2 VOORWOORD VAN DE VOORZITTER Het tweede jaar van CORR is achter de rug. Wetenschappelijke projecten verlopen zoals het aanleggen van een tuin. Er moet worden voorbewerkt, gezaaid en dan is er een periode waarin de tuinman met enige spanning zit te kijken wanneer de eerste loten boven de grond uitkomen. Het bestuur van de Stichting CORR ziet met tevredenheid dat er in het afgelopen verslagjaar al een aantal forse planten zijn opgekomen en daarvan ziet u in dit jaarverslag de weerspiegeling. Het doel van de Stichting is om de samenwerking tussen het Oogziekenhuis en het EMC te bevorderen. De harmonie waarin over die samenwerking werd gesproken in het bestuur en tussen de onderzoeksgroepen is steeds groot, en geeft aanleiding tot grote verwachtingen van de daardoor veroorzaakte synergie. De lange termijneffecten daarvan voor de behandeling en diagnose van oogafwijkingen zullen ongetwijfeld groot zijn als de bijzondere daadkracht en talenten van alle Rotterdamse oogonderzoekers gebundeld worden. In de beschreven projecten is dat al het geval. Voor het bestuur komen er langzamerhand nieuwe uitdagingen. Behalve de opkomende planten verzorgen en water (lees geld) geven moet er ook gekeken worden naar de toekomst na 2014 want het gezamenlijke onderzoek moet doorgaan nadat de huidige bron is uitgeput. Daartoe is nog meer samenwerking nodig en de EMC/OZR researchbesprekingen zijn nog onvoldoende van de grond gekomen. Daaruit zullen de initiatieven voor nieuwe funding moeten komen. Het bestuur is dankbaar voor de grote hoeveelheid werk die voor dit project wordt verzet door Netty Dorrestijn. Zonder haar enorme inzet zou dit project niet zo voortvarend van start zijn gegaan en nu zo goed lopen. Namens het bestuur Prof. dr. A.F. Cohen Voorzitter 2

4 1. ONTWIKKELINGEN IN 2011 De stichting Combined Ophthalmic Research Rotterdam (CORR) is in november 2009 opgericht. In deze Stichting werken de afdeling Oogheelkunde van het Erasmus MC en het Oogziekenhuis Rotterdam samen op het gebied van innovatief wetenschappelijk oogheelkundig onderzoek. De Rotterdamse Vereniging Blindenbelangen (RVB) heeft deze stichting een startkapitaal van 4,5 miljoen meegegeven voor de komende vier jaar vanaf oprichting. Daarmee wil de RVB het wetenschappelijk oogheelkundig onderzoek in Rotterdam een extra impuls geven. Vanaf 2010 worden gemeenschappelijke projecten in beide instellingen uitgevoerd. Activiteiten van het bestuur en de Wetenschappelijke Advies raad van CORR Het bestuur van de stichting CORR werd in 2011 gevormd door Prof. Dr. A.F. Cohen, CEO Center for Human Drug Research (HDR) te Leiden (voorzitter); Prof. dr. J.R. Vingerling, oogarts en voorzitter van de vakgroep Oogheelkunde van het Erasmus MC; Prof. Dr. G. van Rij, emeritus hoogleraar oogheelkunde (Erasmus MC); G. S. Baarsma, oogarts, wetenschappelijk directeur van R.O.I., tevens lid van de maatschap van oogartsen van Het Oogziekenhuis; Prof. dr. J.C. van Meurs, oogarts en lid van de maatschap van oogartsen en tevens onderzoeksleider van Het Oogziekenhuis en onderzoeksleider netvlieschirurgie). De secretaris van CORR was Netty Dorrestijn, managing director van het R.O.I. Het bestuur heeft in 2011 driemaal vergaderd (30 juni, 10 oktober en 31 oktober 2011). De belangrijkste besluiten van deze bijeenkomsten zijn hieronder weergegeven. Naar aanleiding van het Imaging project op het R.O.I. waar gewerkt wordt aan een nieuwe OCT toepassing en waarbij uit de vindingen patenten voortvloeien, is in het bestuur gesproken over de revenuen van patenten die eventueel gegenereerd worden uit dit project. Het bestuur is het eens met het algemene standpunt van Technologiestichting STW in zake de vraag in hoeverre de revenuen van patenten voor het werkgeversdeel aan de financier of aan de werkgever toe komen. STW oordeelt dat dit aan de instelling die de werkgeversrol heeft, behoort toe te komen. Bij subsidieverstrekking door CORR zal aan de hoofdonderzoeker worden gevraagd openheid aan CORR te verlenen over valorisatie van vindingen. Er is een coördinator voor het Oogheelkunde Biobank project aangenomen. In dit jaarverslag staat een verslag van de eerste fase van de opstart van dit project. De wetenschappelijke adviesraad heeft in oktober 2011 zes projecten beoordeeld. Vier van deze projecten zijn, sommige na aanpassingen of voorwaardelijk, goedgekeurd. Twee projecten zijn afgekeurd en voor twee projecten is het voorstel gedaan deze samen te voegen tot een project. In de volgende paragraaf zijn samenvattingen van de gehonoreerde projecten weergegeven. Op maandag 10 oktober vond de gecombineerde vergadering van het CORR bestuur met de wetenschappelijke adviesraad plaats. De zes ingediende voorstellen zijn een maand voorafgaand aan de vergadering naar de WAR leden verstuurd. De onderzoekers hebben hun ingediende voorstel met een presentatie toegelicht waarna gelegenheid was tot het stellen van vragen. Na afloop van de vergadering heeft de WAR de gepresenteerde voorstellen besproken en een advies uitgebracht aan het bestuur. Op basis van dit advies heeft het bestuur van CORR een voorstel werd direct goedgekeurd, voorgesteld twee voorstelen samen te voegen tot een voorstel met beperking van het budget; een voorstel heeft een seeding grant toegezegd gekregen om het onderzoeksprotocol aan te passen aan de voorstellen van de adviesraad, en twee voorstellen zijn afgewezen. Gezamenlijke wetenschappelijke bijeenkomsten gesponsord door CORR Om de samenwerking tussen beide centra te stimuleren, worden bijeenkomsten georganiseerd waarbij het doel is: gezamenlijke inspiratie door over de grenzen van het eigen vakgebied heen te kijken. Hiertoe wordt een programma rondom een thema georganiseerd waarvoor een vooraanstaande spreker wordt uitgenodigd. Hieronder is een beknopte samenvatting van deze twee avonden in 2011 weergegeven. 4

5 Op 8 februari 2011 was er een gezamenlijke bijeenkomst die werd georganiseerd door J. van Meurs. Het thema was: Onderzoeksplannen en mogelijkheden in samenwerking met de TU s. Na een introductie door J. van Meurs sprak Dr. Ir. Paul Breedveld (TU Delft) over de octopus en microchirurgie. Uitgangspunt voor deze spreker is dat de natuur vol zit met vernuftige vindingen die vragen om een technische toepassing. In deze presentatie spraak Breedveld over een darminspectie robot, gebaseerd op een studie van de inktvistentakels en de manier waarop deze tentakels alle kanten op kunnen bewegen, zonder beperkingen. Ir. Arjan Knulst is ook afkomstig van de TU Delft en sprak over diathermie als instrument. Dit onderwerp heeft de specifieke belangstelling van de retinachirurgen. Bij bepaalde transplantatietechnieken (transplantatie van een klein stukje van de sub-retinale laag) is verkleving van het transplantaat aan het instrumentarium een technisch knelpunt, waarvoor diathermie een van de oplossingsrichtingen kan zijn. Prof.dr.ir. R. Dekker (TU Delft) sprak over microchips, en hetgeen hij presenteerde was voor veel aanwezigen een eye-opener. Dekker werkt aan flexibele en uitrekbare chips maken van ultradun silicium waarvoor ontzettend veel medische toepassingen te bedenken zijn. Als laatste spreker vertelde Ir. Nicole Bakker (TU Delft) over haar afstudeerproject: de implementatie van een camerasysteem om de mate van scheelkijken bij kinderen te kunnen meten. Tijdens de afsluitende discussie bleek weer eens duidelijk hoe leerzaam en inspirerend het kan zijn over de grenzen van het eigen vakgebied te kijken en je te laten inspireren daar wetenschappers uit een geheel andere discipline. De tweede gezamenlijke bijeenkomst op 13 september 2011 is georganiseerd door C. Klaver, oogarts Erasmus MC en was een brainstormsessie over Nanotechnology in Ophthalmology. Dit is een onderwerp waar veel over te lezen is, maar de relatie met het eigen vakgebied blijft vaak heel abstract. Dit is echter veranderd na deze sessie. Prof. dr. Gert Storm, Dept. of Pharmaceutical Sciences, Utrecht University hield een boeiende presentatie over Targeted nanomedicines: from design to clinical application". In dit geval betrof het een applicatie voor de toediening van steroiden, een geneesmiddel dat veel wordt toegepast in de oogheelkunde en dit heeft al tot vervolg afspraken en de uitwisseling van onderzoekideeën geleid. De tweede spreker, Prof. dr. Rutledge Ellis-Behnke, Director, Nanomedicine Translational Think TankAugenklinik Medical Faculty Mannheim heeft heel bijzonder onderzoek gedaan op het gebied van hemostase. De presentatie de hij hierover gaf, was heel nieuwe informatie die veel toehoorders heeft verwonderd. Samengevat, een inspirerende sessie waarbij over de doelstelling ruimschoots behaald is en mogelijk ook voor de oogheelkunde in Rotterdam de basis gelegd is voor een onderzoeksproject op het grensvlak oogheelkunde en nanotechnologie. 5

Summary Myopia is a frequent eye disorder which has a 13x increased risk of blindness for high refractive errors. There are currently no treatment options to stop myopia development or to cure the retinal complications. The causes of myopia are mostly unknown, however, it is clear that genetic factors play an important role in the pathogenesis of the disease. In 2008, we started a study investigating the causes of myopia in large epidemiologic settings: the population-based Rotterdam Study; the family-based ERF study; the MYST case control, and the birth cohort study Generation R. We are currently the only myopia study in the Netherlands, and, with inclusion of 21,000 subjects, we are the largest myopia study in the world. Genome-wide association studies (GWAS) have been proven to be effective in finding genetic factors for complex genetic diseases. In this type of study, > 2,5 million genetic markers (SNPs, single nucleotide polymorphisms) all across the human genome can be studied. In 2010, we employed GWAS on the Rotterdam Study and were the first to find a relationship between genetic markers and myopia in the general population. We recently verified our findings for chromosome 15q14 in a metaanalysis of 26 international studies. It is clear that we now have a good start in revealing the genetic background of common myopia, but we are still oblivious of the genetic factors involved in the clinically more relevant high myopia. MYST (MYopia STudy, was particularly designed to fill in this gap of knowledge. In this study, we collect 600 subjects with high myopia ( -6D) and 600 emmetropic control subjects (-1.5D to +1.5D). Participants are thoroughly examined for all characteristics of high myopia, and blood is collected for DNA analysis. The study is still ongoing and is intended to end in early In this grant proposal, we ask for the financial means to perform a GWAS on MYST. This will give us the opportunity to identify specific genes for high myopia and evaluate whether they differ from those leading to lower refractive values. Ultimately, the identification of genetic factors will lead to more insight in the pathogenesis of high myopia, will lead to strategies to slow myopia progression, and prevent the sight-threatening complications of this disease Exploring retinal light reflectivity properties with Spectral Domain Optical Coherence Tomography Project Group H.G. Wefers-Bettink Remeijer, MD, Rotterdam Eye Hospital Summary Optical Coherence Tomography (OCT) is gaining widespread use in clinical ophthalmology, possibly replacing other standards in imaging techniques. Its latest generation, Spectral Domain OCT (SD- OCT), yields non-invasive high speed and high resolution images of the living eye. The macular area is imaged with unprecedented detail. SD-OCT is also gaining in interest for imaging structural damage in glaucoma, a leading cause of irreversible blindness. For glaucoma, the retinal nerve fibre layer (RNFL), which thins progressively, is usually imaged around the optic disc. Detecting such progression is challenging in the clinical management of glaucoma. With progressing disease, the therapy needs to be intensified, whereas without demonstrable progression the disease is well controlled. SD OCT imaging may assist clinicians in early diagnosing glaucoma and detecting progression. To measure its thickness, the RNFL needs to be segmented from deeper retinal layers in SD OCT images of the retina. Therefore, segmentation algorithms have been developed. These algorithms often rely on differences in contrast in reflectivity between the various retinal layers. 6

7 We have recently shown for the first time in a small-scale pilot study (40 eyes) that the reflectivity of the RNFL diminishes with more advanced glaucoma (Van der Schoot J, et al. IOVS 2010;212:ARVO E-Abstract, Vermeer KA, et al. IOVS 2011;3666:ARVO E-Abstract, Vermeer KA, et al., SPIE 2012, accepted, Van der Schoot J, et al. submitted). This novel finding might serve as a typical feature of glaucoma, to be used for improved glaucoma detection, or even for improved progression detection. One of the main aims of the current study is to further explore the reflectivity of the RNFL in glaucoma and to test algorithms that we have already developed and that show promise for accurate detection of (early) glaucomatous damage. We may also fine-tune these algorithms for an improved performance. Another aim of the current study is to explore whether other diseases that might affect the structure of the RNFL, affect its reflectivity, increasing the possible use of our new algorithms. To that end, we will include, for instance, pituitary tumours, retinal detachments and optic neuritis. We apply for the financial support of 0.6 fte physician for 2 years and the research will be carried out at the ROI/REH and the Erasmus Medical Centre. We expect to continue the research for a longer period of time, but that will be outside the scope of the current grant proposal Reduction of anti-vegf intravitreal injections in diabetic macular edema by macular laser photocoagulation. As the visual acuity improving effect of anti-vegf agents is limited in time, monotherapy with repeated monthly or bimonthly anti-vegf injections to prevent recurrent oedema seems unpractical. Laser monotherapy, although reducing the number of patients with severe visual loss over longer periods, is effective in improving visual acuity in a minority (± 1/3) of patients. Many experts suggest combination of laser and anti-vegf to combine the advantage of early visual gain of anti-vegf with the long-term stabilising effect of laser photocoagulation. However, no definite studies are available to prove this strategy. Some early results suggest that this combination might be superior to monotherapy but a proven optimal sequential or combination therapy with laser,anti-vegf and combination is unknown. The objective of this randomized clinical trial (RCT) is to obtain an evidence based optimal treatment strategy for diabetic macular oedema by using the two currently most effective treatment modalities laser photocoagulation and anti-vegf injections. In this study we aim a two-fold goal: 1. to reduce the number of injections to the minimum amount and 2. To obtain the most sustainable long term treatment effect. We will use a multi-disciplinary approach: 1. an assessor-blinded, randomized clinical trial to study the laser photocoagulation as adjuvant to anti-vegf injections 2. extended safety data. In addition to the safety data derived from the RCT, solicited safety data will be obtained from patients who participated in the RCT and continue observation in the Rotterdam Eye Hospital until retreatment will be started. 3. patient outcome assessment 4. an economic model developed on the study arms in the RCT The project group received a seeding grant instead of the required study budget, in order to finalize the full protocol according to the recommendations of the scientific advisory board. 7

Verdijk, Pathology, Erasmus MC, Dr. A de Klein, Clinical Genetics, Erasmus MC The CORR project Genetics of uveal melanoma started March 1, 2011 with the appointment of the PhD student Anna Koopmans, who will collaborate with technical support from Jolanda Vaarwater. With the allocated funds the present uveal melanoma data and uveal melanoma material bank can be maintained and further expanded. Furthermore, we can set up a number of research lines as described in the CORR genetics project. The plan is divided into three sub-questions 1) Detection of genetic abnormalities in uveal melanoma 2) genetic analysis of tumor biopsies and 3) the identification of markers of metastatic uveal melanoma. Uveal melanoma patients and tumor data bank. The old lists and Excel files are converted into an online database filemakerpro UM. In the UM database includes under the project number (EOM) patient data, the pathology report, the research data and an update of patient data at follow-up study. For the latter two, ipads will be used so that the ophthalmologist can add the latest cancer treatment information online at the clinic. We started with a test phase inside the Erasmus MC firewall to test if it is possible to exchange data. In the past year, 16 new patients have been enucleated and blood samples of 64 patients have been received. After determining loss of chromosome 3 and HLA typing, five new patients could be included into the DCT trial (see chapter 3). Detection of genetic abnormalities in uveal melanoma. From the new tumor samples, DNA has been isolated and using SNP's arrays the status of chromosome 3 has been studied. This method is preferred over MLPA FISH, because the presence of two identical copies can be detected using this method. Also in the context of a validation study (Vaarwater et al 2011) a large number of tumor samples are retrospectively analyzed with both FISH and SNP arrays. (Van den Bosch et al 2012). Genetic studies on tumor biopsies. In six tumor biopsies obtained from irradiated uveal melanoma patients, the chromosome 3 status has also been successfully determined using SNP arrays. Identification of markers of metastatic uveal melanoma Using the new technique Next Gen Sequencing, with which in a short time the whole genome or only the protein coding DNA sequences (exomes) can be determined, we want to detect new prognostic markers which predict metastases. Of 20 uveal melanoma and corresponding normal DNAs, an exome DNA sequence has been established. With special software and hardware, these so-called exomes are being analyzed for common mutations in for example GNA genes. These genes are present in approximately 90% of uveal melanoma tumors but unlike previous publications, we cannot establish relationships with prognosis. (Koopmans manuscript in preparation). An interesting chromosome 3 gene is BAP1. This gene has been mutated, particularly in metastatic tumors, and with a specific sequencing technique (targeted) we have determined in 60 uveal melanoma DNAs the sequence of this 11 kb gene. Ophthalmology Erasmus Medical Center In both the Rotterdam Eye Hospital and the Erasmus MC Department of Ophthalmology, research projects aim to study the etiology of eye diseases. These studies are important to improve the detection, prevention and treatment of eye diseases. Such studies are not possible without the cooperation of patients. Patients consent to use of their medical information and waste body materials for research purposes. This could include genetic material from patients for the detection of hereditary eye diseases, but also to serum, anterior chamber fluid, tumour material and RNA. The CORRBI project aims to provide an infrastructure for structured storage of clinical information and physical material of patients with eye disorders. Such an infrastructure has many advantages for the researchers, both institutions and patients because (1) the start of a study is often delayed by collecting materials, (2) collected data and materials are available for multiple studies (3) consent forms of patients are already present and (4) standardized processing and storage of materials improves the quality of the research. Inclusion of patients focuses on both common and rare eye disorders. In the case of the latter group, accurately capturing of the phenotype of patients into the database CORRBI is important to carry out a good selection of patients who are eligible for certain research projects. Early 2012, the Medical Ethical Committee (METC) approved the biobank procedures so inclusion of patients could be launched. Procedures to ensure the issuance of materials and data are established in a way that the interests of patients, physicians and institutions are guaranteed. By April 1, 2012 both institutes started to collect materials. A database for recording clinical data is currently under development. Eventually, the biobank CORRBI will lead to resolving disease mechanisms, cooperation between the two institutions, improve the chances of obtaining grant money and the biobank may play an important role in partnerships with external parties Imaging research: OCT development and clinical applications. 10 Imaging Optical Coherence Tomography (OCT) is a non-invasive optical technique to create cross sectional images of the human retina. OCT is analogous to ultrasound, but uses light to create high resolution images. Commercial OCT systems operate at a wavelength of 850 nm, provide no additional functional imaging, and have a limited depth range. During the project period we have developed an OCT system based on a wavelength tuning technique that operates at a longer wavelength (1050 nm). In addition to structural measurements, the system is also capable of measuring blood flow, in both the retina and, due to the larger penetration depth, the choroid. The unique phase stability of our system is exploited to develop quantifiable parameters for the vascularization and flow in the choriocapilaris and the choroid. These quantifiable parameters might provide a measure of the change in choroidal blood flow that is hypothesized to play a role in the development of AMD. Preliminary results of the choroidal flow capabilities were presented at the 2011 ARVO meeting [2] and updated results were submitted to the 2012 SPIE Photonics West meeting [8]. Visualization of sub-retinal blood flow was used to assess the effectiveness of RPE-choroid graft transplants. This study showed the functional revascularization of a transplanted patch of RPE and choroids. Preliminary, technical results of this method were published in an Optics Express paper [5]. Updated technical results were also submitted to the 2012 SPIE Photonics West meeting [7]. Clinical results, comparing the revascularization from structural OCT and flow measurements, were submitted for presentation at the 2012 ARVO meeting [11]. The developed Doppler flow measurement system is also capable of visualizing retinal blood flow. After optimization of the system parameters for this purpose, the retinal micro-vasculature was visualized. These results were submitted for the 2012 ARVO meeting [9]. In September 2011 a new graduate student (Kari Vienola) has started on the eye tracking project. A line scan ophthalmoscope was designed and a first prototype was constructed to familiarize the graduate student with retinal imaging. Image processing The results from the earlier developed segmentation algorithm for automatic segmentation of retinal layers in OCT images based on a support vector machine were presented at the 2011 SPIE Photonics West meeting and the conference paper was published [1]. In this work, a flexible framework was presented that was able to segment layers, such as the RNFL, based on manually segmented training data. Data from both normal and glaucomatous eyes, acquired with a commercial system (Heidelberg Spectralis OCT) were included in the test set. An extended version of this work was published in Biomedical Optics Express [4]. 10

11 Careful inspection of OCT data suggested that, in glaucoma, thinning of the RNFL is not the only change in this tissue layer. As determined earlier in this project, the OCT signal of the RNFL is reduced in glaucomatous eyes compared to normal eyes. We also showed that these changes can be spatially mapped and reflect the appearance of local RNFL defects [3]. This shows that the OCT signal strength thus carries information about the RNFL s health. Because the OCT signal cannot directly be analyzed quantitatively, we developed a method to model the interaction of the incident OCT probing beam and RNFL and RPE tissue. This model enables us to calculate a specific optical property of the RNFL, the attenuation coefficient, from the OCT signal strength and the segmented OCT data. This model will be presented at the 2012 SPIE Photonics West meeting [6] and spatially resolved attenuation coefficient maps will be shown at the 2012 ARVO meeting [10]. To further investigate the clinical relevance of the attenuation coefficient, a study on the relationship between glaucoma severity and attenuation coefficient was performed. 10 healthy, 10 mildly glaucomatous, 10 moderately glaucomatous and 10 advanced glaucomatous eyes were included and manually segmented. The RNFL attenuation coefficient was calculated and this showed a very significant correlation between increasing disease severity and RNFL attenuation coefficient. It also showed a significant correlation between function (visual field denoted by the mean defect) and structure (RNFL attenuation coefficient). A paper on this was submitted to (and later accepted by) IOVS [12]. Outlook We will incorporate Polarization Sensitivity into the experimental OCT system, continue the image processing as described and incorporate tracking into the system to reproducibly measure retinal structures as a function of time to detect small changes related to glaucoma, diabetic retinopathy and age related macular degeneration. We plan to continue the research on Doppler sensitivity. We will investigate the development of a quantitative measure for choroidal blood flow based on our recent Doppler OCT work to investigate the feasibility to study choroidal flow in AMD patients. In collaboration with prof Austin Roorda from UC Berkeley, CA, who will be visiting our lab for a sabbatical of 6 months, we will develop eye tracking integrated with our 1050 nm OCT system. We will continue to investigate the attenuation coefficient as a novel instrument for diagnosis and monitoring of glaucoma. We will also perform pilot studies to assess the attenuation coefficient in other ophthalmic diseases, such as diabetic retinopathy, AION, neuritis, etc. Other work will focus on analyzing the results from the experimental PS-OCT device, for glaucoma and age related macular degeneration. Introduction We hypothesize that factor Xa and thrombin driven RPE activity plays a role in ocular inflammation and retinal fibrosis in PVR and may thus be attractive targets for therapeutic intervention, given the availability of the clinically used antagonist heparin. The aim of this project is to provide several lines of laboratory based evidence that factor Xa and thrombin play a role in PVR pathology and thus a rationale to initiate a clinical trial to modulate coagulation activity in patients at risk for PVR. Summary of progress in 2011 Stimulation experiments revealed that factor Xa and thrombin dose dependently induced IL-6 and IL-8 production by RPE cells (the cell line ARPE-19 was used). Subsequent experiments using a protein array that allows simultaneous detection of 120 cytokines, chemokines and growth factors in one sample demonstrated that factor Xa and thrombin both induced the production of a broad profile of cytokines, chemokines and pro-fibrotic factors such as GM-CSF, IL-6, IL-8, MCP-3, PDGF-AA, PDGF- BB and TIMP-1 by RPE cells. The data obtained by the quantitative human cytokine arrays were confirmed either by RQ-PCR analysis or ELISA. Remarkably, the cytokines and growth factors induced by factor Xa and thrombin all fit well with the pathophysiological process of PVR. By using specific antagonists for PAR1 and PAR2 in independent stimulation experiments we found that both factor Xa and thrombin both exert their effect on RPE cells via PAR1 activation and subsequent activation of the transcription factor NF-κB. All these data combined demonstrate that factor Xa and thrombin induce the production of proinflammatory and pro-fibrotic mediators by RPE cells via PAR1 induced NF-κB activation. This indicates that factor Xa and thrombin can play an important role in the development of PVR and can thus be considered as treatment targets. These findings resulted in a manuscript (supplement 1) that is currently submitted for publication and was presented as a poster at ARVO Assays have been set up to determine collagen production and alpha-smooth muscle cell production (myofibroblast differentiation). The first preliminary studies to examine the effect of factor Xa and thrombin on collagen production and myofibroblastic differentiation have been performed. This will be presented as a poster and e-poster at ARVO Preliminary experiments have been performed with vitreous fluids on RPE cells in combination with thrombin and/or specific (thrombin) inhibitors like hirudin, PAR1 antagonist and LMW heparins. These preliminary results indicate that the vitreous matrix does not interfere with thrombin activity. Continuation and conforming experiments will be performed in Planning 2012 In vitro studies on the effect of factor Xa and thrombin on collagen production and myofibroblastic differentiation by RPE will be continued and we expect that this will result in a manuscript and submission by the end of

13 Vitreous from patients with and without PVR development will be analyzed for factor Xa and thrombin activity. Furthermore the effects of these fluids on the production of pro-inflammatory/fibrotic mediators, collagen and myofibroblastic differentiation and the contribution of factor Xa and thrombin on this will be examined. ONDERZOEKERS PROFIEL - ROTTERDAMSE OOGMELANOMEN STUDIEGROEP (ROMS) De Rotterdam Occular Melanoma Study Group (ROMS) is een samenwerking tussen het Erasmus MC en Het Oogziekenhuis, waarin onderzoek naar uveamelanomen centraal staat. De afgelopen jaren is de studiegroep actief met onder andere genetische studies naar oogmelanomen (CORR project 1.0.0), de kwaliteit van leven van patiënten na een enucleatie en radiotherapeutische bestraling en sinds kort dendritische celtherapie. De studiegroep bestaat uit de afdelingen Klinische Genetica (Dr Annelies de Klein, moleculair bioloog) en Oogheelkunde (Dr Emine Kiliç, Dr Nicole Naus en Professor Dr G. van Rij) van het Erasmus MC en Dr Dion Paridaens en Dr Hanneke Mensink van Het Oogziekenhuis Rotterdam. Als aanvulling op het verslag over de genetische onderzoeken die door deze groep worden verricht (CORR project 1.0.0), is hieronder een samenvatting van een klinisch onderzoek dat door deze studiegroep wordt uitgevoerd, weergegeven. Met ruim 100 nieuwe patiënten per jaar in Nederland is het uveamelanoom de meest voorkomende primaire intraoculaire tumor bij volwassenen. De uvea bestaat uit de choroïdea, de iris en het corpus ciliare. Choroïdea melanomen komen het meest voor (circa 70-80% van de uveamelanomen). Ongeveer de helft van de patiënten ontwikkelt levermetastasen binnen 3-4 jaar na de primaire tumor. Hier is tot op heden helaas geen effectieve behandeling voor. De overleving van uveamelanoom patiënten is -ondanks veel onderzoek en nieuwe behandelingsmethoden- in de afgelopen decennia niet significant verbeterd: bijna 45 procent van de patiënten overlijdt aan levermetastasen. Daarom wordt veel onderzoek gedaan naar prognostische factoren die kunnen voorspellen welke patiënten uiteindelijk zullen metastaseren en dien ten gevolge overlijden en welke niet. Bij uveamelanomen kan met behulp van chromosoom en/of genexpressie analyses heel goed worden voorspeld welke tumor gaat metastaseren en welke niet (zie figuur). Patiënten met een melanoom, dat verlies van een kopie van chromosoom 3 en/of een klasse II genexpressie profiel toont, hebben een zeer grote kans op metastasering binnen 3 jaar na de primaire diagnose. Patiënten met een hoog risico op metastasen kunnen misschien aanvullend of agressiever worden behandeld. Immuunsysteem Het immuunsysteem speelt een belangrijke rol bij het bestrijden van allerlei ziektes. Het is bekend dat melanoomcellen kunnen worden vernietigd door cellen van het immuunsysteem. Bij deze afweerreactie spelen vele factoren, zoals de T-cellen (lymfocyten) en antigeen presenterende cellen zoals immunoglobulines en costimulatoren, een belangrijke rol. Dendritische cellen vervullen een coördinerende functie in de afweerreactie van het lichaam, met name bij de herkenning van 14

15 kankercellen. Deze herkenning vindt plaats doordat fragmenten van eiwitten van tumorcellen (zogenaamde tumorpeptiden) op het oppervlak van dendritische cellen gepresenteerd worden aan lymfocyten. Door deze activering kunnen de T-cellen op hun beurt de tumorcellen onschadelijk maken. Doel van de studie Het doel van de studie is om door middel van vaccinatie met eigen dendritische cellen een afweerreactie tegen de eventueel in het bloed circulerende micrometastasen te bewerkstelligen. Deze immuunreactie richt zich tegen tyrosinase en gp-100, oppervlakte eiwitten op melanoomcellen. Dit kan, ook met betrekking tot het optreden van bijwerkingen, vergeleken worden met vaccinaties tegen virusziekten, zoals bijvoorbeeld griep of mazelen. In de studie wordt onderzocht of dendritische cel vaccinaties een afweerreactie kunnen bewerkstelligen tegen melanoomcellen, zodat de kans dat metastasen optreden kleiner wordt. Participeren aan studie In de studie van de ROMS gaat het om een experimentele therapie bij patiënten met een choroidea melanoom met verlies van chromosoom 3. Dit is de eerste studie in Nederland om metastasen bij uveamelanomen te voorkomen. Inmiddels nemen verschillende academische centra in binnen- en buitenland deel aan deze studie. 15
