. Extra wear and tear of the musculoskeletal system will be the result. The relationship between the impairment, the cause of the disability and the problematic functioning of the child are not always clear and should be considered together with the development and the environment of the child. If this line of argument (function and participation) is taken to far, the end of the story will be that the patient will be left to himself, (this suits the politicians fine) without therapy or guidance, because he can find out for himself how he functions best. In short I think we should not forget treating the underlying impairment, the primary deficit, because that is why your help is needed in the first place. When witnessing the distorted movements of patients with brain damage, or other system disorders, it can hardly be called optimal function! Again it depends on in which state of development we treat. In the case of neurological or other form of pathological condition, synergies will act as basic units for the control of movement and in this way reduce afferent signals, resulting in less variability, fixed patterns of movement superimposed on voluntary movement. It is the task of preventive therapy to try to create as much, be it adaptive, variety of movement as possible. It should be the skill of the pediatric therapist to facilitate the child and its environment in a playful and purposeful manner to the best possible motor pattern of coordination and try to do so from many different angles, to create active variability. It goes without saying that the tasks should be purposeful and therewith functional, well timed and should be re-enforced by the environment, the so-called carry over. 13 Often the questions we want to get answers to are one bridge too far and this leads to throwing away the child with the bathwater! play. I think we paediatric physio s have been or at least should have been giving functional therapy for a long time!! Clinical experience is needed before research can start I said I saw two bears on the road. My second concern is our fanatical desire to measure everything we do. Of course it was high time we started to use protocols in our work. When we want our profession to be taken seriously we need at least of consensus of opinion about definitions and documenting. Consensus a about treatment is much harder to get. There is no child like the other and there is no therapist like the other. And I think that is exactly were our strength lies! From my point of view there should and will be two sorts of therapists. From the shop floor the questions and solutions should come, trial and error is the best way to learn. Other therapists should try to put a more scientifical base to the profession. May be this is the bottle neck, the way I see it clinical experience is needed before research can start. It seems easy behind a desk to write a research project, to plan measurement techniques you have read about. It is far harder to clarify what it is you want and whether it is possible to measure. Is it a scientifically question you want to ask or is it a question whether your therapy has any effect on this particular child and its environment? Often the questions we want to get answers to are one bridge too far and this leads to throwing away the child with the bathwater! In short: Wat wil je weten en wat ga je meten? What do you want to know and what are you measuring. My view is, that this is our Achilles heel and at the same time our strength!. We should not let ourselves be discouraged by criticism about evidence based results, but likewise we should be critical about ourselves and ask the question what am I doing and what is the purpose of my strategy. Suitable measurement techniques for individual stages of development Now I have come full circle in my talk, because I think the main lead should be the time window of the development of the child. And by saying that I do not mean the so-called milestones, but the individual timetable of that particular child influenced by that particular disease. This will help us to ask the right questions at the right time and to look for the measurement techniques available and suitable for that stage. That should be the expertise of the paediatric physiotherapist. Lets s stick to our special expertise Looking back at my career, I have many things to be grateful for! In the first place the people I met on the way. My long gone ballet teacher Netty van der Valk, who taught me the art of ballet in the first place, but moreover the perseverance and mentality, than my main mentor in developmental neurology Bert Touwen. Later Brian Hopkins who had the patience to listen to my often chaotic talks and ideas and taught me the tricks of translating clinical expertise into research questions. 14 Ron van Empelen: Sociaal functioneren is relevanter dan fysiek functioneren Interview met de scheidend voorzitter van de congrescommisie Liesbeth van Vulpen & Jeannette Cappon, redactie Sinds de uitgave van Kinderfysiotherapie kent iedereen de naam van Empelen. Zoeken we op in van Empelen is te horen in de wandelgangen bij de opleidingen kinderfysiotherapie. Ron van Empelen probeert op verschillende manieren een brug te slaan tussen de wetenschap en de praktijk. We hebben nu al zoveel mensen opgeleid, de meeste zullen het nu toch wel weten (lachend). Zijn jarenlange ervaring en het liefdevolle overbrengen van zijn kennis is een bron van inspiratie voor velen. Na zes jaar gaat Ron de congrescommissie verlaten. Op het volgende congres zal hij niet als organisator maar als spreker optreden. Genoeg redenen voor de redactie om Ron te vragen voor een terugblik op het internationale congres. Met hem kijken we naar de relatie tussen wetenschap en de dagelijkse praktijk van de kinderfysiotherapeut. Drs. Sinds 1979 betrokken bij de opzet van de opleiding Kinderfysiotherapie, vanaf de start (1984) coördinator en sinds 1997 verbonden als kernteamlid en adviseur aan de opleiding Kinderfysiotherapie, Utrecht. Tevens coördinator en docent van verschillende aanvullingscursussen. Twee dagen vol met lezingen over de meest uiteenlopende onderwerpen en diverse invalshoeken zitten erop. Heeft de congrescommissie bereikt wat ze wilde bereiken? Ons doel was om de wetenschap en de praktijk dichter bij elkaar te brengen en daar zijn we in grote lijn wel in geslaagd. Er was een erg goede opkomst (vrijdag 750 deelnemers en zaterdag 500), zodat we een grote groep hebben bereikt. Of we ons doel echt bereikt hebben moet ook blijken uit de uitwerking van de enquête die gehouden is door medewerkers van het wetenschap loket. Tot dusver hebben we al gehoord, dat men over de inhoudelijke kant over het algemeen wel tevreden is, maar dat men nog steeds vraagt naar meer praktijk. Naar mijn idee wordt te weinig gezien dat er verschil is tussen een congres en een studiedag. Als je een studiedag organiseert kun je mensen actief betrekken. Op een congres met zoveel deelnemers kun je niet veel anders dan sprekers een verhaal laten houden. Dat moet wel een beetje gekoppeld worden aan de praktijk maar het kan niet zo zijn dat de deelnemers daadwerkelijk aan de slag gaan. Wel hebben we de sprekers gevraagd naar de praktische relevantie van hun bijdrage. Je kunt wel met videobeelden en casuistieken de link leggen naar de praktijk, zoals bij sommige lezingen gebeurde. Dat is waar en ik denk ook dat die kritiek ter harte genomen wordt. Het vrijdag ochtend programma over nature en nurture liet ons wat betreft de nurture enerzijds het belang van de invloeden van de omgeving zien en anderzijds de in eerste instantie weinig succesvolle invloed van de fysiotherapie. Wel prikkelend, maar niet voor allen motiverend voor de maandag na het congres. Hoe kijk je hier tegenaan? Wij wisten dat Eva Bower een wetenschappelijk goed opgezet onderzoek heeft gedaan naar de effecten van het geven van met name intensieve fysiotherapie bij kinderen met cerebrale parese. Effect studies zijn er nog maar heel beperkt. Het onderzoek van Eva Bower is een voorbeeld van een effect studie in het hoogste niveau van wetenschap, namelijk een RCT (Randomized Controlled Trial), dat wil zeggen een onderzoek met een controle groep waarbij nadrukkelijk gekeken kan worden wat de effecten zijn. Met dit soort onderzoeken krijgen we meer zicht op wat ons vak nu eigenlijk waard is. Daarom denk ik, dat het belangrijk is dat we hiervan kennis nemen. Je leert daardoor kritischer te staan ten opzichte van je eigen werk. Had jij inhoudelijk op het onderzoek je kanttekeningen of verklaringen voor deze uitkomsten? Als we in algemene zin kijken, dan zou je Publicaties: Empelen R. van, Nijhuis van der Sanden R., Hartman A. (red). Kinderfysiotherapie, Maarssen, Elsevier Gezondheidszorg, Diverse abstracts en publicaties Onderzoek: Vanaf 1996 prospectief longitudinaal follow-up naar effecten van epilepsiechirurgie moeten zeggen dat je niet op een deelgebied zoals verbeteren van de motoriek moet focussen. Je moet eigenlijk zo breed mogelijk zien wat het effect is van je behandeling. Daar heb je heel veel meetinstrumenten voor nodig en die heeft ze niet gebruikt. Als je zegt mijn doelstelling van behandelen is verbetering van de grove motoriek dan is de Gross Motor Function Measurement (GMFM) een prima meetinstrument bij kinderen met een cerebrale parese. Als je zegt mijn doelstelling is dat kinderen beter functioneren op school of beter functioneren in de maatschappij, dan is de GMFM geen geschikt meetinstrument. Marjolijn Ketelaar heeft in het verleden over het effect van functionele fysiotherapie onderzoek gedaan met de GMFM en de Pedi, waarbij de Pedi andere uitslagen gaf dan de GMFM. Je zou met andere resultaten kunnen komen, maar dan moet het gekoppeld zijn aan een andere, bredere doelstelling van de behandeling. 12 Kinderfysiotherapie juni 2003

15 Congressen zijn bij uitstek geschikt voor het contact tussen mensen die onderzoek doen en mensen die in de praktijk staan Paul Helders benadrukte dat we vooral ook familiecentered moeten werken. Jij stelde de vraag of we daar dan voor opgeleid zijn. Achter die vraag schuilt vast een antwoord. Ik denk dat het idee heel goed is. We moeten rekening houden met het kind en zijn eigen omgeving, het gezin en de familie. Ik kan me voorstellen, dat je dan meer de familie als doelstelling neemt. Of wij daar voor opgeleid zijn is de vraag, want wij zijn toch erg veel op het gebied van het behandelen van het individu opgeleid en veel minder op de consequenties op sociaal en participatiegebied. In de huidige kinderfysiotherapie opleidingen krijgt de context al duidelijk meer aandacht. Dat wil nog niet zeggen dat de kinderfysiotherapeuten die nu opgeleid zijn, bij uitstek geschikt zijn om familiecentered te werken. Ik denk dat daar nog wel iets meer scholing voor nodig is. Niet behandeltechnische scholing, maar we zouden veel meer sociaalpedagogische aandachtsgebieden moeten aanreiken. Waarbij we niet alleen moeten weten wat andere disciplines kunnen maar dit zelf ook kunnen betrekken in onze behandelmethodiek. Hoe kijk je dan tegen de multidisciplinaire benadering aan? Ik denk sowieso dat je andere disciplines nodig hebt wanneer je vanuit deze optiek van de praktische sociale wetenschappen wil gaan werken, waarbij men meer nog dan multidisciplinair interdisciplinair gezamenlijke doelstellingen kiest. Ook in het zaterdagochtend programma krijgen we als fysiotherapeuten een les in bescheidenheid wanneer we nog hadden gedacht, dat een toename van functionaliteit ook lineair verband houdt Kinderfysiotherapie juni

16 met een toename van de kwaliteit van leven. Hoe zie jij de rol van de fysiotherapeut in relatie tot kwaliteit van leven (KvL)? Eerst moet je weten wat je verstaat onder kwaliteit van leven. Daar heeft professor Bengt Lindstrom op het congres een prachtig voorbeeld van gegeven. Je kunt niet zeggen Dit is kwaliteit van leven. Iedere persoon bepaalt voor zichzelf wat hij of zij KvL vindt. KvL is zo individueel bepaald dat het maar de vraag is of wij als fysiotherapeut daar veel aan bij kunnen dragen. Ik maak nu in mijn onderzoek gebruik van kwaliteit van leven vragenlijsten, en dan zou je kunnen zeggen: ik kan nu meten hoe de kwaliteit van leven is bij een kind. Maar daar ben ik wel een beetje van terug gekomen. Ik kan wel voor wetenschappelijke doelstellingen KvL meten, om een groep te kunnen vergelijken, maar op individueel niveau kan dat eigenlijk niet. Dan moet je een andere vorm voor kiezen, zoals kwalitatieve analyses die veel meer individueel gericht zijn. Een kind vertelt dan zelf wat hij belangrijk vindt. In deze vorm gaat het niet zozeer om de kwantiteit zoals Hoeveel vindt je nu dat dit veranderd is? en Hoeveel vindt je nu dat dat veranderd is? In mijn onderzoek naar het disablement proces bij kinderen die geopereerd worden wegens hun epilepsie, heb ik zowel de lijsten van Jeanet Bruil gebruikt als dat we het kind zelf aan het woord lieten. Hier kwamen de verschillen tussen deze twee methodes mooi naar voren. Ik denk dat we wel een bijdrage kunnen hebben als fysiotherapeut maar dat we altijd met het kind en de ouders moeten bespreken of onze fysiotherapeutische bijdrage die we voor ogen hebben werkelijk relevant is voor het kind. Wat een opvallende uitkomst is van mijn onderzoek is dat er na een operatie een achteruitgang kan worden gezien op functie niveau maar een vooruitgang op participatieniveau en een daardoor verbeterde kwaliteit van leven voor het kind. Je bent voor 50 procent behandelend kinderfysiotherapeut. Hoe gebruik je deze kennis in je eigen behandelingen? Sowieso het kind meer aan het woord laten, dat is heel erg belangrijk. En niet voor jezelf een beeld hebben van Dit lijkt mij belangrijk voor een kind en ouders. Nou had ik toch al van nature de neiging om de mensen zelf te vragen wat ze er van vinden. Ik begin meestal met de vraag Waarom komen jullie bij mij?. Dan kunnen ze zeggen: Ik heb een verwijzing. Maar als je ze zelf vraagt hun Niet iedereen hoeft wetenschapper te worden, maar we moeten de publicaties wel op waarde kunnen inschatten. probleem te verwoorden lukt dit vaak heel goed. En dan, ook heel simpel, vragen: wat voor idee heb je wat er aan gedaan kan worden. Niet alleen wat de ouder vindt maar vooral ook wat het kind daar voor ideeën over heeft. Ik zie in de praktijk dat een kind vaak heel goed kan verwoorden wat hij of zij wel of niet wil. Hier houd ik veel meer rekening mee dan de visie ik zie dat dit kind niet goed beweegt en ik denk dat dat anders zou kunnen, dat is soms helemaal niet relevant. De verwetenschappelijking van de fysiotherapie spiegelt zich af in het soort lezingen op het congres. Een groot aantal van de lezingen doen verslag van wetenschappelijk onderzoek. Om onderzoek goed op waarde te kunnen schatten is een goede kennis van zaken nodig op dit gebied. Denk je dat de gemiddelde kinderfysiotherapeut hiertoe in staat is? Wanneer ik dit niet alleen aan de hand van deze congresdagen beoordeel, maar vooral ook aan de hand van de cursussen methodisch handelen, verslaglegging en met name de evidence baced handelen, dan is dat nog maar heel betrekkelijk. Na 15 jaar doceren op de opleiding voor kinderfysiotherapie had ik gedacht, We hebben nu al zoveel mensen opgeleid, de meeste zullen het nu toch wel weten (lachend). Je denkt dat het nu wel algemeen bekend is, maar dat is het niet helemaal. Het blijkt toch nog erg moeilijk te zijn voor de algemene fysiotherapeut werkzaam in de kinderfysiotherapie om iets kritisch te lezen en te kunnen discussiëren over hoe iets is onderzocht en of dat op de goede manier is gedaan en of het goed is verwoord. Ik heb het dan dus niet over de net opgeleide kinderfysiotherapeut van dit moment. De huidige opleiding besteed hier wel veel meer aandacht aan, maar er is nog steeds een verschil tussen de zeg maar wat meer academisch geschoolde en de doorsnee opgeleide kinderfysiotherapeut. Dat is wel logisch, maar ik denk wel dat we de stap naar academisch niveau moeten gaan zetten. De vertaalslag van wetenschap naar praktijk is net zo belangrijk als de aanlevering van essentiële vragen uit de praktijk naar het onderzoeksveld. Hoe zou de communicatie kunnen verlopen, zonder dat een ieder zowel praktijk als wetenschapsmens dient te worden? Ik denk dat die brug er in ieder geval moet zijn. Niet iedere fysiotherapeut moet wetenschapper worden, dan zouden er veel teveel wetenschappers komen en te weinig praktijk mensen. Maar het zou zonde zijn wanneer wij afhankelijk gaan worden van wetenschappers zonder fysiotherapeutische achtergrond. Om een voorbeeld van het congres te geven: ik vond het onderzoek over oversteek gedrag van kinderen wel erg ver van het praktische werk afstaan, en maak dan maar eens de vertaling naar de praktijk. Dat is het gevaar wanneer we de wetenschap uit handen geven aan andere disciplines in plaats van dat mensen vanuit ons vak groeien naar die wetenschap omdat ze echt geïnteresseerd zijn in wetenschap en dan een volledige scholing doen. Daarnaast moeten de praktijkmensen op zijn minst op de hoogte zijn van de onderzoeksmogelijkheden, en met hun vragen ergens terecht kunnen. Zoals bijvoorbeeld bij het wetenschapsloket. Want er zijn wel degelijk mensen die vanuit het praktisch handelen met vragen zitten, en die ze graag uitgezocht willen hebben. Heb je nog andere ideeën over hoe die brug tussen praktijk en wetenschap gerealiseerd kan worden? Congressen zijn bij uitstek geschikt voor het contact tussen mensen die onderzoek doen en mensen die in de praktijk staan. En dan hoop je dat er meer discussie kan komen. Dit zag je tijdens het congres helaas te weinig gebeuren. Dat is wel iets wat me opviel tijdens het congres. Er zijn heel veel verhalen geweest en toch relatief weinig discussies. Tevens hoop ik dat er kennisoverdracht plaats vindt tijdens de IOF s en op de werkvloer zelf. Hoe komt het dat het zo moeilijk is om tot een discussie te komen, denk je? De grote zaal is denk ik te groot, dat is te lastig. In de kleine zalen hebben we wegens het volle programma niet zoveel tijd gere- 14 Kinderfysiotherapie juni 2003

17 serveerd. Je zou de keuze kunnen maken om twee a drie sessies te laten horen en dan een half uur tijd voor discussies in te plannen. Maar dan moet je er ook vanuit gaan dat er echt gediscussieerd wordt. En dat was bij ons (congrescommissie) nog een beetje de vraag, in hoeverre is de zaal in staat en bereid om in discussie te gaan met degene die onderzoek doet. Voor een volgende keer zou dat voorbereid kunnen worden met bijvoorbeeld stellingen, voor het geval er niet een goede discussie op gang komt. Deelnemers zitten nog teveel als consument in de zaal. Welke ontwikkelingen verwacht je in de naaste toekomst voor het lustrumcongres? Het zou toch prachtig zijn wanneer we over vijf jaar een Europees congres kinderfysiotherapie kunnen organiseren. We moeten een beetje gaan polsen bij verschillende landen. Er is nog heel weinig ontwikkeling op kinderfysiotherapie gebied in verschillende landen, maar er is wel belangstelling. Ik weet van de Zweden en de Engelsen dat ze veel belangstelling hebben voor internationale samenwerking. Zo zijn er nog meer landen die nog niet zover zijn dat ze een aparte kinderfysiotherapie opleiding hebben, maar wel graag aandacht voor kinderbehandeling willen ontwikkelen. Dan vind ik dat we in Nederland het initiatief zouden kunnen nemen. Waarom zou de kennis beperkt moeten blijven tot Nederland. Wij zijn 20 jaar geleden gestart met de opleiding voor kinderfysiotherapie, we hebben erkenning en zijn gespecialiseerd. Terwijl de landen rondom ons nog helemaal niet zover zijn. Heeft deze wens ook te maken met je affiniteit tot doceren? Ik denk het wel een beetje, ja. Ik heb altijd het gevoel gehad dat je kennis moet delen. Het moet niet zo zijn dat je met je kennis op een elite eilandje gaat zitten, en je kennis voor jezelf houdt. Het is ook een soort hobby van me om met mensen aan de slag te gaan en te vertellen wat we hebben ervaren en hebben geleerd. Je kunt dat zien in klein verband, in korte cursussen, maar ik zie het ook in groter verband zoals bijvoorbeeld een internationaal congres. Vanuit de visie van de wetenschap ben je overigens verplicht om je onderzoeksresultaten niet voor jezelf te houden, maar om deze altijd te publiceren en daarmee aan anderen kenbaar te maken wat je gevonden hebt. Daarmee wordt je meteen getoetst. Dat is de andere kant van wetenschap. Wat we hebben geleerd in de loop der tijd is, dat sociaal functioneren veel relevanter is dan fysiek functioneren Wetenschap is iets onderzoeken en uitdragen naar de ander en verantwoorden hoe je het gedaan hebt, wat je gevonden hebt en wat de betekenis daarvan is voor de wereld voor de praktijk. Tegelijk krijgen anderen de gelegenheid om kritisch te kijken naar de wetenschap die is uitgevoerd. Kun je wat over jouw onderzoek vertellen? Zelf heb ik gekozen voor een praktisch relevant onderzoek. Ik ben werkzaam als kinderfysiotherapeut op de afdeling kinderneurologie, waar een belangrijk aandachtsgebied epilepsiechirurgie is. We zijn het eerste ziekenhuis in Nederland aangewezen door het ministerie, dat epilepsiechirurgie mag doen. Inmiddels mag dat ook in Maastricht. Ik heb gekozen voor een longitudinaal onderzoek, dus follow up onderzoek, waarbij kinderen voor en na een ingreep gemeten worden. Deze kinderen worden vergeleken met kinderen die niet geopereerd worden. Als prospectieve studie, dus van te voren opgezet, zo en zo gaan we het doen. Met voldoende meetmomenten, longitudinaal. Om niet alleen te kunnen zeggen wat het korte termijn effect is maar ook wat het lange termijn effect is. De aanleiding was dat ouders en kinderen vaak voor een operatie stonden met het idee van Ik ga dit nu doen, maar wat is het effect ervan?. Ik denk dat het essentieel is dat je ouders en kind kan informeren over wat je kunt verwachten op het gebied van motoriek, leren, en andere gebieden. Niet alleen kijken op stoornis niveau, want het is veel belangrijker om te volgen en te meten wat het voor betekenis heeft voor je dagelijks leven. Wat zijn de gevolgen van de ziekte (disablement proces) en de gevolgen van ingrepen in de ziekte, dat is mijn onderzoek. Wat onder andere uit het onderzoek naar voren komt, is dat functies, en dan met name het sociale functioneren, toenemen daar waar stoornissen gelijk blijven of zelfs ietsje toenemen. Wat we dus ook hebben moeten leren in de loop der tijd is, dat sociaal functioneren veel relevanter is dan fysiek functioneren. Of je met vriendjes kunt omgaan, of je naar school en naar een feestje kunt, is zoveel belangrijker dan het iets moeilijker lopen. Het eerst volgende congres van de NVFK, op 8 november 2003 in Den Haag, heeft het thema epilepsie. Ron van Empelen zal hier onder andere zijn onderzoeksresultaten presenteren. 18 Spasticiteitsteam Academisch Ziekenhuis Maastricht Een overzicht van 5 jaar samenwerking tussen medische en fysiotherapeutische disciplines in de diagnostiek en advisering bij kinderen met Cerebrale Parese A.J.A. de Louw 1, H. Vles 1 Spasticiteit bij kinderen met cerebral palsy kan een zeer invaliderende aandoening zijn, waarbij beperkingen in mobiliteit, verzorgingsproblemen en pijnklachten op de voorgrond staan. In dit artikel wordt de pathofysiologie van spasticiteit bij kinderen met cerebral palsy besproken. Daarnaast wordt er een overzicht van de huidige behandelmethode voor spasticiteit gegeven en de werkwijze en resultaten van de werkgroep spasticiteit binnen het academisch ziekenhuis Maastricht besproken. De behandeling van spasticiteit bij kinderen met cerebral palsy is in de afgelopen decennia aan veel verandering onderhevig geweest. Door een veranderde perinatale zorg voor (premature) neonaten zijn de overlevingskansen van steeds ziekere kinderen aanzienlijk toegenomen. Hierdoor is echter ook een toename van kinderen met een gestoorde motorische ontwikkeling ontstaan. De ontwikkeling van spiercoördinatie en spierontwikkeling is verstoord. Deels komt dit door de spasticiteit. Door deze motorische problemen treden er bij deze kinderen veelal ontwikkelingsachterstanden op. Genezing van spasticiteit bij kinderen met cerebral palsy is op dit moment nog niet mogelijk. Door de introductie van nieuwe behandelmethoden voor spasticiteit gedurende de afgelopen decennia is het scala aan therapeutische opties echter enorm vergroot. De behandeling van spasticiteit is complex, waarbij steeds per individu moet worden gekeken naar de mogelijkheden van verbetering van de spasticiteit en naar de mogelijkheden ter verbetering van vaardigheden na behandeling. De evaluatie van spasticiteit bij kinderen met cerebral palsy en de beslissing om over te gaan tot behandeling berust niet bij een discipline maar vindt bij voorkeur plaats binnen een werkgroep spasticiteit bestaande uit meerdere disciplines. In het academische ziekenhuis Maastricht (AZM) is om deze reden een spasticiteitsteam in het leven geroepen, waarin de volgende teamleden vertegenwoordigd zijn: de kinderneuroloog, de kinderorthopeed, de kinderrevalidatiearts, de anesthesist en de kinderfysiotherapeut. Het hoofddoel van dit spasticiteitsteam is gelegen in het komen tot een gezamenlijk advies met betrekking tot het beantwoorden van de door het kind/ouders of behandelaar gestelde hulpvraag. De leden van het spasticiteitsteam hebben als doel te komen tot een eensluidend advies elkaar aanvullend vanuit een ieders specifieke deskundigheid. De adviezen liggen op het gebied van verder specialistisch onderzoek, specifieke medische behandeling in brede zin van het woord en specifieke paramedische behandeling (therapie en hulpmiddelen). In dit overzicht beschrijven we allereerst de problematiek van cerebral palsy en spasticiteit. Vervolgens gaan we in op de verschillende behandelmethoden en worden kort de resultaten van de werkgroep spasticiteit beschreven. Cerebral palsy Reeds 150 jaar geleden beschreef Little de klinische verschijnselen die bij Cerebral Palsy worden gezien, ook wel Little disease genaamd (Little, 1861). De motorische beperkingen op kinderleeftijd schreef Little toe aan problemen die bij de zwangerschap optraden. Cerebral palsy ofwel infantiele encephalopathie is een frequent voorkomende aandoening met een prevalentie van 1,5/1000 tot 2,5/1000 levendgeborenen (Kuban, 1994). De schade aan het centrale zenuwstelsel kan ontstaan gedurende verschillende stadia van de ontwikkeling; pre-, peri- of post- nataal. Er zijn verschillende oorzaken aan te wijzen voor het klinische beeld van cerebral palsy. In 8 tot 20 procent van de gevallen is perinatale asphyxie de oorzaak van cerebral palsy (Nelson e.a 1986; Pschirrer e.a 2000). Bij perinatale asphyxie is er sprake van een combinatie van ischaemie (geen bloed) en hypoxie (lage oxygenatie van bloed), die weefselschade in alle orgaansystemen kunnen veroorzaken. Het ontwikkelingsstadium van de neonatus en de duur en mate van de asphyxie zijn factoren die de mate van schade aan het centrale zenuwstelsel bepalen (Simom, 1999). 19 voor een cerebral palsy zijn een intra-uterine infectie, (ex)prematuriteit en metabole- of genetische afwijkingen (Kuban 1994; Nelson e.a 2000). Cerebral palsy wordt gekenmerkt door de aanwezigheid van bewegingsstoornissen. In het normale gezonde individu vindt de coördinatie van beweging plaats door aansturing vanuit het centrale zenuwstelsel. Hierbij kunnen drie niveaus van aansturing worden onderscheiden. Ten eerste een niveau bestaande uit de cerebrale cortex, het limbische systeem en de basale ganglia. Het tweede niveau wordt aangestuurd door het cerebellum en de hersenstam. Op een derde niveau speelt het ruggenmerg een centrale rol (Mori 1996). Uit recente dierexperimentele studies naar de effecten van perinatale asphyxie op het ruggenmerg van de rat blijkt dat, naast de cerebrale schade, het ruggenmerg (deels) betrokken is bij de pathofysiologie van spasticiteit (de Louw e.a. 2002). Na perinatale asphyxie werd een toename van geprogrammeerde celdood waargenomen in ondermeer de interneuronen pools van het lumbale ruggenmerg. Behalve door corticospinale banen ofwel piramidebanen en afferente input van spierspoeltjes, spelen ook parapyramidale banen een rol bij de spinale neuronale activiteit. Vooral de Tractus Reticulospinalis Dorsalis (TRD), de Tractus Reticulospinalis Medialis (TRM) en de Vestibulo Spinale Tractus (VST) zijn hierbij van belang. De TRD heeft een inhiberend effect en de TRM en VST hebben beide een exciterend effect op spinale (inter)neuronen. Indien er in een van deze systemen een laesie ontstaat zal dit een effect hebben op de uitvoering van normale bewegingen. Cerebral palsy uit zich in vele vormen, waaronder hemiplegie, diplegie, tetraplegie, maar ook ataxie, dystonie en parese. Het meest voorkomende klinische verschijnsel is spasticiteit, hetgeen zich in 60-70% van de gevallen presenteert en daarmee ook een belangrijk therapeutisch doel vormt. Andere klinische verschijnselen van cerebral paly zijn mentale retardatie, epilepsie, incontinentie, voedingsproblemen, kwijlen, communicatie-, visus- en gehoorstoornissen. Spasticiteit Spasticiteit wordt gedefinieerd als een tonusregulatiestoornis met snelheidsafhankelijke verhoging van de tonische strekreflexen gepaard gaande met een verhoogde weerstand tegen passieve rek (hypertonie), hyperreflexie en beperking van willekeurige Observatie tot stand komen van jongen met spastische diplegie voorafgaand aan botuline-toxine A behandeling bewegingen (Lanc 1980). Spasticiteit ten gevolge van cerebral palsy kan een ernstige invaliderende aandoening zijn. Kenmerkende symptomen hierbij zijn: loopstoornissen, contracturen (blijvende stijfheid van spieren, pezen, ligamenten en gewrichten), pijnklachten en beperkingen bij verzorging voor zowel de patiënt zelf, en met betrekking tot het laatste voor de betreffende verzorger(s). Spasticiteit die ten gevolge van cerebral palsy optreedt maakt onderdeel uit van het het Upper Motor Neuron (UMN). Met het UMN worden alle lange descenderende banen bedoeld, die bewegingen of spiertonus reguleren of beïnvloeden (Alexander e.a. 1991; Sheean 1998) Het UMN syndroom bestaat uit negatieve en positieve symptomen. De negatieve symptomen worden gekenmerkt door afname van motorische activiteit, zoals spierzwakte, snelle vermoeibaarheid en verlies van handigheid. Spasticiteit samen met verhoogde reflexen en pathologische reflexen vormen de positieve componenten van het UMN syndroom en worden gekenmerkt door overvloedige motorische activiteit (Sheean 1998). In tegenstelling tot de huidige veronderstelling is spasticiteit niet alleen het gevolg van een aandoening van de piramidebaan. Geïsoleerde laesies van dit baansysteem resulteerde niet in de combinatie van zwakheid, spasticiteit en hyperreflexie, zoals die in het UMN syndroom wordt gezien (Alexander e.a 1991) De exacte pathofysiologie van spasticiteit ten gevolge van cerebral palsy blijft onduidelijk. Wel is duidelijk dat er sprake is van een verstoorde balans tussen activering en remming van zenuwcellen. Hierbij heeft de activering door middel van afferente informatie uit het ruggenmerg de overhand, waardoor een spier overmatig geprikkeld wordt (Mayer 1997, Sheaan 1998). Deze hyperexcitatie leidt vervolgens tot versterkte reflexactiviteit en spasticiteit, met als gevolg beperkingen die de kwaliteit van leven, van zowel patiënt als verzorger(s), negatief beïnvloeden. Het reduceren van deze positieve symptomen van het UMN syndroom geassocieerd met hyperexcitatie, vormt de basis voor de behandeling van spasticiteit (Goldstein, 2001). Behandeling De behandeling van spasticiteit is complex, waarbij het uiteindelijke doel gericht moet zijn op de hulpvraag van ouder en kind zoals het verbeteren van de beperkingen c.q. mobiliteit, het optimaliseren van de verzorging en het wegnemen van pijn (Richardson 1998). Als voorwaarde geldt echter dat alleen met een behandeling gestart wordt, indien interferentie een kans op verbetering van de beperkingen biedt. Omdat de behandeling van spasticiteit niet onder een discipline kan vallen, hebben wij gekozen tot het samenstellen van het spasticiteitsteam. Dit spasticiteitsteam bestaat bij voorkeur uit een kinderneuroloog, revalidatiearts, kinderfysiotherapeut en orthopedisch chirurg. Door middel van verschillende diagnostische middelen kan tot een volledig overzicht van de huidige toestand van het kind worden Kinderfysiotherapie juni

20 Observatie opstaan van jongen met spastische diplegie voorafgaand aan behandeling met botuline toxine gekomen. Tijdens het spreekuur van het spasticiteitsteam zien alle leden van het team onafhankelijk van elkaar alle kinderen. Daarna wordt in een gezamenlijk overleg de afzonderlijke bevindingen besproken, zodat uiteindelijk een multidisciplinair advies naar de verwijzer ontstaat. Aan de hand hiervan bepaalt het spasticiteitsteam of behandeling geïndiceerd is. Vervolgens worden er behandelingsdoelen opgesteld, indien mogelijk in overleg met patiënt en/of verzorger, waarbij de verbetering de beperkingen van de patiënt wordt vooropgesteld. De keuze van behandeling is afhankelijk van de gestelde doelen, mate van spasticiteit, aanwezigheid van zorg en ondersteuning en de aanwezigheid van co-morbiditeit, zoals mentale retardatie. Met de therapie beoogt men niet alleen de spasticiteit zelf te behandelen, maar ook wordt getracht de complicaties van spasticiteit, waaronder motorische achterstand, contracturen, deformiteiten van botten en subluxaties, te voorkomen of progressie te remmen. Tenslotte vindt er na de behandeling een evaluatie plaats om het behaalde resultaat vast te leggen en eventueel een verder beleid uit te stippelen. Bij spasticiteit moet telkens in het achterhoofd worden gehouden dat alleen de motorische beperkingen kunnen worden behandeld, aangezien cerebral palsy een gevolg is van schade aan de hersenen en/of het ruggenmerg en veel hieruit resulterende problemen slechts gedeeltelijk behandelbaar zijn. Een kleine verbetering van de beperkingen kan echter al een duidelijke toename van kwaliteit van leven tot gevolg hebben voor zowel patiënt als verzorger(s). Met specifieke en effectieve therapie kan dus nog veel vooruitgang geboekt worden. Huidige behandelmethoden Tegenwoordig zijn er grosso modo drie verschillende mogelijkheden om spasticiteit te behandelen: conservatief, medicamenteus en operatief (figuur 1). De conservatieve therapie bestaat voornamelijk uit revalidatie, fysiotherapie en ortheses. Vooralsnog zijn er maar weinig studies gedaan naar het effect van fysiotherapie op spasticiteit. Uit de weinige studies die zijn gedaan blijkt dat er slechts weinig verbetering van de tonusregulatiestoornis behaald wordt (Bower, e.a 2001; Fowler e.a 2001). Wel is gebleken dat het succes van interventies (zowel medicijnen als chirurgisch) sterk afhankelijk is van fysiotherapie en revalidatie (Goldstein 2001). Het onderzoek van de fysiotherapeut draagt bij in het formuleren van de behandeldoelen en de follow-up. De orale medicamenten die gebruikt worden zijn: Diazepam, Dantroleen, Clonidine en Baclofen. Deze medicamenten, behalve Dantroleen, onderdrukken de spasticiteit door remming van excitatoire neurotransmitters of door stimulatie van de inhibitoire neurotransmitters. Een nadeel is echter dat deze orale medicamenten een sedatieve bijwerking hebben door hun effect op neurotransmitters in de hersenen. De orale farmacotherapie therapie heeft dan ook zijn beperkingen. Baclofen intra-thecaal heeft als voordeel dat het lokaal kan worden toegediend, maar het inbrengen van een vreemd lichaam brengt risico s met zich mee, waardoor deze behandelingsmethode momenteel slechts als laatste optie wordt toegepast. Daarnaast is de indicatie voor intrathecal baclofen behandeling afhankelijk van de ernst en uitgebreidheid van de tonusregulatiestoornis, bij een spastische monoparese van een been is intrathecale baclofen bijvoorbeeld niet geïndiceerd. Lokale denervatie is mogelijk met behulp van fenol injecties. De mate van spasticiteit neemt hierdoor weliswaar af, maar deze afname gaat vaak gepaard met het optreden van pijnklachten.
