2 NORMCOMMISSIE NEN Zorg & Welzijn Postbus GB Delft Vlinderweg AX Delft T F Algemene gegevens 1.1 Commissie zorg&welzijn@nen.nl Nederlands Normalisatie-instituut 1.2 Werkgebied Informatievoorziening in de zorg De normcommissie bevordert de ontwikkeling en adoptie van normen op het gebied van informatievoorziening in de zorg en betrekt daar nadrukkelijk de Europese (CEN 1 ) en mondiale (ISO 2 ) ontwikkelingen bij. Primaire taak is het volgen van internationaal werk en het toepasbaar maken van internationale normen in de Nederlandse situatie. Secundaire taak is het ontwikkelen van Nederlandse normen. De normcommissie voert de volgende taken uit: 1. Toezicht houden op lopende nationale en internationale normalisatieprojecten waar Nederland trekker van is; 2. Internationale en/of Europese normalisatieprojecten voorzien van Nederlandse input en beoordelen van deze projecten op relevantie en bruikbaarheid voor Nederland; 3. Nederlandse behoeften in (internationale/europese) normalisatieprojecten vaststellen en inbrengen; 4. Zelf indienen en leiden van projectvoorstellen voor Europese en/of internationale normen waar Nederland direct belangen bij heeft; 5. Verkenning van draagvlak en haalbaarheid van ideeën en voorstellen voor nieuwe nationale normalisatietrajecten; 6. Starten van nieuwe nationale normalisatieprojecten: Nederlandse projectvoorstellen komen beschikbaar ten behoeve van de beleidscommissie. Indien hieruit nieuwe nationale normalisatieprojecten ontstaan, zal een werkgroep het project uitvoeren en treedt de normcommissie op als stuurgroep; 7. Onderhoud, beheer en revisie van bestaande nationale normen; 8. 3 3/ Belang Normalisatie in de zorg bevordert de kwaliteit, functionaliteit en veiligheid van informatievoorziening in de zorg. Voor het zorgveld is het van belang dat internationale en nationale normen op het gebied van informatievoorziening in de zorg en de landelijke infrastructuur niet met elkaar conflicteren. De normcommissie neemt bij het analyseren, bijdragen aan en stemmen over internationale normen dit belang in acht. De normcommissie neemt ook bij het ontwikkelen en reviseren van nationale normen dit belang in acht. Onderwerpen van normering bevinden zich op de volgende vlakken: 1. Het zorgvuldig vastleggen en verwerken van (patiënt)gegevens; 2. Het modelleren en coderen van (patiënt)gegevens; 3. Het veilig met elkaar uitwisselen van (patiënt)gegevens; 4. Het opnieuw gebruiken van (patiënt)gegevens voor verschillende doeleinden zoals managementinformatie, kostenoverzichten, kwaliteitszorg en epidemiologie. 5. De spiegelgroep heeft een continu karakter. De taak van de spiegelgroep is het adviseren van de normcommissie ten aanzien van het stemmen op internationale normen van CEN en ISO. 3 Spiegelgroep 3 over kaarten is enkele jaren geleden opgeheven. Spiegelgroep 5 is opgegaan in spiegelgroep 2 (i.v.m. samenvoeging van ISO/TC 215-werkgroepen)

4 4/26 Werkgroepen hebben een duidelijk gedefinieerde opdracht met een tijdelijk karakter. Werkgroepen hebben een nationale scope en ontwikkelen een normdocument. Platforms zijn nationale expert groepen. In 2013 heeft de normcommissie een symposium georganiseerd om bekendheid te geven aan het werk van de normcommissie en mogelijk nieuwe leden te interesseren. Eén nieuwe partij is hierdoor lid geworden. De volgende groepen stakeholders worden onderscheiden:

De normcommissie identificeert internationale projecten en identificeert Nederlands belang; 2. Normcommissieleden kunnen zich aanmelden als expert vanuit Nederland om aan een internationaal project mee te werken; 3. Parallel: de normcommissie moet stemmen op ISO work items vanuit Nederlands belang; 4. Nederland kan bij ISO voorstellen indien voor normontwikkeling; 5. Voor ISO standaarden dient per standaard een beslissing te worden genomen of deze in Nederland worden overgenomen en gepubliceerd. De scope van de ISO/TC 215 en de werkgroepen worden hierna in het Engels uitgelegd zoals ze staan beschreven in het business plan van de ISO/TC 215. Scope ISO/TC 215 Standardization in the field of health informatics. Health informatics is the application of information and communication technologies (ICT) to facilitate the creation and use of health-related data, information, and knowledge to support and enable all aspects of the health system. ISO/TC 215 adopted the following high-level objectives to guide its activities: 1. make sure that required standards are available to address the needs of the member bodies and stakeholders, by (a) prioritizing and co-ordinating the development of health informatics standards (b) maintaining a coherent view according to an architectural framework (c) harmonizing existing and emerging health informatics standards to establish a global framework of consistent and common standards for health systems and health data

9 9/26 2. adopt or, where identified gaps necessitate original work, develop standards and specifications for health systems design, use and interoperability 3. approve standards 4. with users, review and maintain standards in use to ensure they are fit for purpose 5. provide access and support to all, especially emerging and developing countries. ISO/TC 249/JWG 1 Informatics of Traditional Chinese Medicine This is a joint working group with and hosted by ISO/TC 249 Traditional Chinese Medicine (TCM) To assist ISO/TC 215 prioritise its activities and coordinate its work program, progress its strategic and business planning functions and support collaboration with other SDOs operating internationally within the health informatics field, particularly through the JIC, it has established three committee advisory groups (CAGs) with the following scopes. ISO/TC215 Committee Advisory Group 1 (CAG 1) Scope: CAG1 provides strategic and business leadership to TC215. Formally, the Executive Council meeting is an advisory group to the Committee Chair. The following objectives are within scope for the Executive Council: prioritizing the development of health informatics standards planning, managing, and evaluating the work of the committee

12 12/26 provide access and support for emerging and developing countries ISO/TC215 Committee Advisory Group 2 (CAG02) Scope: To prioritise new work item proposals (NPs) with the goal of harmonizing work within TC215 with the following objectives: (a) co-ordination of the development of health informatics standards; (a) harmonisation of existing and emerging health informatics standards to establish a global framework of consistent and common standards for health systems and health data (together with CAG3); and (b) development and maintenance of operational plans and the overseeing of their delivery; addressing TC logistics and expediting the standards development process. CAG2 is related to TC operations and standards harmonized across WGs. CAG2 develops and maintains operational plans and oversees their delivery. It will address TC logistics and expedite the standards development process: specifically the WGs may identify standards which should be harmonized with another TC workgroup, technical committee or TC 215 liaisons, and will bring these to the CG for consideration. ISO/TC215 Committee Advisory Group 3 / Cross-SDO Harmonization (CAG3) Scope: CAG3 (in the CHARTER signed by all Joint Initiative Council (JIC) SDOs and previously identified as Joint Working Group 9) is a planning, process determination and coordinating group that makes recommendations to the Joint Initiative Council on resolving gaps, overlaps or issues of counterproductive standardization. It is not a decision body, but recommends to the JIC. CAG3 is also the external (Joint Initiative SDOs) coordination group for the TC215 Coordination Group (which is TC215 internally focused). CAG3 contributes to the following TC objective: Harmonizing existing and emerging health informatics standards to establish a global framework of consistent and common standards for health systems and health data (together with the Co-ordination Group). ISO/TC215 Operations Advisory Group [formerly Operations and Harmonization] Scope: Forum for communication and information distribution between the ISO/TC215 secretary and ISO/TC215 work group conveners, vice conveners, and secretaries and other invited guests regarding the policies and procedures for the development of ISO standards. De meeste CEN & ISO projecten duren meerdere jaren. 13 13/26 Scope: The JIC Council operates as a council of equals and as a liaison group under ISO/TC 215, consists of the respective leaders and appointed liaison members of the participating SDO s to a maximum of 3 members per SDO. The JIC is not an ISO/TC215 working group, council or task force but works closely with ISO and other SDOs with the goal of achieving improved harmonization between other health informatics-focused SDOs. 5.2 CEN/CENELEC werk De normcommissie spiegelt het werk van CEN/TC 251 'Health informatics': 1. De normcommissie identificeert Europese projecten en identificeert Nederlands belang; 2. Normcommissieleden kunnen zich aanmelden als expert vanuit Nederland om aan een Europees project mee te werken; 3. Parallel: de normcommissie stemt op CEN work items vanuit Nederlands belang; 4. Nederland kan bij CEN voorstellen indienen voor normontwikkeling; 5. CEN normen worden in principe door Nederland overgenomen en gepubliceerd. De scope van CEN/TC 251 en de werkgroepen I tot en met IV worden hierna in het Engels uitgelegd zoals ze staan beschreven in het business plan van de CEN/TC 251. Scope CEN/TC 251 CEN/TC 251 is actively engaged with many other standards development organisations, consortia and fora to further efforts to coordinate its work with other organisations that have similar goals, such that the stakeholder need for fewer, but more universal, global standards for health informatics that meet the stakeholder needs are recognised in Europe. CEN/TC 251 Health informatics will in general stimulate the development of ISO standards in areas where there is specific need and in doing so ensure they meet the European requirements. Where necessary it may develop European standards to address regional legislative demands. Such standards are essential if healthcare services are to obtain the benefits of open

14 14/26 systems and avoid the constraints of proprietary interfaces. The standards are based on information models - generic models of aspects of health care or health care information. WG I will therefore develop domain model based reference architectures for evolvable information systems. These will facilitate optimised health care processes for maximum patient perceived quality satisfaction, workflow oriented polycentric care and approaches to archiving healthcare information that are not time limited. CEN/TC 251/WG II Terminology and knowledge representation Convener Vice Convener Secretary Scope Davie Hay (United Kingdom) Open Hayley Miller (UK) Semantic organisation of information and representation of knowledge so as to make it of practical use in the domains of health informatics and telematics and provision of information and criteria to support harmonisation in health care and terminological consistency within TC 251. This encompasses clinical, managerial and operational aspects of the health care record and enabling access to other knowledge. The focus of the work is: - terms, concepts and interrelationships of concepts; - structures for concept systems including those for multi-axial coding schemes; - representation of clinical knowledge in health information systems; - guidelines for the production of coding systems; - systematism of the semantic structure behind the names of compositions and headed sections of the health care record. CEN/TC 251/WG IV Technology for interoperability Convener Vice Convener Secretary Scope Alpo Värri (Finland) Björn-Erik Erlandsson (Sweden) Arto Holopainen (Finland) Adoption, promotion and, where necessary, development of European standards for the application of information and communication technology to: - medical devices for plug-and-play interoperability at the point of care, together with facilitation of the efficient exchange of device data in all healthcare environments; - intra- and inter- laboratory, analyser-lis and analyzer-analyser communications; - medical imagery, including as appropriate non-radiological imaging: - such other device related areas as may from time to time be required; - such other areas for technical interoperability as may from time to time be required.

15 15/26 De meeste CEN & ISO projecten duren meerdere jaren. Zie bijlage A (Overzicht internationale projecten) voor een overzicht van de huidige Europese projecten. Nationaal werk De normcommissie neemt actief deel aan de normalisatie activiteiten van (de werkgroepen van) CEN/TC 251 en ISO/TC 215: om mede richting te geven aan Europese en internationale ontwikkelingen. De normcommissie heeft in dit opzicht zowel een breng- als een haalplicht. Zij brengt de resultaten van nationale normalisatie in bij CEN en ISO mede om bestaande / ontwikkelde nationale producten te beschermen. Zij haalt relevante ontwikkelingen uit CEN en ISO voor nationale normalisatie. In toenemende mate wordt bovendien geharmoniseerd met andere standaarden ontwikkelende organisaties (door middel van de JIC). Waar relevant, zal de normcommissie de resultaten van dergelijke harmonisatie betrekken in haar werkzaamheden.

17 17/26 7 Evaluatie en voortgangsrapportage In dit hoofdstuk treft u de evaluatie en geboekte resultaten over Terugblik 2013 Nederland heeft actief deelgenomen aan CEN- en ISO vergaderingen. De Nederlandse experts hebben geparticipeerd in de voor Nederland relevante Europese en/of internationale werkgroepen van CEN/TC 251 en ISO/TC 215. CEN/TC 251 & ISO/TC 215 JWG + ISO/TC 215 Plenary april 2013 Mexico-City CEN/TC juni 2013 Brussel CEN/TC 251 & ISO/TC 215 JWG + ISO/TC 215 Plenary oktober 2013 Sydney 7.2 Publicatie normenontwerpen Sinds 2010 worden alle nationale normontwerpen gratis beschikbaar gesteld via de website Alle belanghebbende partijen kunnen hun commentaar indienen op de gepubliceerde normontwerpen die ressorteren onder de normcommissie. De volgende norm had in 2013 een openbare kritiekronde: - NEN 7512 ' Medische informatica Informatiebeveiliging in de zorg Vertrouwensbasis voor gegevensuitwisseling ' 7.3 Publicatie normen De volgende norm is in 2013 goedgekeurd en gepubliceerd: - NTA 7515 'Medische informatica Berichtenverkeer in de zorg Functionele basisstructuur voor ondersteunende specialismen' 7.4 Werkprogramma internationale normen Bijlage A geeft een overzicht van het werkprogramma aan de hand van de stemmingen in 2013 een lijst van de actieve internationale normontwikkelingstrajecten. 7.5 Persberichten en evenementen NEN heeft in 2013 voor de volgende onderwerpen een persbericht uitgegeven, voor zover relevant voor het werkgebied van de :

18 18/26 - Het certificatieschema voor NEN-EN onder accreditatie - NEN publiceert het certificatieschema voor NEN openbare kritiekronde NEN Begroting en financiering Scope begroting en financiering De begroting betreft de structurele normalisatie activiteiten. Dat wil zeggen: basisactiviteiten die vereist zijn om: - deel te nemen aan- en invloed te hebben op het Europese en internationale normalisatiewerk; - projecten ter ontwikkeling van nationale normen in te richten; - nationale normen te onderhouden,. - het realiseren van maximale overeenstemming over de normen en projecten. De werkzaamheden ter ontwikkeling van nieuwe nationale normen en ten behoeve van revisies liggen buiten de scope van deze begroting. De financiering voor deze projecten zal gedragen moeten worden door de belanghebbende partijen. De leden van de normcommissie hebben in 2013 een financiële bijdrage geleverd ter hoogte van 1.104,= (ex BTW). De ledenlijst is geactualiseerd en potentiële nieuwe leden zijn benaderd. De leden van de normcommissie hebben besloten om voor 2014 de bijdrage te verhogen (bijdrage wordt 2.275,00 exclusief BTW) om de NEN ondersteuning voor het normalisatiewerk te continueren. Ten aanzien van de overige budget posten: De normcommissie besluit dat voor 2013 de voorzitter geen vergoeding krijgt voor het voorzitterschap. Voor 2013 worden geen reis- en verblijfkosten vergoed voor het deelnemen aan de internationale bijeenkomsten. De normcommissie spreekt de noodzaak uit extra inkomsten te genereren. Potentiële mogelijkheden zijn: het organiseren van een symposium, cursussen en het uithuren van expertise. Eventuele extra inkomen van nieuwe normen zouden kunnen worden gebruikt voor het ondersteunen van de normcommissie. De ervaring is echter dat de meeste projectgroepen ook zelf moeite hebben het benodigde bedrag bijeen te brengen. Voor de verdere toekomst. Het onderhouden van NEN is de eerste prioriteit, de internationale reiskosten voor de internationale bijeenkomsten is een tweede prioriteit. Bij voldoende inkomsten in 2014 kan de normcommissie besluiten voor 2014 de internationale reiskosten voor de delegatieleden te vergoeden.

19 19/26 De financiering vanuit de marktpartijen zal ten eerste worden ingezet ten behoeve van de directe activiteiten van de normcommissie. 8.2 Tijdsinvestering door experts De uitvoering van het in dit commissieplan beschreven normalisatiewerk vereist enerzijds expertise en tijdsinvestering van vertegenwoordigers van de belanghebbende partijen (experts) en anderzijds structurele activiteiten van consultants en administratieve ondersteuning van NEN. De tijdsinvestering van experts betreft het reguliere commissiewerk. De tijdsinvestering die experts leveren bij het ontwikkelen van nieuwe normen en omvangrijke revisies valt hier niet onder. Bij het opstellen van de tijdsinvestering van het reguliere commissiewerk voor 2013 zijn de volgende aannames gedaan: - de tijdsinvestering van de experts beslaat 2 dagen per persoon per vergadering, inclusief het volgen van de documenten die worden besproken; - de tijdinvestering van de experts betrokken bij de revisie van bestaande normen is niet opgenomen in dit overzicht. 20 20/ Conclusie De financiering voor de normcommissie voor 2013 bedroeg 9% van de financiering van De basisactiviteiten zijn afgebouwd door het terugbrengen van de activiteiten van NEN. De reiskosten voor deelname aan de internationale vergaderingen en de kosten voor de voorzitter zijn geen onderdeel meer van het budget in NEN doet haar uiterste best om ondanks de teruggang in het aantal dagen een minimum aan kwaliteitsniveau te handhaven zodat de primaire taken van de normcommissie doorgang kunnen blijven vinden. Voor 2013 is er een deficit van 9 dagen ( 9.936). Het voornemen om extra inkomsten te realiseren voor de normcommissie door het organiseren van cursussen en of symposia heeft in 2013 niet tot extra inkomsten geleid. NEN heeft in samenwerking met de normcommissie een symposium georganiseerd. Dit symposium heeft wel bijgedragen aan de kenbaarheid van het werk van de normcommissie maar niet geresulteerd in extra inkomsten voor de normcommissie. Voor 2014 is de jaarlijkse bijdrage voor de leden aangepast om de ondersteuning door NEN te kunnen continueren. De normcommissie is in discussie met het bestuur van NEN over de jaarlijkse bijdrage.
