3 Summary/Samenvatting SUMMARY Gastrointestinal cancer is one of the most important causes of cancer related death. Because curative treatment is only possible in an early stage of the disease, early detection is a sine qua non. Early diagnosis can lead to an important decrease in mortality caused by gastrointestinal cancer. The growing understanding of the molecular genetic alterations underlying the development and progression of gastrointestinal malignancies enable the use of molecular genetic alterations in predicting disease development and progression, and may potentially improve patient management. Germline mutations have been found which can identify patients at risk for the development of gastrointestinal cancer. In addition, investigation of molecular genetic biomarkers in tumors or normal mucosa of patients with gastrointestinal cancer, or individuals at risk for the development of gastrointestinal cancer will contribute to the follow-up and surveillance of these patients in the nearby future. Finally, insights in the genetic defects underlying carcinogenesis may lead to a specific treatment, based on the replacement of the genetic defect. The studies described in this thesis may contribute to the understanding of the role of genetic mutations underlying gastrointestinal tumorigenesis. In chapters 2 and 3 a review is given on the phenotypic and genotypic characteristics of the most important hamartomatous polyposis syndromes, and the problems concerning upper gastrointestinal polyps in FAP patients who have undergone a colectomy are described. Many of these patients die due to upper gastrointestinal tract cancer, which develops from adenomas. Therefore, it is important that these patients are examined on a regular basis by endoscopy and biopsy to assess the mucosal histological changes. In the future it might be possible that in the follow-up of these patients molecular techniques can predict the progression of the disease. In chapter 4 the origin (clonality) of hyperplastic polyps in the stomach of a patient is analysed using various molecular genetic markers. This study shows that in three different polyps in various stages (hyperplasia, high-grade dysplasia and cancer) the same K-ras mutation was present. It is unlikely that this mutation occurred by chance in all three polyps, since K-ras mutations are rare in conventional stomach cancer. Furthermore, an analysis on the involvement oïp53,p21, MDM2, LOH, DNA mismatch repair mutations and EBV in the formation of stomach polyps was performed, but their involvement could not be demonstrated. The relatively early involvement of K-ras shows that this gene is not a prerequisite for the development and presence of dysplasia. In the following chapters 5-10 studies are summarized that focus on germline and somatic mutations in the Peutz-Jeghers syndrome (PJS). The results of the study described in chapter 5 show that K-ras mutations, which are common in colon cancer, are rare in the formation of PJS polyps and dysplasia. Only one out of 52 polyps had a K-ras mutation. In this polyp a small focus of dysplastic cells was present but no K-ras mutation was found in these cells. Also in three other polyps with dysplasia no K-ras mutation was found, indicating that the role of this mutant gene is absent in the transformation towards 96

4 Summary/Samenvatting dysplasia. In chapter 6 a 78-year follow-up is presented of the PJS family that was identified in 1921 by dr Peutz, after whom the syndrome is named. The study describes 6 generations of the pedigree and shows that the syndrome in this family is caused by a mutation in the STK11 gene. The relative high cancer related mortality and the high number of members with illness due to the disease in this family shows that PJS is not a benign disease. The study in chapter 7 describes the role which STK11 can have in the formation of both PJS related disease and sporadic pancreas cancer. In about 6% of 127 sporadic pancreatic and biliary adenocarcinomas 19p LOH occurs which harbors STKll, indicating that this gene is involved in this carcinogenesis. The following chapter 8 describes a study addressing the genetic basis of PJS carcinogenesis. This was done by analysing alterations in the STKll gene and genes known from the adenoma-carcinoma sequence. In this study it is shown by haplotype analysis that the wild-type allele is lost and that the mutated allele is somatically present. This indicates that STKll behaves as a classic tumor-suppressor gene following the two hit hypothesis of Knudson. The data in this study also indicate that STKll mutations are an early event in tumorigenesis. The analysis in this study of genes known from the adenoma-carcinoma sequence show that these genes do not play a major role in tumorigenesis in PJS. In chapter 9 19 PJS families have been analysed on STKll mutations. In 12 families, previously undescribed mutations in STK11 were found comprising a nonsense mutation, 3 frameshift deletions, 3 frameshift insertions, two acceptor splice site mutations and 3 missense mutations; all these mutations lead to a functionally inactive protein product. In the other 7 families no mutations were found, possibly indicating genetic heterogeneity. A relatively large mutation spectrum seems to exist which would hamper screening of individuals. Chapter 10 describes the study that is the continuation of the study described in chapter 8. This somatic study emphasizes the early role of STKll in PJS tumorigenesis. In addition, we found that 19p LOH can be used as bio-marker for cancer-risk. Analysis of the genes from the adenomacarcinoma sequence indicates that a hamartoma-carcinoma sequence is likely and that the genetic pathway in this process differs from the classical adenoma-carcinoma sequence. The final study in this thesis in chapter 11 analysed the possibility to use microsatellite markers as diagnostic markers to establish the diagnosis of the hereditary Muir-Torre syndrome (MTS) in patients with sebaceous gland carcinomas (SGC). Patients with a SGC and a history of intestinal carcinomas show a high percentage of mismatch repair (MMR) defects. In sporadic SGC, no microsatellite instability (MSI) was observed. MMR defects could be confirmed by immunostaining of hmlhl and hmsh2. Both genes were equally mutated in SGC with MSI. Because SGC can occur prior to the development of intestinal carcinomas, one has to consider an analysis of MMR defects in patients presenting with a SGC. A diagnosis of MTS has important consequences for the patient and his or her family. 97

5 Summary/Samenvatting SAMENVATTING Maag-darmkanker neemt in Nederland een belangrijke plaats in als oorzaak voor kanker gerelateerde sterfte. Omdat alleen in een vroeg stadium een adequate behandeling mogelijk is, is het stellen van de diagnose in een vroeg stadium belangrijk. Dit kan leiden tot een vermindering van sterfte ten gevolge van deze ziekte. De toegenomen inzichten in de moleculair-genetische veranderingen die aan het ontstaan en de progressie van maag-en darmkanker ten grondslag liggen maken het gebruik van moleculair-genetische afwijkingen voor het voorspellen van het ontstaan en het beloop van deze ziekte mogelijk, en kunnen potentieel een bijdrage aan een verbeterde patiëntenzorg leveren. Er zijn inmiddels kiembaanmutaties beschreven waarmee patiënten met een verhoogd risico op maag- en darmkanker kunnen worden geïdentificeerd. Tevens zullen moleculair-genetische bio-merkers in tumoren of in het normale slijmvlies van patiënten met een verhoogd risico op het ontstaan van maag- en darmkanker in de toekomst kunnen bijdragen aan het medisch vervolgen en screenen van patiënten. Ten slotte kan het inzicht in de genetische defecten die aan het onstaan van darmtumoren ten grondslag liggen, in de toekomst mogelijk leiden tot een specifieke behandeling die berust op het compenseren van het defecte mechanisme dat tot de ziekte heeft geleid. De studies die in dit proefschrift worden beschreven kunnen een bijdrage leveren aan het begrijpen van de genetische veranderingen die leiden tot gastrointestinale tumorvorming. In de hoofdstukken 2 en 3 wordt een recent overzicht gegeven van de phenotypische en genotypische kenmerken van de belangrijkste hamartomateuze polyposis syndromen en wordt de problematiek van de vorming van dunne darm en maag poliepen bij FAP patiënten die een colectomie hebben ondergaan beschreven. Veel FAP patiënten overlijden aan de gevolgen van kanker in het bovenste deel van het maag-darmkanaal, die zich heeft gevormd vanuit adenomen. Het is daarom van belang dat deze patiënten regelmatig worden onderzocht via endoscopic waarna biopten op histologische veranderingen kunnen worden onderzocht. In de toekomst wordt het wellicht mogelijk dat bij het klinisch vervolgen van deze patiënten gebruik gemaakt kan worden van moleculaire technieken om het beloop van de ziekte te voorspellen. In hoofdstuk 4 wordt beschreven hoe met behulp van genetische merkers naar de oorsprong (clonaliteit) van drie hyperplastische maag poliepen van een patiënt kan worden gekeken. Deze studie laat zien dat in de drie poliepen in verschillende stadia (hyperplasie, high-grade dysplasie en kanker) dezelfde K-ras mutatie wordt aangetroffen. Het is onwaarschijnlijk dat deze mutatie in de drie poliepen van deze patiënt door toeval is onstaan, aangezien K-ras mutaties zeldzaam zijn in maagtumoren. Verder is gekeken naar de mogelijke betrokkenheid van p53, p21, MDM2, LOH, DNA mismatch repair (MMR) mutaties, en EBV bij de vorming van de maag poliepen; een rol in het ontstaan van de poliepen kon niet worden aangetoond. De relatief vroege betrokkenheid van K-ras wijst erop dat mutaties in dit gen niet strikt noodzakelijk zijn voor het ontstaan van dysplasie. In de 98

6 . Summary/Samenvatting volgende hoofdstukken 5-10 zijn de studies beschreven die zich toespitsten op de kiembaan en somatische mutaties in PJS. Uit de resultaten van de studie beschreven in hoofdstuk 5 blijkt dat K-ras mutaties die in colon kanker veel voorkomen niet betrokken zijn bij de formatie van PJS poliepen en dysplasie. Slechts één van de 52 poliepen bleek een K-ras mutatie te hebben. In deze poliep was een kleine focus van dysplastische cellen aanwezig, maar hierin werd geen K-ras mutatie gevonden. Ook in drie andere poliepen met dysplasie was geen K-ras mutatie aanwezig, hetgeen wijst op afwezigheid van een rol van dit gen in een dysplastische transformatie. In hoofdstuk 6 is de 78-jarige en tevens langste klinische vervolgstudie beschreven van de PJS familie die in 1921 door dr Peutz werd beschreven en naar wie het syndroom is vernoemd. De studie beschrijft zes generaties van de stamboom en toont aan dat het syndroom in deze familie is veroorzaakt door een mutatie in het STK11 gen. De relatief hoge sterfte door kanker en het grote aantal personen dat ziek is vanwege het syndroom in deze familie toont aan dat PJS geen goedaardige ziekte is. De studie in hoofdstuk 7 beschrijft de rol die STK11 kan spelen in de vorming van zowel aan PJS gerelateerde pancreas kanker als ook de sporadische vorm. In 6% van 127 sporadische pancreas and biliare adenocarcinomen werd LOH gevonden op 19p, waar STK11 ligt, hetgeen aangeeft dat dit gen betrokken is bij deze carcinogenese. Het volgende hoofdstuk 8 beschrijft de studie die gedaan is naar de genetische basis van de carcinogenese in PJS. Dit is gedaan door te kijken naar de rol van STKU en de rol van de genen uit de adenoma-carcinoma sequentie. In deze studie is m.b.v. stamboom gegevens aangetoond dat in geval van 19p LOH het wildtype allel verloren is gegaan en slechts het gemuteerde en inactieve allel somatisch aanwezig is. Dit wijst er dus op dat STKU inderdaad een klassiek tumorsuppressor gen is hetgeen al eerder gesuggereerd was. De gegevens uit deze studie wijzen er tevens op dat mutaties in STK11 een vroege gebeurtenis zijn in de tumorgenese. De analyse in deze studie van de genen uit de adenoma-carcinoma sequentie laat zien dat deze genen hierin geen grote rol lijken te spelen. In hoofdstuk 9 zijn 19 PJS families geanalyseerd op mutaties in het STKU gen. In 12 families werden niet eerder beschreven mutaties in STKU gevonden die bestonden uit een nonsense mutation, drie frameshift deleties, drie frameshift inserties, twee acceptor splice-site mutaties en drie missense mutaties; al deze mutaties leiden tot een inactief eiwit. In de overige zeven families werden geen mutaties gevonden hetgeen op genetische heterogeniciteit wijst. Er lijkt een vrij groot spectrum van mutaties te bestaan die leiden tot PJS hetgeen het testen van individuen bemoeilijkt. Hoofdstuk 10 beschrijft de studie die een vervolg is van de studie zoals beschreven in hoofdstuk 8. Deze somatische studie benadrukt de vroege rol van STKU in PJS tumorigenese. Tevens vonden we aanwijzingen dat de tweede hit van Knudson's hypothese, in dit geval dus 19p LOH, als bio-merker voor kanker gebruikt kan worden. Uit de analyse van de genen uit de adenoma-carcinoma sequentie blijkt dat APC mutaties frequent voorkomen in de carcinomen. De andere genen blijken een minder belangrijke rol te spelen aangezien de mutatie frequentie van deze genen vrij laag is. De resultaten van deze studie en van de studie in hoofdstuk 8 wijzen erop dat een hamartoma- 99

7 Summary'/Samenvatting carcinoma sequentie waarschijnlijk is en dat deze een andere genetische route kan volgen dan de adenoma-carcinoma sequentie. De laatste studie in dit proefschrift in hoofdstuk 11 analyseert de mogelijkheid om microsatelliet merkers te gebruiken als diagnostische merkers voor het vaststellen van het erfelijke Muir-Torre syndroom (MTS) bij patiënten met een talgklier carcinoom. Talgklier carcinomen van patiënten met in de voorgeschiedenis ook een intestinaal carcinoom blijken in een hoog percentage MMR defecten te hebben, hetgeen blijkt uit de gevonden microsatelliet instabiliteit. In sporadische talkgklier carcinomen is het voorkomen van MMR defecten schaars. De MMR defecten konden bevestigd worden met immunohistochemie van hmlhl en hmsh2. Beide genen bleken in even grote mate gemuteerd in weefsel van patiënten met microsatelliet instabiliteit. Aangezien talgklier carcinomen in MTS patiënten kunnen voorkomen voorafgaand aan de ontwikkeling van intestinale carcinomen moet overwogen worden om talgklier carcinomen te onderzoeken op MMR defecten. De diagnose MTS heeft zowel voor de patiënt als zijn of haar familie belangrijke consequenties. 100
