5 Introduction You have the very first edition of the Vitality & Tourism Management Journal in front of you. In this journal, you could read management articles written by 3rd year students of the BBA program Vitality & Tourism Management. These articles will give you various insights in contemporary issues related to vitality and tourism. Although being characterized by its cross sectorial approach, the articles are categorized by main emphasize. The defined categories in which the articles are presented are Health & Leisure, Tourism and Vitality. One objective of this journal is to inform all interested stakeholders of the vitality and tourism industries. Another objective is to help actual and future vitality and tourism management students to orientate on their possible careers and inspire them in their ambitions and paths to take. 9 Lexy Wagenaar Increasing growth wellness tourism Latest trends, destinations, consumer motivations and implementation Introduction Over the last years there has been a rapid growth of the tourism industry. Due to this growth we see the occurrence of relatively new concepts such as wellness tourism. Wellness is a trending topic (Hoheb & Puzckó) lately and its popularity keeps rising (Rabobank Cijfers & Trends, 35e jaargang), more and more people are regularly visiting spa s. Wellness itself dates back from the ancient times where roman baths were very common and Cleopatra took milk baths to treaten her skin, but the travel for wellness has always been just for the wealthy. Nowadays travel has become more available for everyone and long destination flights are affordable for almost everybody. This results in different types of tourism such as wellness tourism. Wellness tourism signifies all travels that concern the harmony of body, mind and spirit, with self-responsibility, physical fitness, beauty care, healthy nutrition, diet, relaxation (need for de-stressing), meditation, mental activity, education and environmental sensitivity, social contacts as fundamental elements (Mueller & Kaufman, Wellness Tourism: Market analysis of a special health tourism segment and implications for the hotel industry., 2001)This concept is a big opportunity for tourism destinations and its companies and is easily implementable. But before this can be implemented it is necessary to gain some more knowledge about the trends in wellness and tourism and the motivation of the consumers. Trends Wellness trends There are always the basic wellness trends that will never disappear such as the facial treatment, massage, manicure, steam bath and sauna(smith & Puczkó). But wellness has become so much more than just a day in the spa. Spas are an important part of wellness tourism, but wellness is much more than just spas (Global Spa Summit, 2011). There are some new and interesting trends arising for 2013, one of those new trends is called Earthing (Kooistra, 2013). This trend promotes the connection with the earth and to ground to it again, doing this by walking bare foot, treatments in nature and digital detox. This is trending because of the continues surrounding of concrete and technology. Another trend that arises due to our concrete and busy lifestyle is the mindfulness massage (Kooistra, 2013). This treatment provides relaxation for body and mind. The purpose of this treatment is to get out of your own head and to pay attention in a particular way on purpose; in this specific moment. By integrating mindfulness with massage, clients will not only reduce stress during the treatment, but by engaging in the ongoing component of mindfulness, they may also experience the benefit of brain changes that enable them to become more resilient in coping with stress on an enduring basis (Ellis, 2013). The old wellness traditions are regaining awareness and there for Ayurveda is indispensible in the 2013 trends. This treatment or lifestyle ensures a balance between mind and body (Kooistra, 2013). It restores the misbalance in the body by the application of old Hindu health education. Wellness activities do not only attract woman anymore and there is a trend especially for the men. Men are becoming more conscious about their appearance and they want to look good and young too. About 25% of all men in the US are undergoing treatments (Ellis, 2013). They go for massages, facial, manicures and even injectables. This phenomenon is called Brotox. Overall there has been a slight shift in wellness trends. People are in search for inner balance and a healthy lifestyle. Nature and nutrition are becoming more important and people are eager for mindfulness. There is a wide variety of wellness based treatments and activities so it is always possible to implement a suitable one in your business that fits the company profile. Tourism trends Going green and healthy is the big trend not only in the wellness industry but also in the tourism industry. An ordinary vacation is linked to excessive eating and drinking and lack in exercise and sleep. Therefor it s normal for most tourists to return from their vacation unhealthier as they arrived (Ellis, 2013). Due to the rising stress levels and increasing amount of chronic diseases on a global level, the concept of vacation is changing and healthy hotels are arising (Ellis, 2013). These hotels are focusing on healthy eating, exercises, productivity and rest. These hotels are not only healthy for their customers but also for the environment. The emissions are minimized and the water use has significantly decreased. Due to the increasing awareness of the human influence on the environment people have the need to reconnect with nature (WTW, 2013), new trends make this possible. For example there are deprivation holidays where consumers undergo a boot camp to improve their fitness or even weight loss while being outdoors. Hot springs are another possibility to reconnect with nature. This ancient treatment is once more gaining ground in the wellness and tourism industry (Erfurt-Cooper Vitality & Tourism Management Journal Nr.1 9

10 & Cooper, 2009). This can perfectly be combined by hiking through nature and outdoor fitness activities. Sequentially people want to escape from their busy regular life and according to Murphy (2012) 77% of the consumers believe that a holiday without gadgets will be liberating. Giving your technological life a rest will have a positive effect on the state of mind and it relaxes and improves productivity. Conclusion There are a lot of similarities between the trends in the wellness and tourism industry. The interest in nature and how to decrease polluting the environment is rising. As well as personal health which is a hot topic at the moment. We also see that wellness and spas are not only for the ladies anymore because men are going for spa treatments too. People are aware of their physique and they want to improve or maintain their fitness / wellbeing. This can be obtained by healthy nutrition, exercise, treatments and balancing body and mind. By slight modification in the facilities and the offer a tourism destination or company can attract a totally new target group and penetrate a new market. Offer and destinations A lot of hotels are already implementing wellness into their concept. We see a lot of hotels with saunas facilities and the ability to enjoy a massage or facial treatment. Around the Mediterranean you see lots of hotels offering thalasso therapy. And so you see specific treatments at specific areas in the world. There is a wide offer of wellness tourism and a lot of destinations apply wellness to attract tourists. Offer We can segment the different offers of wellness tourism. The most important services are therapeutic, spiritual & holistic and lifestyle based (Hoheb & Puzckó). Puur&Kuur is a Dutch company that arranges wellness trips all over the world. They segment their travels into different types. They offer the possibility to go on an active, detox / weight loss, thalasso, wellness break, ayurveda, yoga / pilates, antistress / burn-out, mindfulness holiday (Griensven, 2013). Destinations Because of the specialization of certain treatments in specific areas and the use of the environment the offer and the destinations differentiate. Products & services in 2020 Most Important Region Therapeutic services Central-, Eastern Europe Spiritual & holistic services Far East, South East Asia Wellness hotels & resort spas Africa, Central and South America, Middle East, Australia, New Zealand Lifestyle-based services Northern, Southern and Western Europe, North America Africa offers mainly spirituality and natural environment. As where Asia offers spirituality, medical and alternative treatments. Australia and the Middle-East provide tourists with traditional and alternative treatments. For Europe the environment plays an important role. In the west and central part of Europe natural healing is offered. Surgical services are an inevitable part in the wellness tourism in all regions in Europe except in Southern Europe. In North America tourist can enjoy almost all segments of wellness tourism, medical services are the most important assets. Alternative treatments and the natural environment are highly valued too. In Central and South America tourist can enjoy traditional and complementary treatments. The environment is of high importance as well (Hoheb et al., 2012). Some popular wellness destinations are Budapest and Hajdúszoboszló who dispose of thermal waters. There are a lot of cities all around Europe who are still very popular due to their thermal waters and hot springs. Hot springs in Asia, South America, New Zeeland and Iceland are also still attracting tourist. India, Thailand, Malaysia and Singapore are very popular among medical tourists (Erfurt-Cooper & Cooper, 2009). Active tourists will find their saturation in spas and safaris in Kenya or spas and golf in Cyprus. Typical destinations for Ayurveda are India, Sri Lanka, Mauritius and Bali. Where detox and weight loss can be found in the USA, Malta, Hungary and Greece. For Holistic treatments the best destinations are Thailand, Malaysia, Sri Lanka, the Maldives and Mauritius. Thalasso therapy can be enjoyed mostly around the Mediterranean in countries like Crete, Cyprus but also in Ireland (Smith & Puczkó, 2009) The health and wellness tourism market is growing (Euromonitor International, 2009) and it doesn t seem to stop any time soon. Therefore it would be a strategic move to implement wellness in tourism destinations and companies. Companies can generate more revenue by implementing this concept because the demand will keep growing. 10 Vitality & Tourism Management Journal Nr.1

11 Motivation The number of people who suffer from chronicle diseases is rising therefore people are becoming more aware of their physical and mental health (Tsui, 2008). The hectic pace of living and the high stress-levels at the workforce causes the need to slow down (Voigt, Brown, & Howat, 2011) and to improve the quality of life (Chen, Prebensen, & Huan, 2008). The changes in the health care system from healing to preventing make consumers more conscious about their health and physique. According to the research of Naylon and Kayser motives for wellness tourism might be the benefits of pampering, self-discovery, fitness, weight loss, experience that other people will envy. This was researched among visitors of fitness resorts. The spa visitors according to the research of Mak, Wong, & Chang (2009) are more motivated by escaping from everyday life, relaxation and relief, selfreward and indulgence, and health and beauty. It depends on the type of person what motivates them towards wellness tourism. According to (Kickbusch & Payne, 2013) there are five types of wellness consumers. The well beings type consider their health and lifestyle importand and are not concerned about the price. Food actives are dedicated to eat healthy with a balanced diet and exercise. They aim for long term solutions and results. The magic bullets are searching for a short term result and they don t mind to obtain that by the use of dietary supplements. This group is somewhat struggling with their health. Fence sitters are a group that is not sure how to deal with health and wellness, they are neutral on most health issues. Finally, the eat, drink and be merrys who do not recognize the need for health or wellness. All these types will be triggered by different offers of wellness tourism. Therefore it is important to target the group of consumers which companies care to attract. Overview The most important elements to motivate consumers towards wellness tourism are from most to least important are: Relaxation Pursuing multiple activities Recreation Experiencing nature Social activity Affordable activity Effortless activity Enhance the quality of life Physical therapy Attractiveness Meditation Health consciousness Mental therapy Word of mouth Curiosity Business engagement These elements show that the lifestyle of wellness tourist is very dynamic. And according to the findings before we can say that wellness tourist are health conscious, looking for relaxation and attractiveness, curious and care to improve the quality of their life (Erfurt-Cooper & Cooper, Health and Wellness tourism, 2009). Implementation There are many ways to implement wellness into tourism. Not only hotels are able to implement on this but also restaurants and companies that organize activities. Depending on the destination and the company profile there are choices to implement trends, treatments and activities. There are only benefits for companies in touristic destinations to apply wellness tourism due to the fact that the demand for wellness tourism keeps on rising. Companies in touristic areas will be able to penetrate a new market and attract a different target group. At the same time they can contribute to a more sustainable way of working with positive effects on all stake holders. This al goes hand in hand with increasing revenue. There are really no reasons not to implement wellness tourism. Conclusion The trends in wellness and tourism are always shifting but the demand to improve life quality in these sectors collide into a new phenomenon. The chance that this relatively new concept will increase its growth is almost certain. For the tourism industry is it inevitable to offer wellness related facilities, treatments and/or activities due to the fact that tourist are becoming more aware of their health and environment. The wellness tourism market has lots of opportunities to attract more and new consumers and to generate more income. 12 Blanca Aarts Communication to Yoga Travelers Opportunity for health sector beginning a yoga retreat in a western country Introduction The aim of this article is to give an insight of globalization of yoga. Did you ever wonder why people are traveling for spiritual meanings? Why this might be interesting for me as a manager of a health sector? Long time ago yoga lifestyle has spread its awareness by Indians who traveled around the world convincing other people to practice yoga for vital benefits. Now days people have become more aware of their health and are actively busy with that. In the western world, yoga awareness is increasing every day and here new concepts of yoga are created. The basic principle stays but the original meaning of yoga is vanishing. By this cause, more yoga practitioners are traveling to India to find the real meaning of yoga. There they stay in a retreat. India is a developing country, where there is an increase in tourists in the country (Aggarwal, Guglani, & Goel, 2008). There are undiscovered places with a lot of spirituality. This attracts tourists where they can be closer to themself. Yoga retreats are mostly located in the nature. By traveling to different kind of country for an experience gives a traveler a new insight of himself. Spiritual and Yoga tourism is foreign tourists that are not looking for luxury but arduous journeys to meet the divine goal or simple life & the technology, which makes traveling easier (Aggarwal, Guglani, & Goel, 2008). Tourism is a fast growing industry now days, where people find it interesting in preventing illnesses and are all the time actively busy with it. They travel all over the places to find their inner peace and themself (Parashar & Dubey, 2012). Seeing the increased awareness of personal health and traveling to a yoga retreats in a far country, western world should implement this idea in their own country where people can find their inner peace by staying in a retreat and live by the original principles of yoga lifestyle. This is a good opportunity for the health management. of yoga. First one is Yama this is self-control, the most important are: truth, nonviolence, don t steel, chastity and renounce possessions. The second one is Niyama, the observance of rules: physical purity, source of supreme joy, burning zeal, self-reflection and surrender to the worship of Ishvara. Third segment is Asanas, physical exercises like stretching to become lean, balanced and vital. Forth is Pranayama, this is the regulation of the breath. Fifth one is Pratyahara, the restraint of senses. Sixth segment is Dharana, steadying of mind. Seventh is Dhyana that is the meditation part. And the last segment is Samadhi where you develop a contemplation of universal consciousness. All these segments help you towards the union of your body mind and spirit. These are the part of classical yoga, yoga before globalization. The Indian tradition has seen yoga as a path to spiritual goal. For few decades there was an evidence-based scientific research where they documented a systematic documentation of the benefits of various yoga-based practices on physiological functioning in terms of physical effects such as metabolic efficiency as well as cognitive aspects such as memory in healthy subjects of different age groups. (Varambally & Gangadhar, 2012). Furthermore, this article describes the benefits of yoga in several psychological disorders. There have been also other researches done with an example of benefits of yoga (Rocha, et al., 2012). They had 2 groups one group was control group and other was the yoga group that where tested on Beck Depression Inventory (BDI), Beck Anxiety Inventory (BAI), and Lipp Stress Symptom Inventory (LSSI). Yoga group was a group that did for 6 months yoga, before and after. At the end yoga has a significant benefit on the specific tested part, it shows low result of BDI, BAI and LSSI, what is good. Therefor yoga is a good way to prevent several sicknesses, physical and mental. It is important to let people know to prevent sicknesses rather than to treat the sicknesses, because while the sickness is treated, unhealthy side effects might occur. Yoga Health awareness of the western Information Yoga is related of religion Hinduism, it is a union of body, mind and spirit. It originates years B.C. from India (Bance, 2005). Yoga is even more than that, it has also eight segments that are part Yoga introduction to the west Yoga has become popular practice that is done by many people in every age worldwide. 13 where body, mind and spirit are combined. In the mid 19th century, Yoga had never left India. Gurus, masters of yoga, started the globalization around year 1900, where they migrated from India to other places. This had an effect on the spiritual seekers that traveled to India in mid 20th century. Celebrities such as the Beatles and Marilyn Monroe have increased the awareness of yoga. Since then yoga have became popular and fashionable (Hoyez, 2007). From here the classical yoga has changed into post-classical yoga. The eight segments were modified into five principles of yoga: Savasana proper relaxation, Asanas proper exercise, Pranayama proper breathing, proper diet and Dhyana positive thinking and Meditation (Bance, 2005). Yoga was originally a meditative practice with little movements, but in the west we are more familiar with the Asanas, these are the body movements. Meditation is still a small part of most yoga practices in the west. Back in India yoga is practiced in nature retreats and isolated village that gives people more relaxation and awareness around them. Here in the west, yoga is practiced mostly in a studio in a city (Kern, 2012). During the globalization of yoga to the world, the west has change the meaning of yoga. All these years people have made yoga an attractive sport, by changing intent of the original yoga. It has become more physical sport, but there are still somewhere yoga practitioners that concentrated on the body, mind and spirit. It is important to grow the awareness towards original yoga. Popular yoga styles Since 1980 s to the present day more and more yoga styles and types appear. Here are the top three: Ashtanga, Bikram and Hatha. Ashtanga is in Sanscrit eight limbs, it is also know as power yoga. Ashtanga provides a useful means for achieving weight loss and performing strength training without using weights. Bikram yoga, known as hot yoga, where 26 different poses are made in a room temperature of 40 degrees. And there is Hatha yoga, the most popular yoga. Hatha is in Sanskrit balance, it is a physical yoga practice that focus on basic postures and breathing techniques (Blank, 2011). There are a lot of different kinds of yoga styles. All these yoga styles are given to people in different ages, it is meant for children, teenagers, adults and elderly people. These styles have been developed recently in the west. It is made on basis of people s needs and wishes. The yoga teachers have listen to their students and create new type of yoga. Spiritual seekers Traveling to India People, who are more interested in ancient yoga, mostly look for spirituality. They really are looking for the meaning of yoga, the tree main objectives, body, mind and spirit. Therefor they travel to country where yoga origin, India, to find the meaning of it. These people are the spiritual seekers, they are also called religious tourism. This tourists can be seen as two different groups: Permanent tourists, where they participate with sacred pilgrimage, a journey driven by faith, religion and spiritual fulfillment, and the other group is the temporary visitors, who may seek to satisfy some personal or spiritual need through tourism (Sharpley & Sundaram, 2005). Furthermore this article, they have research the purpose of tourists in India. The outcome of the research was that the tourist were in India out of curiosity, others to learn yoga and some simply because they had spiritual need that they could find there. Most of tourists are traveling to India to stay in retreats close to Himalaya Mountains. These retreats were built by British people long time ago, to escape from the summer heat of India. If travel and tourism are ways of enlightening the spirit they have been practiced for centuries in India, whose peoples have long expressed a common sense of community in organized religion (Singh S., 2001, p. 138). The retreats are also called Ashrams. It is an Indian word for community and meeting place of religious people, the Hinduisms. These places are meant for life, study and work community (Ashram, 2013). An outcome of a research is that the tourists in India are increasing (Aggarwal, Guglani, & Goel, 2008). In table 2 you can see the increase of tourists during 15 years. The highest peak is in 2007, this eventually will increase in years. This means more travelers will go to India, to find their meaning in their live. There is also a big chance that they will stay there permanent. This gives a good idea of implementing the Ashram concept into western world, where people can stay in a retreat that is located in a peaceful location and live the yoga lifestyle principles. Traveling to other countries There are also other yoga retreats that you can find in other countries. These retreats you can find mostly in a warm country, located in the beautiful nature (Greenwood, 2012). Most retreats, you can stay for minimum 2 or up to 8 weeks. Yoga retreats have their own schedule. Mostly they start at 6 o clock in the morning with meditation and sun salutation (morning exercise). During the day and evenings, the yoga practitioners get several workshops that might be new for them and get help with new movements or breathing exercise. They have also a food pattern, this is mostly a vegetarian meals. The purpose is to detox your body to feel renewed and vital. In the retreats you get also some wise quotes and stories that enriches your wisdom. Beside this, yoga retreats are also organizing trips to spiritual or beautiful places or cultural activities nearby the retreat (Pizer, 2012). But why are there only yoga retreats in warm or beautiful countries? Most people can t even afford such trips to far and warm country or due to economic crisis, where more people are workless (Janssen & Souren, 2011) and not everybody is able to spend money on faraway vacation. Since more and more people are interesting in yoga retreats, is this a good opportunity to have a yoga retreats in their own country, nearby their homes and located in beautiful nature that is offered by their native country. 14 Communication Several ways are used to communicate a new concept into the market (Kaplan & Haenlein, 2010). The old way was always mouth-to-mouth communication, but now days this has been change into the more virtual way. In table 3 you can find the most influenced media towards people. Where people communicate with each other via Internet. Social media is now days the most important tool to reach the target group. This is the best way, to promote and hear the interested people their needs. And the best part is discussing new concepts, to listen to their needs and implement clients ideas. With social media is possible to reach hundreds or even thousands of people (Mangold & Faulds, 2009). There is also some of the good old ways to make a good promotion. Such as visiting yoga schools and ask for co-operations. This is a two way of promoting, promoting each other. Also visiting fairs and workshops and socializing with other people who are interesting in the new concept, yoga retreat. This all is communication towards the potential clients. But there might also occur some problems towards health promotion. These problems are moral and ethical issues, they are mostly sensitive topics, have deeply held beliefs and moral judgments (Guttman, 2008). It is important to know the need of people and to sensitively approach them, by offering something new and attractive to them. Conclusion A yoga retreat is a good opportunity for a health sector. It is a practice that is healthy in physiological, psychological and spiritual way. Here people find themselves and their peace. They are aware of their health and want to be actively busy with it. This is a preventive, of sicknesses, way of living. Yoga has become popular over years and well known by a lot of people. More and more people are traveling to India to a yoga retreat for their inner peace. But not everybody get the chance of doing this. This might be personal issue, shortage in money or due to economic crisis. Where people don t need to travel so far for their needs, here they can safe their money by not traveling too far. Yoga retreats in their own country, western country, could be a good opportunity to offer people something new and interesting. These people can get aware of yoga retreats by good communication, where there is need to discuss about it. 15 Danyel van Zweden De positieve en negatieve groei van medisch toerisme in India Het begrip medisch toerisme, oftewel het reizen naar een bestemming om daar een medische ingreep te laten verrichten, is de laatste 10 jaar enorm gegroeid. Steeds meer mensen bekijken hun mogelijkheden in het buitenland en zijn bereid om te reizen voor medische doeleinden (Connell, 2005). De internationale markt in medisch toerisme wordt ogenschijnlijk gedreven door de huidige druk van de grote vraag naar, en de hoge kosten van behandelingen in het Westen en door de lage kosten en hoge kwaliteit van zorg in een aantal ontwikkelingslanden (Smith, 2012). In dit artikel wordt dieper ingegaan op de niche markt medisch toerisme en wordt er gekeken naar zowel de negatieve als positieve gevolgen voor het land van bestemming die medisch toerisme met zich mee brengt en wat hier aan gedaan kan worden. Hierbij wordt er ingezoomd op het land India, dat zal dienen als voorbeeld in het algemeen. Medisch toerisme Al lijkt medisch toerisme een verschijnsel van de laatste tientallen jaren te zijn, het gaat velen malen verder terug. De oudste vormen van toerisme stonden al direct in verband met het verbeteren van de gezondheid en het welzijn. Voorbeelden hier van zijn de vele diverse spa s die te vinden zijn in Europa waar o.a. de Romeinen al gebruik van maakten (Connell, 2005). Maar het gaat bij medisch toerisme niet alleen over het verbeteren van de gezondheid en welzijn van de mens, maar ook over diverse medische ingrepen. In de literatuur vind je verschillende definities van medisch toerisme. Zo stelt Rayma M. Vijaya, professor Economie aan de Richard Stockton College of New Jersey dat medisch toerisme gaat over het reizen van patiënten naar ontwikkelingslanden waar de medische kosten erg laag zijn (Vijaya, 2010). Een andere definitie van medisch toerisme die gegeven wordt door A. Whitaker is dat medisch toerisme gaat over patiënten die hun land van verblijf verlaten met de bedoeling om de medische zorg binnen te treden, vaak voor een operatie, in het buitenland (Johnston, 2010). Deze laats genoemde definitie zal ook aangehouden worden in dit artikel. De medisch toerist Medisch toeristen kunnen van alle leeftijden zijn, maar vaak betreft het de wat oudere of gepen sioneerde mens uit de Westerse landen waar de prijzen voor medische behandelingen hoog zijn en de wachtlijsten lang. Assenov en Suthin schatte dat in de UK in 2004 meer dan mensen langer dan 6 maanden moesten wachten op een medische behandeling. In the USA zijn zorgverzekeringen erg duur en niet verplicht. Hierdoor zijn veel Amerikanen onder verzekerd of helemaal niet verzekerd en kunnen daardoor geen behandelingen of operaties veroorloven in eigen land (Melanie Smith, 2010). Medisch toeristen vliegen over de hele wereld om die zorg te krijgen die ze willen. In de huidige tijd vindt medisch toerisme plaats tussen zowel rijkere als armere landen en wordt de medisch toerist gemotiveerd door diverse factoren zoals: kosten van de behandeling, kwaliteit van de dienstverlening, lengte van de wachtlijsten en wachttijden, (il)legaliteit van procedures en beschikbaarheid van huidige en alternatieve geneeskunde (Smith, 2012). Een andere reden waarom veel toeristen naar het buitenland gaan is het tropische denkbeeld waarbij men heerlijk in het buitenland, in de zon, aan het strand hersteld van laag geprijsde operaties (Johnston, 2010). Daarbij hoeft ook niemand te weten dat je een operatie heb gehad aangezien je in het buitenland verblijft. Denk hierbij aan cosmetische ingrepen waar nog deels een taboe over heerst. Om een voorbeeld te geven, een Canadees die naar Costa Rica was gereisd voor meerdere cosmetische ingrepen zei het volgende; It was the best experience that anybody could have because while you are healing, you are in the sunshine it is absolutely incredible (Johnston, 2010). Statements als dit werken mee aan het exotische beeld van medisch toerisme, dat er voor zorgt dat de focus niet meer ligt op de ernst van het hebben van een medische ingreep of soms zelfs zware operatie in het buitenland. Globalisatie Zoals al eerder aangegeven gaat medisch toerisme veel verder terug dan je in eerste instantie zou denken, maar is pas sinds de afgelopen 10 jaar echt een opkomende trend te noemen. Medisch toerisme zou nooit bestaan zoals hij nu bestaat zonder de globalisering van onze wereld. Vitality & Tourism Management Journal Nr.1 15

16 Globalisatie kent verschillende definities, maar de definitie die binnen dit artikel gebruikt wordt luidt als volgt: de veranderingen in sociale gedragspatronen en technologie die bedrijven in staat stellen om hetzelfde product over de hele wereld te verkopen (Levitt, 1983). Maar een definitie als deze is vrij abstract. Globalisatie is niet abstract, maar een proces waarbij verschillende maatschappijen, culturen en economieën dichter bij elkaar komen, grenzen vervagen en goederen, technologie, arbeid en kapitaal internationaal integreren (Singh, 2009). Globalisatie heeft dus een grote invloed op medisch toerisme doordat grenzen vervagen en afstanden verkleind worden mede dankzij de steeds vernieuwende technologie. Een reisje naar bijvoorbeeld India vanuit Nederland is tegenwoordig niets geks meer, terwijl dit 100 jaar geleden wel anders was. Maar er is nog een factor die grote invloed uit oefent op medisch toerisme en dat is de opkomst van het internet. De opkomst van het internet heeft het mogelijk gemaakt voor bestemmingsziekenhuizen om informatie over faciliteiten, personeel en diensten te distribueren naar een voorheen ondenkbaar groot publiek, zonder dat ze macht verliezen over de presentatie van de informatie (Johnston, 2010). India Om een beter beeld te krijgen van hoe medisch toerisme in de praktijk werkt en welke gevolgen medisch toerisme kunnen hebben voor het land van bestemming zal er verder worden ingezoomd op India. India positioneert zichzelf als een van de koplopers in de wereldwijde medische markt door het neerzetten van een reputatie in een aantal onderdelen van de gezondheidszorg zoals hart, cosmetische en gewrichtschirurgie. Een van de belangrijkste drijfveren is hun geavanceerde medische technologie die ze aanbieden tegen veel lagere kosten dan het internationale gemiddelde (Smith, 2012). De medisch toerisme industrie groeit snel, vooral in de opkomende markt als India. De Confederatie van de Indiase Industrie voorspelt dat India een omzet van 2 miljard dollar vanuit medisch toerisme zal genereren in In de ban van dit economisch potentieel, is de Indiase Regering actief bezig met het aantrekken van internationale patiënten. Momenteel heeft India wat wordt beschouwd als één van de meest geprivatiseerde gezondheidszorg in de wereld. Indiaas zogenoemde three-tiered gezondheidszorg systeem, bestaande uit primaire gezondheidscentra, district ziekenhuizen en tertiaire ziekenhuizen raken steeds meer overbelast (Shetty, 2010). Volgens Indrajit Hazarika van de Public Health Foundation of India zijn er meerder factoren die verklaren waarom India zo aantrekkelijk is voor de medische toerist. Zo heeft India veel hoog opgeleide professionals, beheerst een groot deel van de populatie en medische staf de Engelse taal, is er een goede toegang tot alternatieve gezondheidszorg, beschikbaarheid van gespecialiseerde behandelcentra, gebruik van geavanceerde technologie en apparatuur en natuurlijk als laatst de meest belangrijke reden die al eerder is genoemd, alles kan tegen een veel lagere prijs worden aangeboden dan in eigen land (Hazarika, 2009). De Indiase overheid De Indiase overheid speelt een grote rol in de enorme groei van medisch toerisme in India. De grote drang naar het verbreden van de economie zorgt ervoor dat de overheid zich actief inzet voor de promotie van het medisch toerisme in hun land. Ze proberen buitenlandse patiënten te werven door grootschalige campagnes waarin India als medische bestemming op de kaart wordt gezet. Ook heeft de overheid een aantal maatregelen getroffen om de groei van het medisch toerisme te stimuleren. Zo bepaalt de Indiase Health Policy dat het verlenen van medische behandelingen en diensten waar voor betaald wordt met buitenlandse valuta wordt gezien als export. Doordat dit gezien wordt als export levert dit een belastingvoordeel op voor de medische klinieken in India die buitenlandse patiënten behandelen. Daarnaast verleent de Indiase overheid een speciaal medisch visum met een maximum van een jaar zodat het makkelijker is voor patiënten om het land binnen te komen. (Singh, 2009) Een van de belangrijkste ethische kwesties die in medisch toerisme speelt in India is de steeds onduidelijker wordende rollen van zowel de publieke als de private sector. In september 2003 introduceerde de overheid het Public-Private-Partnership (PPP) beleid om toekomstige ontwikkelingen in de gezondheidszorg te sturen. Dit beleid schetst een aantal maatregelen omtrent de uitbesteding van het beheer van de openbare voorzieningen van de overheid naar niet-gouvernementele organisaties, de uitbesteding van particuliere gespecialiseerde diensten, en, de uitbesteding van diensten van dochteronderneming ziekenhuizen. Ondanks aanvankelijke pogingen om voorkeuren tussen de publieke en private sector te promoten, blijft het een relatief ad hoc-regeling, waarbij de geplande strategieën en de bewaking van de uitkomsten van de relaties ofwel zwak of afwezig zijn (Smith, 2012). Gevolgen van medisch toerisme in India Medisch toerisme kan niet zomaar plaatsvinden zonder dat dit gevolgen heeft voor het land van bestemming. Deze gevolgen kunnen zowel positief als negatief zijn. Als we kijken naar India speelt de groei en verbreding van de economie een grote rol in de promotie van medisch toerisme. De overheid is hierin geïnteresseerd omdat er veel geld in om gaat. Maar ondanks dat het grote voordelen kent voor de economie van India, kent het vele nadelen voor de publieke sector. 16 Vitality & Tourism Management Journal Nr.1

17 Het Bruto Nationaal Product in India was in 2005 $754 miljard, maar er wordt verwacht, dat dankzij de diensten sector waar medisch toerisme onder valt, in 2035 India tot de top 3 van de wereldeconomie zal behoren (Singh, 2009). De gezondheidszorg is één van de grootste sectoren, in termen als omzet en werkgelegenheid. Tijdens de jaren 90 is deze sector gegroeid met maarliefst 16%. In 2009 was de totale waarde van de sector meer dan $34 miljard. De verwachting is dat dit in 2012 gegroeid zal zijn naar bijna $40 (Hazarika, 2009). Het is dan ook niet gek te noemen dat India hier een groot voordeel in ziet, maar onder dit grote voordeel gaan ook nadelige gevolgen schuil. Doordat de overheid zo gefocust is op de omzet die medisch toerisme kan generen, zal de private sector steeds dominanter worden in India. Hieronder vallen immers de privé klinieken en ziekenhuizen waar de welvarende medisch toerist gebruik van maakt. Maar dit betekent wel dat de publieke sector waar de eigen bevolking het van moet hebben, ondergesneeuwd raakt. De private sector is goed voor 82% van de buitenlandse patiënten, 58% van de intramurale uitgaven en 40% van de geboortes in klinieken. De positieve vooruitzichten van medisch toerisme en zijn verdere groei stimuleren de private sector door de invoering van een toenemend aantal joint ventures en gezamenlijke inspanningen. Positief bekeken kunnen dergelijke partnerschappen bijgedragen tot verbetering van voorzieningen en de invoering van superieure management technieken en informatiesystemen (Hazarika, 2009). De keer zijde hiervan is dat de verbeteringen over het algemeen alleen uitgevoerd worden binnen de private sector omdat hier geld in te verdienen is. In een rapport van het openbaar bestuur van de Indiase regering uit 2005 wordt onderzocht of private ziekenhuizen in Delhi hun belofte hebben gehouden omtrent het verstrekken van een deel gratis gezondheidszorg in ruil voor het kopen van land tegen enorme gesubsidieerde tarieven. Gesubsidieerde ziekenhuizen werd gevraagd om gratis zorg te verlenen aan 25% van de ziekenhuispatiënten en 40% van de poliklinische patiënten. Maar het rapport concludeerde dat de meeste instellingen niet deze gratis zorg hadden aangeboden. Het beleid zou levens redende kansen kunnen bieden voor diegene die levensbedreigend gewond of ziek zijn, maar behandeling hiervan niet kunnen betalen. Het begon als een groots idee om de armere bevolking van India te helpen, maar in plaats daarvan zien we dat deze publieke dollars worden verschoven naar particuliere gezondheidszorg en de voordelen voor de publieke sector erg onduidelijk zijn (Shetty, 2010). De sterke verschillen tussen de publieke en private sector worden gezien als één van de grootste reden van de steeds groter worden kloof tussen rijk en arm in India. Een ander gevolg van medisch toerisme in India is het tekort aan bekwaam en opgeleid personeel in de publieke sector. Volgens een rapport van de Planning Commissie uit 2006 blijkt dat er een tekort is van artsen, 1 miljoen verpleegsters en tandartsen (Planning Commission, Goverment of India, 2006). De beschikbaarheid van medisch specialisten in de lokale Community Health Centres, vergeleken met het aantal goedgekeurde posten is ook zorgwekkend. De bestaande Community Health Centres hebben een hoog tekort aan specialisten zoals verloskundigen en gynaecologen (56%), kinderartsen (67%), chirurgen (56%) en medisch specialisten (59%) (S.K. Satpathy, 2006). Terwijl de publieke sector kamt met een tekort aan personeel en middelen, is geschat dat meer dan 75% van de menselijke hulpbronnen en geavanceerde medische technologie, 68% van de naar schatting ruim ziekenhuizen en 37% van de ruim aantal ziekenhuisbedden in het land zich bevinden in de private sector. Deze cijfers suggereren dat de private sector de belangrijkste werkgever van het personeel in de gezondheidszorg is die voornamelijk zijn opgeleid in instituten voor de volksgezondheid (Hazarika, 2009). Verdere groei in deze sector als gevolg van medisch toerisme zou de interne braindrain verergeren, doordat de beter opgeleide professionals van de publieke sector naar de private sector zullen gaan mede door hogere lonen en superieure infrastructuur. Ook zie je dat ondanks 80% van de Indiase bevolking woonachtig is in landelijke gebieden, dat 75% van de biomedische doktoren en 70% van de ziekenhuisbedden zich bevinden in stedelijke centra. Dit zorgt er mede voor dat een groot deel van de patiënten in de Indiase publieke sector niet de zorg (kunnen) krijgen die zij nodig hebben (Smith, 2012). Als laatst wordt er voorspeld dat in de komende jaren de kosten van gezondheidzorg zullen stijgen als gevolg van medisch toerisme. De laatste jaren hebben verschillende studies de stijging in medische kosten aangegeven. Volgens enquête schattingen van NSSO 2006 zijn de medische kosten tussen en 2004 gestegen met respectievelijk 55, 67 en 77,28%, wat betreft voor de overheid en de private sector op het platteland van India. In stedelijke India zijn de bijbehorende toenames respectievelijk 76,6 en 116,2%. Er wordt voorspeld dat deze trend zich de komende jaren voort zal zetten en dat beetje bij beetje de gezondheidszorg duurder wordt in India. Wel is het zo dat de kosten in India nog steeds lager zullen liggen dan het internationale gemiddelde. Het probleem hierbij is wel dat het voor de bevolking van India ook steeds duurder zal worden terwijl een groot deel van de bevolking zich nu al geen medische zorg kan veroorloven. Dit zorgt er voor dat het gat tussen de arme en rijke bevolking, en tussen de private en publieke sector alleen maar groter zal worden (Hazarika, 2009). Hoe nu verder? Er kan nu wel geconcludeerd worden dat medisch toerisme een grote impact heeft op een land als India. Aan de ene kant heeft het vele voordelen voor de economische groei van India. Maar aan de andere kant brengt het veel nadelen met zich mee voor een groot deel van de Indiase bevolking die zich geen zorg kan veroorloven binnen de private sector. Ondanks sommige inspanningen van de overheid lukt het India niet om het gat te dichten tussen de publieke en private sector. De vraag is ook of ze dit wel willen dichten, aangezien er zoveel geld wordt gegenereerd uit de private sector. In mijn ogen zijn er aantal dingen die waar aangewerkt moet worden, wil India slagen als medisch toerisme bestemming. Vitality & Tourism Management Journal Nr.1 17

18 Er zullen inspanningen moeten worden gedaan om samenwerkingen tussen de publieke en private sector te kunnen vergemakkelijken. Het is belangrijk om deze verbanden vast te stellen, omdat dit zou kunnen helpen bij het vergroten van de financiële capaciteit van de sector van de volksgezondheid, het verbeteren van de algehele beschikbaarheid en kwaliteit van diensten voor de hele bevolking, en het verminderen van de ongelijkheid in normen en arbeidsvoorwaarden tussen de twee sectoren. Er moet meer subsidies komen voor de publieke sector en er dient meer geld dat verdient wordt in de private sector geïnvesteerd te worden in de publieke sector. Hierbij spelen de privéklinieken een grote rol. Zij kunnen bijvoorbeeld investeren in goede doelen binnen de publieke sector om zo de ethische aard van het medisch toerisme te ondersteunen. Er dient meer toezicht te komen op het naleven van regels die door de overheid zijn opgesteld omtrent het verbeteren van zorg voor het armere deel van de bevolking van India. Denk hierbij aan de eerder genoemde regeling van de overheid dat privé ziekenhuizen in Dehli deels gratis zorg moeten aanbieden in ruil voor gesubsidieerd land (Shetty, 2010). De werkomstandigheden en arbeidsvoorwaarden voor personeel in de publieke sector moeten verbeterd worden en de onvrede die heerst onder de werknemers moet geadresseerd en voor zo ver mogelijk verholpen worden. Dit zou namelijk bij kunnen dragen aan het tegen gaan van de brain-drain dat speelt tussen de publieke en de private sector omdat goed opgeleid personeel dan gestimuleerd wordt om te blijven werken in de publieke sector. Al met al heeft India zeker de potentie om uit te groeien tot één van de meeste belangrijke gezondheidzorg centra van de wereld. Maar deze status zal alleen betekenis hebben als de kansen die medisch toerisme voor het land biedt ook de kwaliteit en toegankelijkheid van de publieke sector verbeterd. 18 Vitality & Tourism Management Journal Nr.1

19 Jenny de Back De kansen voor toerisme marketing door culinair toerisme te promoten Introductie Zo veel mooie en verse regionale producten, bijzondere tradities die terugkomen in de keuken en op het bord. De lokale gerechten eten en het bezoeken van de traditionele restaurants brengt de beleving van een land of bestemming op een heel ander niveau. Bovendien is het bijzonder om zulke gerechten te proeven, klaargemaakt door de echte kenners, omdat je ze thuis waarschijnlijk nooit op die manier zult eten. Terwijl er zo een mooi aanbod te krijgen, met in iedere regio weer iets anders, is de komst van de allinclusief resorts niet meer tegen te houden. Grote internationale buffetten domineren, waar de toeristen in hordes op af komen om de kant en klare en veel te gare producten op een bord te leggen. De tradities en verse producten van de bestemming zijn totaal niet meer terug te zien in de buffetten. (Berg, 2013) Is het gemakzucht voor de toeristen dat ze s ochtends, s middags en s avonds gewoon kunnen aanschuiven bij het buffet en onbeperkt drinken kunnen bestellen? Ja, het is gemakkelijk dat alles al in de prijs zit, maar zo mis je wel de bijzonderheden van de bestemming. Wat houdt de toerist tegen om lokale restaurants op te zoeken? En hoe kan een bestemming zich ook van zijn beste culinaire kant laten zien, in plaats van dat men alleen komt voor het klimaat. Daarnaast zijn al deze buffetten niet goed voor het milieu. Er blijft altijd eten over, wat resulteert in dat het allemaal de prullenbak in gaat. Daarnaast worden veel ingrediënten voor de maaltijden zo goedkoop mogelijk ingekocht bij groothandels, wat de druk verhoogt op onder andere boeren om de producten zo goedkoop mogelijk te verkopen (Gössling, Garrod, Aall, Hille, & Peeters, 2010). Culinair toerisme; wat houdt dit in? De term culinair toerisme is ontwikkeld door Lucy Long in de Jaren 98: A participation, including but not limited to the consumption and presentation of food or cuisine, in the special or unique food experiences related to a tourist destination. (Horng, Liu, Chou, & Tsai, 2011) Er zijn drie termen die gebruikt worden om de begrippen voedsel en toerisme aan elkaar te koppelen: keuken toerisme, culinair toerisme en gastronomische toerisme. De keuken (cuisine) focust zich op de manier waarop voedsel bereid wordt. Gastronomie hoort bij de consumptie van voedsel en drinken en het kunnen genieten van goed eten en drinken. Culinair kan ingrediënten, bereid voedsel, drankjes, productie van voedsel, motivatie, activiteiten en voedsel toerisme. (Horng & Tsai, 2009) Dorpen, steden, regio s of zelfs landen kunnen bekend staan om bepaalde unieke keukens of gerechten. (Horng & Tsai, 2009) Denk hierbij bijvoorbeeld aan de Franse keuken, waarvan je door heel het land kunt genieten, of de Nederlandse kust met verse vis uit de Noordzee. Het eten binnen toerisme wordt hierdoor ook vaak gezien als een attractie (Cohen & Avieli, 2004). Waar wordt de toerist door gehinderd? Vele keukens zijn internationaal gevestigd, door heel Nederland bijvoorbeeld kun je genieten van Aziatische keukens, Europese keukens, Amerikaanse keukens, en nog veel meer. Toch zorgt deze toegankelijkheid tot andere keukens er niet voor dat toeristen in een vreemd land gemakkelijker een niet-toeristisch restaurant gaan bezoeken. Toeristen zijn vaak terughoudend met het proeven of eten van onbekend voedsel, waarvan de ingrediënten niet bekend zijn. Cohen en Avieli hebben ontdekt dat zelfs avontuurlijke toeristen vaak kieskeurig zijn tegenover lokaal eten. Elke vakantie brengt zorgen met zich mee; hoe is het klimaat, hoe is het verblijf, wat zijn de gezondheidsrisico s, en hoe is het gesteld met de veiligheid, eetbaarheid en smakelijkheid van het eten en drinken. Zo zijn er verscheidene redenen ontdekt, wat een toerist zou kunnen tegenhouden om een uniek lokaal restaurant te bezoeken, maar juist te gaan voor een meer toeristisch restaurant. (Cohen & Avieli, 2004) Hygiëne en gezondheid De zorgen voor gezondheidsproblemen door het eten van verkeerd eten tijdens vakanties ontstaan voornamelijk door de vele verhalen verteld door andere toeristen. Een gids van de bestemming kan helpen om de verkeerde dingen te vermijden en juist de goede en lekkere dingen niet te missen. In veel landen wordt er afgeraden om kraanwater te drinken, maar de auteur Avieli heeft ontdekt dat Vitality & Tourism Management Journal Nr.1 19

20 sommige toeristen hier wat in doorslaan. Zij weigeren zelfs sla te eten omdat dat wel eens gewassen zou kunnen zijn in vervuild water. Daarentegen geeft een gids over de Filipijnen aan dat je niet paranoïde zou moeten worden maar dat lokaal eten en drinken proberen juist een deel is van de ervaring en beleving van de vakantie. Helaas dragen alle verhalen op het internet of van bekenden en daarnaast alle waarschuwingen om sommige lokale versnaperingen te vermijden, bij aan het compleet voorbij lopen van lokale consumpties, omdat men bang is ziek te worden. Lokale tafelmanieren en gewoonten Bij Westerse tafelmanieren hoort het gebruik van bestek, dit gebruik wordt niet alleen gezien als hygiënisch, maar ook als beschaafd in plaats van eten als een dier. Vaak in gebieden als Zuid en Zuid-Oost Azië wordt er helemaal geen gebruik gemaakt van bestek. Avieli ontdekte dat wanneer toeristen daarvan op de hoogte waren, ze afzagen van het eten daar. In Azië zie je ook vaak dat men gedeelde gerechten op tafel krijgt en daarin allemaal de vingers of het bestek in stopten om er van te eten. Dit is ook iets wat Westerse toeristen vaak afschrikt. Lastig te hanteren bestek kan ook iets zijn dat als obstakel wordt ervaren, zoals het eten met stokjes. Zo zijn er in het Westen ook gewoonten die niet door iedereen gewaardeerd worden. Zoals de kreeften die nog levend in een aquarium zitten, afwachtend tot ze worden uitgekozen om opgegeten te worden, of de nog onbewerkte vissen en zeevruchten die gepresenteerd worden op een flinke laag ijs, alvorens te worden bereid. Maar ook in onder andere China, Hong Kong, Taiwan en Vietnam worden levende dieren bij de ingang van het restaurant neergezet, klaar om bereid te worden. Mogelijkheden van culinair toerisme Toeristen besteden tijdens hun vakantie ongeveer 40% van hun budget aan eten en drinken (Horng, Liu, Chou, & Tsai, 2011), dit is een hoog aantal en is dus een belangrijk onderdeel van de beleving en algemeen oordeel van de bestemming. Verschillende onderzoeken hebben uitgewezen dat interesse en behoefte qua aanbod van eten in een regio, een grote rol speelt in het al dan niet bezoeken van die bestemming. (Mak, Lumbers, & Eves, Globalisation and food consumption in tourism, 2011) Het gedrag van toeristen met betrekking tot eten wordt beïnvloed door vijf belangrijke factoren (Mak, Lumbers, Eves, & Chang, 2011): 1. Invloeden van cultuur en geloof Cultuur heeft invloed op wat de mens geschikt acht als eetbaar, of iets voor hen acceptabel of juist onacceptabel is. Verder bepaalt het welke smaaksensaties als lekker worden beschouwd, zoals pittig of juist absoluut niet pittig. Zo is religie ook bepalend in de keuze om iets wel of niet te eten. Zo is varkensvlees eten bij bepaalde geloven niet toegestaan. 2. Sociaal-demografische factoren Leeftijd kan verschillen laten zien in eetgewoonten, ouderen bijvoorbeeld hebben andere voorkeuren dan jongeren. Zo is ook uit een onderzoek gebleken dat oudere toeristen en hoger opgeleide toeristen zich meer zorgen maken over hun gezondheid, wanneer ze lokale gerechten zouden proberen. Communicatie moeilijkheden Als laatste zou de vreemde taal het lastig kunnen maken om te begrijpen wat sommige gerechten zijn en kan het zelfs moeilijk zijn om een bestelling te plaatsen. Men is onbekend met de ingrediënten of weten de vertaling van de ingrediënten niet. Dit alles kan als te veel moeite worden ervaren en men gaat dan liever naar een restaurant waar meer toeristen komen en waar op zijn minst een taal wordt gesproken die te begrijpen is. Toeristen zijn ook vaak bang om een plek te betreden waar alleen de lokale bevolking zit en gaan dan liever naar een locatie waar meer niet-lokalen zijn. Restaurants met internationale keukens geven een verkeerd beeld aan mensen. De Japanse keuken in de Westerse wereld maakt vaak gebruik van producten gebaseerd op de behoeften van de bevolking daar en maakt geen gebruik van de originele ingrediënten. Zo is de Thaise keuken eigenlijk veel pittiger dan je deze in Nederland vaak proeft. Juist zulke uitgesproken smaken kunnen ziektes als diarree veroorzaken bij toeristen. (Cohen & Avieli, 2004) 3. Voedsel-gerelateerde persoonlijke karaktereigenschappen Hier kunnen twee vormen worden onderscheiden: de angst om iets nieuws te proberen en het zoeken naar variatie. Dit ligt aan je eigen wil om iets nieuws te ontdekken of je te gedragen naar het Nederlandse spreekwoord wat de boer niet kent, dat eet hij niet. 4. Ervaringen uit het verleden Over het algemeen prefereren mensen eten waarmee ze al bekend zijn, maar ook als iets ooit niet goed is bevallen, zal men het niet snel opnieuw proberen. 5. Motiverende factoren Ten eerste zal voedsel een fysieke drijfkracht zijn, omdat het van levensbelang is. Ten tweede kan het worden gezien als een culturele motivatie, omdat het eten van onbekende lokale gerechten bijdraagt aan de gehele culturele belevenis. Het kan een sociale motivatie zijn, interacties tussen mensen tijdens het eten kunnen bijdragen aan het leggen van nieuwe contacten.
