Isaacs School of Veterinary Science The University of Queensland 4072 ABSTRACT NSAIDs are a group of drugs used widely in both animals and humans. The main applications of NSAIDs are for the treatment of pain, inflammation and pyrexia. The primary mechanism of action of NSAIDs is inhibition of cyclo-oxygenase. This results in a reduction of the synthesis of prostaglandins, which are mediators of inflammation. This inhibition also causes a number of side effects, particularly on the gastro-intestinal tract and the kidneys. It is possible to reduce the risk of side effects by knowing the predisposing factors and administering prophylactic medication. This article reviews the side effects of both human and animal approved NSAIDs in dogs and cats. The mechanisms which are responsible for the adverse effects are discussed as well as risk factors, prevention and treatment, and recent discoveries for the development of future NSAIDs. (ISAACS, J.P. (1996). Aust. Vet. Practit. 26: 180) INTRODUCTION Non-steroidal anti-inflammatory drugs (NSAIDs) are widely used in many species of animals. They are as a group one of the most prescribed drugs in man. The main applications for NSAIDs are for the treatment of pain, inflammation and pyrexia. There are now more than 20 different NSAIDs available in Australia for animal and human use (Table 1). Although it is difficult to demonstrate differences in efficacy between these

2 Original English article (continued) drugs in man, significant differences in individual responses to various NSAIDs do exist and this is likely to be the case in animals as well. For example, when one drug is not effective, often a drug from another group will work. The use of NSAIDs is often limited by the high risk of side effects: they have a narrow therapeutic index. The most important adverse effects are on the gastro-intestinal tract and the kidneys, and these can have serious consequences. In addition, significant differences in the pharmacokinetics and pharmacodynamics of NSAIDs exist between species and it is important not to extrapolate dose rates from one species to another. During the 1970s and 1980s, serious adverse reactions were caused in dogs with the use of the newer human NSAIDs because the metabolism and action of these drugs was extrapolated from man to dog. The choice of NSAIDs for dogs and cats has been limited in the past. Phenylbutazone, isopyrin, dipyrone and flunixin have been used for many years in the dog; phenylbutazone, isopyrin and dipyrone have also been used in the cat, but most products containing these drugs are now not recommended for use in cats. In recent years, a number of new NSAIDs have been approved in Australia: copperindomethacin for dogs, ketoprofen for dogs and cats, and carprofen for dogs. Apart from phenylbutazone, which is now rarely used in cats, ketoprofen is the only oral NSAID registered for cats. Some NSAIDs registered for human use are now available as over the counter drugs from pharmacies. These are drugs that have relatively safe profiles in humans, such as ibuprofen and naproxen, but have been associated with serious toxicities in the dog. This easy access to NSAIDs may also have implications for animals. It is not uncommon for pet owners to medicate pets with their own drugs, not realising that they can be quite toxic. De belangrijkste toepassingen van NSAIDs zijn bij pijn, ontsteking en koorts. Het belangrijkste werkingsmechanisme van NSAIDs is de remming van cyclo-oxygenase. Het resultaat daarvan is remming van de synthese van de ontstekingsmediatoren, de prostaglandines. Deze remming kan ook een aantal bijwerkingen veroorzaken, vooral op het maag-darmkanaal en de nieren. Hoewel het moeilijk is om bij mensen een verschil in werkzaamheid tussen deze geneesmiddelen aan te tonen, bestaat er toch een aanzienlijk verschil in individuele reacties op de verschillende NSAIDs en dit is waarschijnlijk ook het geval bij dieren. Wanneer bijvoorbeeld een bepaald medicijn niet werkt, kan een medicijn uit een andere groep wel werkzaam zijn. Het gebruik van NSAIDs wordt vaak beperkt door het hoge risico op bijwerkingen: zij hebben een beperkte therapeutische index. De belangrijkste bijwerkingen betreffen het maag-darmkanaal en de nieren en zij kunnen ernstige gevolgen hebben. Bovendien bestaan er belangrijke verschillen in the farmacokinetiek en farmacodynamiek van de NSAIDs tussen diersoorten. En het is belangrijk doseringschalen niet te extrapoleren van één diersoort naar een ander. In de twee decennia na 1970 werden ernstige bijwerkingen bij de hond veroorzaakt door het gebruik van de nieuwere humane NSAIDs omdat men het metabolisme en de werking van deze medicijnen extrapoleerde van de mens naar de hond. De keuzemogelijkheid in NSAIDs voor honden en katten was voorheen beperkt. Phenylbutazon, isopyrine, dipyrone en flunixine werden vele jaren bij de hond gebruikt; phenylbutazon, isopyrine en dipyrone werden ook bij de kat gebruikt, maar de meeste producten die deze geneesmiddelen bevatten worden nu niet aanbevolen voor gebruik in katten. Diverse nieuwe NSAIDs zijn in de laatste jaren in Australië goedgekeurd: koperindomethacine voor honden, ketoprofeen voor katten en honden en carprofeen voor honden. Behalve phenylbutazon, dat nu zelden bij katten wordt gebruikt, is ketoprofeen de enige orale voor katten geregistreerde NSAID.
