2 VOORWOORD VAN DE VOORZITTER De Stichting CORR, opgericht om de gezamenlijke oogheelkundige research in Rotterdam te stimuleren heeft nu de ruimhartig toegekende gelden van de Stichting Blindenbelangen praktisch volledig toegekend aan projecten. Dat betekent gelukkig niet dat het project ten einde komt. De nu uitgevoerde research loopt nog enige jaren door en de wetenschappelijke resultaten zullen ook nog enige jaren doorlopen. Ik ben blij te kunnen aankondigen dat de stimulans van de eerste CORR subsidie heeft geleid tot het besluit voort te gaan met gezamenlijke research. De daarvoor benodigde faciliteiten voor klinisch wetenschappelijk onderzoek zullen uit vervolgsubsidies worden gesteund. Bij alle partijen bestaat volledige overtuiging dat de toekomst van het onderzoek ligt in samenwerking, ook om de internationale concurrentie aan te kunnen. De combinatie van de grote gezamenlijke klinische praktijk met de geavanceerde faciliteiten van een groot academisch medisch centrum is sterk en wordt, zo hebben wij gehoord van onze internationale adviesraad, ook in het buitenland met enig ontzag bekeken. CORR is op de goede weg en er is nog veel te doen en het is bemoedigend om te zien dat dit ook gaat gebeuren. Dat zal plaatsvinden onder het voorzitterschap van mijn opvolger, Prof. dr. Huug Tilanus. Ik wens hem en de samenwerkende oogheelkundige onderzoekers in Rotterdam daarmee veel succes. Ik zal betrokken blijven bij de oogheelkunde in Rotterdam als lid van de Raad van toezicht van het Oogziekenhuis en van daaruit de toekomstige successen meebeleven. Het voorzitterschap van CORR was voor mij persoonlijk en professioneel een groot genoegen. Dat niet in het minst door de professionele en efficiënte begeleiding van het project door de directeur van ROI, Netty Dorrestijn. De Stichting CORR is haar daarvoor veel dank verschuldigd. 4 1. ONTWIKKELINGEN IN 2013 De stichting Combined Ophthalmic Research Rotterdam (CORR) is in november 2009 opgericht. In deze Stichting werken de afdeling Oogheelkunde van het Erasmus MC en het wetenschappelijk instituut van het Oogziekenhuis Rotterdam, het Rotterdams Oogheelkundig Instituut (R.O.I.) samen op het gebied van innovatief wetenschappelijk oogheelkundig onderzoek. De Rotterdamse Vereniging Blindenbelangen (RVB) heeft deze stichting een startkapitaal van 4,5 miljoen meegegeven voor eerste vier jaar vanaf oprichting. Daarmee wil de RVB het wetenschappelijk oogheelkundig onderzoek in Rotterdam een extra impuls geven. Vanaf 2010 wordt door onderzoekers van beide centra aan diverse grote en kleine onderzoeksprojecten gewerkt. In 2013 heeft dr. W.A. van den Bosch, oogarts van het Oogziekenhuis Rotterdam zijn nevenfunctie als wetenschappelijke directeur van het R.O.I. per 1 oktober beëindigd. Hiermee ontstond ook een vacature in het bestuur van CORR. Met instemming van de raad van bestuur van het Oogziekenhuis Rotterdam is deze vacature vanaf december 2013 tijdelijk ingevuld door de heer J. van Rooij, oogarts van het Oogziekenhuis Rotterdam. Het bestuur heeft in 2013 tweemaal vergaderd (28 maart en 12 december). Dit jaarverslag beschrijft het vierde jaar van de stichting CORR. Om de verslaglegging van de eerste vier jaar compleet te maken, beschrijven we in dit verslag ook twee activiteiten die begin 2014 hebben plaats gevonden: de gecombineerde vergadering met de wetenschappelijke adviesraad en het CORR symposium dat in 2013 gepland was, maar plaats vond op 11 januari Website en ANBI. De stichting CORR heeft vanaf de oprichting de ANBI status. In januari 2014 zijn nieuwe richtlijnen van kracht voor instellingen met een ANBI status. Op hoofdlijnen houdt dit in dat bepaalde informatie zoals jaarverslag, jaarrekening, op de website vermeld moeten worden. De stichting CORR had geen website, dus om aan deze eis te voldoen, heeft het secretariaat zelf een eenvoudige website gebouwd en ingericht. Hierop is alle voor ANBI vereiste informatie te vinden zodat de stichting CORR voldoet aan de nieuwe wetgeving. Het adres van de website is: 4

5 De gecombineerde jaarvergadering 2013 van het CORR bestuur met de wetenschappelijke adviesraad (WAR) 2013 was het laatste jaar dat een subsidie van de Stichting Blindenbelangen wordt uitgekeerd. Het beschikbare bedrag is inmiddels voor het grootste deel toegekend aan projecten. Hoewel deze projecten nog doorlopen tot in 2015 is het niet de bedoeling dat de samenwerking tussen het Oogziekenhuis Rotterdam / Rotterdams Oogheelkundig Instituut en de afdeling Oogheelkunde van het Erasmus MC die door deze subsidie op verschillende terreinen succesvol van de grond is gekomen, ten einde komt. Om die reden heeft het bestuur het resterende bedrag van de stichting CORR bestemd voor startsubsidies die specifiek aangewend dienen te worden om nieuwe gemeenschappelijke onderzoekslijnen te initiëren en hiermee externe financiering van dit nieuwe onderzoek te faciliteren. Hieronder vallen kosten die gemaakt moeten worden om bijvoorbeeld een pilotfase voor een nieuwe onderzoekslijn uit te voeren of kosten voor een andere vorm van vooronderzoek. Hiervoor zijn in oktober 2013 zeven voorstellen ingediend. De wetenschappelijke adviesraad heeft nog dezelfde samenstelling als bij aanvang in 2010, te weten: Prof. J.A. Forrester, Professor of Ophthalmology University of Aberdeen and Professor of Ocular Immunology, University of Western Australia; Prof. S.M. Imhof, Professor Oogheelkunde en Medisch Afdelingshoofd van afdeling Oogheelkunde, UMC Utrecht; De gecombineerde jaarvergadering 2013 van het CORR bestuur met de wetenschappelijke adviesraad (WAR) vond plaats op 10 januari De leden van de WAR hebben de voorstellen vooraf gelezen en beoordeeld. Tijdens de gecombineerde vergadering kregen de hoofdonderzoekers van de voorstellen de gelegenheid de voorstellen met een korte presentatie en discussie toe te lichten aan de WAR. Na afloop van de vergadering heeft de WAR de gepresenteerde voorstellen besproken en getoetst aan het nieuwe criterium van het bestuur, en een advies uitgebracht aan het bestuur. De wetenschappelijke adviesraad heeft in de vergadering van 10 januari drie onderzoeksvoorstellen van de ronde van 2013 de grading A (highest priority, fundable) toegekend. 1. Genetics of primary open-angle glaucoma in Africa: the GIGA study 2. Characterisation of mirnas in Uveal Melanoma and relation with development of metastases 3. Chloroquine and hydroxychloroquine-induced retinal toxicity. Op basis van de beschikbare middelen besluit het bestuur aan deze projecten 85% van het aangevraagd bedrag toe te kennen. De andere projecten worden niet gehonoreerd. Aan de aanvragers van de toegekende projecten is als voorwaarde gesteld dat het volledige project voor het beschikbare bedrag uitgevoerd kan worden. Alle onderzoekers zijn hiermee akkoord gegaan waarna de toekenning officieel is bevestigd. Een samenvatting van de drie gehonoreerde projecten is weergegeven in paragraaf 2. Op basis van dit advies heeft het bestuur van CORR twee voorstellen goedgekeurd voor financiering en voor een voorstel is een bedrag gereserveerd voor 2013 omdat dit voorstel een nadere uitwerking behoeft. De overige voorstellen zijn afgewezen, in sommige gevallen op grond van het feit dat de voorstellen niet volledig in lijn zijn met de doelstellingen van CORR. 5

6 CORR symposium 2013 Het bestuur van de stichting CORR heeft in 2012 besloten om een budget te reserveren voor een jaarlijks symposium ter inspiratie en educatie van (toekomstige) onderzoekers op CORR projecten en andere belangstellenden. VERSLAG CORR SYMPOSIUM Prof. Dr. J.C. van Meurs The CORR symposium on Cell Based Therapy in Ophthalmology was organized on Saturday 11 January 2014, with various speakers discussing the opportunities and limitations of initiating such therapies in the Netherlands. The key note speaker, Prof. Christine Mummery and Prof. Gerard Wagemakers s have been so kind to allow us to show their presentations on the CORR website. With prof. Arnold Vulto as a chairman, Prof Caroline Klaver discussed which hereditary retinal dystrophies might be most amendable to gene-therapy. Prof. Jan van Meurs presented some patients with exudative age-related macular degeneration, with long-term preserved macular function after autologous (tissue from the same patient) retinal pigment epithelium transplantation, as a proof of principle that transplanted tissue can be of benefit and as a rationale to try and produce better tissue. Prof. Christine Mummery, LUMC, presented a overview of the possibilities of stem cells from other fields (also cardiology), but pointed out that setting up such a program for ophthalmology from scratch was extremely expensive, and certification and authorization are real hurdles. It would be more worthwhile to try and co-operate with an established program abroad. As an example Prof Mummery mentioned a project in New York, where doctors are isolating retinal progenitor cells from cadaver eyes. Prof Gerard Wagemakers showed the possibilities of stem cell therapy in combination with gene therapy, where hematopoietic stem cells are infected with the missing gene for several orphan diseases, that currently have an extremely expensive treatment. Dr. Rene de Coo, discussed his planned trial where patients therapy. Dr. Rob Collin discussed a new development in gene therapy in which the transcription of the defective gene is selectively blocked. Mr. Mandeep Singh showed beautifully illustrated data that, in mice, cells taken from a neonate are capable of regenerating rod photoreceptor cells in diseased mice that connect functionally to the visual cortex. Prof Bergen reported experiments in which embryonic stem cells were successfully educated to differentiated retinal pigment epithelial cells. Dr. Peter de Haan (Armana Therapeutics) showed his lab s closed system to isolate mesenchymal stem cells to be used as hematopoietic stem cells, possibly after gene transfer using a novel short RNA vector. Prof Vulto concluded the session by reminding the audience that so far few protocols had been approved by the national central review board (CCMO). 6

7 2. SAMENVATTINGEN GEHONOREERDE PROJECTEN CORR IN Genetics of primary open-angle glaucoma in Africa: the GIGA study. Project group This research is a collaboration between The Rotterdam Eye Hospital (Hans Lemij; Jan van Meurs); the Dept. Ophthalmology (Pieter Bonnemaijer, Caroline Klaver; Henriët Springelkamp; Hans Vingerling) and dept. Epidemiology (Cock van Duijn; Bert Hofman; André Uitterlinden) from Erasmus MC in Rotterdam, the Netherlands; CCBRT Eye Hospital (Alberta Thiadens) and Muhimbili National Hospital (Anna Sanyiwa) in Dar es Salaam, Tanzania; and Groote Schuur Hospital, Capetown, South Africa (Colin Cook). Summary Glaucoma is the most important cause of irreversible blindness worldwide. Primary open-angle glaucoma (POAG) is the most common subtype of glaucoma, and the least understood. Intra-ocular pressure, family history, and race are well-known risk factors. Persons with African ethnicity have a 3x increased risk of POAG; they have a more severe course of disease with a higher risk of blindness. New technologies have identified many genetic risk factors for POAG endophenotypes, but these factors explain only a minor proportion of POAG cases. Strikingly, these studies are predominantly based on study participants of European descent. It is likely that gene finding will be more effective in study populations which have a high frequency of disease and a more severe phenotype. The aim of this study is to identify new genetic factors for POAG in African populations. The results of this study will advance our understanding of the genetic background of POAG and help elucidate its underlying biology. The GIGA study (Genetics In Glaucoma patients of African descent) comprises the following steps: 1. We will establish large cohorts of well-defined POAG cases and controls from African ancestry in Tanzania, South Africa, and Rotterdam. We will use the existing CORRBI framework (biobank of The Rotterdam Eye Hospital and Erasmus MC) to collect clinical data and store biosamples. 2. Genetic variants will be evaluated by Genome-Wide Association Study using a SNP array and imputation to 1000 genomes. We will first investigate the association of known POAG and POAGrelated genes in these populations, and calculate their contribution to the risk of disease in the African study populations. 3. We will identify new genetic factors for POAG in these cohorts by estimating the risk of POAG for each SNP using a logistic regression model adjusting for age, sex and principal components of genetic ancestry. Significant findings will be validated in other cohorts of African ancestry, and a meta-analysis of combined data sets will be performed to increase statistical power. Risk variants will be studied further in European cohorts (Rotterdam Study, IGGC) to evaluate their significance for POAG in other ethnicities Characterisation of mirnas in Uveal Melanoma and relation with development of metastases Project group Dr. E. Kilic and Dr. H. Mensink on behalf of the ROMS; Dr. E.A.C. Wiemer, dept. of Medical Oncology, Erasmus MC Summary Uveal melanoma (UM) is the most common form of primary eye cancer in adults. Nearly 50% of all patients will die from distant metastasis, mainly located in the liver. Metastatic spread of UM occurs haematogenical. To identify patients at increased risk of metastases previous studies have identified prognostic markers. Gene expression profiling classifies the tumours into a high risk profile and low risk profile. Since mrna-expression profiling cannot classify all tumours in the right subgroup, other options should be explored. Micro-RNAs (mirnas) are used in many other types of cancer for diagnostic and classification purposes and have been reported as plasma or serum biomarkers for solid and haematologic tumours. In UM predictive plasma/serum biomarkers for metastasis are not available and biomarkers for early detection are important to identify patients at high risk for developing metastatic disease. 7

8 Little is known yet about the role of mirnas in UM carcinogenesis and UM biology. Our extensive and well defined study population and UM sample set provides an excellent starting point to identify suitable mirnas that can be used as biomarker in a clinical setting and/or mirnas that function as tumor suppressor or oncogene in UM. The aim of this study is to identify differentially expressed mirnas between high risk and low risk UM, to get a better understanding of the involvement of mirnas in the tumour biology of UM and to apply mirna expression differences for clinical purposes. More specifically we will investigate the use of tumour-derived, circulating plasma/serum mirnas as biomarker to stratify patients being at low or high risk for metastasis. In addition we will explore the use of mirnas and their UM relevant targets, for therapeutic purposes using UM cell lines. With this study we start a new collaboration of the ROMS with the Medical Oncology department for finding new targets for detection of metastases in UM Chloroquine and hydroxychloroquine induced retinal toxicity: Mechanisms and treatment options. Of Immunology, ErasmusMC, Rotterdam Summary Chloroquine and hydroxychloroquine are commonly used in the treatment of inflammatory diseases, because they are slow acting but effective drugs with relatively mild side effects. However, a major concern is the occurrence of retinal toxicity which can result in vision loss. As a consequence, regular eye examination by an ophthalmologist is called for when taking these drugs. Unfortunately, retinal toxicity can only be detected after significant damage has already occurred and the retinal damage is largely irreversible. The risk of vision loss obviously presents a major psychological burden for patients and is considered unacceptable by many. Methods to prevent retinal toxicity and loss of vision must therefore be sought. Chloriquine (CQ) and hydroxychloriquine (HCQ) affect many cellular processes but their main effect is thought to be inhibiting Iysosomal function. The mechanistic aspects of CQ and HCQ are not well understood, and the difference in toxicity between the two compounds is still unexplained. Our preliminary results with ARPE-19 cells indicate that CQ and HCQ induce loss of retinal pigment epithelial (RPE) barrier function and we hypothesize that this contributes to retinal toxicity. Further investigation is needed to discover the cause and determine if the barrier function can be restored. Therefore, we aim to expand this in vitro model with primary RPE. We hypothesize that primary RPE could be more susceptible to apoptosis, oxidative stress and stress due to the loss of lysosomal function. Therefore, comparative studies between primary RPE and ARPE-19 cells are absolutely required to gain insight into the mechanisms involved in retinal toxicity caused by CQ and HCQ. Moreover, comparison of primary RPE an ARPE-19 cells using proteomic and genomic approaches may also yield vital, novel insights into retinal toxicity caused by CQ and HCQ. This in vitro model will also be used for the pharmacological testing of various auspicious medicinal targets for their ability to reduce retinal toxicity caused by CQ and HCQ. 8

VOORTGANGSVERSLAGEN Genetics and heriditary of uveal melanoma s Verslag periode tot Samenwerking tussen beide centra oogheelkunde De Rotterdamse Oculaire Melanoma Studiegroep (ROMS) is een intensieve samenwerking op gebied van oncologische oogaandoeningen tussen het Oogziekenhuis Rotterdam (OZR) en het Erasmus MC waarbij er regelmatig overleg plaatsvind over de voortgang van het onderzoek en waarbij er gebrainstormd wordt over nieuwe onderzoeksmogelijkheden. Beide centra zijn evenredig vertegenwoordigd en de taken worden zoveel mogelijk verdeeld. Zo zijn er binnen de ROMS verschillende sub-onderzoekslijnen: dendritische cel therapie (DCT) trial (Paridaens, van den Bosch & Mensink, OZR), bestraling gerelateerde zaken (Naus, Kilic & van Beek, Erasmus MC) en de conjunctiva melanomen (Paridaens, Naus, & Verdijk). Binnenkort zal de ROMS worden uitgebreid met een oncoloog/internist (K. Geul, OZR, SFG), radioloog (C. van Rij, Erasmus MC) en een corneachirurg (T. van Goor, OZR). De ROMS werkgroep komt drie tot vier keer per jaar samen waarbij er (onderzoek)beleid, behandelplannen en nieuwe projecten en samenwerkingen worden besproken. Verder zijn er reguliere werkbesprekingen van de direct bij dit project betrokken medewerkers (twee maal per maand) ten einde een goede en adequate voorgang te waarborgen. De senior investigators (Paridaens, de Klein, Verdijk en Kilic) van de participerende instituten dragen gelijkwaardig bij aan de totstandkoming en review van manuscripten en het schrijven van project voorstellen. Klinische output van het project Beide Oogheelkunde-Oncologie afdelingen van het Erasmus MC en OZR includeren patiënten en tumoren in de DCT trial en verzamelen klinische data en materiaal. In 2013 werden 21 nieuwe uvea melanoom (UM) patiënten genetisch gekarakteriseerd voor de DCT studie en 4 van de patiënten kwamen in aanmerking voor DCT therapie. Het betrof hier ook oogmelanoom patiënten die op basis van een biopt bij een oogsparende behandeling geïncludeerd konden worden. Verder werd het controle cohort (UM patiënten welke niet in DCT studie geïncludeerd zijn) uitgebreid. Verder werden 95 bloedsamples (bij diagnose en follow-up) verwerkt en opgeslagen in de oogmelanoom tissuebank. N.B. de ROMS tumor- en databank bevat nu meer dan 670 unieke patiënten. De karakterisering van het materiaal omhelsde onder meer chromosoomanalyse m.b.v. SNP arrays (n = 105 tumor monsters), targeted FISH testen (n = 33) voor de inclusie van patiënten voor DCT therapie. Verder zijn een aantal samples gesequenced met de nieuwe Next Generation Sequencing (NGS) techniek (n = 12) voor de detectie van specifieke somatische mutaties als voorwerk voor een in 2013 geschreven en in 2014 gehonoreerde KWF aanvraag. BAP1 analyse (bio-marker slechte prognose) en analyses van oogmelanoom specifieke GNAQ/GNA11 mutaties is samen met afdeling Pathologie gevalideerd en de resultaten zijn verwerkt in een manuscript. Daarnaast werden diagnostische tests voor mutatiedetectie van nieuwe oogmelanoom genen zoals SF3B1 en EIF1AX op gezet. Voor de validatie van de immuunhistochemie (BAP1, TERT) en de mutatie analyse van in de NGS gevonden UM tumorgenen BAP1, SF3B1 en EIF1AX, GNAQ of GNA11 hebben we ongeveer 1500 Sanger sequencing reacties en 500 Q-PCR testen uitgevoerd. Dankzij onze lange follow-up hebben deze tests betere en nieuwe inzichten gegeven m.b.t. de prognostische waarden van deze markers. Deze kennis en het materiaal zal worden ingezet om non-invasieve methoden te ontwikkelen ten einde de kans op metastasering beter te voorspellen. Verdere output van het project De AIO/PhD, AE Koopmans (1 fte tot op CORR en van tot op externe fondsen en afdelingsgeld) was verantwoordelijk voor de coördinatie en het verzamelen van het patiënten materiaal en data. Zij werd bijgestaan door J Vaarwater, analist (0,6 fte) en T Brands, analist (0,4 fte). In 2013 hebben tevens diverse studenten (Geneeskunde, S Yavuzyigitoglu; MLO: S Leeggangers, M Mulderen; HLO: M van den Berg) onze groep versterkt. 9

Dit goed gekarakteriseerde materiaal stelt ons in staat om met succes vervolg projecten en subsidie aanvragen bij externe fondsen elders in te dienen. Summary of progress 2013 The ophthalmic biobank CORRBI provides ophthalmologists and researchers a resource for research projects. It will manage, store and safeguard (clinical) data and biological specimens of ophthalmic patients both for the Department of Ophthalmology, Erasmus MC and The Rotterdam Eye Hospital. After obtaining approval by the medical ethical committee (METC), collection of samples started in April Until December 2013 over 2000 samples collected from over 1600 patients have been stored in both institutes. A variety of ophthalmic disorders is represented in the biobank, ranging from common disorders like myopia, glaucoma, uveitis and macular degeneration to rare disorders like drusen papillae and retinitis pigmentosa (see figure 1A). The majority of samples is DNA (from blood), but also aqueous humour, RNA and serum samples have been collected. (Figure 1B) 11

(A) Distribution of ophthalmic disorders (B) Types of samples stored All data of CORRBI biobank is stored in a web-based database that is maintained within the department of Public Health of the Erasmus MC. Database is created using open-source software Lymesurvey and runs on GemsTracker interface. GemsTracker (GEneric Medical Survey Tracker) is a software package for (complex) distribution of questionnaires and forms during clinical research and quality registrations in healthcare. The software allows you to set up your own website for data collection. On a GemsTracker site different users are able to submit, view and send information. For CORRBI the following data is registered: - informed consent - questionnaire - samples - research specific data (subtracted from medical records) The database has quick search options and allows viewing of data of one individual patient. After DNA isolation, concentration of the samples was measured. 98% of first-attempt DNA isolations was successful with yields ranging from 15 to 150 ug of DNA. After a second isolation for a minority of samples the success rate increased to 99.5% of samples. All samples were tested on a XY-PCR, providing information of the usefulness of the DNA and the possible mix up of samples. In 99,5% of cases, the XY PCR was successful with expected results. 1 sample appeared to have a very low concentration of DNA; 1 sample yielded a Y-PCR product in a female patient. We requested a new blood sample from this patient (without mentioning the reason why), but this patient refused another blood draw because of medical reasons. Various DNA samples stored within the biobank have been used within on-going research projects (Prof.dr Klaver, drs V. Verhoeven en drs. G. Buitendijk). These include myopia and age-related macular degeneration. 3 vitreous samples were used for a pilot to investigate the usefulness of stored samples for a specific biochemical measurement. (Prof. dr J. Janssen) Over the last year the biobank shared its knowledge about sample collection, storage and DNA isolation to the students that collect samples in Africa. Furthermore, genetic ophthalmic research projects were supported in both institutes by literature searches, brain storm sessions, organization of 12

Pierrache (The Rotterdam Ophthalmic Institute, Erasmus Medical Center) Introduction Retinitis pigmentosa (RP) is a group of inherited disorders characterized by progressive peripheral vision loss and night vision difficulties. Many genes and pathways involved in the pathogenesis have been revealed and paved the way for experimental trials with gene- and pathway-specific therapies. We need a better understanding of the natural course of disease to assess whether patients are eligible for these therapies and if substantial gain in visual function can be expected. The Rotterdam Eye Hospital and the Erasmus Medical Center have a large number of RP patients with an extensive longitudinal follow-up available for epidemiological research. Summary In August 2013 Laurence Pierrache started as a PhD student on this project. She enrolled in the NIHES research master program with specialization in genetic epidemiology. The study program covers courses on study design, biostatistical methods and genetics (GWAS, NGS and family-based linkage analysis). This theoretical, methodological and statistical knowledge was implemented in the design of this study and will be applied during analysis of future results. A database has been created within the CORRBI (Combined Ophthalmic Research Rotterdam Biobank) ICT infrastructure to collect retrospective and prospective data. Over 900 patients with retinitis pigmentosa have been ascertained from both centers equally. Information on their visual function, genetics and environmental factors is collected from medical charts. Additionally, patients receive a survey by mail or with questions about the onset of disease, risk factors and the use of visual aids. A special interest is taken in patients with mutations in the USH2A gene. This subsample will be used for a pilot study to assess the efficacy of the database set-up. Collaboration Biweekly teleconferences are organized to discuss research findings and future strategies and 13

14 participation in the RP5000 (national retinal dystrophy consortium) meetings. Output Information on 250 patients has been entered in our database, of which 80 have mutations in the USH2A gene. Recycling of CORR grant This research is currently one year ongoing. The grant has been used to pay the salary of PhD student Laurence Pierrache. When results are published, we will apply for other grants to complement and extend this research. The project has generated 13 publications or conference abstracts that describe the new possibilities of OCT for visualizing the vasculature in AMD, degeneration of the nerve fiber layer (attenuation) due to glaucoma, and segmentation of layers for clinical evaluation of retinal structures for AMD and Glaucoma. The results will lead to a better patient care. Dik Introduction We hypothesize that the coagulations factors factor Xa and thrombin drive retinal pigment epithelial cell (RPE) inflammatory and fibrotic responses in proliferative vitreoretinopathy (PVR). Consequently they may be attractive targets for therapeutic intervention. The aim of this project is to provide several lines of laboratory based evidence that these factors are involved in RPE inflammatory and pro-fibrotic activity and PVR, in order to provide a rationale to initiate a clinical trial to modulate coagulation activity in patients at risk for PVR. Summary of progress in The submitted manuscript entitled Factor Xa and thrombin stimulate proinflammatory and profibrotic mediator production by retinal pigment epithelial cells: a role in vitreoretinal disorders? was accepted for publication in Graefes Arch Clin Exp Ophthalmol (see attachment). 2. During epithelial mesenchymal transition (EMT) epithelial cells differentiate into a mesenchymal cell type that is able to produce excessive amounts of extracellular matrix (ECM) molecules. EMT is a major pathologic component of PVR. We examined the effects of factor Xa and thrombin on EMT by RPE cells by examining their effects on α-smooth muscle actin (αsma) collagen (COL) and zona occludens-1 (ZO-1). The last experiments for this study were finished and were processed in a manuscript entitled Thrombin Induces Epithelial-Mesenchymal Transition and Collagen Production by Retinal Pigment Epithelial Cells via Autocrine PDGF-Receptor Signaling which was submitted and accepted for publication in IOVS (see attachment). 3. An assay was set up to determine thrombin activity in the vitreous from patients with and without PVR, patients at risk for PVR development and control groups. Vitreous fluids are being collected at the ROI. Thrombin activity in vitreous samples was determined. In 2014 these vitreous samples will be added to RPE and their effects on RPE will be examined. 4. Experiments have been performed with vitreous fluids on RPE cells in combination with thrombin and/or specific (thrombin) inhibitors like hirudin, low molecular weight heparins (Fragmin) and 15

16 dexamethasone. These results indicate that the vitreous matrix does not interfere with thrombin activity and these inhibitors are able to block thrombin activity in vitreous. In the future, experiments will be performed in which the effect of vitreous (from PVR and non-pvr subjects), in combination with these inhibitors, on RPE activity will be examined. 5. Verena Mulder, starting in November 2013 at R.O.I. has included 160 of 300 scheduled patients with retinal detachment, to study laser flare measurements as a surrogate outcome measure for the development of PVR and a measure for treatment effect to prevent PVR. Recycling of the CORR grant: external funding The CORR funding for the laboratory work of this project ended by the end of However, to proceed with the laboratory work of this project in 2014 we obtained funding from the International Retinal Research Foundation (IRRF; $ for 2014 alone). Planning-progress Finish experiments with vitreous from PVR and non-pvr subjects and control groups in which the vitreous samples will be added to RPE. Process the obtained data into a manuscript and submit it for publication (In June 2014, the manuscript entitled The role of thrombin in proliferative vitreoretinopathy (see attachment) was accepted for publication in IOVS). 2. Finish experiments with vitreous from PVR and non-pvr subjects and control groups in which the vitreous samples, in combination with dexamethasone and Dabigatran, will be added to RPE. Perform a clinical pharmacokinetic study in 24 patients to determine the concentration of Dabigatran in vitreous and subretinal fluid of patients with retinal detachment. METC approval has been obtained. The manufacturer of Dabigatran will perform the concentration assay. The manufacturer also provided the actual active drug (Dabigatran is a prodrug) for the in vitro experiments. 4. Continue the clinical laser flare study. 5. Perform a drug history study in patients with retinal detachment with and without PVR, focusing on their use of thrombin inhibitors (whether they use coumarine derivatives) IgG4-positive Plasma Cells in Inflammatory Orbital Pseudotumors Project group A.M. Breijer, Dr. R.M. Verdijk, Ophthalmopathologist, A.M. Breijer, Erasmus Medical Center, Department of Pathology Aim of the study: To determine if Orbital Pseduotumors can be accounted for as a part of IgG4 Related Disease and to determine the relationship between Idiopathic Inflammatory Pseudotumors of the orbit (IIP) and IgG4-related disease and the response to corticosteroid therapy. Methods: A retrospective case study from Jan 1987 to Dec Clinical data were retrieved concerning demographics, signs and symptoms, lateralization, recurring IIP s, follow-up time, therapy, histological findings and other medical conditions. 73 cases of IIP were evaluated. The tissue slides were re-evaluated and patient tissue was placed in Tissue Micro Array for slicing and processing. Immunohistochemical staining for IgG and IgG4 was performed and IgG4-positive patients were diagnosed if the criteria of >50 positive IgG4 plasma cells per HPF and an IgG4/IgG plasma cell ratio of >0.40 were met. Results: 10 out of 73 patients were IgG4 positive (13,7%). No evidence was found of a correlation between IgG4 positive patients and a better response to corticosteroid therapy or a lower response to corticosteroid therapy in a fibrous pseudotumor. Conclusion: IgG4 positive Idiopathic Inflammatory Pseudotumors of the orbit seem less common than assumed. No evidence was found of a different response to corticosteroid therapy in the IgG4 positive patients compared to the IgG4 negative patients. 16
